技术领域
本发明涉及一种组合药物,其用于治疗晚期非小细胞肺癌患者或伴脑转移的晚期非小细胞肺癌患者,同时含有低用量的乌苯美司(Ubenimex)和阿法替尼(Afatinib)作为有效成分。
背景技术
晚期(III~IV期)非小细胞肺癌患者的脑转移率非常高,约为30%(非专利文献1)。但是,在不能通过外科手术切除转移到脑中的癌时,可以使用伽玛刀(定向放射治疗用的射线照射装置)治疗和癌分子靶向药物,但至今尚未发现令人满意的治疗方法。
阿法替尼是癌分子靶向药物,对在EGFR(表皮生长因子受体)的基因上有突变的非小细胞肺癌有效,但其常规剂量(40mg/天)容易引起全身皮疹和腹泻等副作用,而且对抑制脑转移的效果也不足(非专利文献2)。
乌苯美司是一种蛋白酶抑制剂,具有增强癌症患者免疫力的作用。关于以30mg/天用量的“针对成人急性非淋巴性白血病,在诱导完全缓解后通过与维持/强化化学治疗剂联合使生存期延长”的适应症,在日本已获得批准(非专利文献3),但向鳞状细胞肺癌的术后辅助化学疗法的适应症扩大等尚未实现(非专利文献4)。
另外,通过将乌苯美司的用量从30mg~60mg/天更改为10mg/天,并联合低用量的抗癌剂或癌分子靶向药物等,显示对大肠癌、乳癌、肺癌、胆囊胆管癌、肾癌等的治疗有用(专利文献1)。
再另外,尽管已经报告癌分子靶向药物(EGFR抑制剂)奥西替尼对伴脑转移的非小细胞肺癌的患者有效果,但不能认为效果足够(非专利文献5),此外,有报告称伽玛刀治疗对伴脑转移的非小细胞肺癌患者的脑转移癌的治疗有用(非专利文献6),但治疗效果不足。

发明内容
发明所要解决的问题
为了实现上述目的,专利文献1公开了对于老年癌症患者和晚期癌症患者,联合低用量的乌苯美司或低用量的乌苯美司与抗肿瘤剂或癌分子靶向药物,但是并没有提及低用量的乌苯美司与低用量的阿法替尼的组合可以有效地用于晚期非小细胞肺癌患者或伴脑转移的晚期非小细胞肺癌患者的治疗。
因此,本发明的目的在于提供一种对于晚期非小细胞肺癌患者或伴脑转移的晚期非小细胞肺癌患者,能够实现高疾病控制率(DCR)和平均客观缓解率(ORR)的新治疗手段。
解决问题的技术方案
本发明人为了解决上述问题而进行了深入研究,结果发现:临床应用于针对既往的成人急性非淋巴性白血病,在诱导缓解后通过与维持、强化治疗剂联合使生存期延长的适应症的乌苯美司与作为分子靶向药物的阿法替尼一起以远低于常规剂量的低用量的组合向患者给药,从而晚期非小细胞肺癌患者或伴脑转移的晚期非小细胞肺癌患者的治疗效果等于或优于常规剂量的阿法替尼,且副作用显著被抑制。本发明是基于这些发现而得到的。
即,本发明具有以下特征。
[1]一种组合药物,其是用于治疗晚期非小细胞肺癌患者或伴脑转移的晚期非小细胞肺癌患者的组合药物,作为有效成分,所述组合药物含有对于成人每剂量单位约为10mg/天用量的乌苯美司和每剂量单位约为20mg/天用量的阿法替尼。
[2]根据上述[1]所述的组合药物,其中,上述非小细胞肺癌在表皮生长因子受体(EGFR)上具有Del19型或L858R型的基因突变。
[3]根据上述[1]或[2]所述的组合药物,其是含有上述用量的乌苯美司和上述用量的阿法替尼的套装制剂或复方制剂。
[4]根据上述[3]上述的组合药物,其是胶囊剂、片剂或颗粒剂等口服给药形式。
[5]对于成人每剂量单位约为10mg/天用量的乌苯美司和每剂量单位约为20mg/天用量的阿法替尼在制造用于治疗晚期非小细胞肺癌患者或伴脑转移的晚期非小细胞肺癌患者的上述[1]~[4]中任一项所述的组合药物中的用途。
[6]一种用于治疗晚期非小细胞肺癌患者或伴脑转移的晚期非小细胞肺癌患者中晚期非小细胞肺癌或伴脑转移的晚期非小细胞肺癌的方法,其包括向所述患者每天给予一次上述[1]~[4]中任一项所述的组合药物。

具体实施方式
本发明涉及一种组合药物,其是用于治疗晚期非小细胞肺癌患者或伴脑转移的晚期非小细胞肺癌患者的组合药物,作为有效成分,所述组合药物含有对于成人每剂量单位约为10mg/天用量的乌苯美司和每剂量单位约为20mg/天用量的阿法替尼。本说明书中,关于作为有效成分的乌苯美司和阿法替尼的上述用量术语“约”均指具有“±2mg”的变动范围。
<乌苯美司>
本说明书中使用的“乌苯美司”(Ubenimex)是(2S)-2-[(2S,3R)-3-氨基-2-羟基-4-苯基丁酰氨基]-4-甲基戊酸(CAS注册号58970-76-6)的通用名称,根据情况,氨基和/或羧基也可以形成药学上可接受的盐。
当乌苯美司形成盐时,与其氨基的盐例如是与乙酸、柠檬酸、琥珀酸、马来酸、酒石酸、盐酸等有机酸或者无机酸的盐,另一方面,是与其羧基的盐,例如钠盐、钾盐、铵盐(即,羧基与氨或胺类的盐)等盐。
可以基于现有公知的通常方法制造乌苯美司,例如可以使用作为微生物(例如,橄榄网状链霉(Streptomyces olivoreticuli))的培养发酵产物而生产的产物(H.Umezawaet al.,Journal of Antibiotics 1976;29(1):97-99)。或者可以使用“百士欣(Bestatin)(注册商标)”(日本化药株式会社)等的销售品。优选的乌苯美司是不形成盐的游离体。
乌苯美司是一种氨基肽酶抑制剂,被认为与存在于免疫功能细胞(巨噬细胞、T细胞、骨髓细胞等)表面的氨基肽酶结合,从而在免疫***网络中起连锁作用(F.Abe et al.,Biotherapy 1990;4(11):1708-1718)。此外,推测通过这种生物学作用诱导产生杀伤性T细胞使患者免疫功能增强,由此发挥抗肿瘤作用。
另外,关于乌苯美司的常规剂量,非专利文献3中记载了针对成人急性非淋巴性白血病,在诱导完全缓解后与维持/强化化学治疗剂联合时的乌苯美司的给药量为每天30mg以每天1次进行口服给药。已知乌苯美司的副作用较少,已知有皮疹/发红、瘙痒、恶心/呕吐、食欲不振等副作用。
与此相对,本发明的组合药物中乌苯美司的含量是相当于对于成人每剂量单位约为10mg/天用量的量,已确认基本未观察到该用量的乌苯美司的副作用(图2)。
<阿法替尼>
本说明书中使用的“阿法替尼”(Afatinib)是一种酪氨酸激酶抑制剂,已知是用于治疗具有特定EGFR基因突变的非小细胞肺癌的第二代抗恶性肿瘤剂。
阿法替尼的化合物名称是N-[4-[(3-氯-4-氟代苯基)氨基]-7-[[(3S)-四氢-3-呋喃基]氧基]-6-喹唑啉基]-4(二甲基氨基)-2-丁烯酰胺,CAS注册号为850140-72-6,根据需要,可以是与乙酸、柠檬酸、琥珀酸、马来酸、酒石酸、盐酸等有机酸或者无机酸的药学上可接受的盐,也可以不是盐形式。优选的阿法替尼是二马来酸盐。
阿法替尼的合成法例如记载于WO2007085638A1、WO2014183560A1等中。
已知阿法替尼通过竞争性抑制ATP对EGFR/HER1/erbB1、HER2/erbB2、HER4/erbB4的结合来发挥抗肿瘤作用。
阿法替尼作为商品名“吉泰瑞(Giotrif)”(二马来酸盐;勃林格殷格翰公司),有20mg片剂、30mg片剂、40mg片剂、50mg片剂在销售,常规剂量是每天1次40mg(非专利文献2)。但是,对于阿法替尼,已知除了严重腹泻、严重皮肤障碍之外,还有间质性肺炎、肝功能不全等副作用。
与此相对,本发明的组合药物中阿法替尼的含量是相当于对于成人每剂量单位约为20mg/天用量的量,已确认基本未观察到该用量的阿法替尼的腹泻和皮疹等副作用(图2)。
<晚期非小细胞肺癌患者>
本说明书中的“晚期非小细胞肺癌患者”是指患有晚期非小细胞肺癌的患者,且在用本发明的组合药物进行治疗前无脑转移的患者。
另一方面,伴脑转移的患者在本说明书中被称为“伴脑转移的晚期非小细胞肺癌患者”。
另外,本说明书中[晚期]是指癌症病期为III~IV期,优选为IV期,医生判断为不能手术的症状。
如图1A和1B所示,TNM分类被用作恶性肿瘤的进展程度的指标(UICC·TNM分类,第8版)。T表示原发肿瘤的大小,N表示***转移,M表示远隔转移。此外,M0表示无远隔转移(例如,IVb期以下),M1表示有远隔转移(例如,IVc期)。
即使非小细胞肺癌患者处于晚期状态,也有未观察到转移到脑的患者(图1B)和观察到转移到脑的患者(图1A),通常,脑转移率约高达30%(非专利文献1)。转移到脑中的癌现在还没有令人满意的治疗方法。例如在如图1A所示的患者中,在利用本发明的治疗前,均进行了伽玛刀(定向射线照射装置)治疗,但没有获得治疗效果。
<组合药物>
作为有效成分,本发明的组合药物含有对于成人每剂量单位约为10mg/天用量的乌苯美司和每剂量单位约为20mg/天用量的阿法替尼。
在利用乌苯美司的成人急性非淋巴性白血病患者的治疗中,通常,对于成人给予每天30mg(例如10mg×3)用量的乌苯美司(即,百士欣(Bestatin)(注册商标)),此外,在利用阿法替尼的非小细胞肺癌患者的治疗中,通常,对于成人给予每天40mg或50mg用量的阿法替尼(即,吉泰瑞(Giotrif)(注册商标))。
与此相对,如上所述,在本发明的组合药物中,乌苯美司的用量是常规剂量的约1/3,且阿法替尼的用量是常规剂量的约1/2。虽然本发明的组合药物是这样的低用量,但对伴脑转移或无脑转移的晚期非小细胞肺癌患者(26人,图1A和1B)的晚期非小细胞肺癌和脑转移癌的治疗效果,在疾病控制率(DCR)为100%(26例/26例)方面等于或优于现有治疗的效果,以及,在平均客观缓解率(ORR)为65%以上(17例/26例)方面等于或优于现有治疗的效果,从而可以提供优异的药效。与此相关地,在后述的实施例中,在无脑转移的晚期非小细胞肺癌患者的治疗期间,由于未能确认到转移到脑,由此可推测脑转移被抑制,此外,在伴脑转移的晚期非小细胞肺癌患者的治疗期间,未确认到脑肿瘤大小的增加。
除此之外,如图2所示,在对晚期(III~IV期)非小细胞肺癌的患者的阿法替尼(40mg/天)的治疗中,容易出现腹泻和皮疹等副作用,本发明中,在对含脑转移病例(38.5%)的晚期(III~IV期)非小细胞肺癌患者的阿法替尼(约20mg/天)与乌苯美司(约10mg/天)的联合治疗中,基本未观察到腹泻和皮疹等副作用,特别是完全未观察到在3级以上的腹泻和皮疹。
关于非小细胞肺癌的表皮生长因子受体(EGFR)基因突变,在参与临床试验患者的基因检查结果(图1A和1B)中,由于在EGFR具有Del19型或L858R型的基因突变,因此本发明的组合药物可有效治疗至少具有上述基因突变(伴脑转移的或无脑转移)的晚期非小细胞肺癌患者。
本发明的组合药物的形式只要含有上述用量的乌苯美司和上述用量的阿法替尼,则没有限定,例如是套装制剂或复方制剂。
套装制剂是将对于成人含有每剂量单位约为10mg/天用量的乌苯美司的药物组合物和含有每剂量单位约为20mg/天用量的阿法替尼的药物组合物的分别的多个剂量(例如10~30个剂量)包装在单个包装中的药物。
复方制剂是含有对于成人每剂量单位约为10mg/天用量的乌苯美司和每剂量单位约为20mg/天用量的阿法替尼的药物组合物(即,乌苯美司:阿法替尼的重量比约为1:2)。
上述药物组合物与作为有效成分的乌苯美司或其药学上可接受的盐和/或作为有效成分的阿法替尼或其药学上可接受的盐一起可以包含药物制造中通常使用的赋形剂、粘合剂、崩解剂、润滑剂、稀释剂、增溶剂、助悬剂、等渗剂、pH调节剂、缓冲剂、稳定剂、着色剂、矫味剂、矫臭剂等,可以制成适于口服给药或非口服给药(例如,静脉内给药、动脉内给药、注射局部给药、腹腔或胸腔内给药、经肺给药、皮下给药、肌肉内给药、舌下给药、经皮吸收或直肠内给药等),优选为口服给药。例如,本发明中的上述药物组合物可以制成溶液剂、乳剂、脂质体制剂、注射剂、悬浮剂、软膏剂、霜剂、经皮吸收剂、经粘膜吸收剂、片剂、丸剂、胶囊剂、散剂、粉剂、颗粒剂、细粒剂、糖浆剂等,优选为胶囊剂、片剂或颗粒剂(优选的口服给药形式),但不限于这些。这些中的每一个都可以按照本领域通常使用的方法进行配制、成型或制备。
与未给予该药物或给予前的患者相比,在给予该药物的患者中,可以将以下的(i)~(iv)中的一个或多个作为指标,对本发明的组合药物的效果进行评价。
(i)非小细胞肺癌和根据情况的脑转移癌缩小或消失,或者不再增加;
(ii)观察到生存期延长(寿命延长效果);
(iii)基本没有或完全未观察到因有效成分导致的副作用;
(iv)未观察到转移到脑或转移被抑制。
在将本发明的包装制剂中所含的各药物组合物联合给予患者时,含有乌苯美司的药物组合物和含有阿法替尼的药物组合物在各有效成分可以同时起作用的范围内连续或间隔地给予。此外,这时的各药物组合物的给药形式、给药途径和给药方式可以相同或不同。
在将本发明的组合药物向患者给药时,如果需要,根据患者的年龄、体重、疾病的严重程度、副作用等因素,只要严格遵守上述有效成分的每天上述用量,则每天的给药次数可以例如设为1~3次,或者给药间隔设为每天或每2~10天。在这种情况下,关于给药次数、给药间隔、给药天数等给药条件,可以根据医生的判断进行更改。
其中,对于副作用,包括如图2所例示的腹泻、皮疹、口腔炎、指甲异常、皮肤干燥、食欲减退等。
<组合药物的制造>
本发明还涉及对于成人每剂量单位约为10mg/天用量的乌苯美司和每剂量单位约为20mg/天用量的阿法替尼在制造用于治疗晚期非小细胞肺癌患者或伴脑转移的晚期非小细胞肺癌患者的上述的组合药物中的用途。
关于上述药物的制造,可以将乌苯美司和阿法替尼的各有效成分的预定量与药学上可接受的上述赋形剂和添加剂混合,制备成预定的剂型。对于药物的制剂化,例如可以参照(《雷明顿:药学科学与实践》(Remington:The Science And Practice Of Pharmacy)第23版,2012年,美国学术出版社(Academic Press))的公开。
<治疗方法>
本发明还涉及一种用于治疗(无脑转移的)晚期非小细胞肺癌患者或伴脑转移的晚期非小细胞肺癌患者中晚期非小细胞肺癌或伴脑转移的晚期非小细胞肺癌的方法,其包括向所述患者每天给予一次上述的组合药物。
上述方法中,本发明的组合药物、其用法、用量等给药条件等如上所述。
实施例
下面示出实施例,进一步详细地说明本发明。但是,本发明并不限于这些实施例。
[实施例1]一起联合给予低用量的乌苯美司和阿法替尼的非小细胞肺癌患者的治疗
向具有表皮生长因子受体(EGFR)基因突变的非小细胞肺癌患者联合给予每剂量单位10mg/天的乌苯美司(游离体)和每剂量单位20mg/天的阿法替尼(吉泰瑞(Giotrif)(注册商标);二马来酸盐)每天1次,联合给药的期间至未观察到抗肿瘤效果,考察治疗效果和副作用的降低。
<受试者的背景>
参与上述临床试验(第II期)的受试者(26人)背景如下所述(图1A和1B)。
(1)伽玛刀(定向射线照射装置)治疗无效的伴脑转移的非小细胞肺癌患者:10人;
(2)无脑转移的非小细胞肺癌患者:16人;
(3)性别:女性21人、男性5人;
(4)年龄:53~82岁(平均71.7岁);
(5)吸烟史:有0人、既往有10人、无16人;
(6)发病时间:初次18人、复发8人;
(7)脑转移:有10人(图1A)、无16人(图1B)(脑转移率38.5%);
(8)基因突变型:Del19型13人、L858R型13人。
<评价方法>
1.疾病控制率(DCR)
DCR是与癌症患者的病情控制有关的指标,不仅包括癌的缩小效果,还包括癌大小的不变。
具体地,是RECIST评价(Journal of the National Cancer Institute,2000年,第92卷,第3期,第205-216页)中CR(complete response:完全缓解)+PR(partialresponse:部分缓解)+SD(stable disease:疾病稳定)的比例。CR、PR、SD和PD根据以下标准进行评价。
完全缓解(CR):癌消失状态维持4周;
部分缓解(PR):癌大小缩小30%以上,且该状态维持4周;
疾病稳定(SD):在PR与PD之间的状态;
疾病进展(PD):癌大小增加20%以上。
2.平均客观缓解率(ORR)
客观缓解率(ORR)是具有肿瘤缩小效果的比例。具体地,在RECIST评价中根据如下的合计来判定效果:在CT(计算机断层扫描)检查中病灶完全消失的完全缓解(CR)和肿瘤直径缩小至70%的部分缓解(PR)的合计。也对如下项目进行判定:晚期非小细胞肺癌有无向其他器官转移、转移到其他器官的癌大小、有无转移到脑、脑转移癌大小的变化。
3.副作用
对于如图2所示的副作用(腹泻、皮疹、口腔炎、指甲异常等)中各项目,确定发生率为5%以上的副作用的病例数。
<结果>
本发明的组合药物给药的疗效表示为对(无脑转移的)晚期非小细胞肺癌患者和伴脑转移的晚期非小细胞肺癌患者的综合评价。
疾病控制率(DCR)为100%(26例/26例)。
平均客观缓解率(ORR)为65%以上(17例/26例)。其中,如图1B所示,对于无脑转移的晚期非小细胞肺癌患者,未确认到脑转移。此外,如图1A所示,对于伴脑转移的晚期非小细胞肺癌患者,至少肿瘤大小未增加。
对于副作用,在对含脑转移病例(38.5%)的晚期(III~IV期)非小细胞肺癌患者的阿法替尼(20mg/天)与乌苯美司(10mg/天)的联合治疗中,基本未观察到腹泻和皮疹等副作用。
工业上的可利用性
通过给予本发明的低用量的组合药物对晚期非小细胞肺癌患者和伴脑转移的晚期非小细胞肺癌患者的治疗,由于具有优异的疗效和能够显著地降低副作用,因此在工业上有用。

Claims (6)
1.一种组合药物,其是用于治疗晚期非小细胞肺癌患者或伴脑转移的晚期非小细胞肺癌患者的组合药物,作为有效成分,所述组合药物含有对于成人每剂量单位约为10mg/天用量的乌苯美司和每剂量单位约为20mg/天用量的阿法替尼。
2.根据权利要求1所述的组合药物,其中,所述非小细胞肺癌在表皮生长因子受体(EGFR)上具有Del19型或L858R型的基因突变。
3.根据权利要求1或2所述的组合药物,其是含有所述用量的乌苯美司和所述用量的阿法替尼的套装制剂或复方制剂。
4.根据权利要求3所述的组合药物,其是胶囊剂、片剂或颗粒剂等口服给药形式。
5.对于成人每剂量单位约为10mg/天用量的乌苯美司和每剂量单位约为20mg/天用量的阿法替尼在制造用于治疗晚期非小细胞肺癌患者或伴脑转移的晚期非小细胞肺癌患者的权利要求1~4中任一项所述的组合药物中的用途。
6.一种用于治疗晚期非小细胞肺癌患者或伴脑转移的晚期非小细胞肺癌患者中晚期非小细胞肺癌或伴脑转移的晚期非小细胞肺癌的方法,其包括向所述患者每天给予一次权利要求1~4中任一项所述的组合药物。
