Abstract
Description
The invention relates to transdermal therapeutic systems, which contains the active ingredient moxonidine (4-chloro-N- (4,5-dihydro-1H- imidazol-2-yl) -6-methoxy-2-methyl-5-pyrimidinamine) or an containing alpha-receptor agonists with imidazole structure ten and which for the transdermal administration of this effect substances are suitable for the treatment of high blood pressure.
B. Clonidin. The active ingredient moxonidine is used therapeutically for treatment hypertension (high blood pressure). It is about moxonidine is a centrally acting alpha receptor Agonists. For the administration of moxonidine are oral Dosage forms, e.g. B. film-coated tablets, in the market leads. The invention also relates to other substances zen with effect on the central alpha-2 receptor, the one Contain imidazole structure, e.g. B. clonidine.
Zudem wird nach oraler Verabreichung ein be deutender Anteil des Wirkstoffs bereits während der ersten Leber-Passage metabolisiert ("first pass"-Effekt).It is useful to treat hypertension and he wishes one during the entire application period Ensure lowering of blood pressure. Therefore the Blood plasma level of the active ingredient on a con constant level. Oral administration Moxonidine can cause undesirable peak plasma levels (Peak values) occur, which are common causes of side effects are kungen. In addition, a be significant proportion of the active ingredient during the first Liver passage metabolized ("first pass" effect).
Transdermal therapeutic systems (TTS) enable this Administration of active pharmaceutical ingredients through the skin. This type of administration is opposite to the oral appli cation is beneficial in many ways. For one thing in this way the above-mentioned "first pass" effect on the other hand, these systems enable a continuous animal and constant supply, so that the blood plasma level of the active ingredient at a largely constant level will hold. This can be used for therapeutic purposes adequate patient care with the respective Ensure active ingredient during the entire application period be steady. At the same time, the occurrence of Plas peak values avoided and so the secondary impact risk reduced.
Because these benefits also apply to drug treatment hypertension are of great importance, it is desirable worth providing transdermal therapeutic systems, which for the transdermal administration of moxonidine for Hypertension therapy are suitable.
Transdermal therapeutic systems in general a structure from a drug-impermeable back layer, an associated pressure-sensitive adhesive active ingredient servoir and protection that can be removed before application layer on. The protective layer is removed for application and the TTS with the pressure-sensitive side of the knit fabric reservoirs attached to the patient's skin. Wuh The active ingredient is removed from the Re into the patient's skin.
US 4,882,163 are already transdermal Systems for the application of moxonidine have been proposed. The systems described there are self-adhesive TTS based on optionally networked Polyacrylates or based on polyisobutylenes Combination with synthetic or partially synthetic Resins. Formulations based on Po lyvinyl alcohol, polyurethane and polysiloxane compositions thinks. As a permeation enhancer, which increases the absorption of the Promote active ingredient through the skin, have long-chain Alkane carboxylic acids with at least 12 carbon atoms mentioned.
However, it turned out that the TTS after the above-mentioned prior art available set feed rates (flux rates) too low and therefore unsatisfied are enough. To achieve a therapeutically effective plasma concentration build up and maintain at the required level ten, it is necessary that the highest possible flux rate is achieved.
It was therefore the object of the present invention to provide transdermal therapeutic systems which the transdermal administration of the active ingredient moxonidine with enable high flux rates, so that the applicability at the therapeutic treatment of hypertension improved becomes.
Surprisingly, this object is achieved according to the invention in that with moxonidine-containing TTS with the features of the preamble of claim 1, either (A) the active substance reservoir as base polymers contains such polymers which are selected from the group of hydrophilic poly mers, the polyvinylpyrrolidones, polyvinyl alcohols , Hydroxy propylmethyl cellulose and other cellulose derivatives and polyethylene glycols, or that (B) in the above-mentioned TTS the drug reservoir contains a mixture of at least one longer-chain n-alkane carboxylic acid and at least one longer-chain unsaturated carboxylic acid, where "longer-chain" means, that the number of carbon atoms is at least 12. The two measures mentioned (A) and (B) each cause an increase in the active substance flux; however, they can also be used in combination. As can be seen from FIG. 1 and FIG. 2, the TTS according to the invention have significantly improved flux rates compared to the comparative examples produced according to known recipes.
In particular, it has been found that the invention Formulations of TTS which contain hydrophilic polymers e.g. B. Polyvinylpyrrolidone (PVP) or hydroxypropylmethyl cellulo se, contained as base polymers of the active substance matrix, Deut much higher flux rates compared to those from the prior art Recipes known in the art.
In the TTS according to the invention, an active ingredient can also be a other active substance from the group of substances with effect at the central alpha-2 receptor, which has an imidazole structure included, used; especially comes here the active ingredient clonidine (2- (2,6-dichloroanilino) -4,5-dihydro imidazole).
The drug reservoir that contains the drug moxonidine in contains dissolved or dispersed form is essentially Chen built on the basis of polymers, which a Form active substance matrix; these polymers are also called Base polymers called. The proportion of base polymers carries at least 40% by weight, preferably at least 55% by weight, based on the entire drug reservoir. The Drug reservoir has adhesive properties, wel che by the base polymers used or by additional added tackifying substances are necessary. Furthermore, it is also possible to place the drug reservoir on the To provide the skin side with a pressure-sensitive adhesive layer. The active substance reservoir of the TTS according to the invention is in the simplest embodiment, a single-layer polymer ma trix. In deviation from this, the TTS can be layered Structure of the drug reservoir with at least two polymer Have matrix layers.
The structure of the TTS according to the invention comprises in addition to that above discussed drug reservoir an drug-impermeable back layer as well as an active ingredient-impermeable removable protective film.
A variety of materials are suitable for the backing layer of skin-compatible plastic films, such as B. slides Polyvinyl chloride, ethylene vinyl acetate, vinyl acetate, poly ethylene, polypropylene or cellulose derivatives. Especially Suitable materials for the backing layer are polyester, which are characterized by particular strength. Further In individual cases it may be useful to use foil material existing backing with an additional overlay provided, e.g. B. by vapor deposition with metals or others diffusion-blocking additives such as silicon dioxide, aluminum minium oxide or similar substances known to the person skilled in the art.
For the removable protective layer, the same materials are used as for the back layer, provided that this layer is a suitable one Surface treatment, e.g. B. fluorosiliconization, under is pulled so that it from the pressure sensitive adhesive covered by it layer is removable and removed before application of the TTS can be. Can also be used as removable protective layers other materials can also be used, such as. B. Polyte trafluoroethylene treated paper, cellophane, polyvinyl chloride or the like.
According to the invention, regardless of the use of hydrophilic base polymers for the active substance matrix, the flux rate can also be increased in that the active substance-containing reservoir contains a mixture or combination of at least one longer-chain n-alkane carboxylic acid and at least one longer-chain unsaturated carboxylic acid ( see Fig. 2).

With this recipe, lauric acid is first combined in one suitable solvent, e.g. B. ethanol, dissolved or sus oscillates. Hydroxypro is then added pylmethyl cellulose and moxonidine. With a suitable Lö means, e.g. B. ethanol, a solids content of 28 wt .-% adjusted (also solids contents in the range of 10-50% by weight are possible).
The formulation is spread with a basis weight of approx. 100 g / m 2 (or 50-300 g / m 2 ) on a siliconized PET film or another suitable film. To remove the organic solvents, the film is dried at room temperature and then in the drying cabinet.

The flux of active substance was determined using the TTS prepared according to the comparison formulation or according to example formulations I and II. The flux values were obtained by experiments on Franz cells using human skin. Human full skin (breast) was used for this. The application area was 1.148 cm 2 . Phosphate buffer solution with a pH of 5.5 served as the acceptor medium. The investigation was carried out at a temperature of 32 ° C.
The results of these active substance flux measurements using the various formulations mentioned above are shown in FIGS . 1 and 2.
Regarding the results, it should be noted that the of full-fledged human skin are only partially comparable with test results that with skin samples from other animal organisms the. In general, skins of different origins (e.g. Guinea pigs, cow udder) achieved higher flux values than when using full human skin.
Fig. 1 shows a graphical representation of the flux rates which he was aiming with the various TTS formulations mentioned above, namely with TTS according to the example formulations I and II or with TTS according to the comparison formulation.
It can be clearly seen that the TTS according to the invention according to the example formulations I or II via a free set period of at least 72 hours compared to the same example have increased flux rates.
Fig. 2 shows a graphic representation of drug flux rates, which were obtained with the experimental arrangement described above using TTS according to the invention, the recipes for the drug reservoir contain a combination of oleic acid and lauric acid. Three different formulations were tested, which differ in the quantitative ratio of oleic acid to lauric acid. Particularly high flux rates were achieved with TTS according to the invention in which the quantitative ratio of oleic acid: lauric acid was 1.5: 1.
In addition, the flux rates shown in FIG. 2 are significantly higher than the flux rates achievable with the comparison formulation ( FIG. 1).
Bei der Herstellung von moxonidinhaltigen TTS mit eingeleg tem Vlies wird wie folgt vorgegangen:Inlaid with the production of TTS containing moxonidine The fleece is operated as follows:
The glue line is applied in two steps. A Part of the adhesive is applied to a release liner (as a carrier foil) and coated with siliconized paper covers.
Then a fleece of the required size is con die-cut and placed on the adhesive line, from which before the siliconized paper was removed. The active ingredient-containing enhancer solution, which also in this Trap a mix of saturated and unsaturated car contains bonic acids (oleic acid, lauric acid) is by means of Pi dosed or dripped onto the fleece disc and evenly distributed.
The second part of the adhesive is siliconized Paper and covered with a thin cover film (e.g. PET 19 µm) covered. After removing the paper, this will adhesive-coated cover film of the previously manufactured Backing film (with fleece on top) covered.
