Background
The Western medicine is mainly used for treating burns and scalds, and analgesic medicines and local antibacterial medicines, such as silver sulfadiazine, have obvious toxic and side effects although the curative effect is obvious, and have poor capability of promoting regeneration of skin tissues. The effect of treating oedema after burns is also not ideal and wound infection is one of the common complications of burns and scalds. Taking silver sulfadiazine as an example, the silver sulfadiazine can cause reactions such as neutropenia, aplastic anemia and the like after being absorbed into blood.
In recent years, traditional Chinese medicines are increasingly favored in the treatment of burns and scalds. Compared with western medicines, the Chinese herbal medicine has remarkable advantages in promoting wound tissue regeneration, and has the characteristics of rich preparation types, safety, low toxicity, low price and the like.

The second aspect of the invention provides a preparation method of the burn and scald ointment, which comprises the following steps:
(1) Uniformly mixing radix Sanguisorbae, rhizoma Smilacis chinensis, rhizoma Coptidis, and radix Scutellariae to obtain a mixture, adding water into the mixture, heating for extraction, and filtering to obtain filtrate;
(2) Adding pulverized calculus bovis, scolopendra, os Draconis, succinum, rhizoma Bletillae, fructus Gardeniae, moschus, semen Fagopyri Esculenti, catechu, borneolum Syntheticum, and Cera flava into the filtrate, heating, concentrating to obtain extract, and naturally cooling to obtain burn and scald ointment.
Preferably, in the step (1), the added water is 8-12 times of the mass of the mixture, the heating temperature is 80-90 ℃, and the heating time is 4-5 hours.
Preferably, in the step (2), the crushed particle size is 20 meshes, the heating temperature is 70-80 ℃, and the heating time is 10-15 hours.

Gardenia jasminoides ellis: purging pathogenic fire, relieving restlessness, clearing heat, promoting diuresis, cooling blood, and removing toxic substances; external use detumescence and pain relieving. Can be used for treating vexation, jaundice due to damp-heat, stranguria, hematemesis, conjunctival congestion, swelling and pain, and skin infection due to pathogenic fire.
Bezoar: clearing away heart-fire, eliminating phlegm, inducing resuscitation, cooling liver, calming endogenous wind and detoxicating. Can be used for treating fever, coma, apoplexy, phlegm, convulsion, epilepsy, mania, laryngopharynx swelling and pain, aphtha, carbuncle, and furuncle.
Centipede): has effects of calming endogenous wind, relieving spasm, dredging collaterals, relieving pain, removing toxic substances, and resolving hard mass. Scolopendra is pungent and warm in nature, good in channeling, capable of reaching the inside and outside, and strong in dispelling wind, relieving convulsion and force, and is an essential herb for stopping spasm by extinguishing wind. Centipede is toxic and pungent in flavor and good in property and moving, can dispel toxic substances, has the effects of removing toxicity, resolving masses, relieving swelling and pain.
Musk: inducing resuscitation and refreshing mind, promoting blood circulation and dredging channels, and relieving swelling and pain. Sores and ulcers, scrofula, phlegm nodule and sore throat: the Xin Xianghang powder has good effects of promoting blood circulation, resolving hard mass, relieving swelling and pain, and is effective for oral administration and external use if the above symptoms are treated.
Chinaroot greenbrier rhizome: dispelling wind-dampness, promoting urination and detumescence. Can be used for treating arthralgia, numbness of muscles, diarrhea, dysentery, edema, gonorrhea, furuncle, toxic swelling, scrofula, and hemorrhoid.
Buckwheat: promote appetite and wide intestine, promote qi downward and remove food retention. For diarrhea, gastrointestinal stagnation, chronic diarrhea, dysentery with vomiting, erysipelas with reddish discharge, carbuncle, , furuncle and burn due to decoction fire.
Coptis root: clear heat and dry dampness, purge fire and detoxify. Can be used for treating damp-heat distention and fullness, emesis, acid regurgitation, dysentery, jaundice, high fever unconsciousness, excessive heart fire, vexation, insomnia, hematemesis, epistaxis, conjunctival congestion, toothache, diabetes, carbuncle, and furuncle; it is used externally to treat eczema, wet sore and suppurative ear canal.
Keel: calm heart and tranquilize mind, calm liver and subdue yang, induce astringency and astringe. Can be used for treating palpitation, insomnia, amnesia, convulsion, epilepsy, giddiness, spontaneous perspiration, night sweat, spermatorrhea, enuresis, metrorrhagia, leukorrhagia, chronic diarrhea, chronic dysentery, ulcer, and damp sore.
Amber: relieving convulsion, tranquilizing mind, removing blood stasis, stopping bleeding, promoting diuresis and treating stranguria. It is indicated for convulsion, epilepsy, palpitation, insomnia, hemopuria, urinary obstruction, amenorrhea, postpartum abdominal pain, carbuncle, sore and toxic materials, traumatic injury.
Catechu: has effects of promoting blood circulation, relieving pain, stopping bleeding, promoting granulation, eliminating dampness, healing sore, clearing lung-heat, and eliminating phlegm. Can be used for treating traumatic injury, traumatic hemorrhage, hematemesis, epistaxis, unhealed skin and ulcer, eczema, and cough due to lung heat.
Borneol: has the effects of inducing resuscitation, clearing heat, removing toxic substances, improving eyesight, and removing nebula, and can be used for treating febrile disease, high fever, unconsciousness, apoplexy, phlegm syncope, convulsion, summer-heat dampness, clearing orifices, pharyngitis, deafness, aphtha, odontopathy, carbuncle, infantile hemorrhoid, conjunctival congestion, swelling and pain, nebula, and eye obstruction.
Radix Scutellariae Baicalensis: clear heat and dry dampness, purge fire and detoxify, stop bleeding and prevent abortion. Can be used for treating damp-heat, summer-heat, chest distress, emesis, damp-heat distention and fullness, dysentery, jaundice, cough due to lung heat, polydipsia due to high fever, hematemesis, epistaxis, carbuncle, swelling, sore, and fetal movement.
Bletilla striata: bitter, sweet and astringent in flavor, slightly cold in nature, enter lung, liver and stomach meridians. The bletilla tuber is mainly used for astringing, stopping bleeding, detumescence and promoting granulation.
Beeswax: detoxify, heal wound, promote tissue regeneration and relieve pain. Can be used for treating ulcer, ecthyma erosion, traumatic ulcer, burn and scald.
The burn and scald ointment of the invention follows the compatibility principle of monarch, minister, assistant and guide, wherein the monarch drug is sanguisorba, coptis, chinaroot greenbrier and baikal skullcap root, the ministerial drug is bezoar, dragon bone, amber, bletilla striata and centipede, and the monarch drug, the ministerial drug and the assistant and guide have synergistic effect, and have the effects of clearing heat and detoxicating, cooling blood and removing stasis, removing putrefaction and promoting granulation and relieving pain.

(1) Pulverizing calculus bovis, scolopendra, os Draconis, succinum, and rhizoma Bletillae to 20 mesh, adding 10 times of water, heating at 90deg.C for 4 hr, and filtering to obtain filtrate;
(2) Heating and concentrating the filtrate at 80deg.C for 12 hr to obtain extract, and naturally cooling to obtain burn and scald ointment.
In comparison with example 1, this comparative example has only ministerial drugs.
Comparative example 3
A burn and scald ointment comprises the following components: 10g of gardenia, 2g of musk, 10g of buckwheat, 10g of catechu, 5g of borneol and 6g of beeswax.
The preparation method comprises the following steps:
(1) Pulverizing fructus Gardeniae, moschus, semen Fagopyri Esculenti, catechu, borneolum Syntheticum, and Cera flava to 20 mesh, adding 10 times of water, heating at 90deg.C for 4 hr, extracting, and filtering to obtain filtrate;
(2) Heating and concentrating the filtrate at 80deg.C for 12 hr to obtain extract, naturally cooling and drying to obtain powder.

Patient 2, men, 10 years old, the arm scalded by hot oil shows that the scalded part is whitish, the epidermis falls off, and a great amount of yellow liquid oozes out of the sore surface, the deep II degree scald is diagnosed, the burn and scald ointment of the invention is smeared after being washed by normal saline, the dressing is changed 3 times a day for the first 3 days, the dressing is changed 2 times a day, new granulation is generated after the 3 rd day of the dressing, and the scalded part heals after the 12 th day of the dressing without scars.
Patient 3, male, 35 years old, burned with fire in large area, the burned part has back, arm, the burned part appears as burnt skin, has bad smell, is diagnosed as III degree scald, the burn and scald ointment of the invention is smeared after washing with normal saline, dressing is changed for 5 times every day, new granulation is generated after the 7 th day of administration, and the burned part heals after 20 days of administration without scars.
2. Experiment for scalding mice
The experiment was carried out according to the procedure of the fourth chapter 2.3 mice scald activity experiment in the literature (Shen Yingxin. University of vinca traditional Chinese medicine, 2022) from the study of the effect of treating scalds by the effective substance of classical famous prescription Yuhong Gao. Mice used were purchased from the experimental animal breeding company limited, mitsui.
2.1 establishment of Scald model of mice
The dehairing treatment was performed by applying a proper amount of 5% sodium sulfide liquid to the back of the mice 24 hours before the experiment, and the hairless area of the back of the mice was about 2×2cm. When the scald model is built, after a 50g weight is placed in boiling water for 10min, the model is quickly taken out and tightly stuck to the smooth hairless skin of the mouse for 8s, the deep II-degree scald model is completed, and a blank control group is not treated.
2.2 skin healing Rate
50 mice were equally divided into 5 groups and treated with burn and scald ointment of example 1, comparative example 2, and comparative example 3, respectively. The hair was removed by applying 5% sodium sulfide to the back of the mice in an area of 2X 2cm. The skin of the back of the mice is tightly placed on weights with boiling distilled water, the time is counted by a stopwatch, and each group of mice is scalded for 8s, thus completing the scalding model.
And (3) after the mouse scald model is completed, 0.1g of burn and scald ointment is smeared on the wound skin, 1 time a day, and the wound healing rate is calculated. The results are shown in Table 1.
The method adopts a sulfuric acid paper marking method on the 14 th day after the model is completed, the quality of a marked area is used for replacing the area, and the average healing rate of the wound surface is calculated according to the following specific calculation formula:
wound healing rate = (original scald area-unhealed area)/original scald area×100%
Wound healing judgment standard: the wound crust falls off, new epidermis is generated, and tiny villus appears.

As can be seen from the data in Table 1, the burn and scald ointment of the invention has a skin healing rate of 85% in 14 days in the burn test of mice, and the skin healing rate is higher than that of the burn and scald ointment of comparative examples 1-3.

The burn and scald ointment is characterized by comprising the following components in parts by mass: 10-20 parts of garden burnet, 5-15 parts of gardenia, 1-5 parts of bezoar, 3-8 centipedes, 1-3 parts of musk, 5-15 parts of chinaroot greenbrier, 5-15 parts of buckwheat, 5-10 parts of coptis chinensis, 25-35 parts of dragon bone, 3-8 parts of amber, 5-15 parts of catechu, 3-8 parts of borneol, 5-15 parts of radix scutellariae, 5-15 parts of bletilla striata and 3-8 parts of beeswax.

The burn and scald ointment according to claim 1, wherein the burn and scald ointment comprises the following components in parts by mass: 15 parts of garden burnet, 10 parts of gardenia, 3 parts of bezoar, 5 centipedes, 2 parts of musk, 10 parts of chinaroot greenbrier, 10 parts of buckwheat, 8 parts of coptis chinensis, 30 parts of dragon bone, 5 parts of amber, 10 parts of catechu, 5 parts of borneol, 10 parts of radix scutellariae, 10 parts of bletilla striata and 6 parts of beeswax.
3. The method for preparing the burn and scald ointment according to any one of claims 1 to 2, which is characterized by comprising the following steps:
(1) Uniformly mixing radix Sanguisorbae, rhizoma Smilacis chinensis, rhizoma Coptidis, and radix Scutellariae to obtain a mixture, adding water into the mixture, heating for extraction, and filtering to obtain filtrate;
(2) Adding pulverized calculus bovis, scolopendra, os Draconis, succinum, rhizoma Bletillae, fructus Gardeniae, moschus, semen Fagopyri Esculenti, catechu, borneolum Syntheticum, and Cera flava into the filtrate, heating, concentrating to obtain extract, and naturally cooling to obtain burn and scald ointment.
