Abstract
Description
The invention relates to a patient table according to the preamble of the independent claim.
In modern medicine more and more metal prostheses are used. For example, radiologists in imaging in patients today find coils and clips in cerebral vessels, stents in coronary vessels, stents in the abdominal and thoracic aorta, pacemakers or pacemakers in the atria of the heart, endoprostheses in the shoulder, hips and knees, plates and studs on the skeleton and spine. Particularly in CT scans of patients with these metal prostheses occur in the downstream image reconstruction unwanted image artifacts. Under certain circumstances, these can impair the image quality in the vicinity of the metal in such a way that the diagnosis is only very limited or even impossible.
In computed tomography (CT), some methods are available to reduce metal artefacts (scanning methods and reconstruction algorithms) that the user can select, as long as he knows which patient has the metal in which position and location in the body.
In current patient examinations, the patient is questioned about metals prior to examination or removed from his files. In these cases, the user can select and use the correspondingly optimized scan protocols and reconstruction algorithms or filter cores before the scan or recording. It is more difficult if the patient is unresponsive or no records are available. This usually turns out to be only after the scan, in the conventional reconstruction that metal has been in the patient. Here are the possibilities then only with the optimized reconstruction and filtering methods to search.
The image reconstruction methods (special reconstruction nuclei or iterative image reconstruction) for metal artifact reduction do not always provide the desired high-quality diagnostic image impression and, in the case of iterative image reconstruction, sometimes take up to 30 minutes of computation time per data set.
It is more efficient, for. A special scan mode, e.g. in the so-called dual energy method is possible to choose. With a monoenergetic imaging approach, it is possible to efficiently remove even the largest and heaviest metal artifacts. However, the disadvantage of the method lies either in a limited temporal resolution of the image data sets, as is the case for example in dual-energy CT, or in a greatly inflated radiation exposure, as for example in the so-called "Rapid KV Switching" method by means of a X-ray tube is possible. It is therefore relevant to know exactly in advance when which scan method will be used in which body region.
Magnetresonzanztomographen stattfindet. The invention has for its object to develop an improved technology with which metals can be detected in the body of unresponsive or unconscious patients, ideally before a scan by a computer tomography or magnetic resonance tomography takes place.
The object is achieved with the device according to the independent claim. Advantageous embodiments of the invention are the subject of the dependent claims or can be taken from the following description and the exemplary embodiments.
The invention relates to a patient table with a tabletop supporting a table base, wherein the table top of a retracted state in one or more extended states in the longitudinal direction of the table is movable, characterized in that in the table base at least one induction coil is integrated such that at least one Metal piece is detectable in a body region of a patient to be examined, when the table top is in the retracted state.
An embodiment of the invention provides that the at least one induction coil is designed as a pulse induction coil.
A further advantageous embodiment provides that the table top consists of a conductive plastic material, preferably of carbon.
A further advantageous embodiment provides that the induction coil can differentiate between different types of metal (ferrous or non-ferrous metal) in metal detection. By means of the metal detector (based on pulse induction) it is possible to differentiate between ferrous or non - ferrous metal. Thus, a correct selection of the appropriate metal artifact correction can be made. In one type of metal is more of an optimized reconstruction, in another metal type, the dual-energy method, the method of choice.
A further advantageous embodiment of the invention provides that units of measurement are mounted on the table top such that the position of the detected piece of metal can be determined.
A further embodiment of the invention provides that transmitting and receiving devices are arranged in the at least one induction coil so that the receiving device can recognize the position, position, size and / or depth of the metal or of the metal piece on the basis of the response signal. The position provides the location of the metal in the body of the patient. The situation provides the alignment of the metal in the body. In this case, the response signal can be caused by the pulses emitted by the transmitting device and eddy currents which can be generated by the decay behavior of the metal. It is also conceivable that the recognizable by means of the response signal position, location, size and / or depth of the metal on a so-called topogram of a patient, in whose body the metal is transmitted or registered. A correlation can be established between the position, position, size and / or depth of the metal (P) and a topogram of a patient in whose body (K) the metal (P) is detectable by means of the response signal a suitable image recording protocol and / or reconstruction method for the body region to be examined can be fixed.
In other words, the invention provides a combination of pulse induction coils under the patient table for detecting metallic objects in the patient as well as an image of the eddy current map on the so-called topogram of a patient. This enables the automatic selection of existing optimized scan (image acquisition) or image reconstruction methods. This can be done exactly to the respective body region and possibly free of manual adjustment by the user. Furthermore, the mapping of the eddy current map to the topogram can be used to prospectively and retrospectively adjust the scan and reconstruction parameters for the areas of the patient in which metals adversely affect the image quality.
- Metals in the patient are automatically detected before the scan, for example by means of a magnetic resonance device (MRT) or a computed tomography (CT), which is particularly advantageous in the case of unresponsive patients for carrying out the scans.
- - The metals in the patient are classified and located according to position, location and intensity, enabling precise scan planning to avoid artifacts.
- - A 3D map with the quarter current results from the measurement using the pulse induction method are transferred to the topogram of the patient or superimposed or registered. Modern algorithms that automatically detect the organs and body regions on the topogram provide an accurate anatomical image of the metal artifact data (map) for body region-based scan and reconstruction planning.
- The system automatically selects the optimal (special scanning methods or dual energy or monoenergetic methods) for the body regions affected by metals or filter kernels and iterative reconstruction methods for metal artifact reduction required for the reconstruction.
- - In addition, the method enables a user-independent, efficient and reliable reduction of metal artefacts in patients undergoing CT or MRI examinations, where it is not known beforehand whether the patient carries metals.
In der Zeichnung zeigen: Further advantages, details and developments of the invention will become apparent from the following description of exemplary embodiments in conjunction with the drawing. In the drawing show:
In
With the CT / MR markers in the table, the 3D magnetic field can be correlated (registered) with the exact location of the patient (on the table with markers). The marks also serve as a guide to an image system associated with the CT or MR device. These marks should also be visible in the 3D image of the pulse induction coils. Possibly. these are themselves designed as small coils. This is detected by the metal detector, as well as by the measuring system MR or CT for positional correlation.

Furthermore, the embodiments of the invention are explained in more detail:
The problem of detecting metals and their position in the body of unresponsive or unconscious metal carriers is solved by means of several induction coils in the patient table. Here are several coils (the more, the smaller, the higher the specificity), installed under the patient table top (eg made of carbon). These are stationary installed in a table top bracket or in the table base and remain with moving out table in the rear, stationary part of the patient table. There they detect, in the rear end position of the table, the metals in the patient by field changes of the induction coils. The coils installed in the table are essentially static or non-motion metal detectors based on the pulse induction motion principle.
In this method, short magnetic pulses are emitted from the search coil, which in conductive media such as e.g. create eddy currents for a metal object. The above-mentioned eddy currents remain even after switching off the magnetic pulse for a few microseconds in the metal objects, which is why they can still be clearly detected during the time-delayed receiving phase of the now working as a receiving coil search coil. For this purpose, should an electronics accumulate voltage changes to the coil high, separate from interference and perform a voltage-controlled oscillator, which indicates the located metal by a frequency change. A temporally different decay behavior of the eddy currents generated in the metals allows a more accurate identification of the located metals. The user is thus able to use the response signal to prove position, position, size and, in some cases, depth of the located metal object. The integrated metal detector has a high sensitivity to noble metals such as e.g. Gold, silver, copper and many alloys on. Metal discriminations are indicated by the temporal decay behavior of the eddy currents in located metal objects with a numerical value between 1 and n. Furthermore, iron can be distinguished from non-ferrous metals. The metal discrimination allows detection of the time decay behavior of the eddy currents in the metal, which is displayed on a display with a numerical value e.g. between 00 and 99 can be read. The value of the cooldown allows direct conclusions about the type of metal because it is derived from the conductivity, permeability, and size of the metal object. For the unambiguous identification of iron and non-ferrous, another special evaluation method is possible, which works together with double D probes. The pulse method has many advantages due to the technical time delay between sending and receiving. There is a temporal decoupling, which allows a particularly high transmission power and does not place high demands on the mechanical strength of the coil, which is why almost unlimited search coil sizes can be used. With increasing search coil size, the search depth for large objects also increases. At the same time, the sensitivity for small objects decreases, which in many cases may be desirable. By means of the methods described herein, it is possible to detect and locate the location of metals and non-ferrous metals in the patient's body. The specificity for localization is directly proportional to the number of coils and inversely to their diameters. In other words, the smaller and the more, the more accurate. Whereas the sensitivity of the "pulse induction method" increases with increasing coil size. Therefore, a combination of both methods is advantageous. A large coil first detects the metal and the smaller, many coils then detect the position and position based on this signal. However, in the method described above, other metal detectors may also be used, such as commercially available pulse-induction or sine, VLF, TR metal detectors, which, however, may have loss of sensitivity.
The method described here can also be correlated with a CT topogram in which metal artefacts are noted as extremely high HU values. This method enables the detection and localization of metal artifacts before the scan and before the topogram. If no topogram is created before the scan, the metal map is displayed on a monitor, and the map-patient correlation can be manually performed by the user. The help is a scale, which is embedded on / in the side of the table board. It indicates or indicates on which "length" at the table the metal is in the patient. The user can incorporate this information for the scan and reconstruction planning.
Once known by the coil or coils, the location and location of the metal object or objects are stored in the system. The position is stored relative to the table support made of carbon, under which the coils are located in the rear end position. As soon as the topogram (a projection with static CT with a moving table = topogram) of the patient is taken with the CT, metallic markings in the table allow a correlation of the measured eddy currents with the measured anatomy. Relative to the metallic markers M in the table board made of carbon, a computer program can correlate or register the eddy currents measured by the pulse induction method with the patient topogram (overlaying). This also allows the correlation of individual body regions from the topogram to be found. Thus, e.g. Head, neck, thorax, abdomen, pelvis, lower extremities, heart, lungs, kidneys, spine, already automatically detected and named by the computer program with the help of an algorithm. If a user selects a scan protocol for the body region thorax, then the computing program compares the values of the assignment from the "topogram" with the "eddy current map" and can thus provide the user with a suitable scan protocol for this examination section. Propose "Thorax". For the other regions or body regions to be examined where this alignment also takes place but no metal was found, the standard scan protocols are used. At the same time, metal detection from the pulse induction method (as well as from the potential other metal detection methods) has an influence on the choice of the reconstruction kernel or method, e.g. filtered or weighted backprojection. Thus, it is possible to use in a body region with metal, e.g. the hip, to choose a folding core with a matching filter reducing a special metal artifact. Possibly. In addition, an iterative reconstruction method based on raw data can additionally be used. Whereas, in the remaining other examinations of the patient, abdomen and peripheral angiography, in which no metal was detected, normal standard reconstruction parameters (less time-consuming procedures) can be used.
