Description

Method and device for eliminating peridentitis and periapical periodontitis
Technical Field
The invention belongs to the technical field of periodontal and apical treatment clinic, and particularly relates to a method and a device for eliminating peridentitis and periapical periodontitis.
Background
Peridentitis is a chronic inflammation of the periodontal supporting tissue caused primarily by local factors. The most common cause is that bacterial plaque attached to the tooth surface cannot be cleaned in time, and the common pathogenic bacteria are porphyromonas gingivalis and the like as a bacterial disease. Periodontitis which is not treated in time can cause the formation of periodontal pockets, periodontal pus discharge, tooth loosening and other symptoms.
Acute or chronic inflammation of periapical tissue is called periapical inflammation, and bacterial infection and the like can cause periapical inflammation. As a bacterial disease, common pathogenic bacteria include Porphyromonas gingivalis, actinobacillus, fusobacterium nucleatum, fusobacterium Fusarium and the like. Periapical periodontitis which is not treated in time can cause symptoms such as acute apical abscess, persistent jumping pain and the like.
The treatment of periodontitis and periapical periodontitis consists of relieving symptoms and stopping the pathological process, and the basic process can be divided into two steps: firstly, incision drainage and decompression are carried out on abscess at a focus to eliminate the symptom of jumping pain; subsequently, the focus part needs to be sterilized and washed to prevent the further development of the disease. The current clinical treatment for periodontitis and periapical periodontitis is supragingival scaling or subgingival scaling, with the drug used to rinse the periodontal pocket and the subsequent treatment given a suitable amount of antibiotic to assist in the anti-inflammatory process. For severe patients, the gingiva is opened, and the focus part is exposed for washing and curettage; when the teeth loose, the diseased teeth need to be removed. The debridement anti-inflammation method can diminish inflammation only by injecting or inhaling the medicine for a period of time, has no targeting effect and has a long treatment period. The common photodynamic therapy device and the 'tapered root canal light guide needle' are only suitable for small-area therapy.

The connector is provided with four sections of clamping grooves, the mounting connector is correspondingly provided with four sections of grooves, the clamping grooves are clamped in the grooves, and the connector and the mounting connector are connected and detached in an interference manner.
The rated luminous power of the LED working light source is 25mW; the LED working light source is a red-blue variable light source, and the value range of the red light wavelength is 630nm to 650nm; the blue light wavelength ranges from 430nm to 450nm. Generally, the model is 3535RGB lamp beads and a control chip.
The design principle of the pure flat light guide working tip is as follows: aiming at the requirements of sterilizing and disinfecting a large area of periodontitis and periapical periodontitis in actual treatment, the tail part of the working tip is designed into a smooth surface so as to simultaneously meet the requirements of finer treatment in the oral cavity and irradiation of light rays in the process in a large range. In actual use, the lamp beads emit light rays which pass through the pure flat light guide working tip, a small part of the light rays are scattered to the outside, most of the light rays are emitted from the tail end of the working tip and finally irradiate on the photosensitizer, and therefore sterilization and inflammation diminishing operations for treating periodontitis and periapical periodontitis are achieved by matching with the photosensitizer.
The operating principle of the pure plain light guide working tip for realizing the sterilizing and inflammation diminishing operations of periodontitis and periapical periodontitis is as follows:
(1) Under an aerobic environment, the toluidine blue O photosensitizer used can be attached to bacteria, and the photosensitizer is in a singlet state;
(2) After being irradiated by an external light source, the photosensitizer is excited to jump to an excited singlet state and then rapidly cross to an excited triplet state;
(3) The triplet state of the photosensitizer directly collides with tissue oxygen, energy is released and returns to a singlet state, and the released energy is absorbed by the tissue oxygen to generate singlet active oxygen;
(4) The active oxygen can generate rapid lipid oxidation effect on bacteria, and the metabolism of the bacteria is influenced and rapidly died, thereby realizing the sterilization and anti-inflammation effects.
The creative characteristics and advantages of the pure flat light guide working tip in the technical aspect are as follows:
(1) Aiming at the actual conditions of treating periodontitis and periapical periodontitis, the oral cavity treatment device adopts a shape with relatively large volume and smooth tail end, so that the requirement of realizing sterilization and inflammation diminishing in a large area at one time in the treatment of the oral cavity diseases can be met;
(2) Compared with the existing oral cavity photodynamic sterilization device, the application range is wider, the large-area irradiation and fine operation can be considered due to the reasonable size design of the tail part of the working tip, so that the periodontitis and the periapical periodontitis can be treated simultaneously, and in actual use, the two focuses are injected with the photosensitizer and then irradiated.
Pure flat leaded light work point front end shape is the round platform form, and length is 2cm for the light source that shines, through several times illumination experiments, discovers that the light that the lamp pearl sent can partly scatter to the environment in the in-service use to take place the loss, consequently pure flat leaded light work point actual luminous power is 12mW to 16mW, leads to lamp pearl luminous power to be greater than the work point luminous power. The light power of the pure flat light guide working tip is 12mW to 16mW. The focus part with larger area can be conveniently and directly irradiated to control the photosensitizer to play a role of sterilization. The height of the shell is about 17cm, the weight of the shell is about 150g, and the shell is made of polyvinyl chloride (PVC).
The pure flat light guide working tip is made of bisphenol A type carbonate, namely PC, and is used for irradiation of peri-implantitis and peri-apical periodontitis focuses.
The variable LED lamp bead is hemispherical, light beams are emitted to the periphery, after the variable LED lamp bead is connected with the pure flat light guide working tip, partial light beams are converged at the tail end, and partial light beams are dissipated in the transmission process. Multiple biological experiments prove that the optimal treatment power for periodontitis and periapical periodontitis is 12-16 mW, the teeth are damaged due to overhigh optical power, and the treatment effect is poor due to overlow power. Because of the light power loss, after being replaced for many times, 3535RGB lamp beads with the rated light power of 25mW and a control chip are finally adopted.
The relation between the supply voltage U (V) of the device and the output power P (W) of the pure flat light guide work tip is as follows:
Figure 253162DEST_PATH_IMAGE001
the relation between the output power P (W) of the pure flat light guide working tip and the efficiency E (%) of the Porphyromonas gingivalis of the device is as follows:
Figure 459016DEST_PATH_IMAGE002
the invention has the beneficial effects that:
1. introduces photodynamic therapy to treat periodontitis and periapical periodontitis simultaneously, and solves the defects of the existing clinical technology.
2. The LED light source is used for replacing a semiconductor light source, the LED light source is low in cost and low in power, damage to teeth caused by heat generated by the light source is reduced, and the LED (630 nm) and the semiconductor laser (810 nm) are used for irradiating the Porphyromonas gingivalis, so that the LED (630 nm) can effectively eliminate the Porphyromonas gingivalis compared with the semiconductor laser (810 nm).
3. The disposable pure flat light guide operating tip is arranged at the head of the device main body and used for converging light emitted by the lamp beads, so that the light is ensured to irradiate a focus, the periodontal and tooth apex sterilization operation is more convenient and sensitive, and the possibility of secondary pollution is reduced.
4. The normal light dynamic treatment device is only suitable for small-area treatment, and the pure flat light guide operating tip device adopts a method of directly adopting a photodynamic treatment method to treat focus, so that large-area treatment can be realized, namely, the treatment requirement on large-area focus in the oral cavity such as periapical periodontitis or periodontitis can be met at the same time, two diseases can be realized, the same treatment method is adopted, the treatment is carried out at one time, and the burden of a patient is relieved.
5. Compared with a conical root canal light guide needle, the needle is more suitable for treating large-area focuses, and the focuses need to be exposed properly during actual operation. For example, in the treatment of periapical inflammation, it is necessary to incise the gingiva; when treating peridentitis, forceps are needed to properly open the periodontal pocket to ensure that the light can be sufficiently irradiated.

(2) Compared with the existing oral cavity photodynamic sterilization device, the application range is wider, the treatment of periodontitis and periapical periodontitis can be realized simultaneously due to the fact that the tail part of the working tip is larger, and in actual use, the two focuses are injected with photosensitizer and irradiated.
The front end of the pure flat light guide working tip 3 is in a circular truncated cone shape, the length of the pure flat light guide working tip is 2cm, the pure flat light guide working tip is used for illuminating a light source, and the light power of the pure flat light guide working tip is 12-16 mW. The direct irradiation of the focus part with large area is convenient to control the photosensitizer to play a role of sterilization. The height of the shell 2 is about 17cm, the weight is about 150g, and the material of the shell 2 is polyvinyl chloride, namely PVC.
The pure plain light guide working tip 3 is made of bisphenol A type carbonate, namely PC, and is used for irradiation of peridentitis implantitis and periapical periodontitis focuses.
The variable LED lamp bead is hemispherical, light beams are emitted to the periphery, after the variable LED lamp bead is connected with the pure flat light guide working tip 3, partial light beams are converged at the tail end, and partial light beams are dissipated in the transmission process. Multiple biological experiments prove that the optimal treatment power for dental caries plaque and periodontitis is 11-16 mW, the teeth are damaged due to overhigh optical power, and the treatment effect is poor due to overlow power. Because of the light power loss, after being replaced for many times, 3535RGB lamp beads with the rated light power of 25mW and a control chip are finally adopted.
The relation between the supply voltage U (V) of the battery pack 21 and the output power P (W) of the pure flat light guide work tip 3 is as follows:
Figure 982401DEST_PATH_IMAGE001
the relationship between the output power P (W) of the device pure flat light guide working tip 3 and the efficiency E (%) of the Porphyromonas gingivalis is as follows:
