Abstract

The device for transoral intralaryngeal and pharyngeal injections comprises a dose adjustment unit, an injection needle cover and protection element, an injection depth adjuster, an element for holding the injection needle in the intermediate tube, and a unit for performing the injection.

Description

Bisherige Applikationstechniken wurden auch, z. B. bei der Behandlung der Dystonien, mit einer ungeschützten Injektionsnadel in Lokalanästhesie durchgeführt. Bei Würgreflexen kam es dabei zu Verletzungen im laryngealen und pharyngealen Bereich. Da die Nadelinjektionstiefe bei voller Nadellänge (2 cm) nicht immer sicher zu kontrollieren ist, wurde z. B. die Stimmlippe häufig durchstochen und es mußte nochmals ein Injektionsversuch erfolgen. Da die die Spritze im Applikationsinstrument nicht integriert war, der Therapeut in der einen Hand das Endoskop und in der anderen den Applikator hielt, mußte der Spritzen-Stempelvorschub auf Aufforderung des Therapeuten durch eine zweite Person erfolgen. Die haue jedoch in der Regel keinen Sichtkontakt zum Applikationsort. Der hier vorgestellte Injektionsapplikator Applikator ist sowohl mit einem System für die transorale ambulante Applikation in Lokalanästhesie wie auch im Rahmen für die Anwendung bei einer Mikrolaryngoskopie konstruiert worden. Er ermöglicht eine selbständige Applikation ohne weitere Assistenz durch Integration der Spritze (1) in die Handgriff-Einheit (siehe Fig. 1) sowie eine Dosierungseinheit (siehe Fig. 3) ohne das der Therapeut einen Blickkontakt zur Spritzenskala haben muß. Letzteres wurde durch eine Vorschubeinheit mit Kugelfederungsrasterung mit Steifigkeits-Regelung (2), einer Stabilisierungs­ stange (3) und einer Einsenkung (4) des Spritzenstempels ((13) Fig. 1.) in die Vorschubeinheit erreicht (siehe Fig. 3.). Die Dosierungseinheit erlaubt mehere Applikationen ohne das Gerät aus dem Larynx zu entfernen. An die Spritze ist ein Injektionsschlauch (5) (siehe Fig. 1.) mit Nadel angesteckt. Diese wird nun durch das an die Handgriffseinheit (Fig. 7.) angeschraubte Zwischenrohr ((6) Fig. 1.) durchgeführt. Der Außendurchmesser des Zwischenrohres beträgt 4 mm. Der Innendurchmesser 2,5 mm. Um die starre Nadel um die Rohrbiegung herumzulenken wurde an der Oberseite der Biegung ein längliche Öffnung (7) gelegt (siehe Fig. 8 u. 9). Anschließend wird die Nadel mit der Kunststoff­ fassung am Ende des Rohres nochmal durch ein Loch herausgeführt und letztere an dem Endspalt (8) eingeklemmt. Jetzt kann auch noch die Injektions­ einheit (siehe Fig. 15.) angeschraubt werden. Dadurch wird auch die Injektionsnadel ((9) Fig. 16) eingeschlossen, was eine Gewebetraumatisierung bei transoraler Einführung verhindert. The dosing unit allows multiple applications without removing the device from the larynx. An injection hose ( 5 ) (see Fig. 1) with a needle is attached to the syringe. This is now carried out through the intermediate tube (( 6 ) Fig. 1.) screwed onto the handle unit ( Fig. 7.). The outer diameter of the intermediate pipe is 4 mm. The inner diameter 2.5 mm. In order to steer the rigid needle around the pipe bend, an elongated opening ( 7 ) was made at the top of the bend (see FIGS. 8 and 9). Then the needle with the plastic socket at the end of the tube is led out again through a hole and the latter is clamped at the end gap ( 8 ). Now the injection unit (see Fig. 15.) can also be screwed on. This also includes the injection needle (( 9 ) Fig. 16), which prevents tissue trauma during transoral insertion.

Je nachdem wie weit die Injektionseinheit an das Zwischenrohr ((6) Fig. 9.) angeschraubt wird, ist eine unterschiedliche Injektionstiefe zu erreichen (z. B. wird dadurch eine Durchstechung der Stimmlippen verhindert). Die Injektions­ hülse (10) wird durch einen geringen Druck auf den Applikationsort nach hinten verlagert, während die Nadelspitze in das Gewebe einsticht. Wird die Nadel wieder herausgezogen, so wird durch die Feder (11) in der Injektionseinheit die Hülse wieder über die Nadel geschoben. Der Gewebeschutz bleibt somit bestehen. Läßt der Patient eine ambulante Behandlung in Lokalanästhesie wegen eines starken Würgreizes nicht zu, muß für die Mikrolaryngoskopie ein anderes Zwischenrohr (siehe Fig. 12) eingesetzt werden. Wegen der starren Injektionsnadel sind auch hier an den Rohrbiegungen länglich ovale Löcher sowie das gleiche Schlaucheinklemmverfahren am Ende des Rohres wie in Fig. 8. u. 9. angebracht. Das Applikationssystem kann nicht nur für Botulinum-Toxin sondern auch für andere Substanzen, wie z. B. zur Stimmlippenaugmentation verwandt werden. Ebenso ist hiermit auch eine gleichzeitige EMG Ableitung mit einem EMG Injektionsschlauch möglich.Depending on how far the injection unit is screwed onto the intermediate tube (( 6 ) Fig. 9.), a different injection depth can be achieved (e.g. this prevents the vocal cords from being pierced).
