技术领域
本文中公开的各实施例涉及各种医疗装置和相关部件,包括机器人和/或体内医疗装置和相关部件。某些实施例包括各种机器人医疗装置,包括设置在体腔内并且使用通过体腔中的孔口或开口设置的支撑部件定位的机器人装置。进一步的实施例涉及用于操作以上装置的方法和装置。
背景技术
侵入性外科手术对于解决各种医疗状况是必要的。当可能时,诸如腹腔镜的微创手术是优选的。
然而,已知的微创技术(例如腹腔镜)在范围和复杂性上受到限制,部分原因在于:1)使用通过进入端口***的刚性工具而导致的移动限制,以及2)有限的视觉反馈。诸如da外科***(从位于Sunnyvale,CA的Intuitive Surgical公司可获得)的已知的机器人***也受到进入端口的限制,以及具有非常大、非常昂贵、在大多数医院不可获得并且具有有限的感测和移动能力的额外缺点。
本领域中存在对于改进的外科方法、***和装置的需求。
发明内容

本文中公开的某些实施方式涉及可以以各种构造组装的“组合”或“模块化”医疗装置。出于本申请的目的,“组合装置”和“模块化装置”均应意味着具有可以以各种不同构造布置的模块化或可互换部件的任何医疗装置。本文中公开的模块化部件和组合装置还包括分段的三角形或四边形组合装置。由被连接以形成三角形或四边形构造的模块化部件(本文中也被称为“区段”)构成的这些装置可在使用期间提供杠杆作用(leverage)和/或稳定性,同时还在装置内提供可用于更大的部件或更多可操作的部件的大量有效载荷空间。与以上公开和讨论的各种组合装置一样,根据一个实施例,这些三角形或四边形装置可以以与以上讨论和公开的那些装置相同的方式被定位在患者的体腔内侧。
本文中公开或设想的某些实施例可用于结肠切除、执行为治疗患有下胃肠疾病(例如憩室炎、克罗恩病、炎性肠病和结肠癌)的患者的外科手术。已知结肠切除手术中大约三分之二的是经由完全开放的外科手术执行的,其涉及8至12英寸的切口和长达6周的恢复时间。由于该手术的复杂性,现有的机器人辅助外科装置很少用于结肠切除手术,并且手动腹腔镜方法仅在三分之一的情况中使用。相反,本文中公开的各种实施方式可用于典型地由已知技术执行的“开放”的各种手术的微创方法中,具有改善临床结果和健康护理费用的潜力。此外,可以使用本文中公开的各种实施方式来代替从患者体外进入身体的已知的主机状腹腔镜外科机器人。也就是说,本文中公开的侵入性较小的机器人***、方法和装置具有较小的、自容的外科装置的特征,该外科装置通过患者的腹部中的单个切口以其整体被***。被设计成利用外科医生熟悉的现有工具和技术,本文中公开的装置将不需要专用手术室或专门的基础设施,并且由于其小得多的尺寸,因此预期比用于腹腔镜外科手术的现有机器人替代物显著地花费更少。由于这些技术进步,本文中的各种实施例可以允许当今在开放外科手术中执行的手术的微创方法。

本文中描述的各种***实施例用于执行机器人外科手术。该***用于腹腔中的一般外科手术应用,包括结肠切除术。在某些实施方式中,本文中描述的各种***基于和/或利用在手动腹腔镜外科手术中使用的技术,包括腹腔的吹气和端口的使用以将工具***腹腔中。
各种***实施例的主要部件包括机器人和外科控制台。各机器人实施方式被配置成被***吹气的腹腔中。某些机器人实施例具有捕获外科目标的视图的集成摄像机***。然后,外科医生可以使用在显示器上的该视图来帮助控制机器人的运动。在某些实施方式中,摄像机被设计成使得其能被移除以便其能被清洁并用于其它应用中。
根据一些实施例,外科控制台具有显示器来观察来自摄像机的反馈。该显示器还可以具有画中画(overlay)以向外科医生提供一些附加信息,包括机器人的状态和其它信息。控制台还可以具有用于控制各种***功能的触摸屏。另外,各种控制台实施例还可以具有使用者输入装置(例如触觉操纵杆),外科医生可以使用该使用者输入装置来控制机器人的臂的运动及其它运动。此外,控制台还可以具有用于控制各种机器人控制和功能的一个或多个踏板。
如将在本文中进一步详细讨论的其它实施例中,该***可包括一次性或永久套管、电外科烧灼发生器、***端口、支撑臂/结构、摄像机、遥控外科显示器、末端执行器(工具)、接口盒、光源及其它支持部件。
图1A和图1B描绘了具有机器人或机器人装置10与摄像机12的***1的一个实施例。如图1A所示,机器人装置10具有可操作地耦接到其上的两个机器人臂14、16以及设置在两个臂14、16之间并且在其中可定位的摄像机部件或“摄像机”12。也就是说,装置10具有第一(或“右”)臂14和第二(或“左”)臂16,两个臂都被可操作地耦接到装置10,如下面另外详细讨论的。如图所示的装置10具有壳体(也被称为“盖”或“外壳”)11。装置10也被称为“装置本体”10A并且具有两个可旋转的圆柱形部件(也被称为“肩部”或“转台”):第一(或“右”)肩部14A和第二(或“左”)肩部16A。每个臂14、16还具有上臂(本文中也被称为“内臂”、“内臂组件”、“内连杆”、“内连杆组件”、“上臂组件”、“第一连杆”或“第一连杆组件”)14B、16B以及前臂(本文中也被称为“外臂”、“外臂组件”、“外连杆”、“外连杆组件”、“前臂组件”、“第二连杆”或“第二连杆组件”)14C、16C。右上臂14B在右肩部关节14D处被可操作地耦接到本体10A的右肩部14A并且左上臂16B在左肩部关节16D处被可操作地耦接到本体10的左肩部16A。此外,对于每个臂14、16,前臂14C、16C在肘关节14E、16E处被可旋转地耦接到上臂14B、16B。
如图1B所示,机器人装置10已经以类似于其会被***患者的腹腔6中的方式穿过凝胶端口7被***腹腔6的模型中。凝胶端口7允许在保持吹气压力的同时不规则形状的机器人装置10被***。在该实施方式中,除了机器人10之外还使用了标准的手动腹腔镜端口7。可替代地,可以利用两个或更多这样的端口(未示出)。在进一步的替代方案中,不使用标准手动腹腔镜端口。
在图1B中,装置本体10A被示出为已经以腹侧-背侧定向被***腹腔中使得纵向本体轴线(如由参考箭头A所示)相对于尾侧/前后以及中侧轴线(分别参考箭头B和C)大体上垂直。应当理解,在***之后,装置本体10A可以被各种方式定位,以便相对于腔体6旋转、倾斜或成角度以改变装置工作空间并进入腔体的各种区域,如以下关于图6A至图8C详细描述的。
图2示出了根据一个实施例的带有集成摄像机***的机器人。图2的机器人具有两个臂14、16和具有远端10B与近端10C的本体10A(或躯干)。臂14、16各自具有主动自由度以及附加的主动关节14F、16F以致动末端执行器或工具18、20。应当理解,可以包括更多或更少的自由度。该实施例中的装置具有包括电力、电烧灼和信息/通信信号的连接线8(也被称为“尾线缆(pigtail cable)”)(部分示出)。在某些实施方式中,该装置具有分布式控制电子器件和软件以帮助控制装置10。可以包括一些按钮以支持装置到腹腔中的***和从腹腔的抽出。在该实施例中,集成摄像机12也被示出为被***装置本体10A中。当被***本体10A中时,摄像机12具有从近侧本体端部10C向近侧延伸的手柄或本体12A以及从远侧本体端部10B延伸的柔性摄像机成像器12B。
图3和图4描绘了根据一个实施例的其中移除了摄像机组件12的机器人装置10。在这些实施例中,并且如图2和图3至图4所示,摄像机成像器12B被设计成定位在两个臂14、16之间并捕获该两个臂14、16之间的视图。在这些实施方式中,摄像机12延伸穿过机器人本体10A使得摄像机成像器12B于本体与机器人臂(“肩部”关节14A、16A)之间的关节附近离开。摄像机12具有柔性、可转向末梢部12C以允许使用者调节观察方向。臂14、16的远端上的末端执行器18、20可包括各种工具18、20(剪刀、抓取器、针驱动器等)。在某些实施例中,工具18、20被设计成通过将末端执行器耦接到臂14、16的工具旋钮的小扭转而可移除。
如图3至图4所示,摄像机组件12具有手柄12A和长轴12D,长轴12D具有在远侧末梢部12C处的摄像机成像器12B。在各种实施方式中,柔性末梢部12C并且因此摄像机成像器12B可以在柔性区段12E(轴上的黑色区段)处相对于轴12D在两个独立方向上转向或以其它方式移动以改变观察的方向。在某些实施方式中,摄像机12具有一些如图所示的控制按钮12F。在一些实施例中,摄像机组件12可以独立于机器人装置10使用,如图4所示。
可替代地,如图所示,组件可以穿过通过机器人装置10的本体10A限定的内腔10D被***机器人10中。在某些实施例中,内腔10D包括密封件/端口10E以确保患者的腔体保持被吹气(如关于图1B所示)。根据一个实施例,机器人装置10可以具有传感器来确定摄像机是否被定位在装置10的摄像机内腔10D中。
图5描绘了根据一个实施例的机器人装置10,该机器人装置10处于其中可定位臂14、16被定位成使得工具18、20被定位成与摄像机末梢部12C成一直线的构造。也就是说,在该实施例中,臂14、16被设置在工作空间中以便处于摄像机成像器12B的视场(由参考线“V1”和“V2”表示)内。在图5的实施方式中,装置10以一定角度——也就是说,使得装置本体10A的纵向轴线(由参考线A表示)不垂直于患者的身体(例如,如图1B中所示)地定位在患者的腔体内。
在图5A的实施方式中,装置本体10A因此被定向成具有“顶”、“上”或“正”侧22和“底”、“下”或“背”侧24。应当理解,进一步的构造是可能的,并且如本文中详细描述的,摄像机12和臂14、16能够延伸到任一侧22、24中以便提供较大的工作空间而无需旋转装置本体10A。
在图5B所示的实施方式中,机器人装置10的臂14、16被定位成“***”构造。如图所示,在***构造中,臂14、16和摄像机12都主要与机器人装置本体10A对准使得部件中的每一个的纵向轴线大致上彼此平行(如由参考箭头I所示)用于穿过端口***(例如,如图1B中在7处所示)。应当理解,***构造使装置10的总体“占地面积(footprint)”最小化,以便允许最小的可能切口。在某些实施方式中,在***期间,装置10可以在被***时穿过各种位置,如先前已经在2016年8月3日提交并且题为“Robotic Surgical Devices,Systems,andRelated Methods,”的美国专利申请No.15/227,813中所描述的,该申请内容通过引用以其整体并入本文。
在某些实施方式中,***1的原理优点是对于臂的较宽的工作空间范围,包括其中臂被定位在装置“后面”的实施例。在使用中,增加每个臂的工作空间范围可以减少重新定位到装置的需要,并且因此导致更高的效率和更快的总外科手术时间和恢复。本文中描述了示出增加的臂范围的若干实施方式。
图6A、6B、7A、7B和7C示意性地描绘了根据某些实施例的机器人装置10的整个工作空间30以及臂14、16中的每一个的各自可达到的工作空间30A、30B。在这些实施例中,“工作空间”30意味着围绕机器人装置10的空间30,其中任意臂和/或末端执行器18、20可以在该空间内移动、进入和执行其功能。
更特别地,图6A描绘了装置本体10A的透视图并且进一步示意性地示出了整个工作空间30以及分别示出第一臂14和第二臂16的各自工作空间30A、30B。注意到,每个臂14、16具有运动的范围和相应的工作空间30A、30B,所述工作空间30A、30B从装置的正侧22延伸到装置10的背侧24。因此,对于每个臂14、16,通过相对于装置本体10A的轴线大约180°的空间,第一臂14均等地到达正侧22和背侧24。该工作空间30允许机器人装置同样良好地工作到正侧22和背侧24而不必重新定位本体10A。
如图6B中最佳所示,在这些实施方式中各个臂的运动范围的重叠也允许了相交工作空间30C(也如图6A中所示)。应当理解,在这些实施方式中相交工作空间30C包含在任何独立的装置10位置中由臂14、16和末端执行器18、20均可到达的工作空间30C。同样地,在这些实施方式中,相交工作空间30C包括相对于装置本体10A的轴线的空间的大约180°的范围。
图7A描绘了装置本体10A的侧视图并且进一步示意性地示出了第一臂14的工作空间30A。注意到,第一臂14具有从装置的正侧22延伸到装置10的背侧24的运动范围。因此,第一臂14均等地到达正侧22和背侧24。这允许机器人装置同样良好地工作到正侧22和背侧24而不必重新定位本体10A。关于臂14、16的实际位置,图7A描绘了第一臂14从装置的正侧22延伸出而第二臂16从背侧24延伸出。
类似地,图7B和7C描绘了图7A的装置本体10A和臂14、16的不同视图。例如图7B描绘了本体10A和臂14、16的俯视图。在该实施例中,第一臂14的工作空间30A和第二臂16的工作空间30B均从俯视图示出。此外,图7C从透视图描绘了本体10A和臂14、16,该透视图示出了工作空间30A、30B的另一个角度。
在图7A至图7C中的每一个中,示出了本体10A和臂14、16的相同构造,其中第一臂14从装置的正侧22延伸出而第二臂16从背侧24延伸出(如图7A中最佳所示)。由机器人装置10的两个臂14、16的工作空间30A、30B所例证的该较宽的运动范围给予机器人装置10当与其臂14、16的长度相比时相对较大的工作空间。
图8A、图8B和图8C进一步描绘了根据一个实施例的可由该特定装置10的臂实现的较宽的运动范围。图8A描绘了装置10的背侧的透视图,在该透视图中臂14、16都被描绘在大致上类似于图7A至图7C中所示的单个位置:第一臂14远离装置本体10A的正侧22延伸,而第二臂16远离装置本体10A的背侧24延伸。
图8B描绘了装置10的侧视图,在该侧视图中第一臂14被描绘在多个不同位置,包括第一位置14-1、第二位置14-2、第三位置14-3和第四位置14-4,由此提供臂(在这种情况下,第一臂14)能够的运动范围的一些示例。
图8C的实施方式描绘了装置10的透视正视图,在该图中第一臂14被再次描绘在与图8B中所示相同的位置,包括工作空间30A内的第一位置14-1、第二位置14-2、第三位置14-3和第四位置14-4。本领域技术人员会理解,所示的那些位置之间的许多另外的位置也是可能的,并且第一臂14的这些位置对于第二臂16也是可能的。
图9是带有从装置本体10A的远端10B延伸的铰接或柔性摄像机12的装置10的一个实施方式的透视正视图。在这些实施方式中,摄像机12在末梢部12C上具有远侧透镜12B,以及包围柔性区段12E的柔性护套15。在图9A中,摄像机12和臂总体上被定向在略微“下部”的工作位置,其中末梢部12C远离本体10A的正侧22定向。同样地,应当理解,在这些实施方式中,摄像机12可以因此被定位成最佳地观察末端执行器或工具18、20。还应理解,在这些实施方式中,机器人10在向前表面22上离开本体。
图10描绘了装置10的进一步实施方式,其中臂处于“上部”或“正常”位置,在该位置摄像机略微朝向本体10A的正侧22成角度。此外,图10的装置具有在肩部14A、16A与装置本体10A之间的近侧套管附件32、34。套管附件32、34可以是“凹槽”,在该处两个凸缘32A、32B、34A、34B围绕每个肩部轴36、38设置。应当理解,凸缘32A、32B、34A、34B被配置或以其它方式构造和布置使得永久和/或一次性套管(未示出,但如所并入的参考文献中所讨论的)可以在对应的凸缘32A、32B、34A、34B之间被附接并保持就位。对于每个套管的对应远侧配合区域40、42(未示出)被设置在前臂14C、16C的远端上以及在每个工具18、20的基部处。
图11描绘了具有与图9和10的位置相比大致上“下部”定位的臂14、16的机器人10的进一步实施方式。也就是说,在图11中,摄像机末梢部12C在工作空间30的区域内在背侧24上(与前侧22相反)从机器人本体10A的纵向轴线(参考箭头A)垂直地定向,并且摄像机12被设置成使得臂14、16并且更特别地工具或末端执行器18、20在视场(大体上以参考箭头V示出)内。在该实施方式中,各种操作线缆45也被示出为被连接到装置本体10A和摄像机12。
图12A至图12F描绘了机器人10-1、10-2的替代实施方式。在第一实施方式中,并且如图12A、12C和12E所示,机器人10-1具有倾斜的远侧本体10B-1部分48,摄像机12从该部分内延伸。在第二实施方式中,如图12B、图12D和图12F所示,机器人10-2摄像机12从远侧本体端部10B-2延伸。在这些实施方式中,臂14、16具有在远侧本体端部10B上平行——侧向地和分离地——设置的大体上圆柱形上连杆或肩部14A、16A,使得在肩部14A、16A之间有“间隙”或开口46。在这些实施方式中,摄像机12在开口46内从装置本体10B的远端延伸,以便正好在大体上圆柱形肩部14A、16A之间并且在前侧22与背侧24之间等距。在这些实施方式中,摄像机12可以因此弯曲以均等地向前和向后观察,例如,如关于图6A至图8C所示。
图13至图30描绘了本体10A的内部部件,这些内部部件在这些图中被示出而没有其壳体或外壳11。应当理解在使用中,这些实施方式被覆盖,如关于图1A所示。图13至图30包括本体10A的内部结构或支撑部件。这些部件保持本体12的结构并为设置在本体12中的部件提供结构支撑。
在使用中,有许多方式来致动机器人10及其相关联的部件,例如直流电机、交流电机、永磁直流电机、无刷电机、气动装置、到遥控电机的线缆、液压装置及类似物。关于图13至图30描述了一种可能***的更详细描述。如会被理解的,还可以实施在先前提交和并入的申请和专利中描述的其它技术来致动各种部件。
图13示出了机器人10和一个臂——在此为左臂16的每个关节的一个实施方式。应当理解,该实施方式的右臂14是左臂16的镜像。应当理解,左臂16中的操作/控制/致动左臂16的内部部件与本文中描绘和描述的那些部件大致上相同并且以下提供的描述也同样地适用于那些部件。
在图14的实施方式中,肩部偏航关节100致动机器人肩部14A、16A中的偏航关节100。在该实施方式中,机器人10还具有肩部俯仰关节102,也就是说,机器人肩部14A、6A上的俯仰关节102。在这些实施方式中,还提供了允许关于下面的表1中描述的运动范围的上臂翻滚关节104、肘关节106和工具翻滚关节108。在各种实施方式中,工具致动关节(未示出)与工具(未示出)对接以致动工具的打开和关闭,如先前已经描述的。
在各种实施方式中,这些关节100、102、104、106具有实际限定的运动范围,该运动范围与机器人几何形状一起导致机器人10的最终工作空间。对于本文中给出的示例,关节限制允许显著的机器人工作空间,如上所述。该工作空间允许机器人的各种实施方式以在患者的体腔内的若干位置同时有效地使用臂和手。表1中给出了在图13A至图27的实施方式中限定的关节运动范围。应当理解,进一步的范围是可能的,并且因此该组范围不是限制性的,而是具体实施例的代表。此外,替代实施例是可能的。
旋转方向和零位置在图13A至图13D中示出。在图13A至图13D中,示出了机器人10,其中前四个角度中的每一个都处于零位置。在这些实施方式中,每个关节(肩部偏航关节100、肩部翻滚关节102、上臂翻滚关节104和肘关节106)被示出为具有旋转轴线(点线)和零位置。然后使用箭头来指示围绕旋转轴线的正关节角度的方向。由于工具翻滚关节108和工具致动关节109允许连续旋转,因此零位置是任意的并且未示出。

在图14的实施方式中,本体10A和每个连杆(意味着上臂16B和前臂16C)包含具有嵌入式传感器、放大器和控制电子器件的印刷电路板(“PCB”)110、112、114。一个PCB处于每个前臂和上臂中并且两个PCB处于本体中。每个PCB还具有完整的六轴基于加速度计的惯性测量单元和可用于监控电机的温度的温度传感器。每个关节还可以具有绝对位置传感器或增量位置传感器中任一个或两者都有。在某些实施方式中,一些关节同时包含绝对位置传感器(磁编码器)和增量传感器(霍尔效应)。在其它实施方式中,某些关节仅具有增量传感器。这些传感器用于电机控制。各关节还可以包含许多其它类型的传感器。在此包括了一种可能方法的更详细描述。
在该实施方式中,较大的PCB 110被安装到本体10A的后侧。该本体PCB 110控制基部连杆或本体10A中的电机116(分别用于左臂和右臂的肩部偏航关节100和肩部俯仰关节102)。每个上臂具有PCB 112以控制上臂翻滚关节104和肘关节106。每个前臂具有PCB 114以控制工具翻滚关节108和工具致动关节(未示出)。在图14的实施方式中,每个PCB 110、112、114还具有完整的六轴基于加速度计的惯性测量单元和可用于监控本文中描述的各种电机的温度的多个温度传感器。
在这些实施例中,每个关节100、102、104、106、108还可以具有绝对位置传感器或增量位置传感器或两者都有,如在2012年8月8日提交的美国临时申请61,680,809中所描述和公开的,该申请内容通过引用以其整体并入本文。在一个实施方式中,并且如图15中和其它地方所示,本文中描述的各种致动器或电机116、130、154、178具有设置在电机的表面上的至少一个温度传感器101(例如通过温度敏感的环氧树脂)使得温度传感器(如图22中在101处所示)可以收集来自每个致动器的温度信息以传输到控制单元,如以下所讨论的。在一个实施例中,本文中讨论和描述的任何电机可以是有刷或无刷电机。此外,电机可以为例如6mm、8mm或10mm直径电机。可替代地,可以使用可以被集成到医疗装置中的任何已知尺寸。在进一步的替代方案中,致动器可以是在医疗装置中使用以致动部件的运动或动作的任何已知致动器。可用于本文中描述的电机的电机的示例包括EC 10 BLDC+GP10A行星减速器、EC 8 BLDC+GP8A行星减速器、或者EC 6 BLDC+GP6A行星减速器,所有这些都从位于FallRiver,MA的Maxon Motors可获得。有许多方式来致动这些运动,例如利用直流电机、交流电机、永磁直流电机、无刷电机、气动装置、到遥控电机的线缆、液压装置及类似物。进一步的实施方式可以与2016年8月3日提交并且题为“Robotic Surgical Devices,Systems,andRelated Methods”的美国专利申请No.15/227,813中公开的各种***、方法和装置相结合使用,该申请内容通过以其整体并入本文。
在该实施方式中,关节1-4既具有绝对位置传感器(磁编码器)又具有增量传感器(霍尔效应)。关节5和6仅具有增量传感器。这些传感器用于电机控制。应当理解,各关节还可以包含许多其它类型的传感器,如在并入的申请和参考文献中已经详细描述的。
根据一个实施方式,图15至图16的实施方式中描绘的某些其它内部部件被配置成致动本体10A的肩部偏航关节100围绕轴线1的旋转,如图14所示。应当理解,所描述的部件中的每一个使用了两个内部部件——每个臂一个——但是为了描述的容易,在某些描绘和描述中仅使用一个内部部件。
如图15中最佳所示,肩部偏航关节100、电机116和减速器组合驱动电机齿轮117第一正齿轮组118,其在图16中最佳地示出。第一正齿轮组118驱动由轴承120支撑的轴以驱动第二正齿轮组122。该第二正齿轮组122进而驱动也由轴承126支撑的输出轴124。该输出轴124然后驱动转台14A、16A(代表机器人10的肩部)使得肩部16A围绕轴线1旋转,如图14中最佳所示。
根据一个实施方式,图17至图19的实施方式中描绘的某些内部部件被配置成围绕轴线2致动本体10A的肩部俯仰关节102和/或肩部14A、16A,如图14所示。在这些实施方式中,俯仰关节102被构造和布置成使输出连杆140枢转以便使上臂(未示出)相对于肩部14A、16A移动。
在该实施方式中,电机130和减速器组合驱动电机齿轮131和正齿轮132,正齿轮132进而驱动第一轴134。该轴134然后驱动肩部转台内侧的斜(或伞)齿轮对136、137(在图19中描绘)。斜(或伞)齿轮对136、137相应地驱动直接连接到肩部俯仰关节102、输出连杆140的螺旋正齿轮组138、139,使得上臂16B围绕轴线2旋转,如图14中最佳所示。在该实施方式中,肩部偏航关节100和肩部俯仰关节102因此具有耦接运动。在这些实施方式中,多个轴承141支撑各种齿轮和其它部件,如先前已经描述的。
图20至图23描绘了被构造和布置用于臂16的运动和操作的上臂16B的各种内部部件。在各种实施方式中,多个致动器或电机142、154被设置在前臂16C的外壳(未示出)内。图24至图27描绘了被构造和布置用于末端执行器的运动和操作的前臂16C的各种内部部件。在各种实施方式中,多个致动器或电机175、178被设置在前臂16C的外壳(未示出)内。
在一个实施方式中,并且如图22和其它地方所示,本文中描述的各种致动器或电机116、130、154、178具有设置在电机的表面上的至少一个温度传感器101(例如通过温度敏感的环氧树脂),使得温度传感器可以收集来自每个致动器的温度信息以传输到控制单元,如以下所讨论的。在一个实施例中,本文中讨论和描绘的任何电机可以是有刷或无刷电机。此外,电机可以为例如6mm、8mm或10mm直径电机。可替代地,可以使用可以被集成到医疗装置中的任何已知尺寸。在进一步的替代方案中,致动器可以是在医疗装置中使用以致动部件的运动或动作的任何已知致动器。可用于本文中描述的电机的电机的示例包括EC 10BLDC+GP10A行星减速器、EC 8 BLDC+GP8A行星减速器、或者EC 6 BLDC+GP6A行星减速器,所有这些都从位于Fall River,MA的Maxon Motors可获得。有许多方式来致动这些运动,例如利用直流电机、交流电机、永磁直流电机、无刷电机、气动装置、到遥控电机的线缆、液压装置及类似物。
在图20至图21中示出了被构造和布置成致动上臂翻滚关节104的上臂16B的各内部部件的一个实施方式。在该实施方式中,由PCB112控制的电机142和减速器组合驱动电机齿轮143和对应的正齿轮144,其中输出正齿轮144由轴148和轴承150支撑。输出轴152和输出正齿轮144可以具有配合到肩部俯仰关节102输出连杆140的配合特征146(如图17所示)。
在图22至图23中示出了被配置成操作肘关节106的上臂16B的内部部件的一个实施方式。在该实施方式中,基础电机154直接驱动包括三个齿轮156、158、160的从动正齿轮组。该正齿轮组156、158、160将旋转轴线从电机154的轴线转移到蜗轮166的轴线。
如图23中最佳所示,来自该组的输出正齿轮160驱动电机减速器162,该电机减速器162驱动其上安装有蜗轮166的蜗杆轴164。该蜗轮166然后驱动被连接到关节4输出轴170的蜗轮168。还应注意,上臂单元(如图22所示)在右侧上示出了弯曲的凹部区域172。应当理解,该区域172被配置成允许关节4的更大运动以便允许前臂穿过区域172。
在图24至图25中示出了被配置或以其它方式构造和布置成操作工具翻滚关节108的前臂16C的各内部部件的一个实施方式。在这些实施方式中,工具翻滚关节108驱动保持工具(例如,在图1A-1B中在18、20处示出)的工具内腔174。工具内腔174被设计成与工具上的翻滚特征啮合以造成工具围绕该工具的轴线(如图14中的轴线5所示)旋转。在该实施方式中,带有减速器的工具翻滚电机175被用于驱动电机齿轮176和带有两个齿轮177A、177B的正齿轮链。该链的最后一个齿轮177B被刚性地安装到工具内腔174,以便旋转工具内腔的内表面174A,并对应地旋转任何***的末端执行器。
工具致动关节109的一个实施方式在图26至图27中示出。在该实施方式中,关节6、电机178不会可视地移动机器人。相反,该工具致动关节109驱动与工具(例如,在图1A至图1B中在18、20处所示)对接并且被配置成致动末端执行器打开和关闭的母花键184。末端执行器臂的使得末端执行器打开和关闭的该旋转也被称为“工具驱动”。在一个方面,致动如下产生。提供致动器178,也就是说,在该实施方式中为电机组件178。该电机组件178被可操作地耦接到电机齿轮180,该电机齿轮180在该实施例中是正齿轮。电机齿轮180被耦接到第一从动齿轮182和第二从动齿轮183使得电机齿轮180的旋转造成从动齿轮182、183的旋转。从动齿轮182、183被固定地耦接到由轴承对186支撑的母工具花键184。该母工具花键184被配置成与末端执行器上的公工具花键特征对接以按照指示打开/关闭工具。
根据一个实施方式,末端执行器(在图1A至图1B中在18、20处示出)可以以下列方式快速且容易地耦接到前臂16C和从前臂16C脱离。在翻滚轴线和驱动轴线都固定或保持就位的情况下,末端执行器18、20可以旋转,由此耦接或脱离螺纹(未示出)。也就是说,如果末端执行器在一个方向上旋转,则末端执行器被耦接到前臂16B,并且如果其在另一个方向上旋转,则末端执行器与前臂16B脱离。
***10的各种实施方式还被设计成将能量传递到末端执行器以便在外科手术期间切割和凝聚组织。这有时被称为烧灼并且可以带来许多电气形式以及热能、超声能和RF能,所有这些都是针对机器人的。
在***1和各种装置10的示例性实施方式中,摄像机12被配置或以其它方式构造和布置成允许在工作空间30内的俯仰(意味着“向上”和“向下”)运动和偏航(意味着“从一侧到另一侧”运动)二者,并且在各示例性实施方式中,偏航或“摇摆”功能经由远侧末梢部12C处的机械铰接而不是如先前已经完成的经由旋转摄像机轴12D和/或手柄12A来完成。相应地,该实施方式的摄像机部件12的各种实施方式具有两个机械自由度:偏航(向左/向右看)和倾斜(向上/向下看)。在使用中,摄像机部件12具有由手柄12A中的致动器和电子器件供电和控制的摇摆和倾斜功能,如先前已经在美国专利申请No.15/227,813中所描述的。在***的这些实施方式中,摄像机12因此能够允许使用者在整个扩展的工作空间中观察装置臂和末端执行器。本文中描述了允许这种改进的视觉范围和摄像机运动的若干装置、***和方法。
在图28A至图36C和其它地方示出了摄像机的各种实施方式和部件。如以上所讨论的,某些实施方式的摄像机12被设计成与机器人10一起作用,如图2所示。机器人摄像机12也可以独立于机器人使用,如图4所示。在各种实施方式中,摄像机12被***机器人本体10A的近端10C中,并且如图28A所示,摄像机末梢部12C在本体与臂之间在附接位置附近通过机器人本体10A的远端10B离开,如以上关于图6所描述的。在某些实施方式中,并且如关于图3所讨论的,密封件10E被包括在机器人本体10A中以便在从机器人10移除摄像机12时不会损失吹气。若干直径是可能的,但是一个实施方式具有***机器人中的6mm内腔10D中的5mm摄像机,如图28A所示。
在图28A至图28B的实施方式中,摄像机12被设计成在远端12C处以两个独立自由度弯曲。这允许使用者经由成像器12B观察在机器人工作空间内的任何位置处的机器人工具,如图28B中的1°-V°所示。在这些实施方式中,机器人内腔10D可以相对于机器人本体10A对中,如图28A至图28B所示,这允许相对于机器人臂的对称视点,或者机器人内腔10D可以更加向前,如图1A的实施方式所示,或者向后或处于其它位置。
另外,如图28A至图28B所示,摄像机12末梢部12C包含一个或多个照明部件12F以照明观察目标(如关于图1所讨论的)。在这些实施方式中,照明部件12F可以经由摄像机手柄中的独立灯箱或一些其它已知光源(未示出,但是一个非限制性示例是高亮度LED)或者其它形式的光源照亮。然后可以经由光缆将光引导通过摄像机轴12,如先前例如关于2016年8月3日提交并且题为“Robotic Surgical Devices,Systems,and Related Methods”的美国专利申请No.15/227,813已经描述的,该申请内容通过引用并入本文。
某些实施方式的附加特征允许摄像机12以各种深度被***本体10A中。这些实施方式允许在各种活动期间更好的可视化。例如,图28C至图28E、图28F至图28H和图28I示出了可以以若干深度***的摄像机12的若干实施方式,该摄像机12可以包括固定位置以使用一个或多个突起70(例如设置在摄像机本体12A的外部表面上的弹簧球70)以及设置在本体内腔10D内侧的各种深度处的对应固定环形制动部72(在图28I中最佳示出)来保持摄像机12。在使用中,制动部72以各种程度的***深度(参考箭头H)接合球70。这会允许摄像机相对于机器人臂处于更近侧(图28C至图28E)或相对于机器人臂处于更远侧(图28F至图28H)。应当理解,在替代实施方式中,设置摄像机12的其它方法是可能的,包括利用各种致动和控制机构致动的连续运动和其它***。
在摄像机手柄12的各种实施方式中,为了使用者舒适度可以提供包覆模具。各种连接器和按钮以及尾线缆组合都是可能的。在某些实施方式中,摄像机手柄12A保持至少一个电机以致动柔性末梢部12C。在一个版本中,这些电机然后可以经由外科控制台(如下所述)或其它输入装置控制以控制摄像机12的运动。该控制还可以包括其它摄像机功能,例如变焦、亮度、对比度、光强度以及许多其它特征。
如图29A至图29B所示,摄像机***的柔性铰接末梢部12C允许摄像机12实现在大致上全部机器人工作空间30上的视场(参考箭头V)。在这些实施方式中,矢状面中的一种可能的工作空间的横截面被示出。图29A至图29B例证了机器人臂14、16的运动可以围绕较大工作空间30运动并且摄像机***12必须能够始终观察机器人工具18、20。

转到摄像机末梢部12C的铰接,图34A至图34C描绘了用于实现图31A至图33B和其它地方所示的摄像机12运动的各种内部部件和装置。同样,由于在某些实施方式中可能的较大工作空间(如例如关于图6A至图6B在30处所讨论的),摄像机12的示例性实施方式被配置或以其它方式构造和布置成允许在工作空间30内的俯仰(意味着“向上”和“向下”)运动和摇摆或偏航(意味着“一侧到另一侧”运动)二者。在***的这些实施方式中,摄像机因此能够允许使用者在整个扩展的工作空间中观察装置臂和末端执行器。本文中描述了允许该改进的视场范围和摄像机移动的若干装置、***和方法。如会由本领域技术人员所理解的,本发明各示例是非限制性的,并且是出于说明的目的而示出,其中没有保护性护套(例如,在图9A中15处所示)。
摄像机末梢部12C的俯仰和偏航铰接可以通过如图34A至图34C所示的各种实施方式实现。图34A至图34B示出了连续机构。在图34A的实施方式中,摄像机能够在末梢部12C处铰接。在该实施方式中,摄像机末梢部12C经由铰接部202限定出摄像机内腔204并且在该部的任一侧上包括多个开口206A、206B以便允许装置在可能的方向(如由参考箭头A和B所示)上弯曲。应当理解在这些实施方式中,铰接部分202可以被造成经由穿过摄像机内腔204设置的线缆208A、208B在任一方向(A或B)上移动或铰接,以及经由设置在摄像机手柄12A内的电机致动。还应理解,还可以穿过该内腔204设置诸如电线、光纤及类似物的附加部件。
在图34B的实施方式中,铰接部分具有围绕限定摄像机内腔204的内部管214的若干间隔件212。在这些实施方式中,多根线缆208A、208B、208C、208D穿过间隔件212中的开口216A、21B、216C、216D设置。如会由本领域技术人员所理解的,在这些实施方式中,线缆被固定地附接到最远侧间隔件212并且被允许穿过更近侧的间隔件,使得线缆208的近侧移动导致部分202的铰接。先前已经描述了用于向近侧推动线缆208的各种方法,例如关于2016年8月3日提交并且题为“Robotic Surgical Devices,Systems,and Related Methods”的美国专利申请No.15/227,813所描述的,该申请内容通过引用并入本文。
图34C的实施方式具有设置在部分202内的互锁连杆机构220的“堆叠”。在这些实施方式中,连杆机构220在被配置成允许适当的自由度的相邻连杆220A、220B上具有对应的竖直铰接连杆222A和水平铰接连杆222B,如会由本领域技术人员所理解的。在这些实施方式中,线缆(未示出)可以延伸穿过连杆222中的开口224,如先前已经描述的。应当理解,铰接部分的这些各种实施方式允许摄像机俯仰和偏航以各种自由度的调节以便使摄像机能够观察工作空间内的若干视场而无需重新定位摄像机本体或装置。
此外,摄像机12被***装置10的深度可以变化。图35A至图35C示出了摄像机12的深度可以如何变化以改变有利位置(参考箭头V)。例如,如图35A所示,摄像机12能被完全***机器人10A中,其中成像器12B在***期间与内腔10D同轴以“自我可视化”***过程。在使用中,自我可视化允许使用者在***期间观察工具末梢部。当处于该“***”位置时,成像器12B到达距“***线”230(由参考箭头A示出)的最大距离。
如图35B至图35C所示,向前工作位置(图35B)和向后工作位置(图35C)也是可能的,其中视场(参考区域V)被对应地调节。在所描绘的实施方式中,摄像机12运动可以是手动的或机动的和受控的。如还在图35B至图35C中所示,在装置10的其中摄像机从装置的正侧上的一部分延伸(如图1A中所示)的某些实施方式中,摄像机末梢部深度将在向前和向后观察位置之间变化,如由参考箭头B所示。在某些实施方式中,并且如还关于图28A至图28I所描述的,还可以调节摄像机12在工作空间内的高度以校正这种差异。
该***的各种实施方式具有设置在臂的远端处的各种工具或者末端执行器18、20。各示例性实施方式具有可互换末端执行器或“手”的特征。在这些实施方式中,机器人“手”可以包括各种工具,例如剪刀、抓取器、针驱动器及类似物。在各种实施方式中,各工具被设计成通过工具旋钮250的小扭转而可移除,例如经由1/4转卡口连接。各工具总体上具有相对于前臂的两个致动和受控的自由度。应当理解在各种实施方式中,各工具也可以没有自由度或有一个或多个自由度。在各种实施方式中,各工具经由控制台上的使用者输入装置来控制,如先前已经描述的。第一自由度允许各工具围绕它们自己的轴线翻滚(在参考箭头R处示出)。该机器人中使用的一种类型工具具有一个自由度。图36A至图36B中所示的该工具18、20基于来自手动腹腔镜工具的钩烧灼器(hook cautery),并且具有翻滚接口252和单极滑环254。工具18、20的某些实施方式可以翻滚(参考箭头R),但是没有打开关闭功能。本文中设想到许多另外的末端执行器实施方式,如在若干并入的参考文献中所描述的。
在使用中,根据某些实施方式,装置本体10A的远端10B以及臂14、16被设置在患者体腔内,以便由使用者经由控制台遥控操作,如下所述。使用者(通常是外科医生)将装置10本体于腔体内定位在固定的初始起始位置处,并且在一些实施方式中,此后能够根据需要重新定位装置。在某些实施方式中,并且如本文中所述,本文中描述的各种支撑***利用“遥控对中”或“点追踪”途径通过重新定位来维持机器人相对于特定点(例如遥控点和/或切口或***点)的期望位置和定向。在某些实施方式中,通过在支撑结构通过若干自由度移动时约束支撑结构的移动来维持遥控对中,而某些点追踪实施方式将额外的移动施加到支撑结构上以维持位置。应当理解,某些实施方式可涉及这些和其他途径的组合。本文中描述了用于固定、定位和重新定位装置10的若干说明性***和方法。
如图37所示,在各种实施方式中,机器人10能被支撑在适当位置,图37示出了用于利用附接到手术室台303的已知夹持/支撑***302来支撑机器人10的一种方法或装置。夹持***302允许机器人在对于机器人本体可能的所有六个自由度上的位置的显著调节。应当理解,其它已知的、商业可获得的支撑***可用于保持本文中公开或设想的任何机器人装置实施例(例如,诸如机器人10)。这种已知的装置典型地保持手动腹腔镜器械(例如观测器、工具和牵开器)并且能被类似地用于夹持或以其它方式支撑机器人10或其它这样的机器人装置实施例。
图38至图39示出了遥控对中机构304的一个实施例,其有时被称为“点追踪机构”或“定位***”,其可用于支撑机器人10。根据一个实施方式,遥控对中机构304的一个优点是机构304可用于在机器人10组件的单个点保持在相同位置的同时移动装置10:机构304的遥控中心318在图38中被最佳示出。在使用中,机构304通常地被定位成使得遥控中心318定位在患者中的***点315处,如图39中最佳示出的。在遥控中心318处于***点315处的情况下,机器人10具有围绕该***点318的大约三个自由度和通过***点315与端口301的一个进/出平移自由度。在这些实施方式中,***点315可以以若干方式调节,例如通过相对于手术室床轨移动机构304以使遥控中心318与患者上的***点315对准。在一个实施例中,遥控中心318由机构304的被设计为与该遥控中心318相交的所有关节(在图38中在关节1、2、3和4处示出)而产生。如图38所示,根据一个实施方式,关节1-3是旋转关节(其中关节2是特殊的平行四边形机构)并且关节4是控制机器人到腹腔中的***深度的平移关节。根据如本文中所公开或设想的任何遥控对中机构实施方式,遥控中心318可以消除或减少机器人10与腹壁316之间的可能在机器人10被移动时产生的机械干扰。
图40A和图40B示出了根据某些实施方式的机器人10相对于腹壁316的定位。在这些实施方式中,遥控中心定位装置304(以及本文中公开或设想的任何其它定位装置实施例)允许机器人装置10访问腔体316内的工作空间30的整个范围。在这些实施方式中,定位装置304具有若干连杆机构和连杆305、306、307、308、309,包括与装置10和关节311、312、313机械连通的支撑连杆310,包括与支撑连杆310机械连通的支撑关节314。在这些实施方式中,连杆305、306、307、308、309、310和关节311、312、313、314彼此机械连通并且与支撑枢轴319机械连通,以便能够在至少三个自由度上移动,以及随着装置10的旋转(第四自由度)而移动。
也就是说,定位装置304使得可以将机器人装置10定位在患者的腔体316内,其中装置10的本体10A通过切口315(或设置在切口315中的端口)定位使得被附接到机器人装置10的臂14、16的末端执行器18、20可以到达工作空间30中的任何期望位置,而定位装置304的连杆305、306、307、308、309、310和关节311、312、313、314起到产生遥控中心318的作用,装置本体10A在该遥控中心318处穿过切口315使得机器人装置22的所有运动都在单个点(例如***点315)处穿过遥控中心318。换句话说,无论连杆305、306、307、308、309、310和关节311、312、313、314的定位以及所得到的机器人装置10在患者的腔体316内的定位如何,作为定位装置304的结果,装置本体10A在切口315(遥控中心318)处的部分在所有三个轴线(通过切口311)中保持在相同位置。这允许机器人装置(例如机器人装置10)在腔体(例如腔体316)内的操作,使得末端执行器(例如末端执行器18、20)可以到达腔体内的任何期望位置,同时整个装置10经由穿过并且从不从切口315处的单个点(遥控中心318)移动的装置本体10A而被连接到定位装置304,由此使得可以通过该单个切口(例如切口315)操作和定位装置10。另一个优点是定位装置304使得可以在患者的腔体内使用单个体内机器人装置而不是从患者的腔体延伸并且由此在患者身体外侧占据大量工作空间的已知Da VinciTM***的多个臂。
图41A和图41B示出了可用于支撑机器人10的支撑装置304的进一步实施方式。在这些实施方式中,一个或多个电机301A、301B可以与支撑机构304(如连杆305、306、307、308、309、310和关节311、312、313、314)可操作地集成。应当理解在这些实施方式中,电机301A、301B能够将各连杆机构驱动到各种受控位置,也就是说通过三个或四个(包括装置翻滚)自由度在切口点318上“点追踪”。也就是说,致动器或电机301A、301B可以被配置成通过偏航、俯仰和旋转自由度以协调的方式驱动连杆305、306、307、308、309、310和关节311、312、313、314,以便维持机器人10相对于遥控点318的位置。
根据某些实施方式,图42A至图42C的支撑结构304还利用一个或多个电机301A、301B来维持装置10相对于遥控点318的位置。同样,在这些实施方式中,支撑结构304具有连杆305、306、307、308、309、310和关节311、312、313、314,包括与具有轨道开口324的俯仰轨道322可操作地连通的轨道关节326。应当理解,在这些实施方式中,连杆305、306、307、308、309、310的移动促使支撑关节326通过轨道开口324上的各种位置以重新定位装置10同时在遥控点318处点追踪。应当理解,连杆机构和/或关节的许多实施方式是可能的。
图43和图44的实施方式描绘了被称为“台灯”304的定位和支撑结构实施例。应当理解,该实施方式具有与台灯类似的运动学特征,因为在这些实施方式中,连杆330、332、334、336、338、340、342、344、346能够以受控方式相对于手柄12A和/或机器人10的移动,以便在维持相对于***点318的一致位置的同时调节机器人10的俯仰或其它位置。在某些实施方式中,弹簧可用于抵消机器人10的重量。如图44所示,在这些支撑装置304中的某些中,可以使用多根线缆350、352、354来驱动连杆机构,例如经由被致动的主轴360或其它装置。也就是说,各种实施方式,致动器301A、301B能被可操作地连接到线缆350、352、354以驱动连杆330、332、334、336、338、340、342、344、346的这些运动。
当然,本文中描述的所有支撑机构都可以用电机或其它致动器致动。每个关节或各关节的任何组合可由电机驱动。也可以在一些或所有关节处使用传感器来创建控制***。然后,该控制***可以被连接到机器人控制***使得可以协调支撑机构控制和机器人控制以允许两个***一起工作以便通过控制台上或在单独的控制***中的机器人控制(或其它控制)来扩展机器人装置的工作空间。
如图45所示,在进一步的替代实施方式中,机器人装置10可以由外部机器人360支撑。在此,机器人装置10由外部机器人臂362支撑,该外部机器人臂362具有各具有一个或多个自由度的若干连杆362、364、366,并且可以用于在外科手术过程中遥控对中或点追踪机器人。在各种实施方式中,臂(多个臂)由电机、传感器和控制***(例如本文中所描述的)主动控制。应当理解,在某些实施方式中,该外部机器人360可以是另一个外科机器人360、工业机器人或定制机器人。还应理解,该***1中的外部机器人360可以与其它外科装置和机器人外科***(例如腹腔镜365或其它已知的外科工具和装置)相结合使用。外部机器人支持机器人360的另一个版本可以是并联连杆机构外部机器人370,如在图46的实施方式中所示。
图46的并联连杆机构外部机器人370具有安装在上方的机器人370,其在某些实施方式中被安装到手术室上方的天花板。在各种实施方式中,多个径向设置的近端连杆372由机器人370通过致动关节371致动。这些近端连杆372与对应的关节374机械连通,所述关节374进而支撑或以其它方式定位支撑臂376。在这些实施方式中,支撑臂通过支撑关节378与外科机器人10机械的和/或可操作的连通,使得致动关节371的运动足以侧向地、旋转地和/或竖直地推动支撑关节以便将机器人10推动到各种另外的位置。
图47描绘了使用由支撑杆382支撑的球形关节380以在***点318附近为机器人10提供足够的自由度的进一步替代实施例。在该实施方式中,该球形关节可以被用于调节机器人10的三个旋转和一个平移(进/出),如会由技术人员所理解的。还应理解在某些实施方式中,杠杆锁可用于松开球并允许所有四个自由度移动。
如图48A至图48D-2所示,在进一步的替代实施方式中,“悬吊”支撑结构400被用于支撑机器人10。在该实施方式中,弯曲、枢转的支撑杖402被附接到手术室台303并且在患者腔体316上方延伸。在该实施方式中,支撑杖404与在患者之上延伸的悬挂的铰接“J形钩”404可操作的连通。在该实施方式中,J形钩具有在任一端处带有球形关节408、410的附加伸缩连杆406并且用于支撑和定位机器人10。在各种实施方式中,并且如图48B-1至图48D-2所示,支撑杖的旋转运动造成J形钩404和相关联的连杆406和关节408、410的对应运动,以便“摆动”悬吊结构400并且进而使装置10围绕中心位置318摆动。应当理解,许多替代构造是可能的。

图52E描绘了支撑件580的另一个实施例。在该实施方式中,支撑件580利用球形关节。也就是说,支撑件具有通过三个臂586A、586B、586C耦接的第一或上环582以及第二或下环584。三个臂586A、586B、586C中的每一个在每个端部处具有球形关节588,使得三个臂586A、586B、586C在一端处经由球形关节588耦接到第一环582并且在另一端处经由球形关节588耦接到第二环584。如图所示,机器人10被耦接到第二环584。在一个实施例中,机器人10以类似于以上托架实施例的方式被可滑动地耦接到第二环584使得机器人10可以朝向或远离外科手术空间滑动,由此导致平移自由度。
应当理解,经由球形关节耦接到两个环582、584的三个臂586A至586C的配置可以导致针对机器人装置10在第二环584下方的某个点处的单个旋转中心。这样,如果支撑件580被定位在患者上方,则该旋转中心可以以与以上支撑件实施例类似的方式与外科手术***点(例如切口)对准。

图53中示出了一个控制台720实施方式,其带有示出来自机器人摄像机(例如机器人装置10)的视图的主显示器722。主显示器下方的二级触摸屏724被用于与机器人、摄像机和***的各种功能交互。在该实施例中,两个触觉手控制器726、728被用作使用者输入装置。这些触觉手控制器726、728能够测量如在控制器726、728处施加的外科医生的手的运动并且向这些手施加力和扭矩以便通过该触觉反馈向外科医生指示各种信息。控制台720还具有踏板730以控制机器人的各种功能。外科控制台720的高度可以变化以允许外科医生坐下或站立。触觉反馈的操作的进一步讨论可以关于美国专利申请No.15/227,813以及通过引用并入本文的其它申请找到。
图54示出了***1的各种互操作性和布线可能性。许多构思是可能的,但是在此在图54的上下文中给出了三个示例性实施例。在一个布线实施方式中,外科控制台720(或本文中公开或设想的任何其它控制台)与电外科发生器740交互。然后“魔声线缆(monstercable)”742将外科控制台720连接到外科手术环境附近的分支连接器744。然后摄像机746和机器人10被连接到分支连接器744。在该场景中,电外科单元740的能量在被发送到机器人10之前被路由通过外科控制台720。在该实施方式中,没有提供返回垫。
可替代地,根据另一个布线概念。返回垫748设置成被耦接到分支连接器744使得单极电外科能量在返回到电外科发生器740之前被路由通过分支连接器744、魔声线缆742和控制台720。
在进一步的布线替代方案中,返回垫748被耦接到电外科发生器740,使得由于单极电外科能量被直接路由返回到电外科发生器740,电外科单元的能量在被发送到机器人10之前被路由通过外科控制台720。
在其它实施例中,***1可以具有带有定位在***1上的若干可能位置之一处的接口盒的线缆连接器外壳或集群。例如,图55描绘了***1,其具有悬挂在外科手术台762的台轨上的接口盒(本文中也被称为“盒(pod)”)760。在该实施例中,***1具有支持电子器件和设备,例如耦接到接口盒760的烧灼、照明和其它功能764。控制台720也被耦接到接口盒760。盒760简化了机器人1在外科手术区域中的连接。盒760可以是无菌的,或者不是无菌的并且用无菌盖布覆盖,或者根本不是无菌的。盒760的功能是简化外科手术空间中所需的布线并简化机器人和摄像机1到外科控制台720的连接。接口盒760能被悬挂在外科手术台762上于无菌场内侧或外侧。在一些实施例中,盒760具有通知使用者已经完成了适当连接以及向使用者提供其它形式的反馈的指示器,例如灯或屏幕(未示出)。盒760还可以具有按钮、触摸屏或其它接口机构形式的接口以接收来自使用者的输入。
在某些替代实施例中,盒760能被放置在靠近外科手术台762或与外科手术台762相距一定距离的地板上。可替代地,盒760能被悬挂或连接到其它位置或者被放置在无菌场外侧的地板上。
这种情况的一个用途是将所述盒安装到床轨并且然后在稍后的时间带来无菌机器人和摄像机。然后可以将机器人和摄像机尾线缆交给非无菌人员以连接到所述盒。这允许无菌场和非无菌场之间的清洁接口。盒末端也能被盖布覆盖使得该盒末端可以进入无菌场并且机器人和摄像机连接可以在无菌台处被组装,因此其可以在床边被完全功能地和无菌地提供给外科医生。
接口盒还能被连接到其它支持电子器件和设备,例如烧灼、照明和其它功能,并且接口盒能被设计成在地板或带有支持电子器件的无菌场外侧的另一位置上。

1.一种机器人外科装置,包括:
(a)细长结构;

(ii)第一齿轮对,其中所述第一轴的旋转驱动所述第一齿轮对;以及
