Abstract
Verfahren zur Detektion von HIV-1 und HIV-2 in aus Blut abgeleiteten Proben, umfassend a) zur Verfügung stellen von aus Blut abgeleiteten Proben, b) Konzentrieren der aus Blut abgeleiteten Proben, c) Extraktion der viralen Nukleinsäuren unter der Verwendung eines Lyse-Puffers, der eine interne Kontroll-Nukleinsäure umfasst, und unter der Verwendung von erhitztem Nuklease-freiem Wasser zur Elution, d) Erhalten der viralen Nukleinsäure-Extrakte aus Schritt c), e) Herstellen eines PCR-Mastermixes, umfassend für den Nachweis von HIV-1 einen Satz Oligonukleotide, der spezifisch für HIV-1-Nukleinsäuren ist und mindestens zwei Paare aus jeweils einem Sense-Primer und einem Antisense-Primer und mindestens zwei Paare aus jeweils einer ersten Sonde und einer zweiten Sonde umfasst, wobei die Nukleotidsequenzen der Primer und Sonden von der konservierten 5'-LTR-Region und gag-Region der HIV-1-Genotypen abgeleitet werden, wobei aus folgenden Paaren aus jeweils einem Sense-Primer und einem Antisense-Primer ausgewählt wird: Nukleinsäuren mit der Sequenz von SEQ ID NOs. 1 und 2, Nukleinsäuren mit der Sequenz von SEQ ID NOs. 3 und 4, Nukleinsäuren mit der Sequenz von SEQ ID NOs. 5 und 7, Nukleinsäuren mit der Sequenz von SEQ ID NOs. 6 und 7; Bezüglich der analytischen Spezifität wurden Untersuchungen mit potentiell kreuzreaktiven Erregern (Poliovirus Typ 1, Typ 2, Typ 3, Echovirus Typ 6, Typ 7, Typ 9, Typ 11, Typ 30, Coxsackievirus A9, B1, B2, B3, B4, B5, CMV, Humanes Herpes Virus A1, Humanes Herpes Virus A2, Adenovirus, Staphylococcus aureus, Escherichia Coli, Staphylococcus epidermidis, Pseudomonas aeruginosa, Klebsiella oxytoca, Bacillus cereus, Hepatitis B Genotyp A, B, C, D, E, F; Hepatitis C Genotyp 1A, 1B, 2A, 2B, 2C, 2I, 3, 4, 5) durchgeführt. Dabei zeigte sich keine Kreuzreaktivität mit potentiell kreuzreaktiven Erregern, sondern eine HIV-1 spezifische Amplifkation (Tabelle 1) bzw. eine HIV-2 spezifische Amplifkation (Tabelle 2). Bezüglich HIV-1 wurde eine Untersuchung an Genotypen Panels mit den Subtypen der Gruppe M, A, B, C, D, E, F, G und H sowie der Gruppe O durchgeführt. – Diagnostische Sensitivität von HIV-1 bzw. HIV-2 im Multiplex Ansatz- Diagnostic sensitivity of HIV-1 or HIV-2 in the multiplex approach
Zur Analyse der diagnostischen Sensitivität wurden jeweils 100 Positivkontrollen mit einer Konzentration des Dreifachen der 95% Nachweisgrenze (1800 IU/ml HIV-1; 2400 Kopien/ml HIV-2) analysiert. Dabei zeigte sich eine diagnostische Sensitivität größer 99,9%.

Zur Untersuchung der diagnostischen Spezifität wurden jeweils 500 negative Poolproben analysiert. Untersucht wurden jeweils 500 Pool-Proben im Zeitraum von Januar bis März 2008. Es zeigte sich eine diagnostische Spezifität größer 99,8%.

Die Robustheit wurde alternierend an 8 Reihen von HIV-1 positiven Proben (106 IU/ml) und negativen Proben analysiert, ein identisches Verfahren wurde anschließend für HIV-2 Proben durchgeführt. Es zeigte sich ein robustes Verfahren. Eine Kontamination konnte ausgeschlossen werden.Robustness was analyzed alternately on 8 rows of HIV-1 positive samples (10 6 IU / ml) and negative samples, and an identical procedure was subsequently performed for HIV-2 samples.
