Description

Application of stress disorder test training method and test training equipment thereof
Technical Field
The invention relates to the technical field of treatment and recovery of post-traumatic stress disorder, in particular to application of a stress disorder testing and training method and testing and training equipment thereof.
Background
Post-traumatic stress disorder (PTSD) refers to a mental disorder that is delayed in the appearance and persistence of an individual after the individual experiences, witnesses, or encounters one or more physical deaths involving himself or another person, or is threatened by death, or is severely injured, or is threatened by physical integrity. The clinical manifestations of PTSD are mainly three groups, namely traumatic reexperience, avoidance and numbness, and alertness increase; but the clinical manifestations of children and adults are not exactly the same and some symptoms are specific to children, such as nightmares, repeated re-acting traumatic events, playing a subject game related to trauma, emotional agitation or sadness in the face of associated cues, etc.; avoidance symptoms often manifest themselves in children as separation anxiety, stickiness, reluctance to leave parents; high alertness symptoms often manifest in children as excessive startle reactions, high vigilance, attention disorders, irritability or rage liability, difficulty falling asleep, etc.; and children of different ages may also exhibit different PTSD.
Numerous studies have shown that PTSD patients often show excessive attention to potential threat stimuli in the environment and a tendency to fail to focus on the target task, and positive psychological intervention can effectively reduce the incidence of PTSD. Thus, in exploring the causal relationship between attention bias and anxiety maintenance for threat information, researchers have discovered that by training the attention of the subject to non-threat cues, the level of anxiety in the subject can be reduced. Based on this finding, researchers have begun to consider the relationship between attention bias and anxiety symptoms, and have found that training of anxiety individuals on non-threatening stimuli can reduce the individual's attention bias and reduce anxiety, which is called attention bias training technology, and at the same time, the method uses a forward-guided intervention mode to guide the individuals' attention on non-threatening stimuli during training, and uses a negative avoidance mode to remove the attention on the threatening stimuli, thereby achieving the effect of intervening on the removal of the attention on the threatening stimuli. (attention bias training, also known as attention bias training, attention bias correction, is a systematic training procedure for modifying or correcting an individual's attention bias
At present, a drug treatment mode is adopted, and the side effect is large; if a psychological measurement mode is adopted, although the used psychological scale can be used for performing multi-element evaluation, the form is single and only subjective evaluation is performed; if an evaluation device mode is adopted, the used evaluation device can only evaluate one or a few aspects of child development, cannot realize multivariate evaluation, has single evaluation content, and cannot give a comprehensive systematic evaluation report after one experience is finished.

Detailed Description
Post-traumatic stress disorder clinical manifestations are mainly characterized by a renewed experience of the wound, accompanied by emotional irritability and avoidance.
Point detection experimental mode: point-probe stimuli present two stimuli (500ms) simultaneously on the computer, and then the stimuli disappear, with one probe stimulus appearing; the probing stimulus is present at any one of the locations of the stimulus just presented; requiring the key of the tested person to indicate the position of the stimulation where the detection stimulation appears, and comparing the difference between the response time of the detection stimulation appearing at the position of the negative stimulation and the response time appearing at the position of the neutral stimulation; if the subject is attracted to a negative stimulus, the reaction time of the negative stimulus is shorter than that of the neutral stimulus, and after the anxiety individual judges the information to be threatening according to the attention-avoidance theory, the anxiety individual pays attention to the stimulus so as to further distinguish and analyze the stimulus, and there is a component which is difficult to pay attention to the resolution, and further the anxiety is increased due to the processing of the stimulus.
Psychometric measurement: the psychological measurement is to measure, analyze and evaluate the specific quality of the human through scientific, objective and standard measurement means. The quality as used herein refers to the individual characteristics of perception, skill, ability, temperament, character, interest, motivation, etc. required for or associated with a particular task or activity, which are the necessary basis for performing the task or activity at a certain quality and speed, and mainly include methods such as observation, interview, questionnaire, experiment, psychophysical, etc.
Referring to fig. 1, the present invention provides an application of a method for testing and training stress disorder, comprising the following steps:
step S1, evaluating the stress disorder symptom after the trauma of the tested person to obtain an early evaluation condition report;
step S2, the tested person reads the instruction words and carries out a plurality of training experiments;
step S3, according to the early evaluation condition report, performing an intervention phase: let the testee face the display screen, show the fixation point sign for a period of time earlier on the display screen, show two faces for a period of time again, then the position that two faces disappear appears at random and surveys amazing sign, requires the testee to explain the opening orientation of surveying amazing sign.
Step S4, after 1 intervention phase is finished and 5-8 days are separated, the next intervention phase is carried out;
and step S5, evaluating the stress disorder symptom after the trauma of the tested person to obtain a later evaluation condition report, checking the intervention effect, and making a subsequent scheme.
Compared with the prior art, the method adopts the cognitive training method for intervention, avoids side effects brought by drug treatment, evaluates the stress disorder symptoms of the tested person after trauma before and after the intervention to form two evaluation condition reports and combines the intervention training, can assist therapists in timely mastering the intervention effect, can give out comprehensive evaluation reports, and is visual and convenient to store.
Wherein, step S1 includes: performing data analysis and evaluation on the stress disorder symptom after the trauma of the tested person to obtain an early evaluation condition report, and automatically storing the early evaluation condition report; the follow-up comparison is convenient, and the later period is convenient to check.
Further, step S2 includes: the tested person reads the guide words and carries out 6 times to 12 times of practice experiments, the experiment interval of each time of practice is 700ms to 900ms, (700ms to 900ms is the time interval between two trails and mainly provides short rest time for the experimental subject, and the time is the general time interval used in the experimental mode); preferably, the experiment environment is familiar with 10 practice experiments, each practice experiment being separated by 800ms, (800ms is the time interval between two trails, and mainly provides the experimental subject with a short rest time, which is the general time interval used in this type of experimental paradigm).
In the invention, in order to facilitate training stimulation, in step S3, the fixation point identifier is marked with a "+", the face comprises a neutral expression face, a positive expression face and a negative surface face, specifically, the face selects face emotion pictures of 10 men and women in a revised Chinese emotional face picture system of the Chinese face expression picture system, wherein 20 neutral face pictures, 20 negative pictures and 20 positive face pictures; the detection stimulation mark adopts an 'E' mark; the opening direction of the detection stimulation mark is explained by pressing the keyboard by the testee, specifically, the reaction mode of the testee is a key reaction (pressing the 'F' key towards the left and pressing the 'J' key towards the right), and the reaction time of each key is recorded as the test effect of each experimental stage.
Further, step S3 includes: the method comprises the following steps that a tested person faces a display screen, firstly, a prompt is displayed for 400-600 milliseconds (in a point detection experiment paradigm, the prompt is general time displayed by the prompt of the plus, namely, the prompt attracts the attention of the tested person, the attention is kept, or the attention is directionally accelerated so as to notice the appearance of a next stimulation target), then, a pair of face holes are displayed on the upper side and the lower side of the display screen for 400-600 milliseconds, then, an E appears at the positions where the two face holes disappear randomly, and the tested person is required to explain the opening direction of the E, namely, the tested person is required to explain the left and the right by pressing an F key and a J key on a keyboard; preferably, in the implementation operation process, the tested person is faced to the display screen, and the plus display is firstly displayed on the display screen for 500 milliseconds, and researches show that the attention bias of the anxiety individual is mostly expressed as attention directional acceleration within 500 milliseconds, and the attention bias is changed to attention relieving difficulty after 500 milliseconds; the study adopted pictures as stimulating materials, and compared with words, the study is easier to process. Thus, 500ms may be just in the attention relief phase, and the negative evasion approach is just better suited to correct relief difficulties. In the invention, the intervention can be carried out in two ways of positive guidance and threat avoidance in a short period of the individual suffering from the trauma experience, but the positive guidance mode is relatively good, mainly the patient is trained to take directional attention to the formation of the positive stimulus and is kept to achieve the intervention effect, and the positive guidance mode in the short period of the individual suffering from the trauma experience is because the experimental results show that: teenagers with PTSD symptoms are subjected to first-stage attention bias training after 3 months of earthquake, wherein the period is 4 weeks, 2 times per week, before the first training and after each training, the same anxiety, depression and PTSD scales are used for evaluating symptom degrees, and the results show that the anxiety, depression and PTSD scores of three experimental groups show a descending trend, but the positive group is better than the negative group and the control group. The intervention in the long-term experience of an individual suffering from trauma is mainly carried out by taking a threat avoidance mode as a main training mode, mainly training a patient to relieve the attention of a threat stimulus to achieve the effect of the intervention, and the same intervention training is carried out on the same sample again after 9 months from the first intervention, so that the result shows that the intervention effect of a negative group is better than that of a positive group and a control group.
Aiming at the short-term experience of an individual suffering from trauma, a forward guiding mode is taken as a main mode, and specifically, the method comprises the following steps: in step S3, if the previous evaluation report shows that the subject experienced a short period of trauma, then intervention is performed in a forward guidance mode: the method comprises the steps of enabling a tested person to face a display screen, firstly displaying +/-400-600 milliseconds (preferably 500 milliseconds, within 500 milliseconds, the attention bias of an anxious individual is more shown as attention directional acceleration within 500 milliseconds, and then turning to attention relief difficulty after 500 milliseconds), adopting pictures as stimulation materials, and enabling the pictures to be easier to process compared with words, so that 500 milliseconds can be just in the attention relief stage, the negative avoidance mode is just more suitable for correcting the attention relief difficulty, then displaying a neutral expression face and a positive expression face from top to bottom respectively on the display screen for 400-600 milliseconds, and then randomly displaying 'E' at the position where any one of the neutral expression face and the positive expression face disappears, and simultaneously requiring the tested person to explain the opening direction of 'E'.
Aiming at the long-term experience of an individual suffering from a wound, the method mainly aims at avoiding a threat, and specifically comprises the following steps: in step S3, if the previous evaluation report shows that the tested person suffered from the trauma for a long time, the method of avoiding the threat is adopted to intervene: the tested person is enabled to face the display screen, firstly, the + is displayed for 400-600 milliseconds on the display screen, then, the neutral expression face and the negative expression face are respectively displayed for 400-600 milliseconds above and below the display screen, then, the E randomly appears at the position where any one face of the neutral expression face and the negative expression face disappears, and meanwhile, the tested person is required to explain the opening orientation of the E.
In the present invention, step S4 includes: after 1 intervention phase is finished and 7 days are separated, 1 intervention phase is carried out again, and one intervention course comprises four intervention phases.
The present invention provides an embodiment, as shown in fig. 2 and 4, as follows:
step S1: performing data analysis and evaluation on the stress disorder symptom after the trauma of the tested person to obtain an early evaluation condition report, and automatically storing the early evaluation condition report;
step S2: the tested person reads the guide words and carries out 10 times of practice experiments;
step S3: and performing an intervention phase according to the early evaluation condition report: and if the early evaluation condition report shows that the tested person suffers from the trauma in a short period, adopting a forward guidance mode to intervene: the method comprises the following steps of enabling a tested person to face a display screen, and displaying 500 milliseconds of plus on the display screen (within 500ms, the attention of an anxious individual is biased to be directed to acceleration, and after 500ms, the anxious individual is changed to be difficult to remove, wherein in the research, a picture is used as a stimulating material, and the stimulating material is easier to process compared with words and phrases; and if the early evaluation condition report shows that the tested person suffers from the trauma for a long time, adopting a threat avoidance mode to intervene: the tested person is allowed to face the display screen, firstly, 500 milliseconds of + is displayed on the display screen, then 500 milliseconds of neutral expression face holes and negative expression face holes are respectively displayed above and below the display screen, then, E randomly appears at the positions where any one of the neutral expression face holes and the negative expression face holes disappears, meanwhile, the tested person is required to explain the opening orientation of the E in a key reaction mode (pressing the F key towards the left and pressing the J key towards the right), and the system records the reaction time (as shown in figure 5).
Step S4: after 1 intervention phase is finished and 7 days are separated, 1 intervention phase is carried out again, and one intervention treatment course comprises four intervention phases;
step S5: and then evaluating the stress disorder symptom after the trauma of the tested person to obtain a later evaluation condition report, checking the intervention effect, and making a subsequent scheme.
