Abstract
Die Erfindung beschreibt ein Verfahren zur Bindung und Entfernung von Anti-DNA-Autoantikoerpern aus Koerperfluessigkeiten, so z. B. Serum, Plasma oder Blut, das auf der biospezifischen Bindung an DNA beruht. Hierzu wurde DNA unter Einsatz von biokompatiblen Brueckenverbindungen an geeignete Traeger gebunden. Das Verfahren zeichnet sich durch seine hohe Praktikabilitaet, Biokompatibilitaet und Effektivitaet aus, wobei die so hergestellten Traeger regenerierbar und autoklavierbar sind und somit einen wiederholten und reproduzierbaren Einsatz derselben gestatten. Das Verfahren findet Anwendung in der pharmazeutischen Industrie und in der Medizin.The invention describes a method for binding and removal of anti-DNA autoantibodies from body fluids, such. Die Erfindung betrifft ein Verfahren für die spezifische Bindung und Entfernung von Anti-DNA-Autoantikörpern (DNA-AK) aus Körperflüssigkeiten, das sowohl in vitro als auch in vivo genutzt werden kann, unter wiederholtem und reproduzierbarem Einsatz eines beschichteten Trägers. Autoantikörper gegen verschiedene nukleare Bestandteile sind für Autoimmunerkrankungen des rheumatischen Formenkreises charakteristisch. Autoaniikörpern gegen DNA, insbesondere gogen native DNA, kommt eine große pathogenetische und diagnostische Bedeutung für den SLE zu. Deshalb erscheint es sinnvoll, den Pathomechanismus dieser Erkrankung durch Entfernung der Anti-DNA-Autoantikörper aus den Körperflüssinkeiten zu beeinflussen, um dadurch das Fortschreiten der oben erwähnten Erkrankung zu verhindern, die Krankheitsaktivität zu mindern und die damit verbundenen Symptome zu lindern bzw. zu beseitigen. Die Erfolge der Plasmapheresebehandlung beim SLE werden u. a. auf die Beseitigung bzw. Reduzierung dieser pathogenetisch bedeutsamen DNA-AK zurückgeführt (Apostoloff et al., Dt. Gesundheitsw. 34,64-67 (1979); Z.Sharon et al., Plasma Ther Transfus Technol. 3, 163-169 (1982); J.V.Jones et al., Arthritis Rheum. 24,1113-1120 (1981); J.V.Jones et al., Clinics in Immunol. Allergy S, 13-32 [1986]; E.Schmitt und H. Kiinkmann, z. Klin. Med. 40,21-25 (19851). Nachteile der Plasmapherese sind das nichtselektive und unspezifische Entfernen aller Plasmabestandteile und die dadurch erforderliche Zuführung größerer Mengen Fremdplasma oder Humanalbumin, die auch aus Kapazitäts- und ökonomischen Gründen limitierend sein können. Aus den genannten Gründen existieren seit einigen Jahren Bemühungen um die Entwicklung von Adsorbenzien, die eine selektive und/oder spezifische Entfernung von Autoentikörpern mittels extrakorporaler Immunadsorbtion aus Körperflüssigkeiten erlauben (siehe Übersicht E.Behm und H. Klinkmann, Z. Klin. Med. 41,1501-1610 (1986)). Hierbei wurden im wesentlichen folgende Wege beschritten:Anemia, leukopenia, thrombocytopenia, arthritis) (review F.Hiepe, B-Dissertation, Humboldt-Universität zu Berlin, 1S87). Außerdem besieht die Gefahr des Leakages der DNA und somit der Bildung von neuen pathogenen DNA-AntUDNA-lmmunkomplexen. Erfindungsgemäß wird die Aufgabe dadurch gelöst, daß DNA — als natürliches Antigen — an Polyhydroxymethacrylate (Sepiiron*) oder an zellulosehaltige Träger über Silanhaftmiicel kovalent gebunden wnü und diese trägergebundene DNA zur Bindung und Entfernung von zirkulierenden Anti-DNA-Aui oantikörpern aus Körperflüssigkeiten eingesetzt wird, wobei die an dio DNA gebundenen DNA-AK aus den Körperflüssigkeiten durch Abtrennung der festen Phase sbpariert werden und din feste Phase durch Behandlung mit nichtdenaturierenden Medien funktionell regeneriert wird. Das erfinciungsgemäße Verfahren ist dadurch gekennzeichnet, daß die im menschlichen Organismus vorkommende und für dio Bildung von pathogenetisch bedeutsamen Immunkomplexen mitverantwortliche DNA als physiologisches Antigen der DNA-AK an zelluiosehaliige Träger oder Separor a* mit hinreichend großer Oberfläche und Porosität auf an sich bekannte vorherige chemische Oberfläctanmodifizierunp des Trägers mit Silanhaftmitteln und gegebenenfalls hetero- oder homobifunktionellen Reagenzien gebunden wird und diese trägergebundene DNA für di J i.~. vivo- und in vitro-Entfernung der zirkulierenden DNA-AK aus Körperflüssigkeiten oingesotrt wird und danach die so gebundenen DNA-AK durch nichtdenaturierende Medien wieder abgespalten werden, so daß die mit DNA beladene Matrix nach Sterilisation erneut eingesetzt werden kann. Der Trager hat eine unterschiedliche geometrische Form, wobei kugel- oder faserförmige Träger mit oder ohne Zusätze, die auch durch Packung, Sinterung: 1er Verwendung eines Bindemittels zu übergeordneten Strukturen (wie z. B. zu Filtern, Fritten, Kolonnen und dergleichen) mit definierter Durchlässigkeit angeordnet sein können, besonders geeignet sind. Die nichtdenaturierenden Medien für die Abspaltung der gebundenen DNA-AK setzen sich aus gepufferten Lösungen hoher lonenstärko zusammen, wobei insbesondere 3mol/l KSCN-Lösungen geeignet sind. The matrix thus loaded with DNA is then ligated for binding and removal dt; used in the usual way (hemoperfusion, plasma perfusion), also in the batch process (in the isolation of DNA-AK for in vitro studies) can be used. Im folgenden wird die Elimination von DNA-AK an einem Beispiel erläutert. In the following the elimination of DNA-AK will be explained by an example.
Separon* der Korngröße 500-1000pm wurde mit eiern Silanhaftmittel NB1114 (5VoI.-% in 50% Ethanol reinst) 6 St nden bei 6O0C versetzt. Nach anschließendem zweimaligen Waschen mit 50% Ethanollösung und fünfmaligem Waschen mi 0,1 M Phosphatpuffer PBS pH 7,2 wurde zum Separon* bis zum Bedecken 5% Glutaraldehydlösung zugegeben und 2 Stunden bei 37 0C inkubiert. Nach fünfmaligem Waschen mit PBS pH 7,2 war das Separon* auf die DNA-Bindung vorbereitet. An den so aktivierten Träger wurde Kalbsthymus-DNA nach 24stündigem Löschen in isotoner NaCI-Lösung, Ultraschallbehandlung (4 χ 20sec) und Hitzedenatuiierung (15min bei 1000C, anschließend Abschrecken in Eiswasser) in einer Konzentration von 1,6mg/ml im Verhältnis 1 Volumen fieparon* zu 2 Volumen DNA-Lösung über Nacht bei Raumtemperatur gebunden. Anschließend wurde nach dreimaligem Waschen mit PBS pH 7,2 mit einer 1M Ethanolaminlösung geblockt, um noch eventuell vorhandene reaktive Gruppen am Träger abzusättigen. Der Umfang der DNA-Bindung an dac Separon® sowie die Stabilität der Bindung bei der weiteren Verarbeitung wurde mit P-3'2-markierter mitochondrialer DNA, welche der Kalbsthymus-DNA zugegeben wurde, bestimmt. Unter Verwendung von DNA-Lösungen unterschiedlicher Konzentrationen wurden die in Tab. 1 aufgeführten Worte erreicht (Tab. 1). Die o. g. Versuchsanordnung führt zu einer optimalen DNA-Beladung des Separons*. Purch eine weitere Steigerung der angebotenen DNA-Menge kann nur noch unwesentlich mehr DNA gekoppelt werden. Das nach Beispiel 1 hergestellte Adsorbens wurde bei 1240C autoklaviert. Dadurch wurde Sterilität (t = 30min) und Pyrogenf reiheit (t = 120 min) des Adsorbens erreicht. Nachdem durch Absaugen die restliche Waschlösung vom DNA-Separon* ontfernt wurde, wurde 1 Volumen Adsorbens mit 2 Volumen eines DNA-AK-reichen Serums (eines Patienten mit hochaktivem SLE) versetzt (Reaktion 2 Stunden bei Raumtemperatur unter leichtem Rühren). Das Serum wurde danach abgesaugt, das Trägermaterial 3x mit PBS pH 7,2 gewaschen, und mit 3 Volumen 3M KSCN (t = 15min) wurden unter leichtem Schütteln die gebundenen Antikörper abgetrennt. Nach 3x Waschen mit PBS pH 7,2 und erneutem Autoklavieren wurde der Prozeß der Antikörpergowinnung noch zweimal wiederholt. Die Aktivität der DNA-AK der Seren vor und nach der Behandlung wurde bestimmt. Ein typisches Beispiel zeigt Tab. 2.The adsorbent prepared according to Example 1 was autoclaved at 124 0 C. Die Clearance der verwendeten Seren lag sowohl bei der Erstanwandung als auch beim Zweit- und Drittelnsalz der Adsorbenxien deutlich über 60%. Ein signifikanter Verlust an DNA während der Anwendung und dem Autoklavieren sowie ein Wirkungsverlust durch Mehrfachautoklavieren oder durch Mehrfachvorwendung konnte nicht festgestellt werden.Biokompatibilität: Die Biokompatibilität wurde anhand des Komplementsystems für DNA-Separon* und DNA-freies Separon* bestimmt.
