In the area of cancer therapy, doctors and scientists have been dealing with the targeted overheating of tumor tissue, known as hyperthermia, for over a hundred years. A controlled application of heat should destroy cancer cells and make the tumor more sensitive, for example to further chemotherapy or radiation therapy. Basically, cancer cells are more sensitive to heat than healthy body cells. With long-lasting and repeated hyperthermia, cancer cells form so-called heat shock proteins, which are also called stress proteins. They act as a signal for the body's own killer cells to break down the damaged and mutated cells. For such a treatment of tumor tissue, only temperatures in the range of 40 to 43 ° C are necessary.
In addition to these hyperthermia procedures in the narrower sense, other procedures work with the targeted overheating of tissue, whereby the targeted thermal destruction of tumor tissue is in the foreground. These procedures can be understood as hyperthermia procedures in a broader sense; These include, for example, high-frequency-induced thermotherapy (HITT) and laser-induced thermotherapy (LITT). At HITT, the doctors insert a needle electrode into the patient's tumor during a small procedure. Local anesthesia or a light form of anesthesia are usually sufficient. In the tumor, the electrode generates temperatures between 56 and 100 ° C, at which tissue is immediately damaged. At LITT, a glass fiber light guide is inserted into the tumor tissue. The tissue is then heated and destroyed by long-wave laser light. As with HITT, the temperatures are in an area that directly destroys tissue.
The methods or devices mentioned have in common that on the one hand they require complex applicators (such as a needle electrode with displaceable high-frequency thread, a hot water-operated micro heat exchanger or an electrical heating element located in an elongated element), the size of which is determined by the internal structure and the heating thereof itself runs relatively sluggishly, and on the other hand, an insulation sleeve usually has to be provided around a shaft of the applicator in order to avoid damage to healthy biological tissue when the applicator heats up.
Proceeding from this, the present invention is based on the object of providing an improved device for the thermal treatment of biological tissue and a method for operating such a device, which has an applicator of the simplest possible construction, the size of which is largely independent of its internal structure and thus can be easily changed, especially reduced. The device according to the invention is also intended to permit targeted, relatively rapid heating of the diseased biological tissue to be treated and to protect healthy biological tissue from unintended heat.
This object is first achieved by a device for the thermal treatment of biological tissue, with the features of
The device according to the invention for the thermal treatment of biological tissue is based on generic devices in that it is set up in such a way that the tip of the applicator is heated exclusively with the aid of laser beams generated by the laser generator.
The heating of the tip of the applicator exclusively with the aid of laser beams generated by the laser generator advantageously enables only the tip of the applicator to be heated, the shaft of the applicator advantageously remaining at ambient temperature. In this way, targeted thermal treatment of biological tissue, in particular tumor tissue, is possible, with surrounding healthy tissue advantageously not being injured.
In a preferred embodiment of the invention, the device preferably forms a closed system with respect to the laser beams generated by the laser generator. A system which is closed with respect to the laser beams generated advantageously prevents the uncontrolled emergence of laser beams, in particular the generation of laser scattered radiation. Since there are no free laser beams, the device can be operated without any laser protective device, for example laser protective glasses or the like.
It has also proven useful that the applicator consists at least partially, in particular at its tip, of a thermally conductive material, in particular of medical grade stainless steel. Thermally conductive material can pass the energy absorbed by irradiation with laser beams as heat energy to the environment, preferably to adjacent tissue via heat conduction. In addition to its thermal conductivity, medical stainless steel also fulfills the legal requirements for use in the medical field, especially when it comes into contact with human tissue.
It is preferred according to the invention that the applicator comprises a linear, hollow shaft, one end of which can be connected to the collimator, in particular via a connecting piece, and the tip of which is arranged or formed at the other end. A linear hollow shaft advantageously enables the approximately interaction-free conduction of an (approximately) parallel laser beam through the shaft until the laser beams finally strike the tip arranged at one end of the shaft.
In a further preferred embodiment, it has proven useful that the laser generator generates laser beams of a wavelength which is matched to the absorption properties of the material of the tip, in particular to the absorption maximum of the material of the tip. Laser beams, the wavelength of which is matched to the absorption properties of the material of the tip, enable an optimal energy transfer of the laser energy to the tip and thus a low-loss and rapid heating / heating of the tip of the applicator.
It is advantageous if the laser generator generates laser beams in a wavelength range from 800 to 1600 nm, in particular in a range from 1030 to 1540 nm, particularly preferably with a wavelength of 1064 nm. Laser beams with a wavelength of 1064 nm have proven to be particularly beneficial for heating a medical stainless steel tip.
In addition, it is preferred that the laser generator is designed as a fiber laser. Fiber lasers have unique properties. B. electrical-optical efficiencies up to over 30%, excellent beam quality (with M2 <1.1 for single-mode fiber laser construction, M2 <1.2 for double-cladding fibers), a long service life (> 20,000 h) and a compact, maintenance-free and insensitive construction , The pulse mode extends into the fs range and can advantageously reach a high peak intensity.
In a further embodiment of the invention, it has proven useful that the collimator is set up to focus the laser beams in such a way that they tip the applicator in a range of less than 1.0 mm of the inner diameter of the applicator, preferably in a range of less than 0, 8 mm of the inner diameter of the applicator. The applicator is only heated at the tip and can be varied as desired by the doctor in the temperature range. The primary intention is a temperature range from 2.5 mm to 100 ° C exclusively at the needle tip. The shaft of the applicator itself is advantageously not heated and only has the usual body / ambient temperature, regardless of its length. The thermal damage to the surrounding “healthy” tissue can thus be advantageously avoided.
Finally, an embodiment of the device according to the invention has proven itself in which the device comprises at least one means for generating a visual and / or acoustic signal, in particular a lamp and / or a buzzer. A means for generating a visual and / or acoustic signal, in particular a lamp and / or a buzzer, can be advantageous to the user indicate the start and / or end of the generation of the laser beams. The treatment of the tissue by the user can thus take place in a more controlled and safe manner.
Ein erfindungsgemäßes Verfahren zum Betrieb einer solchen Vorrichtung dadurch aus, dass die von einem Lasergenerator erzeugten Laserstrahlen mit Hilfe eines Lichtwellenleiters zu einem Kollimator geleitet werden; der Kollimator die Laserstrahlen so fokussiert, dass sie sich innerhalb eines Applikators weitgehend geradlinig und parallel zueinander ausbreiten; die Laserstrahlen am dem Kollimator gegenüberliegenden Ende des Applikators auf die Innenwände einer Spitze des Applikators treffen; und die Spitze des Applikators durch das Auftreffen der Laserstrahlen und eine Absorption der Laserenergie erwärmt wirdA method according to the invention for operating such a device is characterized in that the laser beams generated by a laser generator are guided to a collimator with the aid of an optical waveguide; the collimator focuses the laser beams in such a way that they spread largely in a straight line and parallel to one another within an applicator; the laser beams hit the inner walls of a tip of the applicator at the end of the applicator opposite the collimator; and the tip of the applicator is heated by the impact of the laser beams and absorption of the laser energy
It is preferred according to the invention that in one process step the laser beams are focused by the collimator in such a way that they spread largely in a straight line and parallel to one another over a distance of up to 500 mm. Laser beams, which spread largely in a straight line and parallel to one another, do not interact with a shaft of the applicator, in particular with its inner wall, but only with a tip of the applicator. Accordingly, energy transfer by absorption advantageously takes place only at the tip, as a result of which the applicator is heated at its tip in a controlled manner and the shaft advantageously remains largely at ambient / body temperature.
It is also advantageous if the laser beams are generated continuously and / or in the form of pulses. Continuously generated laser beams advantageously enable the tip to be heated continuously, in particular with the aim of achieving a constant tip temperature over a longer period of time, which can be advantageous in particular in the context of hyperthermia treatment in the narrower sense (see above) in which the biological tissue to be treated should be heated to temperatures of 40 - 43 ° C. Laser pulses, on the other hand, advantageously make it possible to briefly heat the tip. Depending on the amount of energy absorbed, high temperatures (temperature peaks) can be achieved for a short time, by means of which targeted tumor cells can be destroyed directly, as is intended in a broader sense in hyperthermia procedures.
In addition, it is preferred according to the invention that the start and / or end of the generation of the laser beams by the laser generator is controlled manually (preferably by means of a foot switch and / or a button) and / or automatically (preferably by means of a timer) in one method step. With manual control, preferably by means of a foot switch and / or a button, the user can determine the start and / or end of the generation of laser beams - as a continuous laser beam or as a pulse sequence - and thus react completely freely to events / changes during the treatment. A foot switch in particular offers the attending physician the possibility of determining the start and / or end of the generation of laser beams without having to use his hands. The button, on the other hand, advantageously allows the control of the start and / or end of the generation of laser beams to be transferred to a third party, as a result of which the attending physician can then concentrate completely on the actual treatment. In this case in particular, the foot switch can then advantageously serve as a type of release module by the attending physician, and thus as an additional safety device. An automatic control, preferably by means of a timer, on the other hand, advantageously allows the user to concentrate completely on the tissue area to be treated and can also advantageously prevent individual areas from overheating.
Finally, it has proven useful that in one process step the generation of the laser beams, in particular the start and / or the end of the generation, are displayed visually and / or acoustically by the laser generator. A visual and / or acoustic display allows people present to be informed of the current status of the process and can, in particular, advantageously enable the user to keep an eye on the treatment field throughout the treatment.
The device according to the invention, or the method according to the invention for operating such a device, advantageously enables targeted, relatively rapid heating of a diseased biological tissue to be treated, at the same time protecting healthy biological tissue from unintentional exposure to heat. Due to the possibility of a network separation and the fulfillment of all requirements for EMC security, it is particularly suitable for medical ambulance use. The relatively simple internal structure of the applicator, which is largely independent of its size, advantageously also enables simple structural changes, in particular a further downsizing of the applicator.
