Der bisherige Stand der TechnikThe current state of the art
Wie aus Langzeitstudien hervorgeht kann als Hauptursache für das Versagen der gegenwärtigen dentalen Implantate die Periimplantitis angesehen werden [10, 11].Despite the prolonged lifespan of the osseointegrated implants and with the new current requirements for advanced implantological procedures, new aspects of their failures are also beginning to show with these implants. As can be seen from long-term studies, the main cause of the failure of current dental implants can be peri-implantitis [10, 11].
Peri-implantitis is defined as progressive bone loss in the implant environment, accompanied by inflammatory changes in the soft tissues. With regard to the classification, a distinction is made between the peri-implant mucositis, i.e. H. the inflammatory changes in the soft tissue around the implant neck, which is not accompanied by a decrease in the alveolar bone, from the actual peri-implantitis, i.e. H. the inflammatory changes in the neck area of the implant, accompanied by an X-ray evaluable bone loss.
Peri-implantitis is referred to as a typical multifactorial disease. Despite everything, bacterial infections and / or mechanical overloading of the implant are currently cited as the main causes.
Bacterial infection occurs when components of the oral environment are infiltrated if the soft tissue around the transmucosal implant part does not form a sufficiently effective barrier against the penetration of the bacterial microflora of the oral cavity, toxins and other active substances into the space between the implant and the bone.
Desweiteren ist aus einer Reihe experimenteller Arbeiten [12, 13, 14] bekannt, daß es zu einer stärkeren Besiedlung der Oberfläche des Zahnimplantats durch die Mikroflora des Mundhöhle und zur Akkumulation bakteriellen Plaques an rauen Implantatoberflächen kommt, als im Vergleich zur glatten und polierten Oberflächen Ra < 0.2 µm.Furthermore, it is known from a series of experimental works [12, 13, 14] that the surface of the dental implant is more strongly colonized by the microflora of the oral cavity and that bacterial plaques accumulate on rough implant surfaces than in comparison to the smooth and polished surfaces Ra <0.2 µm.
For this reason, the contact surface to the soft tissues in the majority of dental implants is currently made smooth, highly polished and not porous, so that contamination with bacterial oral microflora and the accumulation of bacterial plaques are minimal.
The disadvantage of such a solution, however, is that the peri-implant soft tissue lying against the smooth, polished surface is composed of at least two components composed, ie the connective epithelium and the connective tissue, each with a different anatomical and cellular structure, which requires differentiation of the transmucosal implant surface to achieve optimal adhesion of both the epithelial and the connective tissue. While the gingival epithelial cells have a higher adhesion and spread on the smooth titanium surface compared to a rough surface, the observation of the roughness effect was opposite for the adhesion of gingival fibroblasts [17, 20, 21].
However, these known solutions are often not optimal, and the implants do not meet the new, current requirements of advanced implantological processes, and the processes involved in their production are often economically complex and ineffective.
Nonetheless, it can be summarized from the current knowledge of research in this area that peri-implantitis, as the main cause of the failure of current implants, depends significantly on the surface treatment of the transmucosal part of the implant and the optimal relationship between the soft tissues and the dental prosthesis complex formed is.
Das Wesen der Erfindung The essence of the invention
The disadvantages mentioned are eliminated or substantially limited by the presented invention of a two-part tooth-jaw implant which is enclosed during the application of peri-implant tissues, the implant being manufactured from technically pure titanium or its alloy, which comprises at least one element of aluminum, vanadium , Zirconium, niobium, hafnium, tin, iron and tantalum group. The implant consists of: the implant fixation for placement in the bone and the implant build-up (abutment) to support the crown. With their parts facing each other, the fixture and the abutment form the transmucosal part of the implant for contact with the soft tissues, which comprise the connective tissue and the connective epithelium. The essence of this invention is that the transmucosal part consists of two areas, namely the spacing section of the abutment and the tooth neck section of the fixtura. Both transmucosal areas have outer contact surfaces with a chemically and morphologically differentiated surface treatment, namely the outer contact surface of the tooth neck section of the fixture is bioactive for the connective tissue and the outer contact surface of the spacer part of the abutment is bioinert for the connective epithelial tissue. The bioactive contact surface of the tooth neck section has its upper part in the direction of the connection point between the spacer section and the tooth neck section with a height h which corresponds to 0.2 to 0.7 mm, an inclination corresponding to an angle between 1 ° to 85 °, the inclination angle being an angle in positive sense of rotation (counterclockwise).
The main advantage of this invention lies in the development of the surface treatment of the transmucosal part of the tooth-jaw implant, which reduces bacterial adhesion and the accumulation of dental plaque and at the same time enables effective attachment, both for the epithelial tissue and for the connective tissue. A great advantage is the rational and proven solution for the formation of separate areas of the transmucosal part of the two-part tooth-jaw implant for contact with the epithelial and connective tissue in the corresponding dimensions, which are required by the physiological factors of the biological width of the stable soft tissue , that is the thickness of the connective epithelium and the thickness of the connective tissue (possibly also of the thickness of the sulcus) with a minimum value for each of these layers of approximately one millimeter.
The two-part tooth and jaw implant obtained with differentiated surface treatment, formed by chemically and morphologically differentially modified contact surfaces for the adjacent soft tissues of the epithelium and connective tissue, thus enables their specific and improved integration into the transmucosal area of the open dental implant and contributes to reducing the risk of peri-implantitis.
Die abgeschrägte obere Oberfläche des bioaktiven Zahnhalsabschnitts vergrößert so die bioaktive Kontaktoberfläche für das konnektive Bindegewebe um fast 100 %.Another great advantage of the invention is the upper beveled part of the bioactive tooth neck section, which tapers in the direction of the cut line to the healing part of the spacer section. The beveled upper part has tried and tested optimal parameters, namely with regard to the required height and the required angle of inclination. The chamfered upper surface of the bioactive tooth neck section thus increases the bioactive contact surface for the connective connective tissue by almost 100%.
With regard to a very favorable chemical bonding of the collagen fibers, it is advantageous if the bioactive contact surface of the tooth neck section consists of a thin coating of bioactive glasses or bioactive ceramic materials. The bioactive coating of the contact surface of the tooth neck section can bioactive glasses in an optimal mass ratio of 38.0 to 58.0% SiO 2 ; 25.0 to 29.0% Na 2 O; Contain 12.0 to 25.0% CaO and 5.0 to 8.0% P 2 O 5 . A 2% by mass percentage of B 2 O 3 , possibly up to 1% Al 2 O 3, contributes to maintaining the chemical stability of these glasses while at the same time maintaining their bioactivity. When these types of bioactive coatings come into contact with the body fluid, hydroxyapatite is precipitated, which is identical to the patient's hydroxyapatite.
It is advantageous if the bioactive coating on the contact surface of the tooth neck section contains ceramic bioactive material based on a calcium-phosphate type with a molar Ca / P ratio = 1 to 2. With such a surface, the collagen fibers of the connective tissue can form a chemical bond which ensures a firm connection of the bioactive contact surface with the connective tissue.
It is also advantageous if the bioactive coating is thin and smooth with a maximum thickness of 2 mm, with a roughness Ra ≤ 0.2 µm and with a minimum index I b of bioactivity I b ≥ 3, where d represents the number of days within which the bioactive contact surface reaches 50% contact with the bone. The surface smoothness with minimal roughness is favorable because no harmful bacteria can adhere to it.
It is also advantageous if the bioactive contact surface of the tooth neck section in the substrate surface has discrete bioactive particles based on calcium-phosphate salts with a surface coverage of 40 to 70% of the substrate, as well as a roughness of the bioactive contact surface Ra = 0.5 to 1.2 µm. The required proportion of the surface coverage with calcium-phosphate salts expresses the optimally tested degree of coverage and the structured bioactive contact surface with a given roughness enables the collagen fibers to be held on the projections of the substrate surface, whereby a similar bond can be formed with the collagen fibers as it is the binding of the connective tissue in a natural tooth.
It is also advantageous if the two-part dental jaw implant has a bioactive surface for contact with the connective tissue that is highly hydrophilic with a wetting angle below 45 °. The required wetting angle is useful for the penetration of the body fluid to the surface of the dental implant, which leads to the optimal integration of the connective tissue.
Furthermore, it is advantageous if the two-part tooth-jaw implant and its bioactive contact surface for contact with the connective tissue according to measurement B.E.T. has an at least 50 times larger surface area of the unit area compared to the surface of the unit area with a bio-inert contact surface This structured surface with an extremely large measuring surface increases the reactivity of the surface, its bioactivity and improves and intensifies the interaction with the peri-implant tissues.
It is a further advantage if the dental jaw implant has a smooth bio-inert contact surface with a roughness of Ra <0.2 µm and a wetting angle Θ of 60 ° to 110 ° for contact with the connective epithelium. The advantage of such a smooth surface lies in the formation of an effective attachment of the bio-inert surface to the connective epithelium (via the mechanism with hemidesmosomes), similar to a natural tooth. The defined wetting angle contributes to the prevention of an accumulation of plaque on the bio-inert contact surface and at the same time enables an effective adhesion of epithelial cells by the local adhesive contact.
The two-part tooth-jaw implant can be prepared in a manner of the tooth-jaw implant according to the invention, the essence of which is that the contact surface of the transmucosal part is prepared in different ways, on the one hand to obtain the contact surface of the transmucosal part of the implant for the Contact with the connective epithelium, and on the other hand to obtain the contact surface of the transmucosal part of the implant for contact with the connective tissue. The essence of this invention is that part of the fixture of the implant is mechanically processed for contact with the connective tissue to obtain a bioactive surface and then in an inert atmosphere in concentrated hydrochloric acid or sulfuric acid for 20 to 150 min at one Temperature of 30 to 60 ° C is pickled. Then it is etched in a 1 to 10 molar aqueous solution of the hydroxide of an alkaline metal for a period of 1 to 24 h at a temperature of 40 to 70 ° C. The surface obtained in this way is then best leached in deionized water at a temperature of 18 to 30 ° C for 2 to 40 min in an ultrasonic bath until a nano- and macro-structured surface with a roughness of Ra = 0.5 to 1.5 µm and an area that, according to the BET measurement, has a minimum area 50 times larger than the unit area of the surface of the sample than in comparison with the surface of the unit area of the surface of the substrate sample with the smooth bio-inert surface.
The contact surface of the abutment part for contact with the binding epithelium to obtain a bio-inert surface is polished mechanically or electrochemically until a smooth surface with a roughness of Ra <0.2 µm is achieved. The advantage of this process lies in the fact that the technological parameters of the mechanical processes, which form the contact surface for the epithelium and the technological parameters of the chemical processes, which form the contact surface for the connective tissue, can thus be controlled and controlled in order to achieve optimal properties of the surfaces mentioned can be achieved, which in turn form an effective attachment for both, completely differently adhering tissues and thus achieve economic advantages in production. Another advantage of this type of preparation is the generation of defined and different contact surfaces of the transmucosal part, both for the spacer part and for the tooth neck part. If mechanical processing such as surface treatment, grinding, sandblasting or ball grinding is used on the appropriately roughened contact surface of the transmucosal part of the implant, a non-porous macro-structured surface with a roughness of Ra = 0.5 to 1.5 µm is achieved. If acids then act on the surface created in this way, this surface is activated and a surface is created with an articulated micro and macro structure. If this processed surface is etched with an aqueous solution of hydroxides of alkaline metals, a bioactive nano- and macro-structured hydrophilic surface with a high measurable surface is achieved. The chemicals used are removed by washing the implant surface in deionized water under the given conditions, ideally in an ultrasonic bath.
