Description
The The present invention relates to a medical product for the physical Protection of blood vessel segments in situ after they have been removed from specific tissues and before it is reimplanted as part of a by-pass operation become. The medical product contains one biopolymer and one bath liquid, the nutrients and Contains chemoprotectants.
Hintergrundbackground
Angioplastien während coronären By-pass-Operationen werden typischerweise so durchgeführt, dass verschiedene Blutgefäße (meistens aus der vena safena des Beins) entnommen werden und dann als Ersatz für auf Grund von Arteriosklerose oder anderen Beschädigungen verschlossene Gefäße wieder eingesetzt werden.angioplasties while coronal by-pass operations are typically performed in such a way that different blood vessels (mostly from the vena safena of the leg) and then as a replacement for on Vessels closed again due to arteriosclerosis or other damage be used.
The typical sequence of surgical steps leading to a complete By-pass operation, is as follows. While the cardiac surgeon works on the heart, another surgeon isolates a replacement vessel (from the Leg). After the vessel from surrounding Tissue is free, it is usually on a damp cloth deposited and remains there as long as necessary. Then removes the cardiac surgeon removes the segment from its temporary location and reimplanted it accordingly.
On critical disadvantage of the present The procedure used is the fact that the replacement cup is different Risk of injury, not just during removal, but also during the transition period outside of the body is exposed. As is known, conclude the vessels coronärem By-pass often again after transplantation; also occurs frequently Restenosis on, possibly caused by lesions, caused by acute injuries to the inner vessel wall. Once a Blood vessel from its natural Is removed, it will usually collapse and the inner vessel walls stick together. Furthermore, the endothelial cells are not perfused until the replacement vessel is reimplanted and filled with blood again is.
It is therefore for the success of a by-pass operation is essentially open to replacement vessels and maintain the functions of the endothelial cells. For the latter would it be beneficial if during storage and early Reimplantation phase in a physiological or nutrient bath, specific in the presence of chemoprotectants.
Beschreibungdescription
The present invention includes a medical product (a) a biopolymer that enters the lumen of the surgically removed segment of the vessel is injected, there forms a three-dimensional inner polymer gel scaffold, before the segment during a bypass surgery is reimplanted, and (b) a nutrient solution and Chemoprotectant mixture encapsulated in the polymer matrix is and washed around the medical product before storage the vascular segment again from the polymer backbone is released.
On the present invention furthermore relates to a method a biopolymer and a nutrient / chemoprotectant blend to combine in such a way that a three-dimensional framework for vascular segments arises.
The preferred embodiments of the present invention are set out in the subclaims.
- (1) Immediately after removal from a body tissue, the vascular segment is filled with a liquid that polymerizes immediately after being filled in, thus physically protecting the inner vessel wall from collapsing and from adhering to itself.
- (2) The polymer gel structure is saturated with nutrients and chemoprotectants and thus further supports the maintenance of the cellular function of the endothelium.
- (3) With the help of the scaffold, the vessel segment can now be kept in a filled, three-dimensional state in a nutrient solution for a useful and necessary period of time. As a last step, the vascular segment is physically freed from the polymer scaffold immediately before the actual by-pass operation and filled with physiological fluid, or finally with the patient's own blood.
Nährstoffe und Chemoprotectants im Polymergel und Badmedium können Salze, Antioxidantien, Wachstumsfaktoren, Albumin, und spezifische schützende Arzneisubstanzen oder biologische Substanzen, enthalten, oder können aus kommerziellen Transfusions- oder Transplantationsmedien bestehen.nutrient and chemoprotectants in the polymer gel and bath medium can contain salts, Antioxidants, growth factors, albumin, and specific protective medicinal substances or biological substances, or can be obtained from commercial transfusion or transplant media.
Polymer materials with such properties that are available are: a thermoplastic paste biopolymer; a polymer crosslinked in situ; a thermoset polymer; a photocrosslinked polymer; an ionic cross-linked polymer; a biocompatible polymer as a precipitate from organic solution; Particularly useful are biopolymers consisting of water-insoluble biopolymers such as poly (DL-lactide), poly (DL-lactid-co-glycolide), poly (DL-lactid-co-ε-caprolactone), poly (N-isopropyl-acrylamide) , triblock poly (ethylene oxide) -poly (propylene oxide) - poly (ethylene oxide) - copolymer, (Pluronics, Poloxamers).
To of such treatment, the vascular segment is in better shape Condition for the reimplantation compared to the current procedure. Consequently, becomes the result of the by-pass operation in terms of mortality, quality of life and costs positively influenced.
