Description
The invention relates to a stent with a tubular support body, which consists at least partially of a biodegradable material and a method for its production.
A stent (German stent) is a medical implant for keeping open vessels or hollow organs. Often it is a spiral wire prosthesis or a mesh prosthesis in tube form made of metal or synthetic fibers with auxetic or mechanical properties for vasodilation and / or vessel opening.
Stents of various purposes are known in great variety from the prior art. Often, only a temporary retention of the stent in the body is necessary to fulfill the medical purpose. Stents made of permanent materials, ie materials that are not degraded in the body, are often removed again, since in the medium and long term, even with high biocompatibility can lead to rejection reactions of the body.
Hohlorgans kommen. One approach to avoiding further surgery now is to make the stent in whole or in part of a biodegradable material. Degradation means degradation by decomposition or decomposition of a compound into molecules or elements by physical, chemical or biological influences. Biodegradation is understood to mean microbial processes or processes due to the presence of body media, which lead to a gradual degradation of the structure consisting of the material. At some point in time, the stent or at least the part of the stent made of the biodegradable material loses its integrity and mechanical structure. The degradation products are largely absorbed by the body, with low residues are tolerated. However, there is a risk that flat, foil or flap-like fragments arise that tilt or fold into the lumen which one actually wishes to keep open. This can lead to an acute occlusion of the vessel or hollow organ.
The invention is based on the prior art, the task to provide a functionally and application technology improved stent and to show a method for its production.
The solution of the objective part of the task according to the invention in a stent according to the features of claim 1.
Ein erfindungsgemäßes Verfahren ist in Anspruch 10 charakterisiert.A method according to the invention is characterized in
11 bis 18.Advantageous embodiments and further developments of the stent according to the invention and of the method for the production thereof are the subject of the
A stent according to the invention has a tubular support body which at least partially consists of a biodegradable material. The biodegradable material may be metallic basis. Preferably, within the scope of the invention, a biodegradable material based on plastics is used. Thus, the support body of the stent may consist wholly or partly of a polymer of lactic acid. This remains stable for several months depending on the selected strength and is then degraded slowly.
According to the invention, it is provided that the supporting body is produced by an additive manufacturing method and that partial areas of the wall of the supporting body are removed and / or removed by a subtractive manufacturing method.
The invention thus combines the known per se additive manufacturing process for producing the support body or a semifinished product of the support body and the subtractive manufacturing process for packaging and adjustment of the stent, in particular its mechanical support properties and the degradation rate of the biodegradable material. The rate of degradation is specifically made by the wall design and interpretation of the wall thickness or partial areas of the wall. In particular, a targeted wall thickness rejuvenation or dilution by means of a subtractive manufacturing process. Here, the design of the support body by milling or cutting, in particular by electrical methods, preferably laser technology configured.
Overall, the mechanical properties of the stent according to the invention can be better influenced.
According to the invention, the scaffold is tuned individually to the anatomical and biomechanical requirements of a stenosis of a patient. A customized, three-dimensional stent is made. For this purpose, the structure of the affected stenosis is determined by means of an imaging method, for example magnetic resonance imaging (MRI). On the basis of the acquired digital data, the scaffold is assembled.
In particular, the support body is manufactured on the basis of the digital 3D data of a stenosis. For this purpose, first the basic shape or a semi-finished product made of the support body. In this case, preferably based on the digital 3D data, a biodegradable material is deposited in layers and thus the semi-finished product of the support body is constructed tubular. This basic structure of the support body constructed by layered deposition of material is subsequently processed in a subtractive manner.
As already stated, the subtractive production method for removing and / or excluding partial areas of the wall of the support body, preferably a milling or cutting, in particular a laser cutting or cutting. In this way, a dilution of the wall can be done especially in partial areas. Furthermore, the support body can be given a pattern in its wall, which is tailored to the anatomical and biomechanical requirements of the respective stenosis.
In the context of the invention, both partial areas on the inner circumference, as well as on the outer circumference of the wall of the support body can be removed or be.
The shape adaptation and wall thickness adjustment can be made in the axial and radial extent of the support body. A customized 3D printed stent is made to fit the affected body part of a patient.
A wall thickness reduction with respect to the starting thickness of the supporting body constructed by means of an additive manufacturing method can be carried out in the longitudinal direction (axially) as well as in the circumferential direction (radially). By the wall thickness adjustment in partial areas of the support body, the support force can be adjusted. Furthermore, the wall thickness design can influence the rate of degradation of the biodegradable material. Furthermore, it is possible to provide targeted Sollfalt- or kinks in the support body. These can serve as kinking or curling or folding edges for the implantation of the stent. During implantation, the stent can be reduced to a smaller diameter. At its place of use, implanted in a vessel or a hollow organ, the stent unfolds and assumes its supporting function.
Another aspect of the invention provides that the support body is provided with a drug depot. The stent has a pharmaceutically active substance (active substance), which in particular should reduce the risk of restenosis and support the healing process. The active ingredient can be applied in pure form or embedded in a carrier matrix as a coating or in receptacles formed in the supporting body, for example cavities.
Overall, the support body can consist entirely of a biodegradable material. It is also possible that the support body is formed from at least two mutually different materials, wherein at least one material is the biodegradable material.
One aspect of the invention provides for designing the stent such that during or after the degradation of the degradable material, the basic function of the stent is maintained in the form of a supportable residual structure. Due to the residual structure, sufficient vascular or hollow organ supports are still ensured. For example, with a tracheal stent or a bronchial or esophageal stent, minimal ventilation is ensured even after the degradation of the biodegradable material. Collapse of the stent is avoided.
The basic structure of the support body is such that no large or large-scale fragments are formed during the degradation of the biodegradable material.
Furthermore, the basic structure of the support body is designed so that the stent for implantation can be rolled up or folded in accordance with function or implantation. This is achieved by a partial taper of the wall thickness of the support body along a line-shaped area of the wall.
In the stent according to the invention, a shape adaptation and wall thickness adjustment takes place on the support body in axial and / or radial extension. The support body is assembled in such a way that the best possible uniform pressure distribution in the stenosis area takes place with geometrical form closure to the environment. This means that the support body due to the stent geometry encloses the penetrating shape without building up voltage spikes. Outside the loading range, the geometry remains neutral.
The configuration of the stent with the partially ablated and / or recessed areas in the wall of the support body, in particular in the form of recesses in the outer periphery of the wall or openings in the wall, advantageously supports the positional stability or positional fixation of the stent in a body tube or a hollow organ or vessel. By increasing the surface on the outer circumference of the stent, the static friction is increased. It is a high static friction causes a comparatively low surface pressure. The investment of the stent within the stenosis is therefore tissue-friendly, but very stable in storage. Furthermore, the depressions and recesses or openings of the stent cause a additional adhesive effect by being clamped to the surrounding tissue.
About the Wanddickeneinstellung and material distribution in the support body ensures that a functional scaffold of the support body is maintained even with progressive degradation of the biodegradable material until the stent is removed from the vessel or the hollow organ.
An essential aspect of the invention consists in adjusting the supporting body of the stent constructed in layers by means of an additive manufacturing process by means of a subtractive production method with regard to the mechanical properties and the behavior during the degradation of the biodegradable material.
By means of the invention, thin wall regions can be produced in the support body. The production of thin struts within the support body is possible. A thin strut has a thickness which is reduced compared to the additive produced wall thickness of the support body. Otherwise, this is not possible or only possible with difficulty in terms of production engineering.
As already mentioned, the support body can be provided with an agent depot. Furthermore, the support body can be provided at least in regions with a coating or at least partially with a sheathing. The production of a coating or sheathing can be done for example by a dipping process.
In the context of the invention, the combination of an additive manufacturing with a subsequent subtractive production or processing of the support body takes place. Here, first of all, the support body or a semifinished product of the support body is produced by an additive manufacturing process, in particular by a 3D printing. Subsequently, an individualization of the support body by means of a subtractive manufacturing process. In this case, partial areas of the wall of the support body are removed, that is diluted and / or excluded. The lateral surface of the support body can be provided with recesses, that is local Wandausnehmungen. Furthermore, the wall of the support body can be configured with one or more cuts or patterns.
Within the scope of the invention, a further additive manufacturing step may follow the subtractive manufacturing process. In this case, further material, for example a pharmaceutically active substance or a coating or coating, can be applied. Also, the support body can be provided with projections by additional material application.
The invention consequently makes the sequence of additive manufacturing processes and subtractive production processes the same in successive ways.
Furthermore, the surface of the support body can be smoothed and / or polished on the inner circumference and / or on the outer circumference.
