Abstract
Description
Die Erfindung betrifft ein Verfahren zur Registrierung eines mobilen Röntgengerätes mit einer Bestrahlungseinheit, welches dieser zum Zwecke der Patientenpositionierung beigestellt ist. Bei der Bestrahlung mit hochenergetischen Röntgenstrahlen im Zuge einer Strahlentherapie, welche eine mögliche Therapie beispielsweise für Krebspatienten ist, ist für die Qualität der Therapie eine korrekte Lagerung des Patienten bezüglich der Bestrahlungseinheit erforderlich. Üblicherweise wird vor der Durchführung der Therapie ein Computertomogramm (CT) aufgezeichnet und auf der Grundlage eines damit erzeugten 3D-Bilddatensatzes ein Bestrahlungsplan erstellt. Bei der dann unter Umständen erst viel später stattfindenden Strahlentherapie muss der Patient bzw. der ihn tragende Bestrahlungstisch so positioniert werden, dass das zu behandelnde Körpervolumen mit dem entsprechenden Volumen des CT-Datensatzes deckungsgleich angeordnet ist, d.h. exakt die gleiche räumliche Ausrichtung relativ zur Bestrahlungseinheit einnimmt. Um dies zu gewährleisten, werden vielfach Hautmarker verwendet. Diese werden während oder kurz nach der Aufnahme des Planungs-CTs am Patienten angebracht, z.B. in die Haut eintätowiert und ermöglichen eine Registrierung zwischen dem CT-Datensatz und der Patientenanatomie im Bestrahlungsraum mit Hilfe von Laservisieren. Alternativ werden vor der Erstellung des CT-Planungsdatensatzes Marker an der Haut des Patienten angebracht, die sowohl im CT erkennbar sind als auch durch ein Navigationssystem detektiert werden können und somit eine exakte Ausrichtung des Patienten auf dem Bestrahlungstisch ermöglichen. Ein Navigationssystem, das dies leistet, ist beispielsweise das System ExacTrac der Brainlab AG. Ein Nachteil bei Hautmarker verwendeten Verfahren ist, dass sich während des zwischen der Aufnahme des CT-Datensatzes und der Bestrahlungstherapie liegenden Zeitraumes und auch wäh rend der sich oft über mehrere Wochen erstreckenden Bestrahlungstherapie die relative Lage der Hautmarker zum Bestrahlungsgebiet beispielsweise aufgrund einer Gewichtsabnahme des Patienten oder aufgrund von Organverschiebungen verändern kann. Die geschilderten Probleme werden bei sog. Portal-Image-Verfahren umgangen, bei denen der therapeutische Röntgenstrahl genutzt wird, um zweidimensionale Bilder der Patientenanatomie zu erzeugen. Durch visuellen Vergleich eines solches Bildes mit einer Projektion des CT-Datensatzes entsprechend der Röntgenstrahlrichtung des portal imaging wird der Bestrahlungstisch bzw. der Patient in die richtige Position gebracht. Mit therapeutischen Röntgenstrahlen erzeugte Bilder sind jedoch qualitativ schlecht und damit nur bedingt für eine akkurate Patientenpositionierung geeignet.
