Description

Sliding gap retainer and orthodontic device
Technical Field
The invention relates to the technical field of medical appliances, in particular to a sliding type gap maintainer and an orthodontic device.
Background
With the development of economy and the advancement of society, more and more people put higher demands on the beauty and health of the oral cavity. When many patients visit the clinic, oral symptoms are often not a single problem. Even a plurality of oral diseases of carious lesions, permanent tooth implantation and occlusion, tooth defect, dentition deletion, malocclusion and the like of some patients exist at the same time. This places even greater demands on the orthodontist. When orthodontic treatment is carried out, various diseases in the oral cavity of a patient need to be comprehensively analyzed, the beauty of the dentognathic face is improved, the chewing function needs to be recovered, and the unified coordination of the function and the beauty is realized.
In the face of intricate dento-maxillofacial deformities, orthodontists often need to maintain gaps during orthodontic treatment to prevent teeth on both sides of the gaps from moving to the sides of the gap for special treatment purposes. If a person suffering from symptoms such as dental caries, tooth trauma, congenital deficiency or overteeth of individual teeth needs to be implanted or repaired after orthodontic treatment in order to achieve the maximum appearance and function, enough space needs to be reserved for the repair treatment in the orthodontic process. In addition, for window opening assisted sprouting buried impacted teeth, gaps also need to be maintained in the process of guiding the impacted teeth to sprout. It can be seen that gap maintenance is important in some special cases.
The straight wire arch correction technology generally adopts a sliding method to close the tooth extraction gap, and compared with another gap closing method, namely a curve closing method, the straight wire arch correction technology has the advantages of simple operation, convenient re-diagnosis and force application and good arch controllability. In this case, if the gap is required to be maintained with the missing tooth, the fixed gap maintaining system cannot be used, otherwise the free sliding of the main arch wire is affected when the gap is closed, and in this case, the gap needs to be maintained by using the sliding gap maintaining device.
The gap maintaining method in the orthodontic process which is commonly adopted clinically at present comprises the following steps:
(1) the method for bending the orthodontic arch wire to stop bending comprises the following steps: namely, the device is a fixed gap maintaining device which is bent at two sides and stops bending to maintain the gap. The bending stop bending method can effectively prevent teeth on two sides of the gap from moving, but the bending process of the arch wire is complex, the operation time beside the chair is long, the requirements on the clinical experience and the manual ability of a doctor are high, the appearance and the comfort are influenced, and the oral hygiene maintenance of a patient is not facilitated. This approach does not allow for sliding closure of the gap. In addition, the stop bending is difficult to adjust and cannot be bent easily on high-elasticity arch wires such as NiTi wires, so the application range is limited.
(2) The orthodontic arch wire arrangement traction hook method comprises the following steps: the traction hooks are arranged on two sides of the gap to prevent teeth on two sides from moving, so that the gap can be stably and effectively maintained, and the device belongs to a fixed gap maintaining device. However, this method can be performed only on square wires, and the addition of a hook changes the bow shape, so that bow adjustment is necessary, and manipulation of a highly elastic arch wire is not easy. This approach does not allow for sliding closure of the gap. In addition, the arrangement of the traction hook has high positioning requirements, and the gap maintaining effect cannot be realized if the arrangement is inaccurate.
(3) Resin ball: resin balls can be made on the arch wire at two sides of the gap, and the resin balls can prevent the teeth at two sides from moving to the gap side, so that the arch wire belongs to a fixed gap maintaining device. Its advantages are simple operation, small space occupied by resin balls and no discomfort to patient. However, the resin beads have a disadvantage of poor stability and are liable to be broken and fall off when eaten. This approach does not allow for sliding closure of the gap.
(4) Pushing a spring: the gap can be maintained by a push spring with the same width as the space between the tooth brackets at the two sides of the gap, belonging to a sliding gap maintaining device. The method is simple and convenient to operate, and saves time. But the defects are obvious, such as poor stability and easy deformation of the push spring, and the tail end of the push spring is easy to slide out of the arch wire if the treatment is not proper, thus the treatment effect is influenced, and the mouth scraping brings discomfort to patients. In addition, the push spring is easy to retain food residues, difficult to clean and smelly.
