Abstract
Description
The invention relates to an X-ray mammography device for simultaneous breast diagnosis using ultrasound, with a flexible seal between the lower x-ray Radiolucent support plate over the cassette holder for the X-ray detector and the top compression plate to form a sealing seal for the breast examination chamber which can be filled with liquid, being above the made of an X-ray transparent material Compression plate an X-ray tube is arranged.
In such a known from US 5 479 927 Combination device for performing X-ray mammography and sonography in one and the same device, is one of them Most embodiment of the ultrasound head as well as the X-ray gene emitter arranged above the compression plate. This has the consequence that one carries out the ultrasound sub ultrasound gel on the compression plate must bring to a sufficiently good coupling of the Ultra to achieve sound waves. However, this hampers work run, as this gel is always reapplied and used X-ray examination can be removed again got to. Such delays in the combination examination but must be avoided as it is for the patient is very inconvenient to spend a long time under pressure in the Examining chamber squeezed mom.
The arrangement provided in a second embodiment of the ultrasound head in the examination chamber, so that the Radiography of the breast perpendicular to the X-rays from in front, apart from the resulting com complicated construction additionally the disadvantage that the the images are not easily correlated with each other, like this with the same shooting direction with verti x-ray radiation of the breast and the ultrasonic waves is the case.
The invention is therefore based on the object of an x-ray mammography device for simultaneous breast diagnosis with Hil fe of ultrasound of the type mentioned at the beginning ten that correlate with each other in a simplified structure X-ray and ultrasound images are available.
To solve this problem, the invention provides that the lower platen is made of an ultrasonic permeable Material and the top compression plate from an ultra sound reflecting material and that the detector cassette, possibly including a spreading grid, against a a displaceable ultrasound head Recording cassette is exchangeable.
According to the invention, the ultrasound examination is not carried out just like the X-rays from above, but in the Ge opposite direction. The first consequence of this is that the upper one Compression plate from a still X-ray permeable but ultrasound reflecting material must stand, for example as a plexiglass plate while the lower platen is on the other side must be permeable to ultrasound. The training can be there at very simple in a way that the preferred Ultrasound cassette filled with a gas-free liquid with the slidable ultrasound head with a good one Connect the ultrasonic coupling to the lower platen is cash.
The seal that is necessary for the ultrasound examination agile liquid filling of the examination chamber allows light and at the same time with X-ray mammography a decrease in image contrast between the middle of the leads to examined breast and the peripheral areas very easily by at least two sides of the Untersu chamber forming flexible bellows and an insertion side for the mom-forming sealing plate with an on plug opening be formed. In addition to the possibility that the Bellows forms three sides of the examination chamber, too be provided that the upper compression plate on the Sealing plate remote side with a sealing lip on egg ner rigid boundary wall of the examination chamber sliding is movable. In all cases, the liquid should closing lines the wall of the sealing plate remote Enforce examination chamber.
The liquid to fill the examination chamber with is particularly advantageous, especially degassed Water may be preferred from one pump to another Expansion tank from a storage tank into the chamber sucked and in the chamber under a light under kept pressure. This negative pressure may lead to existing air bubbles removed, which is known to be ultrasound interfere with examinations. The bracket device for optional recording of the detector cassette, if necessary finally a scatter grid and the ultrasonic Recording cassette is designed so that the top the ultrasound recording cassette inevitably when inserted fig is pressed close to the lower platen and there through an excellent ultrasonic coupling between at which is guaranteed.

The X-ray mammography device shown in Fig. 1 comprises, as essential components, a lower support plate 1 for the breast to be examined, over which a height-adjustable compression plate 2 is arranged below the X-ray emitter 3 . Under the lower platen is a Kas settenhalterung 4 for receiving a film cassette and an anti-scatter grid arranged above. If necessary, a digital solid-state detector can be used instead of the film cassette. The control device arranged next to the stand 6 of the mammography device is indicated at 5 .
The operation of such an X-ray mammography device is shown in FIG. 2. The X-ray radiation emanating from the X-ray emitter 3 with a cathode 7 for generating an electron beam 8, an anode 9 and a diaphragm 10 comprising X-ray radiation RS passes through the upper compression plate 2 , through which a breast 11 onto the lower platen 12 is pressed. Under this lower receiving plate 12 is a scattered radiation grid 13 and below a film cassette 14 is held in the cassette holder 4 , wherein the film cassette can be provided with a film at the top and a scintillation film at the bottom for converting X-ray photons to light. The radiation penetrating the film cassette can be detected with the aid of a displaceable solid-state detector 15 , the generator 17 being able to be controlled via an automatic exposure device 16 of the X-ray generator 17 , that is to say the control for the X-ray emitter 3 .
According to the invention, the design is now such that, on the one hand, the space between the lower support plate 1 and the upper compression plate 2 is designed as a sealed, liquid-fillable chamber, into which the breast can be inserted through an opening. At the same time, the design of the cassette holder 4 is such that the film cassette 14 is pulled out with the anti-scatter grid 13 and can be set by egg ne shown in FIG. 5 ultrasound recording cassette, in which an ultrasound head 18 on a sliding guide 19 is arranged movably. The ultrasound recording cassette 20 is filled with a liquid 21 , for example degassed water, in order to achieve a good ultrasonic coupling between the ultrasound head 18 and the top cover 22 , which in turn is inserted into the cassette holder 4 by appropriate resilient pressure members the lower platen is pressed so that there is a good ultrasonic transition between the two.
In Fig. 4 an embodiment for the formation of the sealed examination chamber 23 is shown, which in addition to the lower support plate 1 and the upper compression plate 2 one with an insertion opening 25 for inserting the breast two not shown above and below the drawing plane are the whale walls and a rigid Boundary wall 26 exists on which the upper compression plate 2 is slidably displaceable by means of a sealing lip 27 . A pump 28 conveys the liquid 29 with the interposition of a compensating container 30 from a storage container 31 into the chamber and keeps the liquid under a slight negative pressure, where the liquid 29 also degasses again and thus reliably avoids the occurrence of air bubbles which interfere with the ultrasound examination is. Filling the examination chamber 23 with the liquid 29 also has advantages for X-ray mammography, since the liquid filling - before it is again degassed water - to reduce the image contrast between the center of the breast to be examined and the edge areas or the area exposed to direct radiation without water filling. This leads to better detail recognition in the periphery of the breast and, in addition, the beam hardening results in a reduction in the tissue dose in the periphery.
A combined x-ray ultrasound examination with the aid of the device according to the invention is carried out in such a way that the breast is first lubricated with a gel and then inserted into the insertion opening 25 , which is, for example, a sleeve 24 designed as a flexible rubber membrane. The breast is then compressed by lowering the upper compression plate 2 . After sucking entga stem water into the chamber 23 , the X-ray is taken and then the film cassette or the solid-state detector is removed with simultaneous removal of the anti-scatter grid or retraction of the anti-scatter grid into a position outside the radiation path. Then the water-filled ultrasound examination cassette is inserted into the cassette holder and pressed, for example, via a coupling bellows onto the top of the cassette holder and thus onto the lower support plate, so that the ultrasound examination can then be carried out immediately.
The formation of an X-ray mammography according to the invention Guidelines for simultaneous breast diagnostics with the help of Ultra schall has a number of advantages over the previous ones Orders.
For one thing, there are no loose parts except the Ultra sound examination cassette. The arrangement according to the invention can be retrofitted to existing X-ray mammography equipment most. X-ray mammography is either in air or in Water possible, with the inventive design of the X-ray mammography device also for examination with the help of ultrasound no changes for the X-ray examination need to take place, for example none additional absorbent layers are necessary. The breast is still good due to the upper compression plate observable and the fixation of the chest by negative pressure in the Chamber results in good positioning.
At the same time, there is a safe ultrasonic coupling without additional foils between breast and water bath and by sucking the water into the examination chamber disturbing air is sucked out at the same time. The instead the film cassette to be inserted into the cassette holder Ultrasound scanner is very space-saving and disrupts its appearance ne arrangement below the lower platen X-ray process not at all. The pressure plate does not have to be cleaned of coupling gel and preparation for the Ultrasound examination is very quick, which is because of the unbe Quemen painful compression of the chest in the lower search chamber is of great importance. Finally results there is also the additional advantage that because of the sub pressure in the examination chamber water if there are any leaks activities do not expire.
