Abstract
本发明公开了带球囊的静脉插管,包括插管(1)、球囊(2)、导管(3)和单向阀(4),插管(1)里面设置有空腔(11),插管(1)的前端插头(12)的后方外面安装球囊(2),球囊(2)通过导管(3)与单向阀(4)连接,其特征在于:在球囊(2)的后方的插管(1)管壁上设置若干个侧孔(13),侧孔(13)与空腔(11)连通。球囊阻断下腔静脉后,可以充分回收下腔静脉里面血流;同时球囊存在,静脉插管在下腔静脉内成悬空状态,而且下腔静脉远心段涨起,预防静脉插管贴壁,堵塞侧孔影响血流的回收;球囊阻断下腔静脉,有效杜绝VV‑ECMO的再循环问题,提高氧合效率。
Description
技术领域
本发明涉及带球囊的静脉插管,可以用于体外循环或者体外膜肺氧合ECMO。
背景技术
体外膜肺氧合(Extracorporeal Membrane Oxygenation,ECMO)主要用于对重症心肺功能衰竭患者提供持续的体外呼吸与循环,以维持患者生命,体外膜肺氧合(ECMO)的核心部分是膜肺(人工肺)和血泵(人工心脏),可以对重症心肺功能衰竭患者进行长时间心肺支持,为危重症的抢救赢得宝贵的时间。ECMO主要包括血管内插管、连接管、动力泵(人工心脏)、氧合器(人工肺)、供氧管、监测***等部分。ECMO有2种,包括:
一:VV-ECMO:(静脉-静脉)即从大的静脉引流缺氧的血液,在体外装置氧化和脱二氧化碳,回到右心房。氧合的血液进入肺循环部和全身,为机体供氧,使肺休息。VV-ECMO:(静脉-静脉)通常使用股静脉和颈内静脉作为血管通路,前者用引流和后者用于回流。见图1所示,在ECMO的治疗过程中再循环是不可避免的,为了提高治疗的效率我们可以通过导管尖端的位置及治疗的血流量将再循环控制在可以接受的范围,目前也有单针双腔的ECMO静脉导管,建议首选右颈内静脉。
二:VA-ECMO:(静脉-动脉)从大的静脉引流缺氧的血液,在体外装置氧化和脱二氧化碳,回到主动脉。这样,氧合的血液进入体循环部和全身,为机体供氧,提高血流动力学,使心肺休息。VA ECMO需要建立两条血液通路,见图2所示,一般静脉引血通路首选右侧颈内静脉及左右股静脉置管,尖端在上腔或下腔静脉至右心房的入口处。回血通路动脉导管的建立建议选择股动脉,便于置管及护理。置管完毕一定要超声及摄片判定导管尖端的位置,治疗建立后根据需要调整导管尖端的位置。根据临床治疗判断“分水岭”,合理调整导管位置,及建立灌注导管
目前的用于体外循环或者体外膜肺氧合ECMO的静脉插管,是常用的结构,其结构见专利为:03219019.0、专利名称:医用新型静脉插管、公告号为“CN2624935Y”的发明专利,如图3所示,包括管头1A、管身2A和管尾3A,管头1A设置有多个侧孔4A,管头1A后方的管身2A上设置球囊5A,在球囊5A连接导管6A,导管6A连接位于管尾3A的单向阀7A;其工作原理图如图4所示,球囊5A将下腔静脉(股静脉)分成两段,近心段的一段股静脉的血流从管头1A设置有多个侧孔4A流入,然后***管引出。
体外膜肺氧合ECMO使用常规结构的静脉插管,临床上存在一些因素影响效果:如VV-ECMO过程中再循环是一个严重影响氧和的因素,再循环的存在导致氧合血通过引流管流走在并未进入患者体内。VA-ECMO过程中早期由于血容量不足,下腔静脉坍陷,静脉插管容易贴壁,无法回收足够的静脉血导致流量不足。这些问题在临床上依然困扰这医护人员。
发明内容
本发明的主要目的是提供带球囊的静脉插管,解决现有技术中体外膜肺氧合ECMO的静脉插管,在手术中存在再循环的问题严重影响氧合和下腔静脉坍陷,静脉插管容易贴壁,无法回收足够的静脉血导致流量不足的技术问题。
本发明的技术方案是这样实现的:
带球囊的静脉插管,包括插管、球囊、导管和单向阀,插管里面设置有空腔,插管的前端插头的后方外面安装球囊,球囊通过导管与单向阀连接,其特征在于:在球囊的后方的插管管壁上设置若干个侧孔,侧孔与空腔连通。
上述的前端插头是圆锥体。
上述的插管是截面是圆环形的,侧孔是圆孔。
上述的若干个侧孔按着周向间隔分成若干排,每排含有若干侧孔。
上述位于球囊后方的一段插管上设置支撑金属丝。
本发明与现有技术相比,有以下优点:
1.本装置球囊位于插管前端,引流侧孔之前,球囊阻断下腔静脉后,可以充分回收下腔静脉里的血流;
2.本静脉插管尖端成锥形,尖端孔径较小,减少了近端的回流量。
3.由于球囊存在,静脉插管在下腔静脉内成悬空状态,引流侧孔位于下腔静脉中心,而且球囊阻断后下腔静脉远心段涨起,预防静脉插管贴壁而堵塞侧孔,影响血流的回收。
4.球囊阻断下腔静脉后,能有效杜绝VV-ECMO的再循环问题,提高氧合效率。
5.由于球囊大小可控,情况稳定后可以抽空球囊,解除下腔静脉阻断状态。也可部分充盈球囊,起到部分阻断下腔静脉作用,达到增加流量,减少再循环同时不至于影响重要器官静脉回流,实际操作简便,使用效果好。

具体实施方式
下面,结合附图以及具体实施方式,对本发明做进一步描述:
如图5至图7所示,本实施例提供带球囊的静脉插管,包括插管1、球囊2、导管3和单向阀4,插管1里面设置有空腔11,插管1的前端插头12的后方外面安装球囊2,球囊2通过导管3与单向阀4连接,其特征在于:在球囊2的后方的插管1管壁上设置若干个侧孔13,侧孔13与空腔11连通。
上述的前端插头12是圆锥体,便于刺破血管壁。前端插头12尖端设置尖端孔15,尖端孔15的孔径较小,尖端孔15的中心要通过导丝,而且还可能要放一条支撑内芯,因此,近心段的血热回流量可以忽略不计。
上述的插管1是截面是圆环形的,侧孔13是圆孔,结构简单,制造容易。
上述的若干个侧孔13按着周向间隔分成若干排,每排含有若干侧孔13,图中分成4排,周向按90度角间隔布置,每排4个侧孔13,可以均匀使血流回流到插管1里面的空腔11。
上述位于球囊2后方的一段插管1上设置支撑金属丝14,使插管1里面的空腔11充分张开,不至于弯曲时变形导致血流不畅。
如图8所示,本实施例提供带球囊的静脉插管在使用中的工作原理如下:球囊2存在将股静脉(下腔静脉)分成两段,近心段16A和远心段16B,近心段16A的股静脉被阻断,股静脉管道成悬空状态,而且股静脉(下腔静脉)的远心段16B涨起,预防静脉插管贴壁,堵塞侧孔影响血流的回收。球囊2阻断近心段的股静脉(下腔静脉)后,可以充分回收远心段16B的股静脉(下腔静脉)里面血流,有效杜绝VV-ECMO的再循环问题,提高血液氧合效率;由于球囊2大小可控,情况稳定后可以抽空球囊2,解除股静脉(下腔静脉)阻断状态。也可部分充盈球囊2,起到部分阻断股静脉(下腔静脉)作用,达到增加流量,减少再循环同时不至于影响重要器官静脉回流,操作简便,使用效果更好。
以上实施例为本发明的较佳实施方式,但本发明的实施方式不限于此,其他任何未背离本发明的精神实质与原理下所作的改变、修饰、替代、组合、简化,均为等效的置换方式,都包含在本发明的保护范围之内。
Claims (5)
1.带球囊的静脉插管,包括插管(1)、球囊(2)、导管(3)和单向阀(4),插管(1)里面设置有空腔(11),插管(1)的前端插头(12)的后方外面安装球囊(2),球囊(2)通过导管(3)与单向阀(4)连接,其特征在于:在球囊(2)的后方的插管(1)管壁上设置若干个侧孔(13),侧孔(13)与空腔(11)连通。
2.根据权利要求1所述的带球囊的静脉插管,其特征在于:前端插头(12)是圆锥体。
3.根据权利要求1或2所述的带球囊的静脉插管,其特征在于:插管(1)是截面是圆环形的,侧孔(13)是圆孔。
4.根据权利要求3所述的带球囊的静脉插管,其特征在于:若干个侧孔(13)按着周向间隔分成若干排,每排含有若干侧孔(13)。
5.根据权利要求4所述的带球囊的静脉插管,其特征在于:位于球囊(2)后方的一段插管(1)上设置支撑金属丝(14)。
