Description
The invention relates to a method for determining the calcium content in blood vessels in the ECG-triggered sequence mode of a computed tomography device, in which the assignment of an individual risk statement is made by comparison with reference to specific tube voltages reference tables, and a computed tomography device for performing the method.
Computed Tomography (CT) provides a fast, non-invasive technique for obtaining images of vessels showing the location and extent of calcium deposits.
Commercially available data processing programs allow the determination of the calcium content in the coronary vessels based on the 3D image data or volume data of computed tomography. This requires a native CT scan, i. an image of 3D image data of the heart without contrast medium. The user must then mark in the different image representations manually identified as lime deposits identified areas that will later contribute to the quantification of calcium content. As a rule, the different coronary arteries are marked by the user and named accordingly, for example in the usual designations LM (Left Main), LAD (Left Anterior Descending), LCX (Left Circumflexus) and RCA (Right Coronary Artery). The calcium levels are then calculated for the individual coronary vessels from the volumes of the marked areas. The calculation is generally carried out as a weighted sum of all pixels or voxels in the marked areas which are above a threshold value for the HU value, typically 130 HU. The weighting factor depends on the maximum HU value within the range.
To determine the calcium content, a fixed, established in the literature examination protocol is usually followed: Examination in ECG-triggered sequence mode, calculation and evaluation of 3 mm layers, 120 kV tube voltage. In particular, the most widespread clinical measure for calcification of the coronary arteries, the so-called Agatston Score, is defined for a fixed tube voltage of 130 kV and later also for 120 kV. All clinically available reference values and limits are available for this tension, which then give a risk assessment to a measured Agatston score of a particular patient.
If one differs in the investigation of the defined tube voltages, z. For example, by using lower tube voltages, such as 80 kV, the Agatston score changes quantitatively because the X-ray absorption of Ca, and hence the CT value of Ca, depends on the tube voltage (or, in essence, on the average energy of the tube spectrum). The scores obtained in this way are then no longer comparable to the established references and therefore clinically meaningless.
However, the CT examination protocol for the determination of the lime content of the coronary arteries should have the lowest possible radiation dose for the patient, because the Ca-scoring is indeed carried out to determine the risk and is also used in the context of screening (that is, check-up of healthy itself). However, by fixing it at 120 kV tube voltage, the possibility of reducing the radiation dose is limited, because a certain lower dose value (achieved eg by lowering the tube current) can not be undercut technically (because the current can not be lowered further), or the Images have such a poor signal-to-noise ratio due to low dose that the accuracy of determining lime content is affected by image noise.
The object of the invention is therefore to present a method and a device with which ECG-controlled CT scans can be carried out for the determination of the lime content with a significantly reduced radiation dose.
This object is achieved by a method according to
It is proposed to further reduce the radiation dose in ECG-controlled CT scans for determining the lime content of vessels (Ca scoring) to change the tube spectrum of the CT scanner by a prefilter mounted on the tube side.
In this case, ECG-controlled means that the ECG is used as a trigger for recording in order to reduce artifacts resulting from the movement of the heart and to use the optimum cardiac phase for image acquisition.
"Tube side" is to be understood that the X-ray light emitted by the X-ray source first passes through the pre-filter before it hits the object to be examined.
The X-ray light emitted by an X-ray source has an energy distribution of the photons (X-ray spectrum, energy spectrum), which can be influenced by the voltage (tube voltage) applied to the X-ray tube. A prefilter of z. B. 0.6 mm thick tin plate absorbs the low-energy part of the tube spectrum and shifts the average (average) energy of the spectrum to higher values.
The invention provides for use in ECG-controlled CT scans for determining the lime content of vessels, a corresponding tube-side pre-filter, and to make the combination of thickness of the prefilter and selected tube voltage so that the resulting spectrum about the average energy of a standard 120 kV spectrum (for which the Agatston score reference tables are available), but much more monochromatic (due to lack of low energy and high energy contributions).
Such a combination could, for. Example, the use of 90 kV tube voltage with a 0.6 mm tin pre-filter. Due to the retained average energy of the spectrum, a measure for the lime content of the vessels (eg the Agatston score) will not change or only insignificantly change compared to the established case (120 kV) - but the radiation dose can be significantly reduced.
In addition to tin, suitable pre-filters are other suitable materials and other material thicknesses which are suitable for shifting the average energy of an X-ray energy spectrum so that the resulting energy spectrum corresponds to that of a 120 kV tube voltage. For the 120 kV tube voltage, reference tables are available on the basis of which the Ca content or Agatston score can be used to estimate the risk that the patient will have in the foreseeable future of contracting aterial calcification, suffering a heart attack or similar Lime deposits in the blood vessels resulting in diseases.
In summary, the invention relates to a method for determining the calcium content in blood vessels in the ECG-triggered sequence mode of a computed tomography device, in which the assignment of an individual risk statement by comparing matched to certain tube voltages reference tables, as well as a computed tomography device for performing the method.
The invention significantly reduces the radiation dose to which a patient is exposed during such an examination.
