Mikromotoren ohne Welle zur gleichen Anwendung kommen können. Die Auswertung der aufgenommenen (Ultraschall-) Bilder ermöglicht eine Bemessung des totalen Gefäßdurchmessers, der gesamten Restplaquefläche und Restplaquedurchmessers, und des Lumendurchmessers der Stenose 2. Der Durchmesser des Rotablators 4 kann nunmehr so bemessen werden, daß eine optimale und nahezu vollständige Abtragung des Plaque ermöglicht wird, ohne daß die Media des Gefäßes 3 verletzt oder freigelegt wird, oder mögliche Fettinklusionen im Plaque freigelegt werden. Bei Verwendung der bereits vorstehend beschriebenen Überdrahttechnik (over the wire) oder "monorail technik" ist die Bilderzeugungseinheit 21 ihrerseits auf einem koaxial verlaufenden oder koaxial verlaufenden und seitlich austretenden ("monorail-technik") Führungsdraht 23 angeordnet. Der Führungsdraht 23 wird in die durch die Überwucherung der schematisch eingezeichneten Gefäßstütze 24 gebildete Gefäßverengung 2 eingeführt. Sodann wird der Laserkatheter 20 bis an die atherosklerotische koronare Stenose 2 herangeführt und in Betrieb gesetzt. Dadurch wird eine erste Abtragung der Stenose 2 erreicht. Zur Kontrolle des Erfolgs der Abtragung wird, wie dies in Fig. 2B dargestellt ist, die vorzugsweise als Ultraschallsonde ausgebildete Bilderzeugungseinheit 21 in die Öffnung 13 der Stenose 2 eingeführt. Die Bilderzeugungseinheit 21 kann mittels einer Welle 25 in Rotation versetzt werden, wobei sich Bilderzeugungseinheit und Welle in einem Schutzkatheter drehen oder durch ein wellenloses System, wie das elektronische Ultraschallsystem oder Mikromotoren angetrieben wird. Wie bereits beschrieben, dienen die mittels der Bilderzeugungseinheit 21 erzeugten Aufnahmen einer Optimierung und Kontrolle der Abtragung der Stenose 2. Die Kontrolle kann nach jedem Abtragungsschritt, die mit schrittweise größeren Laserkathetern 20 durchgeführt werden, erneut vorgenommen werden.An embodiment using ultrasound control and laser ablation is shown in FIGS. 2 and 3. The check can be carried out again after each ablation step, which is carried out with gradually larger laser catheters 20 .
Bei der in Fig. 3 dargestellten, alternativen dritten Ausführungsform der Erfindung dient die länglich ausgebildete Bilderzeugungseinheit 21 neben der Bilderzeugung gleichzeitig zur Führung des Laserkatheters 20. Dies setzt jedoch voraus, daß die Abmessungen der Bilderzeugungseinheit 21 derart gering sind, daß diese bereits vor Beginn des Abtragungsvorgangs in die Öffnung 13 der Stenose 2 eingeführt werden kann, oder diese Öffnung in einen teilweise oder vollständig geöffneten Verschluß vorher eingelasert wurde. Der Laserkatheter 20 besteht aus einer Vielzahl von Lichtleitungselementen 30, die an seiner stirnseitigen Oberfläche 31 enden. Die Lichtleitungselemente 30 sind nur andeutungsweise eingezeichnet und besetzen die gesamte stirnseitige Oberfläche 31 des Laserkatheters 20. An den entgegengesetzten Enden der Lichtleitungselemente 30 wird Laserlicht einer solchen Intensität eingekoppelt, daß die Abtragung der Stenose 2 ermöglicht wird.
