Description

Anwendungsgebiet der ErfindungField of application of the invention

The invention relates to a wear-resistant acetabulum, which is anchored in one piece cementless and serves for use as an endoprosthesis.

Charakteristik der bekannten technischen LösungenCharacteristic of the known technical solutions

The acetabular cups used in the surgery of the hip joint consist of plastic, preferably polyethylene or metal with polyethylene substitute or all-ceramic. The polyethylene used for acetabular cups (DE-OS 3200340, CH-PS 48747) is relatively well biocompatible in its compactness. It turns out, however, that the abrasion caused by the action of the ball head, the body is difficult or not absorbed. This can cause inflammation that causes reoperations. Despite the applied anchoring profile on the acetabular cup made of polyethylene (CH-P 648747) only an insufficient long-term fixation succeeds.

Cheaper can be two-component acetabular cups, consisting of metal and polyethylene insert (DE-OS 231473) anchored in the form-fitting bone. However, the disadvantage of using polyethylene remains as previously described.

In AT-P 337885 an implantable artificial joint replacement made of ceramic is proposed, wherein head and socket are made of ceramic. Despite the good biocompatibility of the ceramic, the absorbability of the abrasion and the favorable sliding pairings, as with all previously described systems, the geometric disadvantages persist. These are in particular the large dimensions of the joint sockets in relation to the pelvic anatomy.

Often, additional surgeries such as pelvic osteotomies according to CHIARI or acetabulum sculptures according to HARRIS are necessary for secure anchoring. Dysplasia scars can not be treated with the previously offered solutions.

Alle bisher vorgestellten Lösungen berücksichtigen in ungenügendem Maße die relativ hohe mechanische Verschleißbeanspruchung an der Reibfläche Kugelkopf/Pfanne.All previously presented solutions take insufficient account of the relatively high mechanical wear stress on the friction surface ball head / pan.

Ziel der ErfindungObject of the invention

The aim of the invention is to provide a biocompatible universally a settable Hüftgeienkpfanne, which can be safely anchored by known surgical technique and guarantees a long Liegdauer.

Darlegung des Wesens der ErfindungExplanation of the essence of the invention

The object of the invention is to provide a biocompatible, highly wear-resistant, provided with good sliding properties, mechanically stable acetabulum, which forms a good bone contact. According to the invention the object is achieved in that the sliding surface of the existing metal and / or ceramic acetabulum with a dense protective layer and the bone facing the outside is provided with a diffusion-inhibiting bioactive layer.

In particular, barrier-layer-forming pure metals such as titanium, tantalum, niobium, zirconium and their alloys as well as all other metallic or ceramic implant materials can be used as layer-bearing metals.

The sliding layer of the acetabular cup preferably consists of titanium nitride with a layer thickness of up to 5 μm and an average roughness of less than 3 μm

It has been found that the dense wear protection layer, in particular titanium nitride, has a high adsorption capacity against proteinaceous liquids such as synovial fluid, blood and the like. and thus guarantees a firmly adhering stable liquid layer between the Reibpartnern and thus a solid state friction is greatly minimized.

Furthermore, the titanium nitride layer prevents diffusion of metal ions into the implant bed.

It was further found that the previously known materials of the ball head of the endoprosthesis can be used in particular of ceramic or metal as a friction partner.

The outside of the acetabulum is macrostructured with an average roughness of up to 300> m and has a diffusion-inhibiting bioactive layer preferably of titanium nitride with a thickness up to 5μΐη and / or an oxide layer in particular of titanium oxide with a thickness up to ΙΟμ, ιη.

The diffusion-inhibiting bioactive layer may also be a matrix island structure of titanium nitride with a maximum of 40% titanium oxide.

The invention will be explained in more detail below using an exemplary embodiment.

A precision-formed Hüftgeienpfpfanne of pure titanium with an inner diameter of 32.03 mm and a material thickness of 1.08 mm on the outside has a macrostructure of 250 / xm average roughness and on the lobed inside an average roughness of 1 μ, ιη.

On this body a titanium nitride layer with a thickness of 5μτη is applied on all sides. On the outside, the titanium nitride matrix is provided with titanium oxide in which cover 30% of the surface.

Tests with this acetabulum in a simulator resulted in an increase in lifespan of 300% compared to the uncoated acetabular cup.
