Background
With the widespread development of artificial joint replacement, more and more patients need to be operated, and artificial joint revision surgery has become a routine surgical operation. For extensive bone defects of acetabulum after hip replacement and bone loss around knee joint, a plurality of reconstruction modes are available clinically, wherein one mode is bone grafting, and the main purpose is to recover enough bone quantity and mechanical initial stability characteristics as far as possible.
Bone defects are common in the artificial joint revision surgery, and small defects are reconstructed by adopting methods such as granular bone grafting, structural bone grafting or expansion of the diameter of a revision prosthesis, and the like, so that good effects can be obtained. When the defect is serious, the revision is often very troublesome, the difficulty is that an effective supporting structure is lacked, the existing method for filling the defect of the metal bone in the revision of the hip joint is a relatively approved mode, the method can ensure the good support of the metal bone, and can effectively avoid the defect of bone grafting materials.
The metal bone defect filling patch conventionally adopts a spherical crescent structure, the original deformed or diseased acetabulum needs to be ground in the operation, after the matched shape is ground, the patch is fixed on a bone bed of the defect acetabulum socket in a screw mode to form a complete acetabulum socket, and then an acetabulum cup is sunk into the formed acetabulum socket. The acetabulum socket and the patch are bonded by bone cement or are not treated at all, but are only extruded and embedded, so that a good connection mode is not provided to prevent dislocation and slippage between the patch and the acetabulum cup, the future stability is still to be observed, and the function recovery effect of the acetabulum is influenced; meanwhile, any metal filling patch cannot replace the bone of the patient himself, and the function recovery effect of the hip joint is influenced.

As shown in fig. 1 to 3, an embodiment of the present invention provides an acetabular prosthesis including: the acetabulum cup 10 is provided with a relevant nodal surface in the groove of the acetabulum cup 10; a filling structure 20 disposed on the acetabular cup 10 with the filling structure 20 on a side of the acetabular cup 10 facing away from the articular surface, at least a portion of the filling structure 20 configured to fill in a defective acetabular socket to support the acetabular cup 10, at least a portion of the filling structure 20 being made of a resorbable alloy.
By applying the technical scheme of the embodiment, the acetabular cup 10 and the filling structure 20 are arranged in the acetabular prosthesis, and the relevant joint surfaces are arranged in the groove of the acetabular cup 10; a filling structure 20 is provided on the acetabular cup 10 with the filling structure 20 on a side of the acetabular cup 10 facing away from the articular surface, at least a portion of the filling structure 20 being configured to fill in a defective acetabular socket to support the acetabular cup 10, at least a portion of the filling structure 20 being made of a resorbable alloy. Because the filling structure 20 is connected with the acetabular cup 10, after implantation, the filling structure 20 supports the acetabular cup 10, and the filling structure 20 and the acetabular cup 10 cannot be dislocated or slide, so that the stability is high; moreover, since at least a portion of the filling structure 20 is made of absorbable alloy, the absorbable alloy will be gradually absorbed by the human body after long-term implantation and replaced by new bone growth, thereby realizing bone reconstruction. Therefore, the technical scheme can realize the effects of early high-strength filling and supporting and medium and long term bone reconstruction.
In the present embodiment, the filling structure 20 includes a porous structure 21 and a solid structure 22, the solid structure 22 is disposed inside the porous structure 21, and the porous structure 21 is made of absorbable alloy. By providing the porous structure 21, bone tissue may grow into the plurality of pores of the porous structure 21 after implantation, thereby forming a secure connection and fusion. Moreover, the porous structure 21 is made of an absorbable alloy, and is easily absorbed by the human body due to its multiple pores. By means of the solid structure 22, the strength of the filling structure 20 can be ensured.
In the present embodiment, the size of the pores in the porous structure 21 gradually decreases from the outside to the inside of the porous structure 21. This prevents the porous structure 21 from being absorbed too quickly and having less time to grow new bone mass. From the outside to the inside of the porous structure 21, the size of the pores in the porous structure 21 is gradually reduced, so that the effect that the outside of the porous structure 21 is easily absorbed and the inside of the porous structure 21 is relatively not easily absorbed can be realized, and the controllable absorption rate can be realized.
In the present embodiment, the porous structure 21 and the solid structure 22 are integrally formed, and both the porous structure 21 and the solid structure 22 are made of absorbable alloy. With the above arrangement, when implanted for a long period of time, all or a part of the porous structure 21 and the solid structure 22 may be absorbed by the human body and replaced with new bone growth, thereby achieving bone reconstruction.
In the present embodiment, the filling structure 20 further includes: the fastener 23, the solid structure 22 is connected to the acetabular cup 10 by the fastener 23. This allows the solid structure 22 to be coupled to the acetabular cup 10 via the fasteners 23, thereby providing a secure coupling of the acetabular cup 10 to the filling structure 20.
As shown in fig. 3, the fastener 23 includes a first connecting section and a second connecting section connected with each other, wherein the first connecting section is inserted into the acetabular cup 10, the second connecting section has an external thread, the solid structure 22 has an internal thread, and the second connecting section is in threaded connection with the solid structure 22. This allows for a secure connection of the acetabular cup 10 to the filling structure 20 by way of a threaded connection.
Optionally, the acetabular cup 10 and the fastener 23 are both made of a titanium alloy. Such that the acetabular cup 10 and fastener 23 are strong, lightweight, and facilitate achieving biological fusion.

By providing the post 12, a secure connection of the acetabular cup 10 to the filling structure 20 is facilitated. Moreover, when at least a part of the filling structure 20 is absorbed, the column 12 is still in the bone, which can prevent the cup 11 from rotating, thereby improving the reliability.
In this embodiment, the porous structure 21 is a cylindrical structure, the porous structure 21 has a containing cavity, the solid structure 22 is located in the containing cavity and connected to the bottom of the containing cavity, the column 12 is located in the containing cavity and connected to the solid structure 22, and one end of the porous structure 21 is connected to the cup 11. The porous structure 21 is arranged in a columnar structure, so that the bone bed can be conveniently penetrated. Through the arrangement, the part of the filling structure 20 contacting with the bone bed can be completely made into the porous structure 21, so that on one hand, reliable connection and fusion can be realized, and on the other hand, the filling structure is convenient to absorb.
Optionally, porous structure 21 has first end and second end, and wherein, first end has the arcwall face, and the arcwall face of first end and the surface butt of cup 11, the second end has the fillet, through the aforesaid setting, can make porous structure 21 and cup 11 reliable cooperation to set up the fillet and be convenient for porous structure 21 to wear into the bone bed. In this embodiment, the porous structure 21, the solid structure 22 and the column 12 are all cone-shaped structures.
Optionally, the porous structure 21 comprises a first porous section and a second porous section connected with each other, wherein the first porous section is a continuous columnar structure and is filled with a plurality of pores from the surface to the inside, the second porous section is a cylindrical structure, the side wall of the second porous section has a plurality of pores, and the second porous section has the accommodating cavity therein. This facilitates the arrangement and connection of the different components and this allows the portion of the filling structure 20 in contact with the bone bed to be entirely porous 21, on the one hand allowing a reliable connection and fusion, and on the other hand facilitating resorption.
In the present embodiment, the absorbable alloy is an absorbable magnesium alloy. The magnesium alloy is adopted, so that the effects of early high-strength filling and supporting, medium-long term absorption and bone reconstruction are conveniently realized.
In this embodiment, the filling structure 20 is a plurality of filling structures 20, and the plurality of filling structures 20 are spaced apart along the circumference of the acetabular cup 10. This may improve the filling effect of the defect site with the plurality of filling structures 20, thereby improving the support effect of the acetabular cup 10 for reliable support and prosthetic implantation.

The technical scheme has the following advantages: the prosthesis has better adaptability to independent defects and combined bone defects of the ilium region, the ischial region and the pubis region, and can realize good prosthesis fixation. The conical connection and the threaded connection are combined, so that the fixation is more reliable. The length and the diameter of the porous structure can be changed, and the porous structure is suitable for defects with various sizes. The porous structure is made of absorbable magnesium alloy, and any metal filling can not replace the bone of a patient, so that the scheme fully utilizes the advantages of the absorbable magnesium alloy to achieve the effects of early high-strength filling and medium-long bone reconstruction. The device is particularly suitable for the case that the periphery of the acetabulum lacks effective support after revision. The porous structure is made into gradient gaps from outside to inside to adjust the absorption rate of the magnesium alloy, and the solid structure is arranged in the porous structure. After the magnesium alloy is absorbed, the main body on the acetabular cup is inserted in the bone, and the anti-rotation effect can be exerted in a long term. The fixing device is flexible and convenient to install, is fixed and stable, and is a scheme capable of reconstructing bones.
The above description is only a preferred embodiment of the present invention and is not intended to limit the present invention, and various modifications and changes may be made by those skilled in the art. Any modification, equivalent replacement, or improvement made within the spirit and principle of the present invention should be included in the protection scope of the present invention.
