Description
The The present invention relates to angiotensin receptor antagonists Stents and methods of making these stents.
For one, it is characterized by Deposits on the inside of the arteries, which narrow the vessel clearing and lead to reduced blood flow. In extreme cases, such Deposits over time completely occlude the vessel. Such constrictions can be diagnosed by angiographic methods. In terminology this is called arteriosclerotic plaque formation or arteriosclerosis designated.
To the others, there are plaques that do not significantly clog an artery, as it is usually recognized with angiography. But in the plaques occurs an inflammatory Process with formation of a large lipid core, a thin cap and an increasing content of macrophages, foam cells and others Cells. In terms of the pathogenic potential that a sudden Tear causes myocardial infarction due to immediate lumping, These arteriosclerosis herds are called Vulnerable plaques.
For this a balloon catheter in the constricted or introduced locked artery, this is then widened by expansion of the balloon and blood flow thus restored. Here is the acute, right after the PTCA occurring (acute restenosis) or the later, subacute (recurrent stenosis, restenosis) Reclosure of the blood vessel a problem that occurs in about 30% of cases. The same applies for the percutaneous transluminal angioplasty (PTA) for the treatment of peripheral vascular diseases, where peripheral vessels such as the carotid, iliac (ilical), renal (renal), cerebral (cerebral), subclavian and femopopliteal vessels become.
The Risk of acute restenosis may be due to administration of antiplatelet agents be reduced. Furthermore can by mechanical support the coronary wall with a usual cylindrical and expandable braid (stent) done with the help of a Balloon catheter inserted into the narrowed vessel and at the site of the stenosis unfolded to open the narrowed area. Even if through this Method slightly lowered the restenosis risk (a few percentage points) can be, so there is currently no convincing therapy for the recurrent stenosis and late thrombotic operations to disposal.
Einen bedeutenden Beitrag zu den auftretenden Problemen nach der Stentimplatantion liefert der gefäßbiologische Mechanismus der verstärkten Proliferation vaskulärer Glattmuskelzellen (VSMC, vascular smooth muscle cells), welcher zu einem Wiederverschluss der Arterie durch Muskelgewebe führt und die Endothelisierung des Stützgeflechtes des Stents inhibiert, wodurch dieser gar nicht oder nur verzögert in die Gefäßwand integriert wird.a Significant contribution to the problems after stent implantation supplies the vascular biological Mechanism of the reinforced Proliferation of vascular Smooth muscle cells (VSMC, vascular smooth muscle cells) leads to a reocclusion of the artery through muscle tissue and the endothelialization of the supporting braid of the stent inhibits, causing this not at all or only delayed in integrated the vessel wall becomes.
Therefore is in addition to the combination of stent application and systemic Administration of platelet aggregation inhibitors as well as antihypertensives that of cell growth inhibitors or indirectly acting as such Substances such as coagulation factor Xa inhibitors have an established Therapy.
A newer possibility For the treatment of restenosis is the local administration of drugs by means of a stent which releases the active substance. The combination of drug and stent allows a medicinal Treatment and mechanical stabilization in one application.
For this can Stents are coated with active substance-containing paint materials. The drug release takes place by diffusion from the paint or by degradation of the paint when using biodegradable paint systems.
B. EP-A 0 950 386). A other possibility already described is the generation of small cavities or micropores on the Stent surface, be introduced into the active compounds themselves or polymers containing them (see, for example, EP-A 0 950 386). A drug-free layer can still about that be applied. The release takes place by diffusion or Degradation or by a combination of both processes.
Furthermore can active ingredient-containing stents by melt embedding of the active ingredient in a polymeric carrier z. B. using injection molding. The Release of the drug takes place in these stents usually by Diffusion.
So allows the connection of stents with antiproliferative agents is high local concentration of active ingredient, without it being the unwanted systemic side effects comes.
Two stents with the active ingredients paclitaxel and sirolimus are currently on the market, other stents with other drugs are still being tested in clinical trials. (Halkin A, Stone GW, Interv Cardiol 2004; 17 (5): 271-82; McKeage K, Mur but D, Goa KL, Am J Cardiovasc Drugs 2003; 3 (3): 211-30; Dogrgrell SA, Expert Opin Pharmacother 2004; 5 (11): 2209-20) In the paclitaxel-coated stent, about ten percent of the drug is released within the first ten days, then the release rate steadily decreases. After 30 days no more release takes place. This prevents the cells from uncontrollably dividing the vessel wall and closing the vessel again. On the other hand, the normal healing process of the vessel is not disturbed. The equipment of a stent with blood coagulation factor Xa inhibitors is described in WO 03/035133 but has not been developed further to the point of application.
Wagenaar investigated the importance of the angiotensin receptor in the cardiovascular system. (Wagenaar LJ, Dissertation, Groningen 2003, ISBN 90-75092-37-7) In an animal model, the angiotensin II type 1 (AT 1 ) receptor is involved in restenosis following angioplasty. In carotid rats, vascular injury through a balloon catheter causes proliferation of VSMCs, which is AT 1- dependent. (Daemen MJ, Lombardi DM, Bosman FT, et al., Circ Res 1991; 68: 450-456) The renin-angiotensin (RAS) system is also involved in other processes responsible for restenosis following balloon angioplasty via the angiotensin receptor.
(Peters S, Gotting B, Trummel M, et al., J Invasive Cardiol 2001;13;93–97)Restenosis following stents in human coronary arteries differ histologically from restenosis after balloon angioplasty in that in-stent restenotic lesions contain predominantly vascular smooth muscle cells and show greater proliferation of these cells than lesions following balloon angioplasty alone. (Moreno PR, Palacios IF, Leon MN, et al., Am J Cardiol 1999; 84; 462-466). The stimulation of AT 1 leads in the animal model to the proliferation and migration of VSMCs and thus to the formation of a neointima in the stent. (Hafizi S, Chester AH, Allen SP, et al., Coron Artery Dis 1998; 9; 167-175). In human coronary artery samples obtained by atherectomy, smooth muscle cells from in-stent restenotic lesions have been found to contain abundant AT 1 receptors. (Wagenaar LJ, van Boven AJ, van der Wal AC, et al., Neth Heart J 2001; 9; suppl 3: 3) When applied to humans, the AT 1 receptor blocker (ARB) valsartan appears to be the in-stent To reduce restenosis rate. (Peters S, Gotting B, Trummel M, et al., J. Invasive Cardiol 2001; 13; 93-97)
An De-novo-Läsionen nach PTCA und Stentlegung werden Angiotensin-Konversions-Enzym(ACE) und AT1-Rezeptoren in Makrophagen gefunden. Weniger stark wurden auch AT1-positive Glattmuskelzellen beobachtet. In-Stent-Läsionen bestanden hauptsächlich aus AT1 exprimierenden VSMCs. (Wagenaar LJ, van Boven AJ, Van der Wal AC, Cardiovascular Research 2003; 59; 980–987)At de novo lesions following PTCA and stenting, angiotensin conversion enzyme (ACE) and AT 1 receptors are found in macrophages. AT 1 positive smooth muscle cells were also less strongly observed. In-stent lesions consisted mainly of AT 1 expressing VSMC.
Therefore, as medicaments for patients after transluminal (coronary) angioplasty, those with the mechanism of action as angiotensin II type 1 receptor blockers or AT 1 receptor antagonists could be particularly suitable. However, in the event that blood pressure lowering is undesirable, systemic administration of such ARBs to prevent or treat restenosis after PTCA or PTA with or without stent placement should be less suitable.
Surprised has now been found to be a treatment for restenosis with such ARBs through local application through stents that slow these agents excellent is possible, even and especially in the case of stenosis patients the risk of hypotension consists.
While with the previously available active substances and stents in all cases sufficient therapeutic success can be achieved, allows the new combination of ARBs with a stent for a more effective treatment and / or prophylaxis of restenosis and / or vulnerable plaques. By local application of these compounds in combination With a stent, it is possible to achieve the dose required to prevent restenosis to lower the drug. Thus, unwanted systemic effects be minimized. At the same time, the local concentration can be increased and thus increasing the effectiveness become.

Polyurethane eingesetzt:
BIG 1: Thermoplastisches Polyurethan
aus Methylen-bis(4-isocyanatocyclohexan) (z. B. Desmodur® W von
Bayer MaterialScience AG, Leverkusen) und Poly(hexamethylencarbonatdiol)
MW=2000 sowie 1,4-Butandiol als Kettenverlängerer
BIG 2: Thermoplastisches
Polyurethan aus Methylen-bis(4-isocyanatocyclohexan) (z. B. Desmodur
W von Bayer MaterialScience AG, Leverkusen) und Poly(hexamethylencarbonatdiol)
MW=1000 sowie 1,4-Butandiol als KettenverlängererThe following examples are intended to illustrate the invention without, however, limiting it. In these examples, the following abbreviations are used for polyurethanes used:
BIG 1: (. For example, Desmodur ® W from Bayer MaterialScience AG, Leverkusen) Thermoplastic polyurethane from methylene-bis (4-isocyanatocyclohexane) and poly (hexamethylencarbonatdiol) MW = 2000 and 1,4-butanediol as a chain extender
BIG 2: Thermoplastic polyurethane from methylene-bis (4-isocyanatocyclohexane) (eg Desmodur W from Bayer MaterialScience AG, Leverkusen) and poly (hexamethylene carbonate diol) MW = 1000 and 1,4-butanediol as chain extender
Beispiel 1:Example 1:
