Description
技术领域
本发明涉及一种作为有效成分包括组蛋白去乙酰化酶抑制剂(histonedeacetylase inhibitor)和激活因子(priming factor)的干细胞活性促进用组合物。
背景技术
间充质干细胞(Mesenchymal stem-cells,MSCs)是从骨髓((bone-marrow,BM)、脂肪、牙髓(dental pulp)、脐带(umbilical-cord,UC)以及脐带血(UC-blood,UCB)分离出的多功能祖细胞(multipotent progenitor cell)。MSC(Mesenchymal stem cell)的施用可以修复受损的器官的脏器组织,从而提高治疗效果。MSC的功效起因于旁分泌因子(paracrine factor),上述旁分泌因子具有由细胞因子(cytokine)调节的细胞保护效果(cytoprotective effect)、促血管生成(pro-angiogenic)和促动脉生成(pro-arteriogenic)的效果。然而,目前MSC治疗由于治疗有效性低,尤其是体内的植入(engraftment)和移植的细胞的生存率低,因此存在极限。大部分(≥99%)情况下,会将MSCs注射到肺附近的静脉,而在其中只有2-3%被排出到循环***。因此,有必要理解在组织修复过程中对于MSCs的移动和植入的正确的机制。
几种趋化因子,例如基质细胞衍生因子-1(stromal cell derived factor-1,SDF-1)以及生长因子对成体MSCs的移动和植入起到重要的作用,其中上述生长因子例如可以为血管内皮生长因子(vascular endothelial growth-factor,VEGF)、血小板衍生生长因子(platelet-derived growth-factor,PDGF)或者肝细胞生长因子(hepatocytegrowth-factor,HGF)。其中,SDF-1和作为其的受体的CXCR4轴(axis)对于干细胞的归巢(homing)/植入而言是重要因子。事实上,在骨髓中根据SDF-1梯度(gradient),在进行为了移植的预处理(conditioning)后,骨髓中的造血干细胞/祖细胞(hematopoietic stem/progenitor cells,HSPCs)的植入向上调节(up regulation)。对SDF-1梯度的HSPCs的敏感性/反应性通过丰富的分子聚合对受损的组织产生积极的影响(“primed”)。这里包括有活体活性脂质(例如,鞘氨醇-1-磷酸(sphingosine-1-phosphate,S1P)以及神经酰胺-1-磷酸(ceramide-1-phosphate,C1P))、嗜中性粒细胞来源的阳离子抗菌肽(cathelicidin)(LL-37)、β2-抵御素(defensin)以及可溶性膜攻复合物(soluble membrane attack complex,sMAC)(C5b-9)。
本发明人最近确认了,在HSPCs过程中观察到的激活现象也相似地发生在向S1P和CP1活体活性脂质以及作为阳离子肽的LL-37激活的MSCs。尤其,激活的MSCs在细胞培养条件中促进细胞移动、促进集落形成活性以及抗炎症能力,由此能提高肺动脉高压(pulmonary arterial hypertension,PAH)的治疗效果。然而,激活的MSCs依然在受损的组织中表现出较低的体内植入。而且,虽然在施用MSC前进行了密集的清洗过程,但还是没能完全清除触发负面炎症反应的残余激活因子。因此,需要研发在低浓度下的激活策略。

本发明的目的在于提供一种作为有效成分包括组蛋白去乙酰化酶抑制剂和激活因子(Priming factor)的干细胞活性促进用组合物,并还提供利用其的干细胞活性促进方法。
并且,本发明的另一个目的在于提供一种高血压预防或治疗用药物组合物,其作为有效成分包括由组蛋白去乙酰化酶抑制剂和激活因子处理的干细胞或者上述干细胞的培养液。
并且,本发明的又一个目的在于提供一种炎症性疾病或免疫性疾病的预防或治疗用药物组合物,其作为有效成分包括由组蛋白去乙酰化酶抑制剂和激活因子处理的干细胞或者上述干细胞的培养液。

有益效果
本发明涉及一种作为有效成分包括组蛋白去乙酰化酶抑制剂和激活因子的干细胞活性促进用组合物,并且用低浓度的组蛋白去乙酰化酶抑制剂和激活因子处理干细胞时,明显提高干细胞的活性。尤其,激活的MSCs在细胞培养条件下,促进细胞移动、集落(colony)形成活性以及抗炎症能力,从而能能提高高血压、炎症性疾病或免疫性疾病的治疗效果,因此有望成为有效的治疗策略。

图4显示VPA在用S1P激活的UC-MSCs中的效果的结果。图4的A和图4的B分别显示:用VPA单独、S1P单独、或VPA+S1P激活24小时的UC-MSCs的细胞增殖分析(n=3)以及CFU-F分析(n=6)的结果。为了CFU-F分析,将60个细胞接种在培养板,培养14天,并定量集落的数量。贴附细胞的代表性的染色的集落显示在图的右侧。图4的C显示:根据从标记的细胞收获的条件培养基(conditioned medium,CM)的存在与否,由用LPS刺激5小时的大鼠肺细胞巨噬细胞系分泌的TNF-α蛋白质(n=4)的定量结果。所有的结果以如下方式表示:means±SEM,*p<0.05,**p<0.01,***p<0.001与非激活细胞的比较(NT),#p<0.05,###p<0.001与VPA+S1P激活细胞比较,单因素方差分析检验(One-way ANOVAtest)。
图5显示VPA+S1P向上调节的与MSCs的治疗效果相关的基因。图5的A至图5的D分别表示:用0.5mM的VPA单独(VPA)、50nM的S1P单独(S1P)、或者0.5mM的VPA与50nM的S1P的组合(VPA+S1P)激活24小时的UC-MSCs中的MMP12(图5的A)、生长因子(图5的B)、血管新生(图5的C)、抗炎症(图5的D)的相关基因的表达程度。表达程度根据未激活的UC-MSCs(NT)与激活的UC-MSCs的数值比率进行计算。结果以如下方式显示:means±SEM(n≥4);*p<0.05,**p<0.01,***p<0.001与NT组比较(compared to NT group),#p<0.05,###p<0.001与VPA+S1P比较(compared to VPA+S1P)(单因素方差分析及邦弗朗尼事后检验(one-way ANOVA withBonferroni post-test))。

图7为表示VPA+S1P激活的UC-MSC提高治疗(对于HCl-IC大鼠模型中的膀胱功能恢复)可能性的结果。图7的a显示非麻醉膀胱内压测量(awake cystometry)的结果。图7的b显示将用对照组(naive)或VPA+S1P激活的UC-MSCs(每个组中独立存在5只动物)以2.5×105(250K)的细胞数(K=1,000)进行注射后,并在一周后检测的膀胱排尿参数的定量结果。其中,IVP表示膀胱内压(intravesical pressure),IAP表示腹腔内压。Sham表示假手术组(sham-operated)。所有的结果以如下方式表示:means±SEM;*p<0.05,**p<0.01,***p<0.001与HCl-IC组比较(compared to the HCl-IC group);#p<0.05,##p<0.001,###p<0.001与UC-MSC对照组比较,且利用邦弗朗尼事后检验。
图8显示在HCl-诱导的膀胱损伤中,对UC-MSC给药效果的组织学分析结果。图8的a显示施用PBS或250K细胞数之后,经过一周,UC-MSCs HCl-IC鼠的膀胱组织中的(i)细胞角蛋白(cytokeratin)免疫染色(放大倍数×40,比例尺(scale bar)=100μm),(ii)甲苯胺蓝(Toluidine blue)(放大倍数×200,比例尺=100μm),(iii)马松三色(Masson'strichrome)(放大倍数×200,比例尺=100μm)以及(iv)TUNEL分析(放大倍数×400,scalebar=100μm)的结果。箭头表示被浸润的肥大细胞。其中,Sham表示假手术组(sham-operated)。细胞核是用梅氏苏木精(Mayer's hematoxylin)(i、ii和iii)或DAPI(蓝色、iv)进行了染色。图8的b是对组织学实验进行定量化的结果。数据是通过假手术组(shamgroup)(n=15)进行了标准化。并且,结果以如下方式显示:mean±SEM,*p<0.05,**p<0.01,***p<0.001与HCl-IC组比较(compared to the HCl-IC group);#p<0.05,##p<0.001,###p<0.001与UC-MSC对照组比较,且利用邦弗朗尼事后检验(compared to thenaive UC-MSC group with Bonferroni post-test)。
图9显示了用UCB-MSCs和VPA+S1P激活的MSCs(VPA+S1P-MSC)在MCT诱导的PAH大鼠模型中的效果。图9的a显示出MCT诱导了RVSP程度的增加,并且在注入MCT之后,经两周后施用VPA+S1P激活的UC-MSC的结果,明显减少了这样的上升(图9的a的左侧)。MCT还增加了RV/(LV+S)的重量比率,然而VPA+S1P-UC-MSC同样明显减少了上述效果。图9的b显示出MCT增加了肺组织炎症和内壁(medial wall)厚度指数(每血管直径的血管壁厚度),然而VPA+S1P-MSC明显减少了上述增加(图9的b的上部)。未激活的UCB-MSC几乎不显示对RV/(LV+S)和内壁厚度指数的改善效果。图9的b的下部表示在肺动脉中显示α-SMA+平滑肌细胞的免疫荧光染色结果(放大倍数×400)。其中,RVSP表示右心室收缩压(right ventricular systolicpressure);CTL表示对照组;MSC表示人类脐带间充质干细胞(human cord bloodmesenchymal stem cell);S1P-MSC表示用鞘氨醇-1-磷酸激活的人类脐带间充质干细胞;RV表示右心室(right ventricle);LV+S表示左心室以及室间隔(interventricularseptum)。其中,以如下方式进行表示:*p<0.05,**P<0.01单独与MCT比较(compared to MCTalone)(单因素方差分析及邦弗朗尼事后检验)。
具体实施方式
作为干细胞激活的主要标志的CXCR4的表达通过属于DNA-甲基化酶抑制剂的5-氮杂胞嘧啶核苷(5-azacytidine,5-Aza)和属于组蛋白去乙酰化酶抑制剂的丙戊酸(valproic acid,VPA)向上调节。对此,本发明人在提高MSC激活而促进治疗效果的策略中,确认了上述表观遗传(epigenetic)调控因子的作用后完成了本发明。
本发明提供一种作为有效成分包括组蛋白去乙酰化酶抑制剂和激活因子的干细胞活性促进用组合物。
优选地,上述组蛋白去乙酰化酶抑制剂可以是丙戊酸(valproic acid,VPA)、丁酸钠(sodium butyrate,NaB)、烟酰胺(nicotinamide,NAD)或Sirt抑制剂(sirtinol),但并不限定于此。
优选地,上述激活因子可以是鞘氨醇-1-磷酸(sphingosine-1-phosphate,S1P)、神经酰胺-1-磷酸(ceramide-1-phosphate,C1P))、抗菌肽(cathelicidin)(LL-37)、吡格列酮(pioglitazone),但并不限定于此。
优选地,上述VPA可以包括0.1至1.0mM的浓度,上述S1P可以包括10至50nM的浓度,但并不限定于此。
本发明中使用的术语“激活(Priming)”是指为了增进干细胞的治疗效果而提高反应性(活性)的现象,本发明中利用可诱导激活的组蛋白去乙酰化酶抑制剂和激活因子(priming factor)来促进上述干细胞的活性。
在本发明中,“干细胞活性”是指干细胞的细胞移动性、细胞增殖性、多能性(向软骨细胞、骨细胞或脂肪细胞***的体外分化)、集落形成能力或抗炎症活性等。
本发明中促进活性或诱导激活不仅包括对干细胞直接以组蛋白去乙酰化酶抑制剂和激活因子进行处理时诱导干细胞的激活的情况,还包括利用通过上述激活处理(预处理)而提高活性的干细胞诱导其他分化的情况。
并且,通过将上述促进活性用组合物与用于治疗的细胞治疗剂混合后注入至体内,从而不仅能增进细胞治疗剂在体内的效果,还可以以将利用本组合物对干细胞自身进行处理后的功能增加的细胞治疗剂移植到体内的方法使用。
例如,本发明的激活是指能提高与干细胞功能相关的功能,其中包括干细胞的细胞移动性、集落形成能力以及抗炎症活性等。
在本发明中,使用的术语“干细胞”是指具有自我复制能力且具有能分化为2个以上的细胞的能力的细胞,而且干细胞可以分类为全能干细胞(totipotent stem cell)、多潜能干细胞(pluripotent stem cell)和多能干细胞(multipotent stem cell)。
对于本发明的干细胞,可以根据目的适当且不受限制地进行选择,并且干细胞可以来源于包括人类的哺乳动物,优选地,来自人类的公知的所有组织、细胞等成体细胞,例如,骨髓、脐带血、胎盘(或胎盘组织细胞)、脂肪(或脂肪组织细胞)等。
例如,上述干细胞可以是从骨髓、脂肪组织、肌肉组织、体外(ex vivo)培养的自身组织间充质干细胞、同种异体间充质干细胞、脐带血、胚卵黄囊(yolk sac)、胎盘、脐带、骨膜、胎儿以及***皮肤获得的干细胞,而且可以是不受限制地从血液获取的干细胞,还可以是来自胎儿或出生后不久的婴儿或成人的干细胞。
在本发明的优选体现例中,上述干细胞选自由神经干细胞、肝干细胞、造血干细胞、脐带血干细胞、表皮干细胞、胃肠道干细胞、内皮干细胞、肌肉干细胞、间充质干细胞以及胰腺干细胞构成的组,更优选地,可以选自由肝干细胞、造血干细胞、脐带血干细胞和间充质干细胞构成的组,但并不限定于此。
并且,本发明提供一种干细胞活性促进方法,其包括用组蛋白去乙酰化酶抑制剂和激活因子处理分离的干细胞的步骤。
优选地,上述组蛋白去乙酰化酶抑制剂可以是丙戊酸(valproic acid,VPA)、丁酸钠(sodium butyrate,NaB)、烟酰胺(nicotinamide,NAD)或特Sirt抑制剂(sirtinol),但并不限定于此。

另一方面,通过激活使干细胞活性化时,能对包括高血压在内的心血管系疾病起效,并且多个报告记载了关于激活(Priming)的干细胞对心血管系疾病起效的内容(干细胞研究和治疗(Stem Cell Research&Therapy)(2015)6:218;Stroke.2011;42:2932-2939;J.Cell.Mol.Med.Vol 17,No 5,2013pp.617-625;Stem Cells International 2015Article ID 685383)。
并且,本发明提供一种炎症疾病或免疫疾病的预防或治疗用药物组合物,其作为有效成分包括由组蛋白去乙酰化酶抑制剂和激活因子处理的干细胞或者上述干细胞的培养液。
另一方面,通过激活使干细胞活性化时,可参与免疫调节和炎症反应,从而可应用于多种炎症疾病和免疫疾病,并且多个报告记载了关于激活的干细胞对免疫调节和炎症反应的作用(J Orthop Res.2016 Apr 6,1-12;Stem Cell Rev and Rep(2014)10:351-375;Stem Cells International 2016 Article ID 9364213)。
优选地,上述组蛋白去乙酰化酶抑制剂可以是丙戊酸(valproic acid,VPA)、丁酸钠(sodium butyrate,NaB)、烟酰胺(nicotinamide,NAD)或Sirt抑制剂(sirtinol),但并不限定于此。

在本发明中,“培养液”包括能支持干细胞在体外生长和生存的培养基、以及包括在上述培养基的所培养干细胞的分泌物等。用于培养的培养基包括所有适用于干细胞的培养的本领域中使用的常规培养基。可以根据细胞的种类选择培养基和培养条件。用于培养的培养基优选细胞培养基本培养基(cell culture minimum medium,CCMM),通常包括碳源、氮源和微量元素成分。这样的细胞培养基本培养基,例如可以是达尔伯克改良伊格尔培养基(Dulbecco's Modified Eagle's Medium,DMEM)、低限基础培养基(Minimalessential Medium,MEM)、Eagle基本培养基(Basal Medium Eagle,BME)、RPMI1640、F-10、F-12、α最低必须培养基(αMinimal essential Medium、αMEM)、格拉斯哥最小必需培养基(Glasgow's Minimal essential Medium,GMEM)、伊斯科夫改良达尔伯克培养基(Iscove'sModified Dulbecco's Medium)等,但并不限定于此。
并且,上述培养基可以包括青霉素(penicillin)、链霉素(streptomycin)、庆大霉素(gentamicin)等抗生素。
另一方面,本发明能够以如下形态使用:均包括上述干细胞、干细胞的分泌物、培养基成分的形态;只包括分泌物和培养基成分的形态;分离分泌物而单独使用或与干细胞一起使用的形态;或者只施用干细胞而在体内生成分泌物的形态。
对于上述干细胞,可以利用本领域公知的任意方法获取。
使用本发明的组蛋白去乙酰化酶抑制剂和激活因子处理的干细胞可以用作治疗特定疾病的细胞治疗剂,上述处理可以是上述分子的直接处理或预处理。
上述“细胞治疗剂”是指用于复原细胞和组织的功能的以治疗、诊断、预防为目的医药品,其通过对活着的自体(autologous)细胞、同种(allogenic)细胞、异种(xenogenic)细胞进行体外增殖和筛选,或者用其他方法变化细胞的生物学特性等一系列行为来实现。
只要能达到目标组织,可以通过任意通常的方式将上述细胞治疗剂施用到人体。
在本发明的优选体现例中,上述高血压选自由以下高血压构成的组,该组包括:突发性肺动脉高血压;家族性肺动脉高血压;与胶原血管疾病、先天性体肺分流门静脉高血压、HIV感染、药物或毒素相关的肺动脉高血压;与甲状腺障碍、糖原累积病、戈谢(Gaucher)病、遗传性出血性毛细血管扩增、血色素病、骨髓增生障碍或脾切除术(splenectomy)相关的肺动脉高血压;与肺毛细血管血管瘤相关的肺动脉高血压;新生儿的持续性肺高血压;与慢性阻塞性肺疾病(chronic obstructivepulmonary disease)、间质性肺病(interstitial lung disease)、由缺氧症引起的肺泡通气障碍、由缺氧症引起的睡眠障碍、与呼吸或在高处的慢性暴露相关的肺高血压;与发育异常相关的肺高血压;以及由原位肺动脉的血栓栓塞引起的肺高血压,更优选地,高血压是突发性肺动脉高血压、家族性肺动脉高血压或慢性阻塞性肺疾病,最优选地,高血压是突发性肺动脉高血压。
在本发明的优选体现例,上述炎症疾病或免疫疾病可以是骨关节炎、类风湿性关节炎(Rheumatoid Arthritis)、膀胱炎、间质性膀胱炎、哮喘(Asthma)、皮肤炎(Dermititis)、过敏性皮炎、牛皮癣(Psoriasis)、囊性纤维化(Cystic fibrosis)、实体器官移植后的慢性排斥症(Post transplantation late and chronic solid organrejection)、移植物抗宿主疾病(graft-versus-host disease)、移植排斥症、多发性硬化症(Multiple Sclerosis)、全身性红斑狼疮(systemic lupus erythematosus)、舍格伦综合征(Sjogren syndrome)、桥本甲状腺炎(Hashimoto thyroiditis)、多肌炎(polymyositis)、硬皮病(scleroderma)、艾迪生病(Addison disease)、白斑症(vitiligo)、恶性贫血(pernicious anemia)、肾小球肾炎(glomerulonephritis)、肺纤维化(pulmonary fibrosis)、炎性肠病(Inflammatory Bowel Diesese)、克罗恩病(Crohnsdisease)、自身免疫性糖尿病(Autoimmune Diabetes)、糖尿病视网膜病(Diabeticretinopathy)、鼻炎(Rhinitis)、缺血再灌注损伤(Ischemia-reperfusion injury)、血管成形术后再狭窄(Post-angioplasty restenosis)、慢性阻塞性肺疾病(Chronicobstructive pulmonary disease;COPD)、格雷夫斯病(Graves disease)、胃肠道***反应(Gastrointestinal allergy)、结膜炎(Conjunctivitis)、动脉粥样硬化(Atherosclerosis)、冠状动脉疾病(Coronary artery disease)、心绞痛(Angina)、癌转移、小动脉疾病或线粒体病(mitochondrial disease),但并不限定于此。
对于本发明的药物组合物,除了有效成分以外,可以使用符合药剂学并在生理学上允许的辅助剂来实现制备,并且作为上述辅助剂可以使用赋形剂、崩解剂、甜味剂、结合剂、包覆剂、膨胀剂、润滑剂、增滑剂或芳香剂等增溶剂。对于本发明的药物组合物,优选地,除了有效成分以外还可以包括1种以上的符合药剂学的载体,从而实现药物组合物的制剂化,以便进行施用(给药)。对于液态溶液的组合物进行制剂化时,作为符合药剂学的载体应灭菌且适用于机体,可使用生理盐水、灭菌水、林格氏液、缓冲盐溶液、白蛋白注射液、葡萄糖溶液、麦芽糊精(maltodextrin)溶液、甘油、乙醇、以及混合这些成分中的1种以上成分,而且根据需要可以添加抗氧化剂、缓冲液、抑菌剂等其他常用的添加剂。并且,可以进一步添加稀释剂、分散剂、表面活性剂、结合剂以及润滑剂,从而实现如悬浮液和乳状液等注射用剂型、药丸、胶囊或片剂等的制剂化。
本发明的药物组合物的药物制剂形态可以是颗粒剂、粉剂、包衣片、片剂、胶囊剂、栓剂、糖浆(syrup)、汁、混悬剂、乳剂、点滴剂或可注射的液剂和活性化合物的缓释型制剂等。本发明的药物组合物可以通过静脉内、动脉内、腹腔内、肌肉内、胸骨内、经皮、鼻腔内、吸入、局部、直肠、经口、眼球内或皮内途径以常规方式进行给药。本发明的药物组合物的有效成分的有效量是指疾病的预防或治疗所需的量。因此,可以根据以下多种因子对有效量进行调节,上述因子包括疾病的种类;疾病的严重程度;包含在组合物的有效成分和其他成分的种类及含量;剂型的种类和患者的年龄、体重、平时健康状态、性别和膳食;给药时间、给药途径以及组合物的分泌率;治疗时间;同时使用的药物等。
并且,本发明提供一种用于促进干细胞活性的组蛋白去乙酰化酶抑制剂和激活因子的用途。

以下,为了理解本发明,通过实施例进行详细地说明。然而,以下实施例仅仅示意性的示出了本发明的内容,并且本发明的范围并不会被以下实施例所限定。本发明的实施例是为了向本领域技术人员更完整地说明本发明而提供的。
<实验例>
以下的实验例是能共同适用于根据本发明的各个实施例的实验例。
1.培养来源于人类脐带的MSCs
根据韩国峨山医院的生命论理审查委员会承认的准则,在得到父母的书面同意后,从健康且正常足月的新生儿获取了人类脐带(UC)。在获取UC之前得到了所有产妇的同意书。将脐带来源的MSCs(UC-derived MSCs,UC-MSCs)利用低葡萄糖的达尔伯克改良伊格尔培养基(Dulbecco's modified Eagle's medium,DMEM)(海克隆,匹兹堡,宾夕法尼亚州(HyClone,Pittsburgh,PA)),在5%的CO2的条件和37℃温度条件下进行培养,其中上述培养基添加有2mM的L-谷酰胺、20mM的4-(2-羟乙基)-1-哌嗪乙磺酸(4-(2-hydroxyethyl)-1-piperazineethanesulfonic acid,HEPES)(pH为7.3)、低限基础培养基(minimum-essential medium,MEM)、非必须氨基酸溶液、青霉素/链霉素(Corning Cellgro,Pittsburgh,PA)、1mg/ml的抗坏血酸(Sigma-Aldrich)、10%热灭活的胎牛血清(fetalbovine serum,FBS)(HyClone)、5ng/mL的人类上皮细胞生长因子(Sigma-Aldrich,St.Louis,MO)、10ng/ml的基本的成纤维细胞生长因子以及50mg/ml的long-R3***-1(ProSpec,Rehovot,Israel)。为了维持多能性而使用了进行5代以下的传代扩增的UC-MSCs。根据之前的报告分析了表面蛋白质的表达(Stem Cells and Dev.24(2015)1658-1671)。
2.分析细胞移动
用1.0%的明胶(Sigma)50μl对8μm的聚碳酸酯膜包覆1个小时。将UC-MSCs与VPA(0.5mM,Sigma-Aldrich)单独、或5-Aza(1μM,Sigma-Aldrich)单独、或与50nM S1P进行混合来激活了1天。对于UC-MSCs,用TrypLE溶液(Thermo Scientific,Pittsburgh PA)分离后,进行清洗并在含有0.5%牛血清白蛋白(bovine serum albumin,BSA)的DMEM中再次混悬,并且以3×104细胞/孔(Well)密度接种在细胞迁移侵袭试验插件(Transwell inserts)(Corning costar,pittsburgh,PA)的上部腔室。下部腔室使用添加至含有0.5%BSA的DMEM中的150ng/ml的SDF-1(R&D Systems)进行填充。一天后,从细胞迁移侵袭试验板清除小室。用脱脂棉刮出残留在上部腔室的细胞,之后用溶解于磷酸盐缓冲溶液(phosphate-buffered saline,PBS)中的4%多聚甲醛(paraformaldehyde,PFA)溶液固定移动的细胞,之后用0.5%结晶紫染液(crystal violet)(Sigma-Aldrich)进行染色。然后用Image Pro5.0软件(Media Cybernetics,Rockville,MD,USA)分析数字图像,从而对于在膜的下部面的染色细胞进行定量。
3.分析MSCs的体外(in vitro)特性
MSCs的细胞增殖、成纤维细胞集落生成单位(colony-forming unit-fibroblast,CFU-F)、多功能(向软骨细胞、骨细胞或脂肪细胞的***的体外分化)以及体外抗炎症分析可根据之前的报告(Stem Cells and Dev.24(2015)1658-1671)执行。
4.蛋白质印迹法和实时定量PCR(real-time quantitative PCR,RQ-PCR)
将用50nM的S1P和0.5mM的VPA激活的UC-MSCs,在含有0.5%BSA的DMEM中,以37℃条件饥饿(starvation)12小时,用150ng/ml的SDF-1刺激5分、10分、20分或30分后,使用30μg-细胞提取物,根据蛋白质印迹法,分析丝裂原活化蛋白激酶(MAPK)p42/44和蛋白激酶B(Ser473)(AKT(Ser473))的磷酸化。为了分析基因表达,逆转录从标记的细胞获取的全部RNA,标记的转录体则通过RQ-PCR来定量。
5.统计分析
为了确认统计学显著性差异,对数据使用非参数曼惠特尼检验(non-parametricMann Whitney test)或单因素方差分析及邦弗朗尼事后检验(one-way ANOVAwith theBonferroni post-hoc test)而进行分析。本发明人使用专业医学绘图分析软件(GraphPadPrism 6.0software)(GraphPad Software,La Jolla,CA)执行所有的分析,并且定义p<0.05、0.01或0.001具有统计学差异。
6.实验设计
本发明的目的不仅是检测V1P+S1P激活的人类脐带来源的MSCs(umbilical cord-derived MSCs,UC-MSCs)在老鼠模型中治疗间质性膀胱炎/膀胱疼痛症候群(Interstitialcystitis/bladder pain syndrome,IC/BPS)的效果,还要广泛的观察移植的细胞在体内(in vivo)的细胞特性。将对照组或V1P+S1P激活的UC-MSCs施用在膀胱损伤的老鼠,并检测对于膀胱排尿功能、尿路上皮脱落(urothelium denudation)、肥大细胞浸润、组织纤维化、细胞凋亡(apoptosis)和肿瘤形成的影响。在所有的实验中,对每个组中独立存在的5只动物进行了2次独立实验。这些动物被随机分成损伤组、细胞移植或施用赋形剂(vehicle)的组和膀胱内压测量(cystometry)组,并进行相应处理。并且,没有向参与手术过程的研究人员告知关于施用的细胞的形态和施用量的信息。所有的膀胱内压测量、组织学和基因表达检测均由不知道关于组的信息的研究人员来执行。在所有的分析中将由于膀胱损伤或导管***而意外死亡的动物排除。所有的动物实验均根据韩国蔚山大学医学院的动物实验论理委员会的准则和规定执行(IACUC-2014-14-167)。
7.动物模型及UC-MSCs的移植
HCl注入的IC/BPS大鼠模型之前已经有过报告(干细胞与发育(Stem Cells andDevelopment)24,2015,1648-157)。HCl损伤一周后,切开下腹部,利用500μm注射器及26-G(gauge)的针,将2.5×105(250K)的UC-MSCs或PBS直接施用到膀胱的前壁外层和上壁。在施用干细胞的前一天,为了切断Wnt或IGF-介导信号传递,分别在皮下注入吲哚美辛(indomethacin;PMG Pharm Co.,Ltd.Ansan,Korea;每12小时按2.5mg/kg给药)或者吉非替尼(Gefitinib;Santa Cruz Biotechnology,Santa Cruz,CA,USA;每天按5mg/kg给药)。
8.收集非麻醉(Unanesthetized)和非抑制(unrestrained)的膀胱内压测量图(非麻醉的膀胱内压测量方法)
对代谢笼内的非麻醉和非抑制的大鼠执行膀胱内压测量(Cystometrograms)。为了记录并测量膀胱内压(intravesical pressure)和腹腔内压(intra-abdominalpressure,IAP),在测量膀胱内压的三天前***导管。简单来说,诱导麻醉后,通过切开腹部将带有留置带(cuff)的聚乙烯导管***至膀胱上壁。为了记录IAP,使导管尖端的留置带附近的腹腔球囊(Latex;Daewoo Medical,Incheon,Korea)位于膀胱近端部,并捆绑在带有丝线(silk thread)的其他导管。在温水中加热聚乙烯导管(PE-50),将***部的尖端延长为原有长度的1.5倍左右,并且使用由肝素处理的生理盐水(100IU/mL)填满。当移植完膀胱导管时,拉长的导管将被***到大腿静脉内。之后,上述导管穿过皮下空间,通过动物的背部穿出,并附着在背部的皮肤上。手术后,每只鼠单独被饲养并且以相同的方式维持。
为了非麻醉膀胱内压测量分析(awake cystometric analysis),通过连接于压力传感器(pressure transducer)(研究级别血压传感器;哈佛仪器,Holliston,MA,USA)和微量注射泵(microinjection pump)(PHD22/2000pump;哈佛仪器)的T形管,使用双重阀将留置导管连接在膀胱。为了记录IAP,填满体液并连接有腹腔球囊的其他留置导管连接于其他压力传感器。将无菌生理盐水以0.4mL/min的速度注入到膀胱内,并通过连接有肌张力变位换能器(force displacement transducer)(研究级别等距换能器;哈佛仪器)的体液采集器持续记录排尿量。使用安装有Acq Knowledge 3.8.1软件的MP150数据采集***(MP150data acquisition system)(Biopac Systems,Goleta,CA,USA),以50Hz的采样速度持续记录IVP,IAP和排尿量。对每只动物检测8分钟的所有排尿周期数值均用于评估。
在没有排尿的状态下,IVP从基准值增加至15cmH2O以上时,将其测定为非排尿收缩(NVC)。BP表示填充膀胱时的最低膀胱压力,MP表示排尿周期内的最高膀胱压力,MV表示排尿时的尿液量,RV表示排尿后的残余尿液量。BC定义为NV+RV,MI表示排尿收缩之间的间隔。
9.肺动脉高压(Pulmonary artery hypertension,PAHPAH)动物模型
根据先前的报告(干细胞与发育24,2015,165-1671)确立了野百合碱(monocrotaline,MCT)诱导的PAH大鼠模型。对于雄性无特异性病原菌Lewis大鼠(8周,250-280g),在饲养温度及光照(12小时明暗周期)的调节下,以不受食物和水的限制的方式进行饲养。通过MCT(60mg/kg,西格玛(Sigma))的皮下给药来诱导PAH。对于对照组的大鼠施用了相同体积的PBS。MCT或PBS给药两周后,将未进行激活诱导的UC-MSCs或者用VPA+S1P预处理的UC-MSCs,以每200μl的PBS中存在2.5×105个细胞的密度施用于尾静脉。在仅用溶剂处理的对照组(vehicle control)中,施用没有细胞的200μl的PBS。在注入之前,用温热的PBS清洗两次细胞,并使用根据7-氨基放线菌素D(7-AAD)(BD Biosciences)排除染色的流式细胞(FACS)分析,观察施用细胞的存活力。
10.检测收缩期的右心室压(right ventricular systolic pressure,RVSP)及右心室肥大症(right ventricular hypertrophy,RVH)
施用MCT或PBS四周后,通过人工呼吸机(哈佛仪器,Holliston,MA)维持呼吸,并通过舒泰(zoletil)(40mg/kg)和隆朋(rompun)(10mg/kg)进行麻醉的情况下,通过膈膜的直接穿刺检测收缩期的右心室压,其中膈膜的直接穿刺使用连接有患者监护器(MDE EscortII patient monitor,Arleta)的26G的针。对于右心室压(Right ventricular pressure,RVP),通过人工呼吸机维持呼吸。为了检测RVH,从室间隔(interventricular septum)分离心脏的右心室,并检测包括右心室(RV)和室间隔的左心室(LV+S)的重量。
11.分析组织学和基因表达
为了膀胱组织的组织学分析,通过对细胞角蛋白(cytokeratin)的免疫染色检测上皮细胞脱落;通过甲苯胺蓝染色(Toluidine blue staining;8544-4125;DaejungChemicals&Metals,Seoul,Korea)检测肥大细胞浸润;通过马松三色染色(Masson'strichrome staining;Junsei Chemical,Tokyo,Japan)检测组织纤维化;以及通过TUNEL染色(1 684 795;Roche,Mannheim,Germany)检测细胞凋亡。对于肺组织,在H&E染色或α-平滑肌肌动蛋白(α-smooth muscle actin,α-SMA)-染色的部位,对具有4μm厚度和25-100μm直径的随意选择的血管中的随意意选择的区域,以400倍放大倍数拍摄(capture)5次以上。根据制造商的推荐标准适用了抗-α-SMA(1:100,Abcam)抗体,并使其进行反应。对于细胞核用4',6'-二脒基-2-苯基吲哚(4',6'-diamino-2-phenylindole;D9542;DAPI,Sigma-Aldrich)进行复染色(counterstain)。使用NIH ImageJ program(http://rsbweb.nih.gov/ij/)检测内壁(Medial wall)的厚度。内壁厚度指数由相对于外部直径的外部直径-内部直径的比率所定义。
为了分析基因表达,使用RNA提取试剂盒(Qiagen Inc.,Valencia,CA)获取了全部的RNA;使用taqman逆转录试剂盒(TaqMan Reverse Transcription Reagents)(AppliedBiosystems)执行逆转录;使用IQ荧光定量PCR(iQ SYBR Green PCR Master Mix)(Bio-Rad,Hercules,CA)执行实时定量PCR(real-time quantitative PCR,RQ-PCR)。对于每个处理组独立存在的5只动物,从随意选择的3部分的区域(n=15)用于定量数字图像。相似地,对每个组中随意选择的5只动物,反复两次RQ-PCR(n=10),从而获得基因表达数据。
<实施例1>基于S1P的人类UC-MSCs激活中5-Aza的影响
5-Aza和VPA的低浓度处理((1μM和0.5mM)大幅增加在UC-MSCs中的CXCR4的表达(图1的A)。其次,本发明人确认了对于UC-MSCs的基本特征的上述表观遗传(epigenetic)调控因子的效果。与50nM的S1P的激活不同地,5-Aza和VPA均没有对表面标记蛋白(CD29、CD73和CD90阳性、CD34和CD45阴性)的表达起到多少影响(参见图1的B和图2的A)。并且,一起处理5-Aza(5-Aza+S1P)或VPA(VPA+S1P)的S1P激活几乎不影响多***分化能力,对于多***分化能力而言,分别检测葡萄糖胺聚糖(glycosaminoglycans;阿尔新蓝(Alcian blue))、矿物质沉积(mineral deposition;茜素红S(Alizarine Red S))以及脂质蓄积(lipidaccumulation;油红O染色(Oil red O staining))的增加程度,将其作为分析软骨细胞、骨细胞以及脂肪细胞***的体外(in vitro)分化的基础(参见图1的C和图2的B)。
与CXCR4的向上调节冲突地(参见图1的A),在transwell迁移分析中,用5-Aza+S1P激活的UC-MSCs对SDF-1反应的移动活性相比于未激活的细胞减少约30%左右(参见图1的E)。并且,5-Aza+S1P激活严重损害克隆性(clonogenic)CFU-F的活性,其中上述CFU-F表示向克隆性前体细胞自我再生的频率(参见图1的F)。对于5-Aza+S1P的激活,MSCs的细胞增殖几乎没有变化(参见图1的D)。
<实施例2>在低浓度的S1P处理时,能强化UC-MSCs激活的VPA
接着,本发明人检测了在低浓度的S1P处理时,VPA对UC-MSCs激活的影响。为此,本发明人适用了50nM的S1P,其用量相比于为了激活脂肪和UCB来源MSCs而使用的最佳用量(200nM)小4倍。与VPA单独处理和S1P单独处理不同,VPA+S1P处理时UC-MSCs激活增加对SDF-1的化学趋向性(chemotactic activity),其中较未激活的细胞相比增加2~3倍(图3的A以及图3的B)。基于VPA+S1P激活的对SDF-1的强化的反应,与5-Aza+S1P激活的情况完全不同。接着,本发明人确认了与HSPCs的移动相关的信号传递途径的状态。与化学趋向性分析结果一致,暴露在VPA+S1P的UC-MSCs增加MAPKp42/44和AKT蛋白质的磷酸化,其表示信号传递途径的活性化(参见图3的C)。上述结果表示与5-Aza不同地,VPA在低浓度的S1P中也能活性化MAPKp42/44和AKT信号传递途径,从而可以促进UC-MSCs的激活。
<实施例3>用VPA+S1P激活的UC-MSCs的治疗能力提升
本发明人检测了对与MSCs的治疗效果相关的其他细胞活性的VPA+S1P激活效果。与5-Aza+S1P不同(参见图1),用VPA+S1P激活的UC-MSCs的细胞,在增殖(参见图4的A)和克隆性CFU-F活性(参见图4的B)方面均有提高。但是,在VPA单独处理和S1P单独处理的UC-MSCs中,没有观察到细胞增殖能力和CFU-F活性的增加(参见图4的A和图4的B)。MSCs影响抗炎症能力和免疫调节能力,因此本发明人检测了VPA+S1P激活是否对UC-MSCs的抗炎症效果有影响。对此,本发明人准备了从未激活的UC-MSCs或者从用单独的VPA或用VPA+S1P激活的UC-MSCs收集的条件培养基,并检测了该条件培养基是否能抑制MH-S细胞的肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)的分泌,其中MH-S细胞为用脂多糖(lipopolysaccharide,LPS)刺激的肺泡巨噬细胞(alveolar macrophages cell)。发明人发现,从UC-MSCs收集的条件培养基能有效地减少LPS刺激的MH-S细胞分泌TNF-α(图4的C)。尤其,由VPA+S1P激活的UC-MSCs收集的条件培养基相比于未激活的UC-MSCs或者VPA单独激活的UC-MSCs,能更有效地抑制TNF-α的分泌。本发明人将从作为人类肺纤维原细胞的IMR90收集的条件培养基(CM)作为对照组使用,其结果显示出在体外抗炎症分析中进一步增加TNF-α的分泌。
为了确认VPA+S1P激活的效果的分子学机理,本发明人检测了由MSCs分泌的生长因子、前-炎症细胞因子和抗-炎症因子的表达。与之前所述的细胞学特性实验结果一致,用VPA+S1P激活的UC-MSCs明显向上调节了与生长因子及其的受体[例如,PDGFB,PDGFRB,cMET](参见图5的B)、前-血管新生-[例如,VEGFB,VEGFC,ANGPT1,ANGPT2](参见图5的C)、抗-炎症-[例如,LIF,TSG6,IDO1,IDO2](参见图5的D)以及干细胞移动-[例如,MMP12](参见图5的A)相关的因子的表达。但是,在VPA单独处理和S1P单独处理的UC-MSCs中,没有观察到对上述基因的表达的显著变化(参见图4的A和图4的B)。并且,5-Aza+S1P激活向下调节了由VPA+S1P激活影响的如上所述的基因。综上所述,上述结果显示通过最小浓度的VPA和S1P处理的MSCs的激活,能有效促进MSCs的移动、增殖、自我再生以及抗炎症能力,这将对MSCs的治疗潜力起到重要的作用。
<实施例3>根据多种VPA和S1P浓度的SDF-1a的趋化性(Chemotaxis)分析
本发明人分析了根据多种VPA和S1P浓度的SDF-1a的趋化性,从而确定VPA和S1P的最低浓度。
如图6所示,将VPA固定为0.5mM的情况下,采用5-50nM的S1P浓度进行检测的结果,S1P的最低浓度被定为10nM(参见图6的a)。并且,将S1P固定为50nM的情况下,采用0.05-0.5mM的VPA浓度进行实验的结果,VPA的最低浓度被定为0.1nM(参见图6的b)。
<实施例4>在IC/BPS治疗中,确认VPA+S1P激活的UC-MSC的体内(in vivo)治疗可能性
接着,本发明人使用通过注入HCI而建立的IC/BPS动物模型,确认了UC-MSCs的体内效果。使用非麻醉膀胱内压测量方法(awake cystometry)的膀胱功能分析结果,注入HCI而被诱导的IC/BPS鼠(HCl-IC group)相比于对照组(假手术组(sham-operated),sham)显示出不规则的排尿,且尿间隔缩短(micturition interval,MI),而且还显示出排尿量(micturition volume,MV)、最大压力、排尿压力(micturition pressure,MP)以及膀胱容量(bladder capacity,BC)均发生了降低(参见图7的a和图7的b)。2.5×105细胞数的对照组(naive)UC-MSCs(UC-MSC 250K)或VPA+S1P激活的细胞(UC-MSC 250K+VPA+S1P)的单一移植,改善了受损的排尿参数。重要的是,VPA+S1P激活的UC-MSCs相比于对照组细胞显示出优异的效果(参见图7的a和图7的b)。尤其,在HCI-IC组的动物中,非排尿期间(non-voidingperiods,NVC)显示出收缩频率增加,然而这样的症状在VPA+S1P激活的UC-MSCs中得到了明显的改善,但是对照组UC-MSCs中几乎没有改善(参见图7的a和图7的b)。通过非麻醉膀胱内压测量方法(awake cystometry)的结果也与上述功能改善效果一致,并且显示出由于对照组或VPA+S1P激活的2.5×105UC-MSCs的施用,大鼠模型中如尿路上皮脱落(urotheliumdenudation)、肥大细胞浸润、纤维化、细胞凋亡的非正常的组织学现象逐渐恢复正常(参见图8的a和图8的b)。这样非正常的组织学现象也是在人类IC/BPS膀胱中出现的特征。尤其,对于组织学变形的恢复而言,相比于对照组细胞,VPA+S1P激活的UC-MSC在移植的膀胱组织中显示出更好的改善效果。综上所述,上述结果显示出,在IC膀胱治疗中,利用VPA+S1P激活能明显地改善UC-MSCs的治疗可能性。
<实施例5>在PAH动物模型中,UC-MSC的VPA+S1P-介导治疗可能性的提升
接着,本发明人使用由MCT诱导的PAH动物模型,在体内条件下确认了VPA+S1P激活的效果。类似于之前的报告,MCT施用4周后,RVSP明显增加。与对照组(naive)UC-MSC(PAH+MSC组)不同,用VPA+S1P激活的UC-MSC(PAH+VPA+S1P-MSC组)的施用,明显减少了由MCT诱导的RVSP的上升(参见图9的a)。并且,施用MCT后,RV/(LV+S)也会增加,然而VPA+S1P-MSC明显减少了RV肥大症(hypertrophy),但是未激活的UC-MSC几乎没有效果(参见图9的a)。并且,VPA+S1P-MSC施用,减少了由MCT诱导的肺组织炎症、内壁厚度指数以及α-SMA+平滑肌细胞的增加(参见图9的b)。综上所述,上述结果显示通过VPA+S1P的UC-MSC的激活能促进新生血管的形成,并能诱导抑制炎症反应的微环境(microenvironment)。
以上,对本发明特定部分进行了详细的说明,对于本领域技术人员显而易见的是,上述具体技术仅仅是优选的实施例,本发明的范围并不限定于此。因此,本发明的实际范围应当由所附加的权利要求及具有同等效果的内容所定义。
