Description
Hintergrundbackground
The invention relates to a surgical jaw instrument, the type mentioned in the preamble of claim 1, as well as a mouth sector for a jaw part of such a surgical jaw instrument. Furthermore, the invention relates to a method for producing a mouth industry.
Surgical jaws of the type according to the invention are typically equipped with an elongate or curved rigid shaft and are preferably used for abdominal laparoscopic surgery. The jaw head of such an instrument is, depending on the application, e.g. designed as a pair of pliers, as a pair of scissors or as a gripper. Typically, the jaw head is opened or closed by a distally or proximally directed displacement of a pull or push rod displaceably mounted in a longitudinal passage of the shaft. Alternatively, the surgical jaw instruments but also scissor-like, without a longer shaft, be pronounced.
Problems arise with the high holding forces, which lead to a considerable load on various parts of the jaw instrument, especially in the area of the forceps jaw. A break in this area, the z. B. leads to the loss of a mouth industry in the patient's body, but is not allowed. It is therefore desirable to produce jaws that are resilient to bending and not easily break, but at the same time have a high rigidity and low wear. In the prior art, therefore, certain parts of the metal jaws, which are resilient and stretchable on bending, and other parts made of materials which have higher rigidity and less wear, were manufactured and then joined together by brazing.
The production of such composite mouth sectors is complex, error-prone and thus relatively expensive. As a rule, first the semi-finished metal product must be brought into the appropriate shape and then with the harder material, e.g. Tungsten carbide, which was previously molded and sintered, are soldered. The brazing is done e.g. manually with a blowtorch lamp at a melting temperature of about 750 ° C. Under these extreme conditions, the joining process is difficult to normalize or validate and the microstructure in the hardened jaw part is often adversely affected. So it can be e.g. come to grain growth and precipitation at the grain boundaries. Finally, after brazing, a post-processing of the brazed parts is required.
Against this background, the present invention seeks to provide mouthpieces for a jaw instrument, which are extremely stable, but can be produced simultaneously in a cost-effective and normalizable manner.
Beschreibung der ErfindungDescription of the invention
The object is achieved by a surgical jaw instrument with the features of claim 1, by a jaw industry with the features of claim 7 and a method for producing a mouth industry with the features of
Thus, an advantage of the invention is that the mandrels of the present invention comprising sintered metal-ceramic composite have significantly improved wear resistance, e.g. for use as a needle holder.
Furthermore, the costly and error-prone soldering process previously required for state-of-the-art products is eliminated in the manufacture of the mandrels of the present invention. Also, the elaborate post-processing, in which, for example so far removed excess solder and a Edge rounding had to be performed on hard metal plates, is omitted for the mandrels invention, since they can be optimally sintered in the desired shape.
Another advantage of the invention is that sintered metal-ceramic composites are generally biocompatible so that immune reactions against the material are avoided.
In addition, the surfaces of sintered metal-ceramic composites are treatable or sealable in a variety of ways, for example by means of coating or glazing techniques. As a result, the mouth sectors can be easily adapted to the properties desired in the individual case, in particular with regard to appearance, surface structure and / or durability.
In a first aspect, the invention thus relates to a surgical jaw instrument, at the distal end of a jaw part is arranged, which is formed with two mouth branches, wherein at least one mouth sector is pivotally mounted, and at the proximal end of the jaw instrument, a handle for actuating the jaw part is arranged , characterized in that the mouth sectors comprise sintered metal-ceramic composite material.
In other words, the mandrels according to the invention are characterized in that they consist at least partially of sintered metal-ceramic composite material. Preferably, the mouth branches are 50% or more, 60% or more, 70% or more, 80% or more, 90% or more, 95% or more of sintered metal-ceramic composite. In a particularly preferred embodiment, the jawed branches are 95% or more of sintered metal-ceramic composite material. In other words, the mouth sectors consist of 50% -100%, 60% -100%, 70% -100%, 80% -100%, 90% -100%, or 95% -100% of sintered metal-ceramic composite.
According to the invention, the mouth sectors preferably consist of one or more sintered metal-ceramic composite materials. Sintered metal-ceramic composites are composites, i. Materials made of two or more bonded materials that have different material properties than their individual components. In particular, the term "sintered metal-ceramic composite" refers to a material comprising one or more ceramic components and one or more metallic binders.
The sintered metal-ceramic composites of the invention are sintered materials, i. Materials that can be sintered to a desired shape (the shape of a mouth industry). Preferably, the metal-ceramic composites are also formed by a metal injection molding (MIM) process. Corresponding sintering and MIM processes and components for making sintered metal-ceramic composite are known to those skilled in the art and described elsewhere herein.
The sintered metal-ceramic composite material according to the invention comprises one or more ceramic components or ceramics. The terms ceramic component and ceramic are used synonymously herein. Preferably, the sintered metal-ceramic composite comprises a ceramic having a Young's modulus E of 135 GPa or more. Such a high modulus of elasticity of the ceramic component ensures that the mouth sectors can withstand the heavy mechanical load. Such a ceramic may e.g. an oxide ceramic or non-oxide ceramic. By contrast, a silicate ceramic according to the invention is generally unsuitable since it has a modulus of elasticity of usually 40 to 130 GPa. The ceramic used in the invention has e.g. an elastic modulus of 135 GPa or more, preferably 150 GPa or more, more preferably 200 GPa or more. In other words, the ceramic may have a modulus of elasticity of e.g. 135 GPa to 500 GPa, 150 GPa to 480 GPa or 200 GPa to 450 GPa.
If the sintered metal-ceramic composite comprises a plurality of ceramics, the specified preferred values for the modulus of elasticity refer to the ceramic with the largest weight fraction in the metal-ceramic composite. Preferably, substantially all of the ceramics contained in the metal-ceramic composite material have a modulus of elasticity as described above.
The sintered metal-ceramic composite may e.g. comprise a ceramic selected from the group consisting of oxide ceramics and non-oxide ceramics. The ceramics in the sintered metal-ceramic composite are also referred to as the reinforcement phase.
Suitable oxide ceramics are e.g. Alumina, magnesia, zirconia, titania, titanate, lead zirconate titanate, zirconia reinforced alumina, zinc oxide and barium titanate. In a preferred embodiment, the metal-ceramic composite comprises alumina.
Suitable non-oxide ceramics include silicon carbide, silicon nitride, aluminum nitride, boron carbide, boron nitride, titanium carbide, tungsten carbide, tantalum carbide, niobium carbide, molybdenum carbide, zirconium carbide, titanium nitride, hafnium carbide and chromium carbide. While tungsten carbide is a widely used reinforcement phase, there are other carbides with particularly advantageous properties. Thus, metal-ceramic composites containing titanium carbide as the main carbide component have even better heat resistance and hardness, and a low tendency to diffusion and adhesion. These metal-ceramic composite materials are called cermets in non-German terminology and are preferred according to the invention. Thus, in one preferred embodiment, the sintered metal-ceramic composite comprises titanium carbide. In another preferred embodiment, the metal-ceramic composite comprises niobium carbide. In yet another preferred embodiment, the metal-ceramic composite comprises tungsten carbide.
The one or more ceramics make up the largest proportion in the sintered metal-ceramic composites. The sintered metal-ceramic composite contains e.g. 5% to 97% ceramic, preferably 20% to 97%, more preferably 50% to 97%. Unless otherwise stated or mandatory, all percentages herein are to be understood as wt .-%.
Here, it is preferable that tungsten carbide, titanium carbide and / or niobium carbide constitute more than 40% of the ceramics in the sintered metal-ceramic composites. A combination of titanium carbide and titanium nitride is particularly preferred.
In addition to the ceramic component or ceramics, the sintered metal-ceramic composite material comprises one or more metallic binders. The sintering is usually carried out at a temperature above the melting point of the metallic binder and below the melting point of the ceramic. Thus, the metallic binder may act to bind during the melting process, e.g. penetrate into cavities. In other words, the metallic binder is preferably fused to the ceramics in the sintered metal-ceramic composite.
The metallic binder (s) may e.g. be selected from the group consisting of cobalt, nickel, iron, chromium, nickel-chromium and molybdenum. The metallic binders cobalt, nickel and iron are particularly preferred. Iron can e.g. be contained in the metal-ceramic composite in the form of a stainless steel or steel alloy as a metallic binder. Particularly preferred stainless steel or steel alloys are the alloys with the material numbers 1.4542 and 1.4404.
The sintered metal-ceramic composite preferably comprises 1% to 20% metallic binder, more preferably from 3% to 10%.
In a preferred embodiment, the sintered metal-ceramic composite according to the invention comprises tungsten carbide, tantalum carbide, niobium carbide and / or titanium carbide as a ceramic component and iron, cobalt, nickel and / or chromium as metallic binders. Such sintered metal-ceramic composites may be manufactured or purchased by those skilled in the art, e.g. as cermet of Formatec Ceramics. The sintered metal-ceramic composite may e.g. Niobium carbide and iron, e.g. in an alloy with the material numbers 1.4542 and 1.4404, comprise or consist of these components. Alternatively, the sintered metal-ceramic composite may include or consist of tungsten carbide, cobalt, and tantalum carbide. In a preferred embodiment, the sintered metal-ceramic composite consists of about 88 wt% tungsten carbide, 11 wt% cobalt and 1 wt% tantalum carbide.
In addition to the ceramics and metallic binders, the sintered metal-ceramic composite material may contain further stabilizing additives, which prevents segregation of the components.
The surgical jaw instrument has a jaw part at its distal end. The jaw part according to the invention is designed with two mouth sectors, wherein at least one mouth sector is pivotally mounted. The pivotable mouth sector is movable against the second mouth sector and touches it in the closed state and / or is able to hold an object to be manipulated between the mouth branches. In one embodiment, the jaw part is formed with two jaw sectors pivotally mounted about an axis. The mouth sectors are movable relative to each other and can touch each other and / or an object to be manipulated in the closed state.
In one embodiment, it is envisaged that the jaw part is designed as a forceps or gripper with two, suitable for gripping and holding, movable mouth sectors. In other words, the surgical jaw instrument is preferably a grasping instrument. In a particularly preferred embodiment, the surgical jaw instrument is a needle holder. Alternatively, the jaw part can be designed as a pair of scissors with at least one mouth sector having a cutting element. In particular, for use as a needle holder, a single rigid and a single movable mouth industry may be provided. In other alternatives is also a Jaw part with three or more than three mouth sectors conceivable, for example, to form a multi-armed gripper.
Furthermore, a handle for actuating the jaw part is arranged at the proximal end of the jaw instrument according to the invention. The handle preferably consists of two handle elements, which are each associated with a mouth industry. Alternatively, a first jaw branch may be associated with a shaft of the jaw instrument and a second jaw branch may be associated with a handle member, e.g. by attachment to an operating element connected to the handle element, e.g. an actuating rod.
In one embodiment, the surgical jaw instrument according to the invention is characterized in that it comprises a shaft formed as a tube, at the distal end of the jaw part and at the proximal end of the handle is arranged, and characterized in that the jaw part operable by a shaft passing through the actuator is, which is longitudinally displaceable by the arranged at the proximal end of the shaft handle in the direction of the shaft. With the elongated shaft such needle holders are particularly suitable for laparoscopic applications in the abdomen, in which there is a relatively large distance between the surgeon and the site of intervention. The actuating element is preferably designed as an actuating rod.
In an alternative embodiment, the surgical jaw instrument according to the invention is characterized in that the mouth branches are connected to one another via a joint and are movable towards each other by compressing two gripping elements of the handle connected to the jaw parts. In this embodiment, the mouth sectors and the associated gripping elements are preferably made in one piece.
In a second aspect, the present invention relates to a mouth sector for a jaw part for a surgical jaw instrument, in particular for a surgical jaw instrument according to the invention, characterized in that the jaw industry comprises sintered metal-ceramic composite material. In other words, the mouth industry according to the invention, as already described elsewhere, is characterized in that it consists at least partially of sintered metal-ceramic composite material. The sintered metal-ceramic composite may have the composition described elsewhere herein.
In a further aspect, the present invention relates to a method for producing a mouth industry according to the invention, characterized in that it comprises the steps of: (a) mixing one or more ceramic powders and one or more metallic binders in powder form or a sinterable mixture of one or more ceramic powders and one or more metallic binders in powder form; (b) shaping the mixture; and (c) sintering the mixture; wherein step (c) takes place during or after step (b).
In step (a) of the process according to the invention, one or more ceramic powders and one or more metallic binders are mixed in powder form or a sinterable mixture of one or more ceramic powders and one or more metallic binders in powder form is provided. As described elsewhere, the sintered metal-ceramic composite of the present invention may further contain other components. Such further components can be added to the mixture or to the ceramic powder or the metallic binder in powder form before, during and / or after step (a).
Methods for mixing such powders are known to the person skilled in the art. For example, the mixing in step (a) may be carried out by co-grinding the ceramic powder and the metallic binder in powder form. In this case, first the powder (s) are weighed out and, e.g. milled in a ball mill or in an attritor for several hours to days. The milling process serves to distribute all components as evenly as possible and to break larger particles. Alternatively, finished, sinterable mixtures of ceramic powders and metallic binders in powder form can be purchased from various manufacturers.
In step (b) of the process according to the invention, the mixture is shaped. In particular, the mixture is placed in the shape of a mouth industry (or part of a mouth industry). Methods of forming materials to be sintered are known to those skilled in the art. The mixture may e.g. by molding in a suitable mold (e.g., dry pressing), by 3D printing, or by powder injection molding.
The step (b) may include a metal powder injection molding (MIM) step comprising a step (b1) of injection molding the powders with a plastic (to the desired shape); and a step (b2), in which the plastic is removed from the injection molded powder / plastic blank (green part). The mixture to be sintered in step (c) is then in the form of a blank. This method allows a particularly fine and complex shape.
In step (b1), the powder with the plastic is already injection molded in the form of a mouth industry or part of a mouth industry. In this case, the powders (ie the mixture of the ceramic powder and the metallic binder in powder form) are preferably mixed in a ratio of about 10: 1 with the plastic. Suitable plastics and injection molding processes are known to the person skilled in the art.
In step (b2), the plastic is removed without the blank losing its shape, i. "Binder removal". Suitable, e.g. Thermal, methods for removing the plastic from the blank are known in the art.
In step (c) of the process according to the invention, the mixture is sintered. Step (c) may take place during or after step (b). In other words, the shaping can take place simultaneously or before sintering. Preferably, step (c) takes place after step (b). Suitable sintering processes are known to the person skilled in the art. During sintering, the mixture is heated to temperatures above the melting temperature of the metallic binder but below the melting temperature of the carbide. The heating of the metallic binder to its liquid phase causes the binder to fill in the existing interstices and surround the carbides. Suitable temperature ranges are for example over 1000 ° C. Optionally, the sintering is carried out under elevated pressure.
