背景技术
抗磷脂综合征(“APS”)是一种与血栓形成和流产相关的自体免疫性疾病,可导致肺部和大脑出现危及生命的血栓。APS是50岁以下人群中风的主要原因。在孕妇中,它通常会导致流产和死产。

本发明的方法适用于治疗所有类型的APS,例如原发性APS、继发性APS和灾难性APS。
药物组合物以任何形式施用,包括颗粒、片剂、胶囊、丸剂、粉末、溶液、悬浮液或糖浆。优选地,以每天100mg至800mg(例如120mg至600mg、150mg至500mg,以及180mg至450mg)的S-HCQ的剂量向患者施用。
S-HCQ是指化合物本身及其药学上可接受的盐。其盐的实例为盐酸盐、硫酸盐和磷酸盐。
本发明的数个实施方案细节在下面的描述和附图中进行阐述。本发明的其他特征、目的和优点将从该描述以及所附的权利要求中变得显而易见。最后,本文引用的所有出版物和专利文献均经由引用整体并入。

具体实施方式
如上所述,提供一种治疗APS的方法,通过向APS患者施用含有高纯度S-HCQ和药学上可接受的赋形剂的药物组合物而达成,该药物组合物实质上不含R-HCQ。
S-HCQ的纯度系以其对映体过量值来测量,定义为S-HCQ和R-HCQ之间的摩尔百分比差异,其中S-HCQ和R-HCQ的总摩尔百分比为100%。例如,对映体过量值为99%的S-HCQ的纯度包含99.5%摩尔的S-HCQ和0.5%摩尔的R-HCQ。当药物组合物含有对映体过量值为99%或更高(例如,99.2%或更高、99.5%或更高,以及99.8%或更高)的S-HCQ时,该药物组合物被视为实质上不含R-HCQ。
在美国专利申请号17/176,679和美国专利号5,314,894中描述具有此种高对映体过量值的S-HCQ制剂。
药物组合物中的S-HCQ为游离碱或药学上可接受的盐。药学上可接受的盐可为但不限于硫酸盐、磷酸盐和盐酸盐。优选地,其为硫酸盐。
该药物组合物含有按重量计5%至95%,例如30%至80%、40%至70%、40%至55%,以及60%至70%范围的S-HCQ。
该药物组合物实质上不含R-HCQ,例如,含有按重量计2%或更少的R-HCQ(例如,1%或更少,以及0.5%或更少)。
示例性药物组合物含有50%至70%的S-HCQ,以及30%至50%的一或多种药物赋形剂。
为了实施本发明的方法,通常向患有APS的受试者施用有效量的药物组合物,其对应于100mg至800mg(如200mg和400mg)的S-HCQ的日剂量。
与R-HCQ和外消旋HCQ(即S-HCQ和R-HCQ的等摩尔混合物)相比,施用S-HCQ具有较少的副作用,特别是在心脏毒性方面。
除了S-HCQ,药物组合物亦包括药学上可接受的赋形剂,其可为制药领域中使用的任何生理惰性的赋形剂,包括但不限于黏合剂、稀释剂、表面活性剂、崩解剂、润滑剂、助流剂,以及着色剂。赋形剂的示例请参见美国专利申请公开号2008/020634。
药物组合物以任何形式提供,例如颗粒、片剂、胶囊、丸剂、粉末、溶液、悬浮液或糖浆。其可依照许多文献中描述的常规方法制备,请参见例如美国专利申请公开号2018/0194719。
令人惊讶地发现本发明方法在治疗患有原发性APS(血栓性APS和产科APS)、继发性APS和灾难性APS的受试者中相当有效。
原发性APS是一种不存在任何合并症的血栓形成倾向状态,其特征是复发性动脉和静脉血栓形成、复发性流产以及存在导致血栓形成倾向和妊娠发病的循环aPL抗体。在患有继发性APS的患者中,预先存在有自身免疫疾病。灾难性APS为最严重的APS形式,是一种与aPL抗体相关的多***自身免疫疾病,其特征为血管血栓形成或流产,同时伴有小血管阻塞的多重器官衰竭。
APS症状因患者而异,包括血栓、流产、皮疹、慢性头痛、痴呆、癫痫发作、动脉血栓形成、自身免疫性血小板减少症、常染色体显性遗传、视力模糊、视网膜中央动脉闭塞、虹膜炎、角膜炎、狼疮抗凝物、视网膜脱离、视网膜血管炎、巩膜炎、静脉血栓形成、视力丧失和玻璃体炎等病征。
不受理论的束缚,一般相信HCQ通过与β2-糖蛋白I(“β2-GP1”)(一种与APS相关的血液蛋白)结合来治疗APS。β2-GP1在血液中以高浓度循环,即0.2mg/mL,能够调节凝血。请见McDonnell等人。
β2-GP1以两种构象存在,即封闭的环形和开放的线形。目前尚不清楚是什么触发β2-GP1在这两种形式之间改变其构象。其中,90%的β2-GP1以环形在血液中移动。请参见Agar等人,Blood 116,1336-43(2010)。在其线形中,β2-GP1暴露出两个结构域,即N-端结构域I(“DI”)和C端结构域V(“DV”)。DI为接收抗体例如aPL抗体的主要区域。DV负责与血液细胞膜结合。当β2-GP1从环形变为线形时,它会促进抗体与血液细胞膜结合,因而引发凝血反应。这是通过形成β2-GP1-抗体复合物(此为APS的关键致病途径)而实现的。
人们认识到,β2-GP1-抗体复合物通过破坏血液细胞上的抗凝血膜联蛋白V屏障等机制发挥凝血作用。请参见McDonnell等人。
膜联蛋白V是一种细胞蛋白,通过与血液细胞膜的磷脂结合,形成阻止抗体攻击磷脂的屏障来抑制血栓的形成。在APS患者中,膜联蛋白V屏障会被抗体通过β2-GP1而破坏。
HCQ被认为通过阻止β2-GP1将其构象变为线形,而破坏上述致病途径。
无需进一步详述,一般相信本领域技术人员可基于本文的公开内容,最大限度地利用本发明内容。因此,以下具体实施例应被解释为仅为描述性,而不以任何方式限制本发明的其余部分。
实施例
实施例1:S-HCQ和R-HCQ在β2-糖蛋白I上的分子对接
进行分子对接模拟以评估S-HCQ和R-HCQ与β2-GP1的结合稳定性,β2-GP1的结构来自蛋白质数据库(PDB:1AV1),此为全球开放获取的生物大分子结构数据档案。Discovery Studio软件(Dassault Systemes,加利福尼亚州,圣地亚哥)用于比对和编辑蛋白质结构和氨基酸序列。SwissDock是由瑞士生物信息学研究所提供的对接服务,通过计算机分析预测β2-GP1的疏水性区域。UCSF Chimeram(加州大学,圣地亚哥)软件用于可视化和分析分子结构。
分子对接有助于了解HCQ如何与β2-GP1结合并抑制其构象变化。S-HCQ和R-HCQ分别与β2-GP1进行分子对接模拟。当与抗体结合时,β2-GP1经历从封闭形式(环形)到开放形式(线形)的构象变化。后者促进β2-GP1-抗体复合物的形成,导致血栓形成。因此,了解HCQ的结合是否会干扰β2-GP1的构象变化至关重要。
分子对接结果如图1所示。如图所示,β2-GP1包含一个假连续、两亲性α-螺旋,其具有五个结构域和四个接合部(图中显示为1、2、3和4)。两个相邻的α-螺旋结构域经由一个接合部连接。这四个接合部一起或单独地使α-螺旋移动,因而改变β2-GP1的构象于环形和线形之间。
分子对接显示HCQ分子可与该四个接合部(1、2、3和4)中的任一者结合。其中,与接合部3的结合可有效抑制构象变化。结合强度通过分子对接研究中计算的亲和能来测量。更高的亲和能显示更强的结合。
S-HCQ以12.6kcal/mol的亲和能与接合部3结合,此为非常高的数值。通过固定β2-GP1α-螺旋的接合部3,S-HCQ会抑制其构象从环形变为线形,而线形为抗体与β2-GP1偶合形成复合物所必需,此为APS致病途径所必需的。因此,S-HCQ可阻断该途径,因而有效治疗APS。
相较之下,R-HCQ以仅10.5kcal/mol的亲和能与该接合部3结合,该能量等级对于阻止β2-GP1α-螺旋的构象变化并不是很有效。此外,R-HCQ与β2-GP1α-螺旋的结合角度与S-HCQ不同,这使得R-HCQ在抑制构象变化方面效果较差。
实施例2:HCQ对β2-GP1-抗体形成的抑制
在一项体外研究中,对S-HCQ和R-HCQ进行测试,以显示使用THP-1(一种衍生自患有单核细胞白血病患者的人类单核细胞系)减少aPL抗体和β2-GP1结合的有效性。
THP-1是一种人外周血单核细胞,其高表达β2-GP1,并且与患者的APS增加有关。在以HCQ处理之前,将THP-1细胞于3.7%聚甲醛溶液中进行固定反应、在含有1%牛血清白蛋白(BSA)的磷酸盐缓冲液(PBS)中进行封闭,并与小鼠aPL抗体(A500-006A,BethylLaboratories,蒙哥马利,德克萨斯州)一同孵育。细胞免疫荧光染色用于确定β2-GP1表达。
如下制备用于斑点印迹测定的单独样品:将THP-1总裂解物加载到聚二氟亚乙烯膜上,以含有1% BSA的PBS进行封闭,并与小鼠抗β2-GP1A500-006A以及S-HCQ、R-HCQ或外消旋HCQ,浓度各自为10mg/mL,一同孵育。除了不添加HCQ之外,依照上述相同流程获得对照样品。
随后,进行ELISA测定以确定S-HCQ或R-HCQ是否抑制β2-GP1-抗体复合物与THP-1膜的结合。如此处理的THP-1细胞以3.6x 105细胞/mL的密度重新悬浮于含有80% RPMI-1640(Thermo Fisher Scientific,沃尔瑟姆,马萨诸塞州)、有20%抗-β2-GP1免疫球蛋白G(“IgG”)(0.2mg/mL)的HEPES-缓冲盐水(HBS,pH 7.45)和S-HCQ的培养基中。三个样品各由具有不同S-HCQ浓度(即1μg/mL、2.5μg/mL或5μg/mL)的培养基制备。除了不添加S-HCQ之外,依照上述相同流程获得对照样品。在450nm处测量吸光度。
上述所有测定均进行三次重复。报告结果以平均值±S.E.M表示。所有统计分析均使用商标为GraphPad(Version 8.0.GraphPad Software Inc,圣地亚哥,加利福尼亚州)的软件进行。用于两组之间的比较,使用学生检验。p值<0.05被认为具有统计学显著性。
使用细胞免疫荧光染色发现本研究中的THP-1细胞以高水平表达β2-GP1,由绿色圆点与THP-1细胞重叠证实,该细胞显示为圆形单细胞形态,带有核酸蓝点。
斑点印迹分析显示S-HCQ抑制β2-GP1与抗体结合的惊人水平为97%(±2%,p<0.05)。作为比较,R-HCQ仅抑制23%(±17%)的β2-GP1与抗体的结合,而外消旋HCQ抑制80%(±4%)的结合。
ELISA测定显示,S-HCQ以剂量依赖性方式抑制β2-GP1与抗体结合,即在1μg/mLS-HCQ时抑制率为25%,在2.5μg/mL S-HCQ时抑制率为60%,以及在5μg/mL S-HCQ时抑制率为90%(p<0.05)。
发现S-HCQ可有效抑制β2-GP1与抗体的结合,令人惊讶地比R-HCQ和外消旋HCQ更有效。
实施例3:以HCQ修复细胞膜上的膜联蛋白A5抗凝血屏障
如上所述,膜联蛋白A5(一种内源性蛋白质)在血液细胞表面形成屏障,抑制血液凝固,因而降低APS的风险。在这个例子中,S-HCQ显著修复被aPL抗体破坏的膜联蛋白A5抗凝血屏障。
THP-1细胞首先保持在含有10%胎牛血清、2mM L-谷氨酰胺和50U/mL青霉素-链霉素抗生素的RPMI1640培养基中(Thermo Fisher Scientific,沃尔瑟姆,马萨诸塞州)。之后将其以8×104个细胞/孔的密度接种在96孔培养板中,并使其达到汇合。随后,在0.5μg/mLHCQ存在下,以抗β2-GP1 IgG处理THP-1细胞。在以HBS-CaCl2溶液漂洗经IgG处理的THP-1细胞,以去除游离的膜联蛋白V,仅留下附着在细胞表面的膜联蛋白V后,通过吸光度测定细胞表面的膜联蛋白V水平。
以三种HCQ(即S-HCQ、R-HCQ和外消旋HCQ)溶液中的一种各自制备三种样品。除了不添加HCQ之外,依照上述相同流程获得对照样品。含有β2-GP1抗体的患者血清作为对照样品。
细胞免疫荧光染色证实膜联蛋白V的高表达水平,在免疫荧光图像中显示为绿点。此外,通过蛋白质印迹证实THP-1中膜联蛋白V的表达。
经S-HCQ处理的THP-1细胞具有相对水平为3.45的膜联蛋白V,相比之下,经R-HCQ处理的THP-1细胞具有相对水平为2.54的膜联蛋白V,而经外消旋HCQ处理的THP-1细胞具有相对水平为3.09之膜联蛋白V。比较样品显示膜联蛋白V的相对水平仅为1。
上述结果显示,在治疗APS方面,S-HCQ比R-HCQ和外消旋HCQ更惊人地有效。
实施例4:HCQ减少体内血栓形成
通过APS-相关血栓形成动物模型评估HCQ对于抑制血栓形成的治疗作用。
抗β2-糖蛋白I的分离
选择来自6名APS患者(即患者1-6)的血清和血浆参与研究。测量抗心磷脂(aCL)、抗β2GPI和狼疮抗凝物(LA)活性,以确认APS。使用rProtein A/Protein G GraviTrapTM(CytivaTM,Merck KGaA,达姆施塔特,德国)纯化血清样品,获得APS-衍生的抗-β2GPI样品。通过酶-联免疫吸附测定(ELISA)(Eagle Biosciences,Inc.,阿默斯特,新罕布什尔州),使用与β2GPI的结合来测试每一样品中抗-β2GPI抗体的浓度。
经由上述获得的抗-β2GPI抗体诱导体外内皮细胞活化。接种内皮细胞(即HUVEC)并与APS-衍生的抗-β2GPI抗体一起孵育。作为阳性对照,一些HUVEC系以脂多糖(LPS,3mg/mL)处理。检测E-选择素、细胞间黏附和血管细胞黏附分子1(VCAM-1)的表面表达,发现与β2GPI IgG的量呈正相关。
APS-相关血栓形成的小鼠模型
本研究使用8-12周龄的C57BL/6雄性小鼠(购自台湾BioLasco)。所有程序均经台北医科大学机构动物照护委员会核准。
使用内皮损伤模型在小鼠中诱导静脉血栓形成。小鼠以100、200或300AU(测试组)的剂量进行静脉内(IV)注射7.5% FeCl3(阳性对照)、生理盐水(阴性对照),或APS-衍生的抗-β2GP1抗体(来自患者2)。小鼠在注射后72小时被麻醉。暴露出右股静脉并以1500g/mm2的压力挤压以诱导血栓形成。
已确定抗-β2GP1抗体在所有三个浓度,即100、200和300AU下,均有效诱导血栓。因此,将抗-β2GP1抗体以100AU注射至小鼠中,用于以下所有研究。
外消旋-HCQ在APS-相关血栓形成小鼠模型中减少血块形成
依照上述程序,以来自患者5的100AU的抗-β2GP1抗体和2000μg外消旋HCQ(200μl,10mg/ml)注射小鼠(即HCQ处理的小鼠)。作为对照,一组小鼠(n=4;APS诱导的小鼠)仅注射100AU的抗-β2GP1抗体。72小时后,将每只小鼠的右股静脉暴露出,并以1500g/mm2的压力挤压以诱导血栓形成。记录血栓形成时间(单位为分钟)。健康小鼠组(n=2)的血栓形成时间为5分钟,而APS-诱导的小鼠显示平均血栓形成时间为2分钟。相较之下,经HCQ处理的小鼠血栓形成时间为5分钟,与健康小鼠相同。结果显示,抗-β2GP1抗体加速APS-诱导小鼠的血栓形成,另一方面,外消旋HCQ则抑制抗-β2GP1抗体的功能。
挤压5分钟后从股静脉去除血块。与未使用HCQ处理的小鼠组相较,外消旋HCQ显著降低血栓大小。
结果显示,包括S-HCQ和R-HCQ二者在内的外消旋HCQ,可通过减少APS患者的血块形成而用于治疗APS。
S-HCQ在APS相关血栓形成小鼠模型中减少血块形成
APS-相关血栓形成和静脉血栓栓塞(VTE)的危险因子重叠,主要与内皮功能障碍(ED)相关。E-选择素和VCAM-1与APS-相关血栓形成的高风险相关。
基于上述APS动物模型,向小鼠施用R-HCQ和S-HCQ,以减少血栓形成。将小鼠分为6组,每组注射如下:(1)IgG作为对照组,(2)100AU的抗-β2GPI抗体作为比较组,(3)100AU的抗-β2GPI抗体和300μg的S-HCQ作为处理组3,(4)100AU的抗-β2GPI抗体和200μg的S-HCQ的组合作为处理组4,(5)100AU的抗-β2GPI抗体和200μg的R-HCQ作为处理组5,或(6)100AU的抗-β2GP1抗体和100μg的R-HCQ的组合作为处理组6。
测量APS-相关血栓形成的两种生物标志物——E-选择素和VCAM-1的血清表达水平。基于E-选择素和VCAM-1相对于比较组的表达水平而计算处理组3-6中的抑制百分比。结果请见下表1。E-选择素或VCAM-1的低表达水平显示血栓形成的高度抑制。
如表1所示,与经R-HCQ处理的小鼠相比,以300μg或200μg的S-HCQ处理的小鼠,其E-选择素和VCAM-1的表达要低得多。令人惊讶地发现,与经200μg的R-HCQ处理的小鼠(其仅抑制26.5%的血栓形成)相比,以200μg的S-HCQ处理的小鼠可抑制51.4%的血栓形成。

Claims (8)
1.一种治疗抗磷脂综合征(APS)的方法,所述方法包括:
辨识出患有APS的受试者,以及
向所述受试者施用有效量的含有(S)-(+)-羟氯喹和药学上可接受的赋形剂的药物组合物,从而治疗APS,
其中所述药物组合物实质上不含(R)-(-)-羟氯喹。
2.如权利要求1所述的方法,其中所述S-羟氯喹为药学上可接受的盐形式。
3.如权利要求2所述的方法,其中所述药学上可接受的盐为盐酸盐、硫酸盐或磷酸盐。
4.如权利要求1至3中任一项所述的方法,其中向所述受试者施用S-羟氯喹的剂量为每日100mg至800mg。
5.如权利要求4所述的方法,其中所述剂量为每日150mg至500mg。
6.如权利要求1至5中任一项所述的方法,其中所述药物组合物为颗粒、片剂、胶囊、丸剂、粉末、溶液、悬浮液或糖浆的形式。
7.如权利要求1至6中任一项所述的方法,其中所述APS为原发性APS、继发性APS或灾难性APS。
8.如权利要求1至6中任一项所述的方法,其中所述APS为血栓性APS或产科APS。
