背景技术

发明内容
本发明提供一种活动支承件膝盖假体,其包括:用于连接患者胫骨的胫骨盘;以及设置在胫骨盘邻近的胫骨嵌入物;与胫骨嵌入物底面相关联的第一配合件和与胫骨盘顶面相关联的第二配合件;其中,胫骨嵌入物能够相对于胫骨盘运动,并且胫骨嵌入物相对于胫骨盘的运动至少包括枢转运动;该枢转运动是围绕一旋转轴进行的,所述旋转轴由在胫骨盘顶面上的一基本呈圆形的凸起位置限定;所述基本呈圆形的凸起位置沿着胫骨盘的中间-侧向轴线和前-后侧轴线朝着远离第二配合件的方向向下弯曲;当向胫骨盘的中间边缘靠近时,中间边缘的至少一部分向上弯曲,当向胫骨盘的侧向边缘靠近时,侧向边缘的至少一部分向上弯曲;以及当胫骨嵌入物和胫骨盘在中间-侧向轴线和前-后侧轴线的方向上都对准时,胫骨嵌入物的底面被设置成与胫骨盘的顶面基本互补;所述第二配合件被设置在所述胫骨盘顶面上的所述基本呈圆形的凸起位置上;所述第一配合件包括从胫骨嵌入物底面延伸的柱件,并且所述第二配合件包括设置在胫骨盘顶面上的孔。

在另一实施例中(该实施例是例证性的而非限制性的),在图7B所示的区域“A”中形成了比区域“B”中更为紧密的间隙。
现在参考图8A-8C,图中示出了一有助于旋转约束和/或控制的实施方案。
特别地,活动支承件膝盖假体800包括胫骨盘802,胫骨嵌入物804,以及与胫骨嵌入物704相连的股骨部件(未示出)。活动支承件膝盖假体800还可以具有由与柱件808配合的空腔806形成的枢转位置。此外,十字销806A可以与凹槽808a配合,从而在胫骨嵌入物804旋转的过程中,十字销806a可用作转动停止件,并且柱件808可用作弹簧(即对旋转产生阻力的扭簧)。
在一实施例中(该实施例是例证性的而非限制性的),可以改变十字销806a的直径和/或凹槽808a的尺寸,以形成不同程度的旋转约束和/或控制。
在另一实施例中(该实施例是例证性的而非限制性的),可以在植入胫骨盘802之前安装十字销806a(借此,凹槽808a能够将胫骨嵌入物804与胫骨盘802一起安装在体内适当的位置上,如胶合在适当的位置上)。
现在参考图9A-9D以及10A-10E,图中示出了有助于旋转约束和/或控制以及/或者有助于防止胫骨嵌入物翘起的各种其它实施例。
特别地,如图9A-9C所示,当胫骨嵌入物904旋转时,它可以接触到胫骨盘902的旋转限制突出部902a(rotation limiting tabs)(借此可以将胫骨嵌入物904楔入到适当的位置,而且能够进一步抑制旋转)。
此外,每个旋转限制突出部902a都包括底切902b,当达到旋转的极限值时,所述底切有助于防止翘起(从这一点讲,胫骨嵌入物904可以包括一个或多个唇部904a,用以接合旋转限制突出部902a和/或底切902b)。
此外,胫骨嵌入物904还可以包括位于胫骨盘902中空腔910内的柱件908,从而空腔910就具有凹部912以用来容纳柱件908的凸起部分914。凹部912和凸起部分914相互配合,这样有助于防止胫骨嵌入物904的翘起。在一实施例中(该实施例是例证性的而非限制性的),柱件908和空腔910之间的运转间隙大约为0.005至0.010英寸。
图10A和10B示出了在胫骨盘1002外壁部分内侧的一段距离“A”;图10C示出了与胫骨嵌入物1004相关联的距离B1,B2和C(其中距离B1和B2大于距离A,距离C小于距离A);图10D示出了当胫骨嵌入物1004旋转(在这一实施例中是顺时针方向旋转)时,胫骨盘1002和胫骨嵌入物1004之间的接触点。

现在参考图12A和12B,可以看到按照本发明实施方案的活动支承件膝盖1200包括胫骨盘1202和胫骨嵌入物1204。需要注意的是,图12A和12B中的这些设计能够通过用于后十字韧带(PCL)的间隙保持住后十字韧带(这里不需要利用PS脊柱(例如,一些其他的实施方案中可能需要)提供的后部稳定性)。
在另一实施方案中,胫骨嵌入物可由超高分子量的聚乙烯(“UHMWPE”)制成。在一实施例中(该实施例是例证性的而非限制性的),UHMWPE可以是模制成的UHMWPE(据认为,这种模制成的UHMWPE具有比机加工成的UHMWPE低的磨损率)。
现在参考图13A-13F,14A-14D,15A-15C,16A,16B以及17A-17C,需要注意的是在本发明的这些实施方案中,活动支承件膝盖假体可包括双凹面的分界面。
从这一点讲,需要注意的是这种双凹面分界面有助于提供一种最佳的膝盖解剖学构造,同时还可提供出足够厚(如根据耐磨性)的胫骨嵌入物关节连接结构(如聚乙烯关节连接结构)。在一实施例中(该实施例是示例性的而非限制性的),这种关节连接结构大约为6.5mm厚。
接下来参考图17A,可以看到胫骨嵌入物关节连接结构具有均匀的或恒定的厚度(即图17A中的厚度“X”),参考图17B,可以看到胫骨嵌入物关节连接结构还具有不均匀的或非恒定的厚度(如图17B中“A”区域和“B”区域处的厚度“Y”)。
此外,这种双凹面分界面有助于克服患者运动过程中由于翘起而引起的潜在剪切应力(参见图17C,示出了本发明所涉及的防止翘起以进行自动定心,以及减少或消除对枢转部件(如轴向的柱件)的剪切应力)。
还需要注意的是,图13A-13F,14A-14D,15A-15C,16A,16B以及17A-17C所示的这些实施方案可以包括上述各种枢转、平移、锁定、旋转约束和/或控制部件中的部分或全部。
现在参考图18A和18B,按照本发明实施方案的活动支承件膝盖假体包括由胫骨盘上的上凸面和胫骨嵌入物上的下凹面组成的分界面(如具有一定球面半径的分界面)(需要注意的是,这种球面半径具有自动调整的内在倾向)。
值得注意的是,上述结构通过从分界面中产生出磨料(如由分界面处的相对运动产生的聚乙烯颗粒),能够有助于减少胫骨嵌入物和胫骨盘之间的分界面的磨损(参见图18A)。
当然还需注意的是,正如图18B中所看到的,根据需要(如为了获得最优的耐磨性能并便于运动)可以改变形成胫骨嵌入物的材料厚度。在一实施例中(该实施例是例证性的而非限制性的),标记“A”和“B”的区域要比标记“C”的区域厚。
还需注意的是,图18A和18B所示的实施方案包括上述的各种枢转、平移、锁定、旋转约束和/或控制部件中的部分或全部。
现在参考图19A和19B,再次注意的是按照本发明的活动支承件膝盖假体可以与偏心(或偏置)枢转部件(如轴向的柱件)结合。特别地,在一实施方案中,这种偏心枢转部件可用于(如在患者运动的过程中)减少与内髁(medial condolyte)相关联的向前移动,并且增加与外髁(lateral condolyte)相关联的返回(向后移动)。从这一点讲,如图19A和19B所看到的,图19A示出了与中间枢轴(如相对于胫骨使股骨向外旋转70°)相关联的大幅度向前移动,图19B示出了与中间的偏心枢轴(如相对于胫骨使股骨向外旋转70°)相关联的小幅度向前移动。特别地,图19A和19B示出了偏心枢轴引起的与内髁相关联的小幅度向前移动和与外髁相关联的大幅度向后移动。
此外,需要注意的是,图19A和19B所示的实施方案包括上述的各种枢转、平移、锁定、旋转约束和/或控制部件中的部分或全部。
最后,参考图20A-20E,应该注意,本发明的该特定实施方案涉及与其他部件相结合的偏心枢轴部件的使用(如使用偏心的轴向柱件),其中这些部件也可以被构造成与非偏置枢轴部件一起使用。
例如(该实施例是例证性的而非限制性的),可采用一不对称的部件(如偏心轴向柱件)与对称的胫骨盘和对称的支承件(如聚乙烯支承件)相联以“凸轮”原理运行。图20A示出了按照这一实施方案的聚乙烯支撑件(在不旋转的条件下)的平面图,图20B示出了按照这一实施方案的胫骨盘(在不旋转的条件下)的平面图。此外,图20C示出了与向外旋转(右膝)相关联的偏心的旋转中心(点A),图20D示出了与向外旋转(左膝)相关联的偏心的旋转中心(点B)。
此外,图20E示出了采用“梁”形的平面图。
而且,需要说明的是图20A-20E所示的这些实施方案可以包括一个或多个内部停止机构(如用于旋转约束和/或控制的机构)。此外,图20A-20E所示的这些实施方案可用在与胫骨嵌入物和/或胫骨盘相关的方面,其中该胫骨盘具有包括平面部分、凹入部分和/或凸起部分的分界面。
此外,还需要注意的是图20A-20E所示的这些实施方案当然也可以包括上述的各种枢转、平移、锁定、旋转约束和/或控制部件中的部分或全部。
应注意的是,对于同样尺寸的膝盖假体来说(如3号尺寸),用于“波形”设计中的胫骨嵌入物和胫骨盘之间的接触面积比用于平面设计中的大(如高出10%的接触面积)。在一定的情形下,期望能够使这一接触面积最小化。因此,在一实施方案中,通过减少第二支承件(即在活动支承件膝盖中,胫骨嵌入物与胫骨盘之间的分界面,该分界面与胫骨嵌入物和股骨部件之间的分界面相对)的同余因子(congruencefactor)来使胫骨嵌入物和胫骨盘之间的接触面积最小化。还应当注意到是,这一解决方案不能用于平面设计,因为平面设计中的同余因子总是等于一。还要注意的是当接触仅仅发生在负载区域上时,这种同余因子方法具有优势。
在另一实施方案中,可以减小由柱件和孔之间的接触所引起的磨损,这是因为剪切应力能够被胫骨盘吸收(如受到“波形”内侧部的缓冲)。
尽管已经对本发明的多个实施方案进行了说明,应当理解的是这些实施方案仅仅是实例性的,而非限制性的,因此许多改进对本领域的普通技术人员来说变得更加直观。例如,可以采用一个或多个适当的紧固件来装配本发明的活动支承件膝盖假体(如采用螺钉或螺栓使胫骨嵌入物相对于胫骨盘保持正确的定位)。此外,本发明的活动支承件膝盖假体可带有能够预测位置、自动调整以及/或者能够自动定心的支承件。而且,可采用任意所期望的技术(如带有抛光剂的钻机)对金属进行抛光。而且,胫骨嵌入物可以比胫骨盘小(至少在某一尺寸上),从而可防止旋转过程中出现悬垂现象(例如,通过减小胫骨嵌入物中间/侧向的尺寸来实现)。可采用聚乙烯、金属、金刚石、陶瓷、聚醚醚酮(“PEEK”)以及/或者任何其他所需要的低摩擦关节用材料形成一个或多个彼此相配合的关节连接面。例如,胫骨盘、胫骨嵌入物和/或股骨部件可采用模制金属的构造(如UHMWP的模制金属)。胫骨盘、胫骨嵌入物和/或股骨部件可包括钴、铬和/或钛。可采用任何所需要的机械装置(如接合剂、一个或多个底切以及与其相匹配的凸起、机械紧固件(如螺钉)等)将股骨部件连接(如缚)到患者的股骨上。还可采用任何所期望的机械装置(如接合剂、一个或多个底切以及与其相匹配的凸起、机械紧固件(如螺钉)等)将胫骨盘连接(如缚)到患者的胫骨上。另外,按照本发明的活动支承件膝盖假体中的一个或多个部件可用来“更新”现有的假体/部件。此外,术语“活动支承件膝盖假体”还包括(但不限于)“旋转平台”式机构和“mensicalbearing”式机构。

Claims (14)

1.一种活动支承件膝盖假体,其包括:
用于连接患者胫骨的胫骨盘;以及
设置在胫骨盘邻近的胫骨嵌入物;
与胫骨嵌入物底面相关联的第一配合件和与胫骨盘顶面相关联的第二配合件;
其中,胫骨嵌入物能够相对于胫骨盘运动,并且胫骨嵌入物相对于胫骨盘的运动至少包括枢转运动;
该枢转运动是围绕一旋转轴进行的,所述旋转轴由在胫骨盘顶面上的一呈圆形的凸起位置限定;
所述呈圆形的凸起位置沿着胫骨盘的中间-侧向轴线和前-后侧轴线朝着远离第二配合件的方向向下弯曲;
当向胫骨盘的中间边缘靠近时,中间边缘的至少一部分向上弯曲,当向胫骨盘的侧向边缘靠近时,侧向边缘的至少一部分向上弯曲;以及
当胫骨嵌入物和胫骨盘在中间-侧向轴线和前-后侧轴线的方向上都对准时,胫骨嵌入物的底面被设置成与胫骨盘的顶面互补;
所述第二配合件被设置在所述胫骨盘顶面上的所述呈圆形的凸起位置上;
所述第一配合件包括从胫骨嵌入物底面延伸的柱件,并且所述第二配合件包括设置在胫骨盘顶面上的孔。
2.如权利要求1所述的活动支承件膝盖假体,其中,胫骨嵌入物的上表面被构造成容纳连接患者股骨的股骨部件。
3.如权利要求1所述的活动支承件膝盖假体,还包括一机构,该机构对胫骨嵌入物相对于胫骨盘的运动提供旋转控制和旋转约束的至少其中之一。
4.如权利要求3所述的活动支承件膝盖假体,其中,所述对胫骨嵌入物相对于胫骨盘的运动提供旋转控制和旋转约束的至少其中之一的机构包括设置在所述孔内的十字销和设置在所述柱件上的凹槽,其中,该十字销被容纳在凹槽内。
5.如权利要求3所述的活动支承件膝盖假体,其中,所述对胫骨嵌入物相对于胫骨盘的运动提供旋转控制和旋转约束的至少其中之一的机构包括至少一个沿着胫骨盘的中间边缘设置的第一突出部,以及至少一个沿着胫骨盘侧向边缘设置的第二突出部,其中,在沿第一方向转动的过程中,第一突出部与胫骨嵌入物接触以提供旋转控制和旋转约束的至少其中之一,在沿第二方向转动的过程中,第二突出部与胫骨嵌入物接触以提供旋转控制和旋转约束的至少其中之一。
6.如权利要求5所述的活动支承件膝盖假体,其中,第一突出部包括用以容纳胫骨嵌入物第一边缘的第一底切,第二突出部包括用以容纳胫骨嵌入物第二边缘的第二底切。
7.如权利要求1所述的活动支承件膝盖假体,还包括防止胫骨嵌入物从胫骨盘上翘起的机构。
8.如权利要求7所述的活动支承件膝盖假体,其中,防止胫骨嵌入物从胫骨盘上翘起的机构包括柱件上的凸起部分,该凸起部分与胫骨盘上限定孔的部分中的凹部相配合。
9.如权利要求7所述的活动支承件膝盖假体,其中,防止胫骨嵌入物从胫骨盘上翘起的机构包括至少一个沿着胫骨盘的中间边缘设置的第一突出部,以及至少一个沿着胫骨盘侧向边缘设置的第二突出部,其中,第一突出部包括用以容纳胫骨嵌入物第一边缘的第一底切,第二突出部包括用以容纳胫骨嵌入物第二边缘的第二底切。
10.如权利要求1所述的活动支承件膝盖假体,其中所述孔和柱件中的至少其中之一呈圆形。
11.如权利要求1所述的活动支承件膝盖假体,其中,所述孔和凸起位置沿着胫骨盘中间-侧向轴的中央位置设置。
12.如权利要求1所述的活动支承件膝盖假体,其中,所述孔和凸起位置沿着胫骨盘前-后侧轴的中央位置设置。
13.如权利要求1所述的活动支承件膝盖假体,其中,胫骨盘的上表面由金属构成,胫骨嵌入物的下表面由聚乙烯构成。
14.如权利要求13所述的活动支承件膝盖假体,其中,胫骨盘由金属构成,胫骨嵌入物由聚乙烯构成。
