It should be noted that the embodiments described below are some embodiments and not all embodiments. The components of the embodiments generally described and illustrated in the figures herein may be arranged and designed in a wide variety of different configurations.
Referring to fig. 1(a), 1(b), 2, 4 and 7, a first aspect of an embodiment of the present invention provides a ligation device assembly. The ligation device assembly comprises an endoscope 14 and a ligation device, wherein the ligation device comprises a barrel body 11, the ligation device is fixed on a lens body of the endoscope 14, and the endoscope 14 is at least partially inserted into the barrel body 11. The ligation device can be used with an endoscope to perform ligation and even resection.
The following text focuses on the ligation device in the ligation device assembly in conjunction with the drawings of the specification
Referring to fig. 1(a) and 1(b), the ligation device includes a ligation device body, a ligation device mounting assembly, a ligation device releasing assembly and a loop set. The drawings in the present specification show only the main structure of the ligation device, and the manipulation end of the ligation device and other components of the endoscope 14 are omitted.
The body of the ligation device has elasticity 10. It should be noted that the elastic body 10 of the ligation device can be made of shape memory alloy with super elasticity, besides common elastic materials such as rubber.
With continued reference to fig. 1(a), 1(b) and 2, the ligation device mounting assembly includes a barrel 11, the barrel 11 having a first end and a second end in an axial direction. Referring to fig. 1(b) and fig. 2, the operation end of the pulling member 12 is located at the first end of the upper cylinder and is far away from the operation end relative to the second end, so that the first end of the cylinder is located below the second end. In other words, in the placing position shown in fig. 1(b) and fig. 2, the first end of the cylinder is the bottom end, and the second end of the cylinder is the top end. It is noted that embodiments of the present invention also have the proximal end at the end of the overall ligation device where the control handle is located (i.e., the control end) and the distal end at the other end of the overall ligation device. Accordingly, the components of the ligation device are also separated into distal and proximal ends by the distance from the handle of the ligation device. For example, the first and second ends of the barrel 11 are distal and proximal ends of the barrel 11, respectively.
In some embodiments shown in fig. 1(a) to 4, the ligator body 10 is sleeved on the outer wall surface of the barrel 11. The cylinder 11 is used for pulling the target object to be bound to be close to the cylinder inlet of the cylinder 11 or enter the inside of the cylinder 11.
Referring to fig. 1(a), 1(b), 2, 3 and 4, the release assembly of the ligation device comprises a pulling member 12, and the pulling member 12 is used for pulling the ligation device body. The handling end of the pulling member 12 is relatively far from the first end of the barrel 11. The body of the ligation device forms a ring shape, and the body of the ligation device shrinks towards the inner side of the cylinder body 11, namely the area surrounded by the ring formed by the body of the ligation device is reduced. The body of the ligation device is separated from the outer wall surface of the cylinder 11 due to shrinkage, and then the body of the ligation device is used for sheathing the target object.
Referring to fig. 1(b), the ligation device further includes a snare 15 made of a conductive wire, the snare 15 includes a first loop portion 151 and a second loop portion 152, an insulating sheath is disposed around the second loop portion 152, the cylinder 11 is provided with a first working channel 115, and the first working channel 115 is used for the second loop portion 152 to pass through. The insulating sheath may be a conduit made of an insulating material (as shown in fig. 1(b), 2 and 7), or may be an insulating protective film laid on the outer surface of the conductive wire. In fig. 1(b), 2 and 7, the conduit 153 passes through the first working channel 115, and the second snare portion 152 of the snare unit 15 is located inside the conduit 153. The first working channel 115 may be disposed between the inner wall surface and the outer wall surface of the cylinder 11, may be disposed on the outer wall surface of the cylinder 11, and may be disposed on the inner wall surface of the cylinder 11 as well. The ligation device equipped with the snare member 15 can perform an operation of excising the object in addition to the ligation operation. It should be noted that snares are not specific to existing snares and devices that can act to snare a target and perform excision can be referred to as snare sets.
In some embodiments, with continued reference to FIGS. 1(a), 1(b), and 2, pull element 12 is at least partially disposed within a second working channel 141 of endoscope 14 itself. The traction piece is arranged in a second working channel of the endoscope, so that the whole ligation device can effectively traction the ligation device body without additionally adding an auxiliary device. It will be appreciated that the traction element 12 may also be placed within the traction channel member provided with the barrel 11 without passing through the second working channel of the endoscope itself, thereby occupying no working space.
Referring to fig. 2, 5(a) and 5(b), further, an annular limiting structure 113 is disposed on the inner wall surface of the cylinder, and the first loop portion 151 of the loop assembly 15 is wound around the annular limiting structure 113, so that the loop assembly 15 is supported by the annular limiting structure. Specifically, the inner wall surface of the cylinder 11 between the first end and the second end relatively close to the first end is provided with an annular limit structure 113. The annular limiting structure 113 may be a ring of flange surrounding the inner wall surface of the cylinder 11 and extending from the inner wall surface of the cylinder 11 to the center of the cylinder 11, or may be a plurality of protrusions arranged at intervals on the inner wall surface. It should be noted that the ring-shaped stopper structure 113 means that its central region is hollow and its edge region is solid, but the meaning of "ring-shaped" does not mean that the edge regions as solid are equal in width in the circumferential direction.
The first loop portion 151 of the loop set 15 is placed behind the annular limiting structure 113 to facilitate the excision of objects (e.g., early cancers and submucosal tumors), etc. The annular limiting structure 113 is used for supporting the ring set 15, and as mentioned above, the annular limiting structure 113 is also used for limiting the ligation device body in the axial direction of the cylinder.
In some embodiments, the ligator body includes a ligation ring of resilient elements and a piercing structure disposed on the ligation ring. Compared with a rope belt with a locking structure such as a cable tie, the elastic ligation device body has the advantages that although the structure is simple and the release is easy, the ligation effect of the elastic ligation device body is generally considered to be not as firm as the rope belt with the locking structure. The puncture structure is arranged to be matched with each other, after the elastic ligation ring is used for sheathing the target object, the puncture structure can puncture the target object, and the ligation effect is further strengthened.
Referring to fig. 1(b), fig. 6(a) and fig. 6(b), in some embodiments, the ligation ring is an elastic band 101, the puncturing structure is a puncturing element 102, one end of the puncturing element 102 has a sharp portion, the sharp portion punctures the elastic band 101 from the outer side of the elastic band 101 to point to the inner side of the elastic band 101, and the end of the puncturing element 102 located at the outer side of the elastic band 101 is fixed to the outer side of the elastic band 101. Under the action of the elastic restoring force of the elastic band, the puncture piece is pressed into a target object to be sleeved and tied, so that the sleeving and tying are further firmer and more reliable.
With reference to fig. 1(b), fig. 6(a) and fig. 6(b), the body 10 of the ligation device further includes a fixing pad 103 and a fixing cap, the puncturing element 102 has a threaded connection structure matching with the fixing cap, and the fixing pad 103 is tightly attached to the inner side of the elastic band 101. The mounting pad provides a relatively rigid mounting surface for the lancet on the elastic band, which facilitates mounting. The piercing member is fixed to the elastic band by means of the fixing pad and the fixing cap.
With continued reference to fig. 1(b), 2, 5(a) and 5(b), the barrel 11 is further provided with an escape structure 117 at a position facing the sharp portion of the puncturing member 102. The avoiding structure can be a notch positioned in an edge area in a through hole or blind hole mode, or a through hole or blind hole positioned in a non-edge area. The arrangement of the avoiding structure enables the puncture piece to be located in the avoiding structure, so that the structure of the whole ligation device is more compact and interference can be avoided.
Referring to fig. 7, 8(a) and 8(b), in another embodiment, the ligation ring is an elastic band 101, the puncturing structure is a metal sheet 105 with a sharp portion, the material of the metal sheet 105 is a shape memory alloy with elasticity, one end of the metal sheet 105 is a fixed end fixed on the inner side of the elastic band 101, the other end of the metal sheet 105 is a free end, and the metal sheet 105 is tightly attached to the inner side of the elastic band 101 when the elastic band 101 is placed on the outer wall surface of the cylinder 11 in a tensioned state.
The elastic band is sleeved on the outer wall surface of the barrel body at a position close to the first end, and meanwhile, the metal sheet is extruded to be positioned between the elastic band and the outer wall surface of the barrel body. The elastic belt has elasticity and can be contracted and restored to be sleeved on the target object. Meanwhile, the metal sheets also have elastic restoring force based on the shape memory effect, and mutual pressing action can be generated between the adjacent metal sheets. Under the action of the three acting forces, the body of the ligation device is easier to shrink.
With continued reference to fig. 7, 8(a) and 8(b), further, the free ends of the metal sheets 105 have sharp portions, and the number of the metal sheets 105 is at least two, and the sharp portions of two adjacent metal sheets 105 point to each other.
With continued reference to fig. 1(b) and 7, in some embodiments, the pulling member 12 is a pulling string that passes through the interior of the barrel 11 in a direction toward the first end of the barrel 11, then extends over the outer wall surface of the barrel 11 in a direction toward the second end of the barrel 11, and is captured between the body 10 of the ligation device and the outer wall surface of the barrel 11. The distal end of the extension of the pull cord has a stop 121, the stop 121 abuts the ligation body 10, and in both the stop 121 and the ligation body 10, the ligation body 10 is relatively closer to the first end of the barrel 11. When the pulling rope is pulled, the stopper 121 is driven to move towards the first end of the barrel 11, and meanwhile, the stopper 121 pushes the ligation device body 10 to move towards the first end of the barrel 11, so that the ligation device body 10 is separated from the outer wall surface of the barrel 11 at the first end of the barrel 11. In fig. 1(b) and fig. 7, the stopper 121 is a stopper ball. It will be appreciated that the stop member 121 may also be a block-like structure for urging the movement of the ligation device body 10.
The following text describes the use of the ligation device in an embodiment of the invention, taking the body of the ligation device with a piercing element as an example:
ligation step
1. The ligation device is fixed at the distal end of the endoscope 14 by an endoscope fixing part;
2. a ligation device mounted on the body of the endoscope 14 is fed into the stomach or intestinal cavity along with the endoscope 14;
3. the first end of the cylinder 11 is attached to the mucosal tissue of the target object to be ligated, and then the target object is sucked into the cylinder 11 through the working channel of the endoscope 14. After the target is completely sucked into the barrel 11, the pulling member 12 is pulled to push the ligator body 10 out of the outer wall surface of the distal end of the barrel 11, so that the ligator body 10 is detached from the barrel 11. The body 10 of the ligation device, which loses the support of the outer wall surface of the cylinder 11, will be retracted under its own elastic restoring force, thereby tightly fitting over the object. Meanwhile, the puncturing member 102 fixed to the inner side of the elastic band 101 of the ligator body 10 is pressed into the object by the elastic restoring force of the elastic band 101, so that the ligator body 10 is tightly ligated to the object.
The ligation device with the loop set can also comprise the following cutting steps on the basis of the ligation step:
4. after the target object is covered with the ligation device, the loop member 15 wound around the annular stopper structure 113 is tightened to tightly cover the target object. The conductive snare 15 is connected externally to a high frequency electrical device for energizing, so that the snare member 15 cuts off the target laterally from the upper edge of the ligation site and at a distance therefrom.
While embodiments of the invention have been shown and described, it will be understood by those of ordinary skill in the art that: various changes, modifications, substitutions and alterations can be made to the embodiments without departing from the principles and spirit of the invention, the scope of which is defined by the claims and their equivalents.
