Abstract
In order to solve the problem that the left atrial appendage occluder in the prior art is not closed tightly, the invention provides a left ventricular occluder and a use method thereof, wherein the left ventricular occluder comprises the following steps: the plugging disc, the connecting part and the fixed disc; wherein, the plugging disc is used for plugging the left auricle; the fixed disc is connected with the inner side wall of the left auricle, and the plugging disc is fixed at the mouth part of the left auricle; the connecting part is arranged between the fixed disk and the plugging disk and is used for connecting the fixed disk and the plugging disk into a whole; the outer ring of the plugging disc is provided with a sucking disc part for adsorbing the left auricle part and fixing the plugging disc at the left auricle part to prevent residual leakage. The invention solves the problem that the left auricle occluder in the prior art is not tight in occlusion, and has the advantages of simple structure and good occlusion effect.
Description
Technical Field
The invention relates to the technical field of left atrial appendage occlusion, in particular to a left ventricular occluder and a using method thereof.
Background
Atrial fibrillation (atrial fibrillation) is one of the most common clinical arrhythmia, and at present, about 800 tens of thousands of patients with atrial fibrillation exist in China. The main harm of atrial fibrillation is that blood in an atrium deposits and easily forms thrombus, and the thrombus falls off to cause complications such as cerebral apoplexy, peripheral vascular embolism and the like, thereby obviously increasing disability rate and lethality rate. Stroke is the most common and most harmful complication of atrial fibrillation. Anticoagulation therapy is the standard method for preventing atrial fibrillation stroke complications at present, but the anticoagulation therapy has the limitations of high bleeding risk, poor medication compliance and the like. Therefore, the method has important significance for searching more effective and safe measures to prevent atrial fibrillation stroke. Research suggests that 60% of patients with atrial fibrillation of rheumatic heart disease have cardiogenic thrombi from the left atrial appendage, and more than 90% of patients with non-valvular ward fibrillation have thrombi formed in the left atrial appendage. Therefore, the intervention of the left atrial appendage to prevent the thromboembolism of patients with atrial fibrillation, especially the stroke, has important theoretical basis and clinical significance. The left atrial appendage occluder prevents atrial fibrillation stroke and is suitable for atrial fibrillation patients who have high stroke risk, anticoagulation treatment contraindication and high bleeding risk or are unwilling to take anticoagulant drugs for a long time.
The left auricle is the residue of the original left atrium in the embryonic period and is in a long and narrow bent shape, abundant pectinate muscles are arranged inside the auricle to form a small muscle beam, gaps among the pectinate muscles form a plurality of holes, and the internal structure is very complex. Clinically, there are generally four main left atrial appendage shapes, namely "chicken wings", "cactus-like", "wind vane-like" and "cabbage-like". In addition to these four major left atrial appendage shapes, a very few bottom multilumen left atrial appendage morphologies are clinically encountered. The size, shape, internal structure and the like of the left atrial appendage also have great difference due to different individuals, which brings great challenge to the design of the left atrial appendage occluder.
The left atrial appendage occluder on the market at present mainly comprises an inner plug and an outer occlusion. The inner plugging left auricle plugging device is used for plugging the left auricle by an instrument, and the outer plugging is used for covering a plugging disc on the mouth of the left auricle to achieve the purpose of plugging the left auricle.
Many interior stopper left atrial appendage occluders, the limitation mainly lies in: when the instrument is plugged into the left auricle, the shape of the left auricle part is irregular, and the deformation capability of the instrument is limited, so that the left auricle part cannot be completely plugged, and residual leakage occurs.
Disclosure of Invention
In order to solve the problem that the left atrial appendage occluder in the prior art is not closed tightly, the invention provides a left ventricular occluder and a using method thereof, which can effectively solve the technical problem.
The technical scheme adopted by the invention for solving the technical problems is as follows: a left ventricular occluder, comprising: the method comprises the following steps:
the plugging disc is used for plugging the left auricle part;
the fixed disc is connected with the inner side wall of the left auricle to fix the plugging disc at the mouth part of the left auricle;
the connecting part is arranged between the fixed disc and the plugging disc and is used for connecting the fixed disc and the plugging disc into a whole;
the outer ring of the plugging disc is provided with a sucking disc part for adsorbing the left auricle part and fixing the plugging disc at the left auricle part to prevent residual leakage.
The fixed disk includes:
a fixing part detachably connected to the connection part;
a claw part arranged at one end of the fixing part far away from the connecting part;
the claw part is used for being connected with the inner side wall of the left auricle to prevent the plugging disc from plugging untight.
The claw portion includes:
the extension rod is bent towards the inner side wall of the left auricle;
the anti-slip hook is arranged at the end part, far away from the connecting part, of the extension rod and is used for contacting the inner side wall of the left auricle and preventing slipping.
The mousing-hook is a bulge or a diamond frame towards the inner side wall of the left atrial appendage.
The claw portion includes:
the extension rod is bent towards the inner side wall of the left auricle;
the anti-drop hook is arranged at the end part of the extension rod, which is far away from the connecting part, and is used for contacting with the inner side wall of the left auricle to prevent the plugging disc from plugging untight;
and the tensioning mechanism is arranged on the extension rod and is used for providing force for the extension rod to move towards the inner side wall of the left auricle.
The tensioning mechanism comprises:
one end of the tensioning rod is hinged with the extension rod, and the other end of the tensioning rod penetrates through the plugging disc;
and the adjusting bolt is connected with the end part of the tensioning rod penetrating through the plugging disc.
A method of using a left ventricular occluder, comprising:
placing the fixed disk into the left auricle, and contacting the inner side wall of the left auricle by utilizing the self-expansion property of the fixed disk;
the sealing plate is fixed at the opening part of the left auricle to play a role of sealing by contacting the fixed disk with the inner side wall of the left auricle and preventing slipping; the sucking disc part is closely contacted with the tissues of the left auricle part, so that the residual leakage is prevented.
The invention has the beneficial effects that: according to the invention, the outer ring of the plugging disc is provided with the sucking disc part for adsorbing the left auricle part, and the plugging disc is fixed at the left auricle part, so that the residual leakage is prevented; furthermore, the plugging disc is firmly fixed in the left auricle by using the tensioning mechanism to prevent looseness. The invention solves the problem that the left auricle occluder in the prior art is not tight in occlusion, and has the advantages of simple structure and good occlusion effect.

The invention will be further described with reference to the accompanying drawings.
Specific example I: 1-4, a left ventricular occluder, comprising: the plugging device comprises a plugging disc 1, a connecting part 2 and a fixed disc 3; wherein, the plugging disc 1 is used for plugging the left auricle; the fixed disc 3 is connected with the inner side wall of the left auricle to fix the plugging disc 1 at the mouth part of the left auricle; the connecting part 2 is arranged between the fixed disc 3 and the plugging disc 1 and is used for connecting the fixed disc 3 and the plugging disc 1 into a whole; the outer ring of the plugging disc 1 is provided with a sucking disc part 4 for adsorbing the left auricle part, and the sucking disc part is used for fixing the plugging disc 1 at the left auricle part to prevent residual leakage.
It should be clear that the suction cup portion 4 may be preferably made of medical rubber, and is relatively soft and not prone to rejection. Meanwhile, the structure of the sucker part 4 is very easy to adsorb tissues at the left auricle part, so that the plugging disc 1 is prevented from being insufficiently adsorbed.
The fixed disk 3 includes: a fixing portion 301 and a claw portion 302; wherein, the fixing part 301 is detachably connected with the connecting part 2; a claw portion 302 is provided at one end of the fixing portion 301 away from the connecting portion 2; the claw part 302 is used for being connected with the inner side wall of the left auricle to prevent the plugging disc 1 from plugging untight.
Preferably, the fixed disk 3 takes a self-expansion nickel-titanium alloy frame as a stent, and the stent is covered with a film.
The claw portion 302 includes: an extension rod 3021, an anti-drop hook 3022;
wherein, the extension rod 3021 is bent towards the inner side wall of the left auricle; the anti-slip hook 3022 is arranged at the end of the extension rod 3021 far away from the connecting part 2 and is used for contacting the inner side wall of the left atrial appendage and preventing slipping; preferably, the anti-slip hook 3022 is a protrusion or a diamond frame facing the inner side wall of the left atrial appendage.
The connecting part 2 comprises a spring 201, a flexible rod 202 and a nut 203; one end of the flexible rod 202 is connected to the fixing portion 301, and the other end of the flexible rod passes through the plugging disc 1 and then is fixed to the plugging disc 1 through the nut 203. The distance between the fixed disc 3 and the plugging disc 1 can be adjusted within a certain range through the combined action of the nut 203, the extension rod 3021 and the spring 201, so that the tissue connection between the claw part 302 and the inner side wall of the left atrial appendage is adjusted to be firm.
Specific example II: as shown in fig. 5 to 6, the claw portion 302 includes: an extension rod 3021, an anti-drop hook 3022, and a tightening mechanism; wherein, the extension rod 3021 is bent towards the inner side wall of the left auricle;
the anti-disengaging hook 3022 is arranged at the end part of the extension rod 3021 far away from the connecting part 2 and is used for contacting with the inner side wall of the left atrial appendage to prevent the plugging disc 1 from being blocked; and the tensioning mechanism is arranged on the extension rod 3021 and is used for providing force for the extension rod 3021 to move towards the inner side wall of the left auricle.
The tensioning mechanism comprises: a tensioning rod 3023 and an adjusting bolt 3024; one end of the tensioning rod 3023 is hinged to the extension rod 3021, and the other end of the tensioning rod 3023 passes through the plugging disc 1; the adjusting bolt 3024 is connected to the end of the tension rod 3023 passing through the plugging disc 1, and the contact angle between the extension rod 3021 and the inner side wall of the left atrial appendage is adjusted by adjusting the length of the tension rod 3023, so as to improve the gripping force of the claw portion 302.
In use, the occluding device of the specific embodiment I, II comprises:
placing the fixed disk 3 into the left auricle, and contacting the inner side wall of the left auricle by utilizing the self-expansion property of the fixed disk 3;

a fixing part detachably connected to the connection part;
a claw part arranged at one end of the fixing part far away from the connecting part;
the claw part is used for being connected with the inner side wall of the left auricle to prevent the plugging disc from plugging untight;
the claw portion includes:
the extension rod is bent towards the inner side wall of the left auricle;
the anti-drop hook is arranged at the end part of the extension rod, which is far away from the connecting part, and is used for contacting with the inner side wall of the left auricle to prevent the plugging disc from plugging untight;
the tensioning mechanism is arranged on the extension rod and used for providing force for the extension rod to move towards the inner side wall of the left auricle;
the tensioning mechanism comprises:

the anti-slip hook is arranged at the end part, far away from the connecting part, of the extension rod and is used for contacting the inner side wall of the left auricle and preventing slipping.
3. A left atrial appendage occluder as in claim 1, wherein: the mousing-hook is a bulge or a diamond frame towards the inner side wall of the left atrial appendage.
4. A left atrial appendage occluder as in claim 1, wherein: the hook claw part is coated with a film by taking an extension rod as a framework.
