Abstract

Description
Die Erfindung betrifft eine Chirurgieeinrichtung zur Stillung von Blutungen.The invention relates to a surgical device for stopping bleeding.
Viele chirurgische Operationen, insbesondere offene, laparoskopische oder endoskopische Operationen erfordern eine aktive Stillung von auftretenden Blutungen. Treten Blutungen auf, so müssen diese gestoppt werden, da sonst der Operations-Situs schnell mit Blut bedeckt ist und eine effektive Fortführung der Operation oft unmöglich ist. Bei schweren Blutungen besteht darüber hinaus eine akute Verblutungsgefahr des Patienten, wenn die Blutung nicht innerhalb kurzer Zeit zum Stillstand gebracht wird. Im Falle schwerer Blutungen werden oft Blutkonserven benötigt, die prinzipiell Mangelware sind und außerdem von Patienten häufig aus religiösen oder anderen Gründen abgelehnt werden. Darüber hinaus besteht bei der Verwendung von Blutkonserven immer ein gewisses Restrisiko einer Infektion. Herkömmliche Verfahren und Vorrichtungen zur Blutstillung bedienen sich mechanischer, elektrochirurgischer oder laserchirurgischer Mittel. Weiterhin besteht die Möglichkeit, Fibrinkleber oder Kollagenauflagen zur Blutstillung zu verwenden. Mechanisch kann eine Naht oder ein Hämoclip zur Blutstillung eingesetzt werden. Die mechanische Blutstillung ist gegenüber der elektrochirurgischen und der laserchirurgischen Blutstillung mit einem erheblich größeren Zeitaufwand und schwierigerer Handhabung verbunden. Ebenfalls können Naht- und Hämoclip-Unverträglichkeiten wie Fremdmaterial, insbesondere durch Fadenmaterial und metallische Werkstoffe auftreten. Die Blutstillung mittels des Lasers und mittels der Elektrochirurgie zielen auf eine Erwärmung der blutenden Gewebestelle ab, wobei bei der Laserchirurgie eine exogene Erwärmung und bei der Elektrochirurgie eine endogene Erwärmung des Gewebes zur Blutstillung führt. Nachteilig ist hierbei das Auftreten hoher lokaler Temperaturen von bis zu 300°C und ein damit einhergehendes Risiko der thermischen Schädigung von angrenzendem Gewebe. Darüber hinaus heilen die Wunden, welche durch Laser und Elektrochirurgie erzeugt werden im Vergleich zu einer mechanischen Schädigung des Gewebes durch ein Skalpell nur verzögert. In vielen Fällen konnte gezeigt werden, dass auch thermisch induzierte Wunden Verwachsungen von Organen untereinander oder Verwachsungen mit der Bauchwand verursachen können. Besonders problematisch ist dieser Befund bei fast allen Operationen im Bereich der Gynäkologie, aber auch bei anderen operativen Fachdisziplinen. Hiervon sind besonders viele junge Frauen betroffen, da die typischen Operationsindikationen Frauen im gebärfähigen Alter betreffen (Ovarialzysten-Entfernung, Myom-Entfernung, Tubenchirurgie). Wenn Adhäsionen postoperativ auftreten, können als Folge chronische Schmerzzustände, Fertilitätsstörungen und mechanisch bedingte Darmverschlüsse auftreten. Adhäsionen und die sich hieraus ergebende Symptomatik bzw. Folgeoperationen stellen einen erheblichen Kostenfaktor für das Gesundheitswesen dar. Der Laser ist als Blutstillungsverfahren nicht sehr weit verbreitet, da das Verfahren apparativ sehr aufwändig und teuer ist. - – ein Blutstillungsinstrument mit mehreren Blutstillungskomponenten, wobei wenigstens eine thermische Blutstillungskomponente und eine biochemische Blutstillungskomponente vorgesehen ist,
- – wobei eine optimierte Einstellung und/oder Kombination der einzelnen Blutstillungskomponenten jeweils in Abhängigkeit von den durch den Biosensor erfassten Bluteigenschaften des Patienten erfolgt.

Die biochemische Blutstillungskomponente ist hingegen vorzugsweise zur Erzeugung einer stillen elektrischen Entladung mittels wenigstens einer Entladungselektrode und einer Isoliereinrichtung ausgebildet, wobei die Isoliereinrichtung zwischen der Entladungselektrode und dem zu behandelnden Gewebe angeordnet ist und die Entladungselektrode mit einem HF-Strom gespeist wird. Darüber hinaus ist vorzugsweise eine weitere Blutstillungskomponente in Form einer Zufuhreinrichtung zum Zuführen von die Blutgerinnung beeinflussenden Stoffen an das zu behandelnde Gewebe vorgesehen. Eine weitere Blutstillungskomponente ist vorzugsweise in Form einer Zufuhreinrichtung zum Zuführen von Edelgas, insbesondere Argon, an das zu behandelnde Gewebe vorgesehen. Darüber hinaus kann noch eine weitere Blutstillungskomponente in Form einer Erwärmungseinrichtung zu endogenen oder exogenen Erwärmung des Blutes während einer stillen elektrischen Entladung auf eine Temperatur unterhalb der Koagulationstemperatur von biologischem Gewebe vorgesehen sein. Darüber hinaus kann die Chirurgieeinrichtung einen Gewebesensor aufweisen, der entstehende Gewebeeffekte während des Einsatzes ein oder mehrere Blutstillungskomponenten umfasst und der vorzugsweise im chirurgischen Instrument angeordnet ist.
