本发明总地涉及矫形外科领域,更具体地涉及采用功能性椎间假体进行椎骨再造的设备和方法。为了便于理解本发明的原理,下面将参照附图中所表示的实施例或例子进行本发明的具体说明。但是应当理解,本发明的范围并非仅限于此。可以对所述的实施例进行任何改变和进一步的变型,本发明领域的技术人员可以对本文所述的本发明的原理进行任何进一步的应用。
首先来看图1,标号10表示有一个或多个关节部位12的人体解剖图,这些关节部位可能由于受伤或疾病而受损。如图2所示,在一种典型的关节成形术过程中,可能除去关节12的全部或一部分,在两块完好的骨头14和16之间形成一个空隙。然后可将植入体18***骨头14和16之间,以至少部分地填充该空隙。

下面来看图4,在这一实施例中可以是椎间盘假体的关节假体20包括置于两个端板组件(endplate assembly)24和26之间的中间构件22。端板组件24可具有外表面28和内表面30。可以从内表面30延伸出一个凸起32之类的关节连接机构。在这一实施例中,这个凸起可以是半球形的,但是可以设置各种形状的凸起,其中的几种将在其他各实施例中描述。表面28和30可以是平的、有角度的或弯曲的。在这一实施例中,外表面28可以是相对平的,或者可以构形为与邻接的椎骨端板表面相匹配。内表面30可以是向着凸起32逐渐上升或下降的锥面。

中间构件22,可根据要采用这一植入体的具体关节或假体20用于矫正的具体畸形,在形状、尺寸、组成成份和物理性质上有某些变化。中间构件22的形状可以与端板组件24和26的内表面30和34的形状互补,以允许被更换的特定关节能够作适当范围的平移、弯曲、伸展、旋转和侧向弯曲运动。在此实施方式中,中间构件22可包括具有与凸起32的形状大体相匹配的凹腔40的表面38。中间构件22还可具有表面42,在此实施方式中,表面42的形状可与内表面34大体一致。
端板组件24、26和中间构件22可用任何适当的生物相容材料制成,包括钴-铬合金、不锈钢、钛合金、氧化铝、氧化锆、多晶金刚石、热解碳、聚醚醚酮(PEEK)、超高分子量聚乙烯(UHMWPE)、交联UHMWPE和/或其他合适的材料。表面28、36可包括能够增强植入假体的固着力的结构或涂层。例如,可以将羟基磷灰石(HA)之类的生物相容性和骨传导性(osteoconductive)材料涂敷在表面28的全部或一部分上。其他合适的涂层或处理可包括多孔珠粒涂层、多孔网目涂层、生骨肽涂层、生长因子涂层、rh-BMP涂层和/或喷砂处理。其他合适的结构可包括锯齿状、销钉状、凸脊状、翅片状和/或其他表面纹理。
在一些实施方式中,中间构件22可以用上述相对刚性的材料制成,而在其它实施方式中,中间构件22可允许具有一定程度的弹性或缓冲性,因此可将弹性材料用于这种中间构件。尽管中间构件22可以有一定的柔性,但其也应足够硬,以便能与端板组件有效地协同,以限制运动不超出所容许的范围。中间构件22的表面还应是足够耐久的,以提供可接受的耐磨性。在一个实施方式中,可以通过使中间构件22的表面区域的硬度大于接近其芯部的中心体材料的硬度,来达到这些性质的组合。因此,这一构件22可包括具有硬化的表面的生物相容复合材料或弹性材料。
下面来看图5,可通过使凸起32啮合于凹腔40以及将中间构件22的表面42定位在端板组件26的表面34上,来将椎间盘假体20的各构件装配起来。可将构件26、22、24沿纵向轴线44对中地对齐。
下面来看图6,可将椎间盘假体20用作植入体18并***椎间盘截除手术(discectomy)所产生的脊柱12a的空隙里(图3)。在一个实施方式中,表面36可与椎骨14a的端板接触,而表面28可与椎骨16a的端板接触。在其它实施方式中,可将假体倒置。
如图7的剖视图所示,当构件26、22和24沿纵向轴线44对中地对齐时,椎间盘假体20可处于中位(neutral position)。凸起32可具有曲面50,在这一实施方式中,曲面50可以是有相对不变的半径52和中心点54的弧。表面34可具有曲面56,在这一实施方式中,曲面56可以是有相对不变的半径58和中心点60的弧。可以测量中心点54和60之间的距离55。在这一实施例中,半径52小于半径58,因此弧50比弧56更为弯曲(tighter)。在中位时,中心点54和60可沿纵向轴线44对齐,以及较小的弧50可位于弧56之内,在这一实施例中,可以是位于半径58所扫过的区域57内。
图8表示出椎间盘假体20处于沿着例如前-后轴线(anterior-posterior axis)62移动了的一个位置。移动可以是例如由弯曲-伸展运动而产生。当端板组件24和26运动成不再相对于轴线44对齐时,中间构件22可在端板组件的内表面30和34之间作关节运动。在患者的体重作为沿着纵向轴线44的负荷64并且较小的曲面50是位于较大曲面56之内的情况下,假体20可能发生位移而返回到更稳定的中位,在这一位置,曲面50和56沿纵向轴线44对齐。在此实施方式中,当中心点54,60沿纵向轴线44排列时便是对齐了。在这一实施例中,当中心点54和60沿纵向轴线44对准时就是对齐了。该实施方式描述了乃是圆弧的曲面,但在其他实施方式中,这些曲面可以是其他曲面的一部分,例如一个椭圆球面的一块弧面。在这些替换实施例中,例如以椭圆形为例,当焦点对齐时或者当平分曲面的中心线对齐时,即为对齐了。
假体20的这种自校正脊椎前移或其他移位的趋势使得关节可以更自由更自然地运动,同时能够防止可能导致假体20不稳定的过度移动。不稳定会在相邻的关节上带来无法承受的负荷,或可能会造成假体20解体。假体20的这种对准倾向性可减轻邻接的关节处由于端板组件24和26的长时间过分位移而可能形成的过度的负荷。尽管在此实施方式的取向中,是把较平缓(wider)的弧置于较弯曲的弧的上面,但是在另一实施方式中,可把这一取向倒过来,就是把较弯曲的弧放在较平缓的弧的上面,但是使较弯曲的弧仍然处在较平缓的弧之内。
应能理解,可把对准倾向的程度,与中心点54和60之间的距离55关联起来,因而稳定性的大小也与中点54、60之间的距离55相关。随着距离55增大(例如一个球体在一个平面上),稳定性、假体20内的约束能力、以及自对准趋势都可能降低。随着距离55减小(例如一个球体在一个半径很小的凹坑里),稳定性、假体20内的约束能力、以及自对准趋势都会增大。尽管该实施方式是针对在前-后方向62的移位进行说明的,但是采用本发明的其他实施方式还可矫正在其他方向或方向组合中由移动、弯曲和/或旋转引起的移位。例如,端板组件26在侧向相对于端板24的侧向移位也可产生约束力,这个力会驱使中心点54和60返回到对准状态。各构件22-26可选自使得外科医生能够为患者设计具有适合患者的约束力和倾向性的特定假体的一整套元件。
在涉及截除多级椎间盘的实施方式中,可通过外科手术去除或损失掉韧带和其他软组织支撑结构。在这些实施方式中,用假体20之类的组件替代椎间盘,可至少在某种程度上弥补因除去软组织而丧失的稳定性。这种恢复的稳定性可以防止邻接的关节受到过度的负荷和磨损,还可鼓励患者作运动学上更准确的运动。
下面来看图9,在此实施方式中,椎间盘假体70可包括置于两个端板组件74和76之间的中间构件72。端板组件74可包括一个具有曲面80的凸起78。在此实施方式中,曲面80可以是具有中心点81和恒定半径的弧。端板组件76可包括具有曲面84的内表面82。在此实施方式中,曲面84可以是具有中心点86和恒定半径的弧。
下面来看图10,在此实施方式中,椎间盘假体90可包括置于两个端板组件94和96之间的中间构件92。端板组件94可包括具有曲面100的凸起98。在此实施方式中,曲面100可以是具有中心点101和恒定半径的弧。端板组件96可包括具有弧面104的内表面102。在此实施方式中,曲面104可以是具有中心点104和恒定半径的弧。
假体90的材料、装配和作用与假体20的类似,因此不再详细说明。凸起的形状相对于所接触的内表面的形状可对应于假体内的约束的程度。例如,在图9中,与相对弯曲度大的曲面104相比,弧形曲面84很平缓,而图10中,弧形曲面104更紧密地匹配于曲面100,与图10的假体90相比,图9的假体70可受到更大程度的约束。约束的增大可使假体更加倾向于返回中位而使中心点沿纵向轴线44对中地对齐。
下面来看图11,在此实施方式中,椎间盘假体110可包括置于两个端板组件114和116之间的中间构件112。端板组件114可包括具有曲面120的凸起118。在此实施方式中,曲面120可以是有焦点121和变化的半径的半椭圆球面或其他类型的曲面。端板组件116可包括内表面122,而内表面122又可具有曲面124。在此实施方式中,曲面124可以是具有焦点126、变化的半径、有角度的平的部分和/或平行的平的部分的U形表面。假体110的材料、装配和作用可与假体20类似,因此不再详细说明。在使用中,可使假体110倾向于使焦点121、126沿纵向轴线44对齐。
下面来看图12,在此实施方式中,椎间盘假体130可包括置于两个端板组件134和136之间的中间构件132。端板组件134可包括具有曲面140的凸起138。在此实施方式中,曲面140可以是具有焦点141和变化的半径的半椭圆球面。端板组件136可包括可具有曲面144的内表面142。在此实施方式中,曲面144可以是具有焦点126、变化的半径、有角度的平的部分和/或平行的平的部分的U形表面。
假体110、130的材料和装配可与假体20类似,因此不再详细说明。在使用中,可使假体130倾向于使焦点141和146沿纵向轴线44对齐。如图11和12所示,在一些实施方式中,曲面124、144的形状可以不是对应于恒定半径的圆弧,而是曲面的形状可以是例如U形的、半椭圆球形的或椭圆球形的曲面。图11中的U形曲面124比图12中的U形曲面144平缓,因此假体110受约束的程度比假体130的小,在假体130中,U形曲面154相对地更加弯曲,因而更紧密地匹配于曲面140。应当理解,由于U形壁可使假体110返回中位的倾向性增大,所以假体110(图11)受约束的程度比假体70(图9)的大。

对于假体150、170,与曲面80(图9)相比,曲面164、184相对地较为尖陡。图13中的尖陡的曲面164比图12中的较为弯曲的曲面184平缓,因此假体150受约束的程度可比假体170的小,在假体170中,U形曲面184相对地更为弯曲,因而更紧密地匹配于曲面180。

根据要应用植入体的具体关节,中间构件202的形状、尺寸、组成成份和物理性质可略有变化。中间构件202的形状可分别与端板组件204、206的内表面210、214互补,以允许被替换的具体关节能够作适当范围的移动、弯曲、伸展、旋转和侧向弯曲运动。在这一实施例中,中间构件202可包括表面218,表面218具有大致匹配于凸起212的形状的凹腔220。中间构件202还可有表面222,在此实施方式中,表面222可与内表面214的形状大致匹配。
构件202、204、206可分别用以上针对构件22、24、26所述的那些材料来成形。下面来看图19和20,可以通过使凸起212啮合于凹腔220以及将中间构件202的表面222定位在表面214上,来把椎间盘假体200的各个构件装配起来。可将各构件202-206沿纵向轴线44对中地对齐。可将椎间盘假体200***切除手术形成的(图3的)脊柱12a的空隙里。假体200的定位和功能可与假体20类似,因此将不再详细说明。如上文对假体20的说明,假体200也倾向于返回沿轴线44对中地对齐的中位。另外,在此实施方式中,凸起212在侧向66的延伸使关节能够作更稳定更可控制的侧向移动,同时可降低中间构件202发生错位的危险。

根据要采用植入体的具体关节,中间构件242的形状、尺寸、组成成份和物理性质可略作改变。中间构件242的形状可以分别与端板组件244、246的内表面250、254互补,以允许被替换的特定关节能够作适当范围的移动、弯曲、伸展、旋转和侧向弯曲运动。在此实施方式中,中间构件242可包括一个表面258,表面258上有一个与凸起252的形状大体相匹配的凹腔260。凹腔260可包括约束机构261,在此实施例中可以是一个突块(boss)。可采用一个以上的约束机构261(对应于一个以上的约束机构253),并且一个或多个约束机构261可定位在中间构件242上的几个交替的部位。突块261可沿前-后方向62延伸过凹腔260,以限制沿轴线66的运动,但在其他实施例中,可设置一个约束机构来限制沿轴线62的运动。中间构件242还可具有一个表面262,在此实施方式中,表面262可大体上与内表面254的形状相匹配。

根据要采用植入体的特定关节,中间构件272的形状、尺寸、组成成份和物理性质可略作变化。中间构件272的形状可分别与端板组件274、276的内表面280、284互补,以允许被替换的特定关节能够作适当范围的移动、弯曲、伸展、旋转和侧向弯曲运动。在此实施方式中,中间构件272可包括一个与凹坑282的形状大体相配的表面290。中间构件272还可具有一个表面292,在此实施方式中,表面292可与凹穴288的形状大体相配。
如图29所示,当各构件272-276沿纵向轴线44对中地对齐时,椎间盘假体270可处于中位。表面292可包括具有半径296和中心点298的弧294。表面290可包括一个具有半径302和中心点304的弧300。在图29所示的中位,中心点298、304沿纵向轴线44对齐。在此实施例中,半径302小于半径296,相应地,弧300比弧294更加弯曲。距离306延伸于中心点298和304之间。
各构件272、274、276可用以上分别针对构件22、24、26所述的那些材料来成形。具体地参见图28-30,可通过使表面290啮合于凹坑282并进一步使表面292啮合于表面288,来将椎间盘假体270的各个构件装配起来。可使各构件272-276沿纵向轴线44对中地对齐。可将椎间盘假体270置入由于截除椎间盘12形成的(图3的)脊柱12a的空隙里。表面278可与椎骨16的端板接触,以及表面286可与椎骨14a的端板接触。
下面来看图30,椎间盘假体270可由于例如弯曲、伸展和/或平移运动而作关节运动。由于这种运动,中间构件272可在端板组件的内表面284和280之间作关节运动。由于是较弯的弧300处于较平缓的弧294之内,作关节运动的假体270受到约束,并在承受诸如患者体重的负荷时倾向于返回到更为稳定的沿纵向轴线44对齐的中位。假体270的这种自对齐倾向可允许更自然的关节运动,同时可防止可能会导致假体270解体的过度移动。还有,这种对齐倾向可减轻如果没有这种倾向就可能由于中心点298和304之间的长期过度错位而在邻接的关节中形成的过度负荷。凹坑282和凹穴288除使得中间构件272能够作平滑的关节运动之外,还可起限制或禁止沿着轴线66的侧向运动的作用。表面282和290以及292和288之间互相匹配的曲率可以分布负荷并增强各构件272、274、276的耐磨性。各构件272、274、276可以是模件化的,这可允许选择具有一定厚度的中间构件272,从而将假体270调整至所需的高度。

1.一种关节假体,它包括:
用来啮合于第一骨部分的第一构件,该第一构件包括具有第一曲面的第一表面;
用来啮合于一个第二骨部分的第二构件,该第二构件包括具有第二曲面的第二表面;
其中,所述第一构件可相对于所述第二构件移动,并且所述第二曲面是被定位在所述第一曲面内,使得所述第一曲面和第二曲面倾向于沿着通过所述第一骨部分和第二骨部分的第一轴线对齐。
2.如权利要求1所述的关节假体,其特征在于,所述第一曲面具有第一恒定半径和第一中心点,以及所述第二曲面具有第二恒定半径和第二中心点。
