Description
Embodiments relate to a device, a method and a computer program for influencing the respiratory muscles of a person, in particular, but not exclusively, to a concept for respiratory muscle training of a exercising person or a patient based on electromyographic signals of his / her respiratory muscles.
Various concepts are known in conventional technology that cause training of the respiratory muscles. As a rule, these are based on the visualization of a breathing performance that exceeds the normal breathing performance. In this way, the exercising person is encouraged to breathe more intensely, which strengthens the respiratory muscles beyond normal use. A difficulty in breathing is also known, which is achieved, for example, by narrowed cross-sections of mouthpieces or the like. As a rule, the difficulty of breathing can be adjusted, so that from a slight difficulty in breathing, increasingly higher resistance for breathing can be gradually adjusted. Such type of respiratory muscle training is known both for increasing performance during sporting activities and for recovery support and rehabilitation after illnesses in which the respiratory muscles have been weakened or affected.
Types of respiratory muscle training (AMT) - (English Inspiratory Muscle Training - IMT) Basically, different training modalities / modes are known to carry out respiratory muscle training. The various training modalities sometimes differ in the type of pneumatic intervention. In addition, the different training modalities also have different therapeutic goals in some cases. While some training modalities are more aimed at improving muscle strength, other training modalities are more aimed at improving endurance (muscle endurance). Some types of training / modes / modalities are also said to serve both goals in medical research. An overview of different types of training / modes / modalities can be found in the book "Textbook of Pulmonary Rehabilitation" in the chapter "Inspiratory Muscle Training" by Daniel Langer. Sections 18.3.4 to 18.3.7 and Table 18.3 are of particular interest.
With the exception of the special case of normocapnic hyperpnea, these methods have the common feature that a passive resistance element is introduced into the airway. This element is partly regulated / adjustable. In a broader sense, this also includes flow limitation, occlusion and pressure limitation. In this case, passive means that the respiratory muscle training device does not itself generate a pressure and / or volume flow. In the electrical equivalent circuit diagram, the respiratory muscle training device would be a passive element (resistor, potentiometer, the resistor may have special non-linear voltage / current characteristics or is regulated). However, it is not an active voltage or current source (no energy is introduced). Various devices are commercially available for this type of respiratory muscle training with a pneumatic resistance. In some cases, these are mobile (handheld) devices that the patient / person to be trained holds in front of their mouth in order to breathe.
In addition, the detection and recording of the performance of the respiratory muscles are known, especially in the medical field. If, for example, a patient is artificially ventilated, the mechanical work normally applied by the respiratory muscle culture is at least partially taken over by a ventilator. The respiratory muscles can be damaged by the constant inactivity after just a few hours. So it is often not easy to get a patient back into independent breathing after a long period of artificial ventilation. Even if the patient's illness that originally led to ventilation is no longer present, it is often not possible to simply disconnect the patient from the ventilator. The respiratory muscles that have receded during inactivity are then no longer even able to perform the breathing work necessary for the patient at rest - that is, lying in bed inactive.
Another special feature of respiratory muscle training in connection with ventilated patients is the fact that the respiratory muscle strain is partly / usually realized through less relief. A ventilated patient is usually not able to do the breathing work that is necessary even for the physical state of rest. For this reason he / she is connected to a ventilator. The ventilator takes over the breathing work of the patient wholly or at least partially and thereby relieves him / her. In this environment, respiratory muscle training can already be implemented by temporarily stopping or reducing the relief. Thus, even less relief is effectively a burden on the respiratory muscles.
In addition, in the case of a ventilated patient, impairments from the ongoing ventilation must be taken into account. These include maximum pressures, minimum volumes, etc. In addition, alarm limits may have to be adjusted during respiratory muscle training.
Examples include monitoring the degree of muscular exhaustion, the proportion of different muscle groups involved in a movement, and training-induced changes in muscle condition. Another prominent field of application are biofeedback methods, in which the visual representation of EMG-based activation measures for the patient is used to control and optimize training.
The design and control of respiratory muscle training exclusively via respiratory parameters, which essentially depend on the overall mechanical performance of the respiratory muscles, does not take into account detailed information about the condition of the respiratory muscles to be trained. This applies to both the adaptation of the training regime and the evaluation of the training performance of the person or patient.
Due to the fact that only a very limited amount of information about the state of the muscles is available in the prior art, the existing methods are also only adaptive to a small extent. This low adaptivity concerns both the possibility of adapting the training load within a training session in which the training mode is applied, and adaptations for the following training sessions. Both cannot be optimally adapted to the degree of exhaustion or recovery of the muscles; Over- or under-training with serious consequences, especially for people or patients, on training equipment and, in particular, on ventilators are possible. Optimal training is important for a good weaning result. Good weaning, on the other hand, is essential for a good clinical result.
The interaction between the training person or patient and the training device (for example a ventilator) is currently not optimal in two respects: On the one hand, the training person / patient receives little or no feedback about their current performance in relation to the target performance, what can lead to a lack of motivation and thus reduced training success and less coordinated exertion and thus, for example, incorrect stress, for example to undesirably high expiratory activity. On the other hand, it is currently not possible for the patient in particular to determine the point in time and degree of difficulty of the training sessions themselves; it is therefore externally determined, which can also have strong negative effects on the patient's motivation.
Accordingly, there is a need to record the degree of exhaustion or recovery of the muscles before, during and after training more precisely and to take it into account for planning and monitoring the training phases and for displaying and evaluating the training results. Embodiments of an apparatus according to
A respiratory muscle training to be carried out with the aid of a respiratory muscle training device or ventilator is therefore proposed. Information about the condition of a muscle is generated on the basis of EMG measurements. On the one hand, it is assumed that a ventilator is used as a training device. On the other hand, an EMG-based muscle condition monitoring unit is also carried out with conventional respiratory muscle training devices (for example handheld devices that are also suitable for home use).
Pressure Time Product).
Lungenvolumina
- • Pressures, volume flows and variables derived from them, such as volumes or time products (eg Pressure Time Product). In particular, these are Paw (airway pressure at the patient connection), Peep (positive end-expiratory pressure), Pinsp (inspiratory pressure), patient flow, tidal volume, minute volume, pneumatic lung resistance, lung elasticity, respiratory performance, time constants of breathing.
- • Gas properties such as concentrations, partial pressures, fractions or temperatures, especially oxygen 02, carbon dioxide CO2, water H2O. In the case of capnographic signals (CO2), the end-tidal CO2 (etCO2) should also be mentioned in particular. Substances introduced into the breathing gas (nebulized drugs, anesthetics, helium)
- • Lung properties derived from pressure and / or volume flows (flow) and gas properties, such as lung volumes
The inclusion of EMG before, during and between the individual training sessions enables the consideration of more precise information about the condition of the muscles to be trained. This means that the course of the training can be better adapted to the individual needs of people or patients. In addition, the feedback of the information obtained from the EMG to the training person / patient enables an improved interaction with the training device.
Accordingly, embodiments of the present invention are based on the core idea of detecting an improved detection of the muscle state of the respiratory muscles of a person or a patient who is ventilated or exercising. The recorded muscle condition is used to plan and control training to strengthen the respiratory muscles, which degenerate during ventilation or lead to increased performance of the respiratory muscles. The use of electromyography (EMG) enables the muscle states to be recorded more precisely, which allows for more refined training parameters and monitoring.
It is the subject of current research how the weaning from artificial ventilation (English weaning) can be carried out as successfully as possible. A measure proposed in this context is the training of the respiratory muscles by respiratory muscle training (English inspiratory muscle training, IMT). In respiratory muscle training, the patient's respiratory muscles are challenged by training sessions with a temporarily increased load. These training sessions are intended to restore the strength or endurance of the respiratory muscles to such an extent that the patient can breathe permanently again without support.
Since, in addition to chronic under-strain of the respiratory muscles, their long-term excessive strain is also harmful for the patient or the person exercising, the monitoring of the activity and the general condition of the respiratory muscles has recently increasingly become the focus of clinical research.
- ◯ The training should be as effective as possible in order to enable the person / patient to breathe independently again as quickly as possible.
- ◯ The respiratory muscle training should be as sustainable as possible. The aim was to prevent the patient from relapsing and having to be reconnected to a ventilator.
- ◯ The respiratory muscle training should be as gentle as possible for the patient.
- ◯ The effort for the hospital should be kept as low as possible.
These requirements can also be applied in a narrower sense to rehabilitation or the Weaning patients from ventilation systems or, in a broader sense, also transferred to training the respiratory muscles of people, especially athletes.
On the basis of the EMG alone and, if necessary, in combination with pneumatic data and information about the physiology of the respiratory system, various therapeutically relevant observations can be made about the condition of the respiratory muscles. In order to obtain extensive status information about the muscles relevant to breathing, it is helpful to evaluate not just one but several EMG recordings. In addition to recording the diaphragm activity, this also includes EMG measurements of the intercostal muscles, other auxiliary respiratory muscles and the antagonists (e.g. abdominal muscles, straight abdominal muscles (rectus abdominis)).
Amplitudeamplitude
- o A physiologically not sensible relationship between the activity of the intercostal and diaphragm muscles or, more generally, the relationship between the various muscles involved in breathing. This division of muscle groups can also be determined separately for the inspiration and expiration phases.
- ◯ Strong expiratory activity.
- ◯ Anticyclical activity (asynchrony of patient and ventilator), or more generally:
- the temporal course of the neural activation of the respiratory muscles within one breath.
- ◯ Thoracic tension without breathing, for example when standing up the upper body. The diaphragm, the intercostal and expiratory muscles are active. This can be changes in posture, for example.
mechanische Belastungmechanical load
- ◯ The temporal course of the respiratory effort exerted by the person or the patient as well as the temporal course of the pressure exerted by the patient.
- ◯ The temporal course of the overall decrease in respiratory performance (applied by the person / patient and the training system) in the respiratory system,
- ◯ The maximum pressure difference that can drop across the lungs. This also includes the maximum force, the maximum pressure.
- ◯ Consideration of the influence of antagonistic activity on the respiratory muscles (e.g. abdominal muscles, straight abdominal muscles (rectus abdominis).).
The pressure difference that drops across the lungs or the pressure difference that “opens” the lungs is also referred to as driving pressure. If the driving pressure is too low, the lungs will not be sufficiently recruited. Alveoli collapse. If the driving pressure is too high, the lung tissue may be overstretched and thus damaged.
Using various signal processing methods, it is also possible to detect EMG-based unusual muscle contractions such as cramps or coughs or hiccups.
FatigueFatigue
In addition, the state of fatigue of the respiratory muscles (muscle fatigue) can be detected from the EMG. Muscle fatigue is a condition in which the muscles are only able to generate or maintain force to a limited extent. Developing muscle fatigue can be diagnosed by changes in the EMG signal before the muscles can no longer perform a certain task. To this end, various methods are known to calculate a fatigue index from the EMG signal, which describes the degree of fatigue that occurs. Various methods for calculating an EMG-based fatigue index are known. Muscle fatigue can, in particular, also be examined as a function of time both within a session and over several sessions.
Finally, signal classification algorithms can be used to detect various pathological muscle states on the basis of EMGs. Pathological conditions of the entire respiratory system (for example Cheyne-Stokes breathing) can also be detected based on EMG. Additional EMG diagnostic tools include neuromuscular efficiency and muscle regeneration. For example, muscle regeneration can be quantified by DOMS (English Delayed Onset Muscle Sornes) can be quantified. Muscle regeneration is particularly important when planning your next workout. Further means are electromuscular delay and the detection of the risk of atrophy.
Weitere Informationen, die beim EMG Signal mit anfallenFurther information that arises with the EMG signal
The information contained therein can also be evaluated and used. This applies to stress affecting the whole body, for example. For example, the heart rate can be determined for this purpose. Heart rate variability HRV is particularly suitable for quantifying the stress level.
It is also possible to calculate a stress level from the transition impedance of the electrodes. The perspiration associated with increased stress leads to changed electrical effects on the electrodes and can be detected.
In the previous section, various methods were described with which EMG-based information about the condition of the respiratory muscles can be determined. In order to be able to use this information meaningfully in the context of respiratory muscle training, this information must be evaluated, weighted and summarized. In this context, the individual status information is processed in such a way that adaptation recommendations for respiratory muscle training are generated from them.
- ◯ As part of the online analysis, possible adaptation steps are determined during an ongoing training session that have an immediate effect on the ongoing training session.
- ◯ As part of the offline analysis, each training unit is evaluated retrospectively. The knowledge gained in this way can be used by a planning unit when planning the next training session.
In essence, both units solve an optimization task with secondary conditions: By adapting the training intensity and the training mode, the training benefit should be maximized, under the secondary condition that no health-damaging states of the muscles or the respiratory system should be achieved. The result is in each case a recommendation for the choice of the training intensity in the next time unit, with the training history in particular also being taken into account. In particular, it can be taken into account which effect certain training intensity levels or changes in training intensity had on the person or the patient in the past. In the case of the online analysis unit, this recommendation is addressed to the training control and should have an immediate effect on the current training session. In the case of offline analysis, the recommendation is addressed to the planning unit and should be included by it when planning the next training session.
Embodiments create a device for influencing the respiratory muscles of a person, with a detection device for detecting an electromyographic signal of the person; a respiratory influencing device; and a control device for controlling the detection device and the respiration influencing device. The control device is designed to determine, based on the electromyographic signal, information about a muscular condition of the respiratory muscles of the person. The control device is also designed to operate the breathing influencing device as a function of the information about the muscle condition in a time-limited training mode with a training intensity. In this way, the training mode can advantageously be planned taking into account the state of the muscles and also corrected while it is being carried out.
In some further exemplary embodiments, the device for influencing the respiratory muscle of a person can be designed as a respiratory muscle training device, which in exemplary embodiments can be designed as a mobile respiratory muscle training device. In this way, improved handling of the device can advantageously be achieved, which also allows mobile use independent of supply networks, for example can be operated independently of the electricity network or in home use.
In further exemplary embodiments, the control device can also be designed in the device in order to execute the training mode as a difficulty in breathing for the person in order to train the respiratory muscles. As a result, systematic training can advantageously be carried out in which, in particular, the duration or the intensity / difficulty can be continuously increased, for example.
In some further exemplary embodiments, the control device can be designed to adaptively adjust a breathing influencing of the person by the breathing influencing device in the training mode as a function of the information about the muscle condition. By adapting the training mode, both a Significant overstrain as well as unnecessary underload of the trained muscles can be reduced or completely avoided.
For example, in exemplary embodiments, the control device can also be designed to influence the training mode with regard to a training duration or the intensity / difficulty. It is advantageous to influence the load on the trained muscles particularly efficiently with the help of these parameters.
In some exemplary embodiments, the control device can be designed to terminate the training mode as a function of the information about the muscle condition. In this way, it is advantageous to avoid excessive demands on the muscles to be trained, which under certain circumstances can lead to permanent damage to the muscles.
In some exemplary embodiments, the training mode can be terminated as a function of a signal. The signal can contain information about at least one element of the group from a change / deviation in the division of muscle groups, an expiratory activity, an inspiratory activity, an anti-cyclic activity, harmful pressure conditions in the lungs, persistent cramps, coughing or hiccups, muscle fatigue over a level considered intolerable respected threshold and detection of pathological conditions that can harm a person. By monitoring the typical symptoms, an adverse effect on the person or the patient can advantageously be effectively recognized and avoided.
In further exemplary embodiments, the control device can be designed to output parameters of the training mode and / or parameters of a respiratory performance of the person and / or information about the muscle state during the duration of the training mode or after the training mode of the respiratory influencing device. The information output can advantageously increase the patient's motivation and / or provide the treating doctor or trainer with diagnostic information.
For example, the control device can also be designed to carry out the output optically and / or haptically and / or acoustically and in relation to a target variable for this person. In this way, the information to be output can advantageously be prepared so that it can be easily grasped for the patient, the treating doctor or trainer.
In some exemplary embodiments, the control device can be designed to enable the person to initiate and / or parameterize the operation of the training mode of the respiratory influencing device. Advantageously, the support of other people to prepare and carry out the training can be dispensed with and the training can be ideally adapted to the needs of the patient.
In some further exemplary embodiments, the control device can be designed to interrupt the training mode for a predetermined period of time as a function of the information about the muscle condition. In this way, it is advantageous to react to temporary or weaker excessive demands on the muscle group to be trained without having to stop training completely.
In further exemplary embodiments, the control device can be designed to determine the information about the muscle condition as a function of signals that indicate the intensity of fatigue and / or cramp and / or expiratory activity. In this way, the parameters of the person or the patient that are essential for well-being can advantageously be taken into account when planning the training and, in particular, when performing the training.
For example, the control device can also be designed to determine an acceptable target intensity of fatigue and / or spasm and / or expiratory activity, the acceptable target intensity being designed as a target area or target point that is constant or changeable over time. In this way, a further optimization of the training can advantageously be achieved, since a certain intensity of the above-mentioned effects is acceptable during the training and does not cause any damage.
In some exemplary embodiments, the control device can be designed to receive the information about the muscle condition as a function of signals that include information about the diaphragm and / or the intercostal muscles and / or antagonists, and / or other auxiliary respiratory muscles, which in exemplary embodiments can be designed as a sternocleidomastoid , to investigate. By taking these additional parameters into account, excessive demands on the person or the patient can advantageously be better avoided and a change in the patient's posture can be distinguished from respiratory muscle abnormalities.
Weiterhin ermöglicht dies die Bestimmung der weiter oben erwähnten „Druckverhältnisse in der Lunge“.In further exemplary embodiments, the control device can be designed to determine the information about the muscle condition as a function of a pneumatic signal. The device for influencing respiration can thus advantageously take into account further essential parameters of the state of the person or patient, which the Increases the predictive reliability of the muscle condition and enables the determination of further information about the muscle condition. This enables in particular the determination of the neuromuscular efficiency (ratio of Pmus / EMG), which is currently a lot of research and research. This also enables the determination of the “pressure conditions in the lungs” mentioned above.
In some further exemplary embodiments, the control device can be designed to carry out an evaluation, weighting and summary for at least some signals when determining the information about the muscle condition. The training mode is determined depending on the combined signals. Advantageously, parameters that are particularly important for the person or the patient can be weighted more heavily or unimportant parameters can be weighted less or not at all. The respiratory influencing device can thus be better adjusted to the state of the person / patient.
For example, the control device can also be designed to carry out a diagnostic maneuver on the muscle state of the person by means of targeted loading. Here, in exemplary embodiments, the diagnostic maneuver can include a time-limited, predetermined control of the respiratory influencing device that deviates from the normal mode. A further operating mode of the device can thus advantageously be used, which can be used, for example, to set the training parameters for a person or to facilitate diagnosis for the attending physician and to further increase the precision of the determination of the information about the muscle condition.
In some exemplary embodiments, the control device can also be designed to output the results of the diagnostic maneuver and / or information about the muscle condition and / or parameters of respiratory performance, optionally in each case based on target specifications for the person, and / or output parameters of the training mode. In this way, a target / actual comparison can advantageously be carried out by the person exercising, the trainer, the patient or the treating doctor, which can be determined in relation to a training plan.
Embodiments further create a breathing device for ventilation of a person comprising a device for influencing the respiratory muscles of the person according to one of the previous embodiments, wherein the control device is also designed to control the breathing device depending on the information about the muscle condition at a first point in time in a normal mode with a first training intensity and operate at a second point in time in a training mode with a second training intensity. Furthermore, the control device is designed to adapt the training mode and the second training intensity during the normal mode of the breathing influencing device as a function of the information about the muscle condition for future second points in time. The device can therefore advantageously be used alternatively for ventilation of patients and for training, which at least saves equipment costs, since many components, for example the EMG sensors, mouthpieces, etc., can be used together.
In some further exemplary embodiments, the control device can also be designed to execute the training mode as a time-limited change in respiratory support for the person for training the respiratory muscles of the person. A training mode for rebuilding the respiratory muscles can thus advantageously be inserted into the ventilation phases, since the respiratory muscles recede after a short time when the patient is ventilated and breathing may no longer be sufficiently possible without the device.
Embodiments further provide a method for influencing the respiratory muscles of a person, comprising detecting an electromyographic signal of the person; generating information about a muscle state of respiratory muscles of the person based on the electromyographic signal; an operation of a respiratory influencing device as a function of the information about the muscle condition in a time-limited training mode. In this way, the training mode can advantageously be planned taking into account the state of the muscles and also corrected while it is being carried out.
Exemplary embodiments also create a program with a program code for performing the method of the previous exemplary embodiment when the program code is executed on a computer, a processor or a programmable hardware component. In this way, the training mode can advantageously be planned taking into account the state of the muscles and also corrected while it is being carried out.

As a mobile respiratory muscle training device, a portable device can be understood in embodiments that can be used independently of stationary available elements such as electricity, compressed air, oxygen or coupling elements to other machines. It typically has a handle or a corresponding housing design in which it can be held during use and, if necessary, also during transport. The weight also makes it easy to carry and does not exceed five kilograms. It is equipped with output / output devices that allow the device to be operated and various parameters to be read out.

The difficulty of breathing is achieved by means of pneumatic structures that operate through constricted cross-sections or controlled flaps, air-permeable membranes or the like is effected in breathing tubes which the person or the patient takes in the mouth or which are connected to corresponding mouthpieces. The degree of difficulty can be adjusted from light to maximum difficulty of breathing. In addition, the difficulty depends on the breathing speed. Thus, with a given cross section of the pneumatic device, a higher difficulty arises at a high air speed of the breath than at a lower air speed of the breath. Correspondingly, sensors in the pneumatic device can determine the air speed of the breath and take this into account when setting the mechanical elements to make breathing difficult.
A breathing difficulty can also be understood as a reduction in the respiratory support, which in exemplary embodiments can be provided by a ventilator. In exemplary embodiments, this reduction can lead to residual breathing support, which, however, already acts as training for the patient. Correspondingly, in exemplary embodiments, breathing difficulty is not limited to breathing difficulty based on free breathing without a ventilator. Instead, the training can also be carried out as free breathing, i.e. as breathing without a ventilator or also as a reduced breathing load, which, based on a low breathing support by a ventilator, now comprises a slight difficulty during training, the difficulty arising from the lack of support and the actual breathing difficulty.
The following steps are carried out breath by breath as part of the online analysis. The intensity can optionally be optimized and adjusted by breath. Exceptions are the recognition of termination criteria and the execution of the resulting termination, which can also take place at any time within a breath.
Schritt 1: Empfehlungen basierend auf einzelnen BeobachtungenStep 1: Recommendations based on individual observations
In the first step of the online analysis, a recommendation for the further training course is generated according to the result of each of the EMG-based status signals, which in this step is still independent of the other status signals.
- ◯ Eine sehr starke Änderung/Abweichung in der Muskelgruppenaufteilung. Ein möglicher Fall wäre zum Beispiel, wenn der Anteil der durch das Zwerchfell aufgebrachten Muskelarbeit unter einen geringen Prozentsatz in Bezug auf die Gesamtmuskelarbeit fällt;
- ◯ Sehr starke exspiratorische Aktivität;
- ◯ Sehr starke antizyklische Aktivität;
- ◯ Anhaltende Krämpfe oder Husten oder Schluckauf;
- ◯ Muskelfatigue über einer als nicht tolerabel angesehenen Schwelle;
- ◯ A very large change / deviation in the division of muscle groups. A possible case would be, for example, when the proportion of muscle work applied by the diaphragm falls below a small percentage in relation to the total muscle work;
- ◯ Very high expiratory activity;
- ◯ Very strong countercyclical activity;
- ◯ Harmful pressure conditions in the lungs, which can be detected, for example, using an EMG-based estimate of the pressure conditions in the breathing system.
- ◯ Persistent cramps or coughing or hiccups;
- ◯ muscle fatigue above a threshold considered intolerable;
- ◯ Detection of pathological conditions that can damage the muscle.
For conditions that are likely to last only for a short time, for example isolated cramps or coughs or short-term anti-cyclical activity, it is also possible to recommend a short break of a few minutes instead of the abort flag with a break flight before training again is started.
In addition to the detection of situations that suggest an interruption in training or a break in training, the main task of the evaluation units is to derive a recommended training intensity from each status signal. In order to enable a consensus to be found from the various status signals, not only is a specific, preferred training intensity determined, but an acceptable intensity range is output, which can be weighted according to the requirements of the respective status. Intensity ranges that are absolutely to be avoided can be excluded with a weight of zero. For example, this could be The result of the fatigue analysis will be that the muscle is currently not exhausted, i.e. both an increase and a decrease in the training intensity are in principle acceptable. At the same time, the analysis of the relative activation of the diaphragm and intercostal muscles could show that the relationship between the two is currently not optimal, but also not yet critical. A slight reduction in intensity would therefore be preferable; a slight increase would also be acceptable, but not really desirable. The weighting can be implemented in different ways, for example by means of representation as fuzzy sets (English "fuzzy sets") or fuzzy logic.
When determining the intensity recommendations, in particular the previous training course can also be taken into account in order to estimate, by means of comparison with previous training courses, what effect a change in intensity of a certain size could have on the various muscle condition parameters.
In the second step of the online analysis, the recommendations of the various observation units are weighted relative to one another in order to control the influence of the various observations on the final intensity recommendation. For example, the spasm detection could have a greater influence on the final recommendation than the fatigue detection. Missing status information can be ignored in the weighting (weight: zero). In embodiments, a user in
The weights of the individual observations can be dependent on all other observations - for example, taking into account the EMG magnitudes when cramps or coughs occur does not make sense; the weight of the magnitude observation should therefore be reduced in this case. Furthermore, a fatigue index used, for example, can provide imprecise information when muscle recruitment occurs and should therefore be weighted less heavily in this case.
A possible special case of the above-mentioned general method is to set all but one of the weights to zero in order to implement the training control using only a single variable. In this way, for example, methods can be implemented in which the training intensity is automatically adapted (regulated) in such a way that the person / patient has to apply a fixed, predetermined EMG amplitude or a fixed, predetermined work of breathing.
Step 3: Combining all information into a final intensity recommendation In the third step, a final recommendation to change the training intensity is made based on the weighted intensity recommendations based on the individual observations. For this purpose, an optimization problem is solved, whereby the training intensity should be chosen so that it has the greatest possible weight in as many of the isolated recommendations as possible (i.e. follows the individual recommendation as closely as possible), under the secondary condition that it is at least must be acceptable. In addition, it can be taken into account that a) greater training intensities are to be preferred (in order to maximize the training stimulus), and b) deviations from the existing training intensity are to be avoided in principle in order to enable training sessions that are as constant as possible and the person / patient not to confuse. On the other hand, it may also be desirable to give preference to individual changes in intensity in order to a) enable more variable training, and b) to be able to examine the effect of such intensity changes on the muscle condition.
When solving the optimization problem for the online analysis, it can make sense to prevent deviations from the existing training intensity as far as possible in order to enable training sessions that are as constant as possible. It is advisable to carry out this recommendation by breath.

In further exemplary embodiments, the training mode can be terminated as a function of a signal that contains information about at least one element of the group of a change / deviation in the muscle group division, an expiratory activity, an inspiratory activity, an anti-cyclical activity, harmful pressure conditions in the lungs , persistent cramps, coughing or hiccups, muscle fatigue above a threshold not considered to be tolerable, and detection of pathological conditions that can harm a person. The intercostal muscles, the diaphragm muscles, other auxiliary respiratory muscles and the antagonists (e.g. abdominal muscles, straight abdominal muscles (rectus abdominis)) can be taken into account for the division of muscle groups.
The interruption can optionally be coupled with the establishment of a time for the next training session. The parameters mentioned above can be taken into account. Accordingly, at the end of a training session, based on the evaluations, a prognosis for the expected necessary recovery time can be made and the next training session scheduled. In addition, the EMG data can optionally also be used to check during normal operation whether the recovery of the muscle has progressed so far that further training appears sensible. In this way, the set training time can be corrected again, taking into account the current patient condition.

On the basis of the EMG-based information about the current muscle state and the applied neuronal muscle activation, it is possible to give the patient individual feedback on the course of the training in real time. The following information can be included: How close is he to achieving the training goal? Has the strength and / or neuromuscular efficiency of his muscles increased or decreased compared to previous training sessions? This feedback can for example take place visually by means of a display, acoustically, haptically or in combination. In addition, the person or the patient can optionally receive feedback on the course of the training and / or the achievement of goals through maneuvers or special ventilation artifacts of the ventilator. Furthermore, the patient should be able to initiate training sessions independently using a suitable input or respiratory muscle training device and to be able to modify the duration and intensity of the training. This can be done, for example, via a conventional touch display or via more special input devices such as a button or a pressure ball that the patient can press in a certain rhythm in order to initiate a training session. It would also be conceivable that the patient can initiate a training session via a specific breathing rhythm or special breathing maneuvers.

The normal mode includes ventilation of a patient, which supports breathing in a known manner over the long term, so that, in contrast to the training mode, the normal mode ensures optimal lung-friendly breathing for the patient in the long term.
The adaptation of the training mode during normal mode is also called offline analysis. In contrast to the online analysis, the offline analysis is only carried out at the end of a training session. As part of the offline analysis, the signal curves of the individual muscle condition indicators must therefore be temporarily stored. The process is basically similar to the online analysis, see the description of the online analysis above. However, there are sometimes different evaluation criteria, different weightings and different constraints in the optimization task.
In contrast to the online analysis, the evaluation pays more attention to the course of the EMG states during the training session. The aim is to determine whether problematic conditions have developed over the course of the training. Especially in the case of changes in the training intensity from one training session to the next, it can be assessed here whether and how the change in the Exercise has had an impact on muscle condition. It is examined how the relative change of the mechanical training task behaves in relation to the relative change of the EMG performance. It can also be examined whether an increased training task also causes more muscle contractions (with the right muscles). In contrast to the online analysis, it can make sense to require deviations from the existing training intensity to a certain extent. This makes the training more varied and more information can be collected during the evaluation.
In addition, the findings of the current normal mode can also be incorporated into the offline analysis. Changes in the patient, for example long-lasting coughing stimuli or the like, can lead to a weakening of the planned training task. On the other hand, a significantly improved EMG performance, for example, can reinforce the planned training task.
Finally, a fundamental difference to the online analysis is that in the offline analysis not only a different training intensity, but also a different training mode can be recommended. This can be useful, for example, if a patient or person does not tolerate a certain mode well, or to increase the training variability. The recommendations of the offline analysis unit can be processed automatically (in the planning unit) and displayed to the device user (clinical staff, trainers). In the latter case, the decision-making is made by the device user, supported by the recommendations and information provided by the evaluation unit.
Update of the individual evaluation criteria In addition to the recommendation of a new training intensity, an update of the evaluation criteria is carried out as part of the offline analysis. For example, the threshold values can be adapted, from which a certain level of fatigue or a certain ratio of diaphragmatic and intercostal activity is assessed as critical.
Possible reasons for adapting these threshold values are a changed training intensity, a changed training mode and measurement changes, for example if the EMG electrodes are in different positions or the conductivity of the skin has changed (for example due to sweat). The question of the fundamental comparability of measurements in different training sessions should also be taken into account when creating the respective intensity recommendations.
- o the training modality, for example negative minimum inspiratory pressure, WOB (work of breathing) -controlled, EMG threshold value, etc., also called training mode here,
- o the training intensity, which is expressed differently in the different training modalities, and
- o the duration of the training.
- o Auswertung der zurückliegenden Trainingsphase, um ggf. für die nächste Trainingsphase eine Änderung zu empfehlen (Dauer, Intensität, weitere Trainingswege);
- o Generation von Trends und anzeigbaren Daten für das klinische Personal;
- o Berechnung eines Ready-To-Wean-Indikators.
- o Evaluation of the previous training phase in order to recommend a change for the next training phase if necessary (duration, intensity, further training paths);
- o Generation of trends and viewable data for clinical staff;
- o Calculation of a ready-to-wean indicator.
Based on the results of the analysis units, the ventilation mode, which is also referred to as normal mode, can of course also be adapted outside of the training sessions.
With regard to a visualization of the training progress in a WOB display with fatigue and atrophy information, it should be noted that additional information is obtained by the EMG-supported monitoring and analysis of the respiratory muscle training. Provide this information an added value for the clinical staff when assessing the patient's condition. However, in order to be able to interpret it easily, it is necessary to prepare and display the data accordingly.
In an alternative (not shown) illustration, the vertical axis is indicated relative to the WOB necessary for independent breathing.
When determining an EMG-based ready-to-wean score, it should also be noted that the ready-to-wean score expresses how far the patient is able to breathe sustainably himself. In contrast to a weaning indicator, which is based only on the proportion of self-performed WOB or other pneumatic data, the weaning indicator presented here can incorporate further EMG-based muscle condition information. As a result, he not only expresses whether the patient was able to do enough of his own breathing work for a short time in the training phases, but can also take into account how the respiratory muscles were doing and thus has a much better possibility of making lasting predictions whether the patient is able to perform the necessary work To do breath work permanently.
The improved Ready-To-Wean Score is also basically based on the proportion of self-performed WOB in relation to a WOB, which would be necessary for completely independent work. However, the score also takes into account the condition of the muscles during the short-term training sessions. In particular, it is taken into account if there are signs that the load that has to be performed during the short training periods cannot be applied over the long term.
- o Perseverance / duration (the longer the patient persevered, the better)
- o Devaluation, if fatigue developed (EMG-based)
- o Devaluation if expiratory activity occurs (including EMG-based)
- o Devaluation if there is heavy recruitment (including EMG-based)
- o Devaluation, for cramps (EMG-based)
- o Consideration of the power distribution within a breath (EMG-based)
This would continuously work towards a weaning trial. For a person who is Ready to Wean, one would expect that even with 100% own WOB share, there would be no fatigue symptoms for a medium time.

- • Visuelle Kommunikation über die Ergebnisse einer Trainingssession oder eines Diagnosemanövers und des Zustandes der Muskeln und des Atemapparates;
- • Visuelle Kommunikation von Ereignissen, die maßgeblich zu einer Änderung der Trainingsintensität geführt haben;

- • Feedback from the user for the acceptable value ranges. The user can set the
acceptable range 320 restrict / adjust. In9B-1 this is shown as an example under "cramps". In exemplary embodiments, however, it can also be adjustable for at least one additional value, for several values, or for all values. - • Influence of the user on the weighting (not shown in the graphic) can also be possible via the graphical user interface.
- • The individual training sessions can be marked in such a way that a coupling can be established between the various displayed parts AD. For example, this coupling can be resolved by displaying rainbow-colored dots. Alternatively, shapes, numbers, flashing patterns or the like would be conceivable as identification.
- • In exemplary embodiments, the user can interactively select which values are to be displayed on the axes (indicated by the drop-down menu). These can be individual values derived from the EMG / pneumatic data (e.g. fatigue, EMG performance of a muscle group or WOB) as well as values / scores derived or summarized therefrom.
Darüber hinaus können in Ausführungsbeispielen der
- 1. Amplitude, 2. mechanische Belastung, 3. außergewöhnliche Kontraktionen, 4. Fatigue, 5. „innere“
- 1. amplitude, 2. mechanical stress, 3. extraordinary contractions, 4. fatigue, 5. "internal" states and 6. further information that is also incurred in the EMG signal.
- ◯ Ein bewertetes Trainingsergebnis (englisch Score Trainingscompliance) als Zusammenfassung der Trainingsleistung, das unter anderem aus den EMG-Leistungen der Muskelgruppen abgeleitet werden kann;
- ◯ An evaluated training result (Score Training Compliance) as a summary of the training performance, which can be derived from the EMG performance of the muscle groups, among other things;
- ◯ An evaluated training result of internal evaluated muscle condition results as a summary of those derived from the EMG internal status information of the muscle. This internal status information can include fatigue, neuromuscular efficiency, muscle regeneration (for example quantified by DOMS (Delayed Onset Muscle Sornes)), electromuscular delay, risk of atrophy and max. Include pressure / force.
- • In the diagrams, further information can be displayed in exemplary embodiments at the points belonging to the individual training sessions. This can for example be in a
bladder 280 or as a local addition to the data point, shown here as anarrow 300 is shown down to visualize an adjustment of the reduced training intensity for the next training.

- • dem Nutzer weitere Details des Trainings kommunizieren, also zum Beispiel auch die Werte, die gerade nicht in einem der Grafen zur Anzeige selektiert wurden;

- • Communicate further details of the training to the user, for example also the values that were not currently selected for display in one of the graphs;
- • In addition, part D also has the important task of explaining the decisions of the automatic training adaptation to the user. The user should thus be able to understand the automatic decisions. This can include, for example, the following:
- ◯ Communication of problems that led to the training session being terminated, shown in the example as a
warning triangle 310 in case of "cough", - ◯ Communication of effects that have significantly led to a change in the training intensity. In the example this is the
green arrow 300 in the case of fatigue, which serves as information that a decision was made to increase the training intensity due to very low fatigue.
- ◯ Communication of problems that led to the training session being terminated, shown in the example as a
Weiter können Trainingsdetails folgende Zwecke erfüllen:

- ◯ Trainingseigenschaften;
- ◯ Atmung (hauptsächlich basierend auf EMG Leistung);
- ◯ innere Muskeleigenschaften;
- • In the case of summarized weighted results (English scores), the origin of the weighted results (scores) can be communicated to the user;
- • Individual values can be grouped. For example, the following groupings can be made:
- ◯ training characteristics;
- ◯ breathing (based mainly on EMG performance);
- ◯ internal muscle properties;
- ◯ Properties affecting the whole body, which can be recorded, for example, as stress measured by heart rate variability.
