一种人工髋关节股骨柄假体
技术领域
本发明属于人工假体置换技术领域,具体地说,涉及一种人工髋关节股骨柄假体。
背景技术
在人工全髋关节置换手术中,需要通过获得良好的髋臼及股骨的联合前倾角来达到髋关节的安全稳定活动。
在实际操作过程中,髋臼侧假体的放置需要兼顾宿主骨的覆盖。在髋臼骨折、髋臼旋转截骨术后、发育性髋臼后倾等病例中,假体大多需要偏中立甚至后倾安放。而股骨自身前倾角变异较大,尤其以髋关节发育不良的患者居多。
由此可见,在全髋关节置换术中,对股骨侧前倾角的调节存在着很大的需求。由于股骨柄与股骨髓腔形态匹配的问题,现有的单楔形或双楔形股骨柄改变前倾角的范围非常有限。
为了尽量满足股骨侧调节前倾角的需求,现在主要有以下三种实现方案:
第一,水泥型股骨柄。由于水泥柄本身无需与髓腔形态匹配,因此可以调节前倾角。
但是,水泥型股骨柄存在诸如:固定远期失败率高、术中产热可能导致骨坏死、可能产生潜在的磨损颗粒、翻修困难等弊端。因此,这种水泥型股骨柄的应用效果并不好。
第二,以Wagner cone(Zimmer)为主要代表的一体式圆锥型带棱股骨柄。这种股骨柄整体为圆锥型设计,其圆形的横截面便于以不同的前倾方向植入股骨柄。
但是,这种股骨柄圆锥型的外观在实际应用中会带来正反两方面影响:一方面,在植入过程中能够与股骨髓腔进行较好的适配和充填(fit-and-fill);而另一方面,在手术后有进一步下沉的风险。此外,在高脱位髋关节发育不良的病例中,常常需要进行粗隆下截骨;而这种股骨柄的柄长有限,因此对截骨端的抗旋转能力不足,需要使用额外的器械如钢板或将截除骨块劈开捆绑在截骨端。增加了手术的复杂性和操作难度,同时也增加了成本。
第三,组配式假体。根据组配界面位置的不同,包括颈组配假体,如:Kinectiv(Zimmer)、Profemur(Wright)等;以及袖套组配假体,如:S-rom(Depuy)等。前者通过选择不同前倾方向的颈部组件来调节前倾角;后者则可实现柄和袖套之间的自由旋转,在更大范围内调节前倾角,因此应用更广泛。
但是,无论上述哪种组配式假体,组配的交界面都会带来隐患。组配界面的微动会产生金属磨屑,导致局部异物反应;此外,两者均出现过界面断裂等机械并发症。
因此,目前急需一种应用于人工全髋置换手术中的能够不局限于股骨髓腔形态而调节前倾角的股骨柄假体。
发明内容
为了解决现有技术中存在的问题,本发明提供了一种人工髋关节股骨柄假体。
根据本发明的一个方面,提供一种人工髋关节股骨柄假体,所述股骨柄假体包括依次连接的股骨柄锥、股骨颈和股骨柄;
所述股骨柄包括:一体形成的股骨柄近端、股骨柄身和股骨柄远端;
所述股骨柄近端按照其横截面的圆心角的角度分为股骨柄近端内侧和股骨柄近端外侧,其中,所述股骨柄近端内侧所对应的圆心角为120°;
所述股骨柄近端内侧的锥度沿所述股骨柄由近至远的方向递减;
所述股骨柄近端外侧的锥度恒定不变;
所述股骨柄远端为圆柱结构或锥形结构。
根据本发明的一个具体实施方式,所述股骨柄近端内侧的锥度沿所述股骨柄由近至远的方向从10°至5°递减。
根据本发明的另一个具体实施方式,所述股骨柄近端外侧的锥度为5°。
根据本发明的又一个具体实施方式,所述股骨柄近端外侧的外表面经过喷砂处理。
根据本发明的又一个具体实施方式,在股骨柄近端外侧的外表面设置两个鳍状突起。

本发明提供的人工髋关节股骨柄假体采用一体成型结构,规避了组配界面带来的如柄交界面断裂,金属磨屑及局部异物反应等潜在风险;近端内侧的阶梯状微结构处理,将假体-骨界面之间的剪应力转化为压应力,提高初始稳定性;近端表面的多孔涂层具有良好的摩擦系数、孔隙率和骨诱导性,促进骨长入,提供远期稳定性。
所述股骨柄创造性地在股骨柄近端内侧设置渐变锥度区域,一方面大大降低了传统股骨柄假体整体圆锥型设计带来的下沉风险;另一方面又可以在尽量减少骨质破坏的基础上实现股骨髓腔内旋转角度的调节。更重要的是,渐变锥度区域与袖套组配假体的近端三角形袖套组件相比更偏向外侧,对近端内侧骨质破坏更少,植入更容易,且可避免出现类似于袖套在股骨近端高悬的现象。后者使肢体过度延长,带来血管神经损伤的风险。由于这种设计更便于植入髓腔,这也间接降低了进行粗隆下截骨的可能。即使必要时行粗隆下截骨,这种设计提供了足够的柄长发挥髓内钉稳定截骨处,此外,柄远端突出表面的棱可以嵌入骨质,进一步提供旋转稳定性。

所述股骨柄近端31按照其横截面的圆心角的角度分为股骨柄近端内侧和股骨柄近端外侧。其中,所述股骨柄近端内侧所对应的圆心角为120°。可以理解,圆心角对应240°的即为股骨柄近端外侧。
为了促进骨生长,优选的,在所述股骨柄近端31的表面涂覆有多孔钛金属涂层、钽金属涂层或钛喷浆+羟基磷灰石涂层。为了获得最佳的骨长入效果,不同的涂层有不同的孔隙率、孔径等参数。若采用多孔钛金属涂层,则孔隙率55-70%,孔径300μm-640μm。若采用钽金属涂层,则孔隙率80%,孔径550μm。若采用钛喷浆+羟基磷灰石进行涂覆,具体的采用330μm钛+50μm羟基磷灰石,表面粗糙度在21μm-28μm之间。
进一步的,为了将假体-骨界面之间的剪应力转变为压应力,提高初始稳定性,优选的,在所述股骨柄近端31的内侧120°锥度渐变区做阶梯状微结构处理。
为了使股骨柄30更容易调节前倾角和植入髓腔,所述股骨柄近端内侧的锥度沿所述股骨柄30由近至远的方向递减。更为优选的,所述股骨柄近端内侧的锥度沿所述股骨柄30由近至远的方向从10°至5°递减。
参见图3和图4,首先在图3所示的股骨柄近端31上沿所述股骨柄30由近至远的方向选取三个截面,分别是A-A、B-B、C-C。以图4中截面A-A为例,圆心角对应120°的一侧是股骨柄近端31的内侧,另外一侧则为股骨柄近端31的外侧。截面B-B和截面C-C同理。
从图4中可以看出,不同的截面,股骨柄近端内侧的弧度不同,因此可以看出,股骨柄近端内侧的锥度是在变化的,而且是沿所述股骨柄30由近至远的方向递减的。而在不同的截面图中,股骨柄近端外侧的弧度却是相同的,即所述股骨柄近端外侧的锥度恒定不变。优选的,所述股骨柄近端外侧的锥度为5°。
上述阶梯状的微结构处理,可有效地将股骨柄30与髓腔之间的剪切力转变为压应力,提高初始稳定性,而所述股骨柄近端31的渐变锥度设计降低了传统股骨柄假体(如:Wagner cone)整体圆锥型设计带来的下沉风险。此外,所述股骨柄近端30的渐变锥度区域,相比传统的三角形袖套(如:S-rom),整***置更偏向外侧,对骨质破坏更少,不仅股骨柄假体的植入更便捷,而且其在股骨髓腔内的旋转角度也可灵活调节。此外,得益于其植入的便捷性,应用所述股骨柄相对于三角形袖套可以在股骨髓腔植入更深,避免肢体的过度延长,降低行粗隆下截骨的可能。
为了更便于植入,且进一步降低成本,可以仅在所述股骨柄近端外侧的外表面进行喷砂处理。
此外,为了进一步增加股骨柄近端30植入后的抗旋转能力,优选的,在股骨柄近端外侧的外表面设置两个鳍状突起。更为优选的,所述两个鳍状突起对称设置。
所述股骨柄远端33为圆柱结构或锥形结构。
当股骨柄远端33采用锥形结构时,优选的,所述股骨柄远端33呈2°的锥度。可以理解,股骨柄30的整体结构都是沿着股骨柄30由近至远的方向向内缩进,形成一个锥形,以便于植入操作。
优选的,所述股骨柄远端33的外表面设置有凸棱。通过这些棱与骨皮质的咬合,能够显著提高股骨柄远端33的旋转稳定性。随着凸棱个数的增多,所述股骨柄远端33的抗旋稳定性也在提高。但是过多的凸棱,会增加生产成本,因此优选的,所述凸棱的数目为8个,且均匀分布与所述股骨柄远端33的外表面。更为优选的,所述凸棱的高度在1mm-1.25mm之间,例如:1mm,1.15mm或1.25mm。
此外,为了降低股骨柄30的刚度,减少大腿疼痛的发生率,所述股骨柄远端33的尖部呈音叉形结构,即所述股骨柄远端33的尖部设置有开槽。
本发明提供的人工髋关节股骨柄假体能够与股骨髓腔形态高度匹配,前倾角可调,植入便捷、抗旋能力强、术后风险低,易于大规模推广使用。
虽然关于示例实施例及其优点已经详细说明,应当理解在不脱离本发明的精神和所附权利要求限定的保护范围的情况下,可以对这些实施例进行各种变化、替换和修改。对于其他例子,本领域的普通技术人员应当容易理解在保持本发明保护范围内的同时,工艺步骤的次序可以变化。

Claims (10)

1.一种人工髋关节股骨柄假体,其特征在于,所述股骨柄假体包括依次连接的股骨柄锥、股骨颈和股骨柄;
所述股骨柄包括:一体形成的股骨柄近端、股骨柄身和股骨柄远端;
所述股骨柄近端按照其横截面的圆心角的角度分为股骨柄近端内侧和股骨柄近端外侧,其中,所述股骨柄近端内侧所对应的圆心角为120°;
所述股骨柄近端内侧的锥度沿所述股骨柄由近至远的方向递减;
所述股骨柄近端外侧的锥度恒定不变;
所述股骨柄远端为圆柱结构或锥形结构。
2.根据权利要求1所述的股骨柄假体,其特征在于,所述股骨柄近端内侧的锥度沿所述股骨柄由近至远的方向从10°至5°递减。
3.根据权利要求1所述的股骨柄假体,其特征在于,所述股骨柄近端外侧的锥度为5°。
4.根据权利要求1所述的股骨柄假体,其特征在于,所述股骨柄近端外侧的外表面经过喷砂处理。
5.根据权利要求4所述的股骨柄假体,其特征在于,在股骨柄近端外侧的外表面设置两个鳍状突起。
6.根据权利要求1所述的股骨柄假体,其特征在于,当所述股骨柄远端为锥形结构时,所述股骨柄远端呈2°的锥度。
7.根据权利要求1所述的股骨柄假体,其特征在于,所述股骨柄远端的外表面设置有凸棱。
8.根据权利要求1所述的股骨柄假体,其特征在于,所述股骨柄远端的尖部呈音叉形结构。
9.根据权利要求1所述的股骨柄假体,其特征在于,所述股骨柄近端的表面涂覆有多孔钛金属涂层、钽金属涂层或钛喷浆+羟基磷灰石涂层。
10.根据权利要求1所述的股骨柄假体,其特征在于,所述股骨柄近端内侧的外表面采用阶梯状微结构进行处理。
