Description

一种外用膏药及其制备方法和应用
技术领域
本发明涉及中药技术领域，尤其涉及一种外用膏药及其制备方法和应用。
背景技术
痹证：是由风、寒、湿、热等引起的以肢体关节及肌肉酸痛、麻木、重着、屈伸不利，甚或关节肿大灼热等为主症的一类病证。《素问·痹论》所说:“风寒湿三气杂至，合而为痹。”根据感受邪气的相对轻重，常分为行痹(风痹)、痛痹(寒痹)、着痹(湿痹)。若素体阳盛或阴虚火旺，复感风寒湿邪，邪从热化，或感受热邪，留注关节，则为热痹。总之，风寒湿热之邪侵入机体，痹阻关节肌肉筋络，导致气血闭阻不通，产生本病。此外西医学的风湿热(风湿性关节炎)、类风湿性关节炎、骨性关节炎等同属痹症范围。
目前，治疗软组织损伤、风湿热、风湿性关节炎、类风湿性关节炎、痛风、坐骨神经痛、骨质增生、骨刺、腰痛、腿痛、肩周炎、颈椎病、膝骨关节炎、半月板损伤等疾病的方法多种多样，中医中药、各种膏药、西医药物治疗、微创手术、高科技仪器理疗等。这些疗法都对其中某一种疾病有所疗效，但疗效单一，疗程长，效果不明显，后期恢复缓慢，费用高，操作复杂，尤其是对复合叠加的疾病治疗存在一定盲区。近几年，随着中医药的快速发展，许多治疗相关疾病的药物相继问世，目前治疗中医传统外治疗法具有“简便廉验”的独特优势，但需要相关专业人士的调配和制作，尤其是传统的黑膏药，市面上所见的传统膏药多以黑膏药为主，黑膏药使用时非常不方便，多存在使用时需加热软化，如非专业人员操作，药效往往会根据软化的程度不同而发生改变，软化不足容易出现药效降低，软化过度会引发烫伤等二次损伤。如果去火毒不彻底，容易引起局部红肿等症状。以及更换清理时不方便，摘下后皮肤残留难以清洗，贴敷时间长，易引起皮肤瘙痒过敏等现象。黑膏药在配料和火候上也相当讲究，真正的质量好的黑膏药由于制作工艺和药物成本的约束，导致价格也相对昂贵，且揭去膏药后的膏药残留难以清理。并且现有的膏药一般用于治疗单一疾病，应用范围相对狭窄；因此针对上述问题，研制一种疗效好，应用范围广，使用方便的外用膏药。

本发明所要解决的技术问题是：针对现有技术的不足，提供一种疗效好、应用范围广、使用方便的外用膏药。

本发明所要解决的另一个技术问题是：针对现有技术的不足，提供一种疗效好、应用范围广、使用方便的外用膏药的制备方法。
为解决上述技术问题，本发明的技术方案是：
一种外用膏药的制备方法，所述制备方法包括以下步骤：
(1)按照重量份数计将依次称量的生川乌、生草乌、生南星、洋金花、细辛、鸡血藤、七叶莲、桑寄生、当归、威灵仙、红花、天花粉、马钱子粉碎，加入白酒，每5-7天搅拌一次，密封浸泡49天以上得到药酒；
(2)按照重量份数计依次称量土元、三七、乳香、没药、延胡索、蒲公英、栀子、大黄，再取蜈蚣5-9条以及焙干的全虫5-8条，将上述组分混合后粉碎，过筛，得到药粉；
(3)取步骤(1)中的药酒，过滤、除渣，收集的上清液，备用；
(4)取步骤(2)中的药粉，再加入步骤(3)中上清液，搅拌过程中加入生姜汁，混匀，密封浸泡49天以上，即得。
作为一种改进的技术方案，步骤(4)中药粉和上清液按照料液比1:1.5-2的比例加入。
作为一种改进的技术方案，步骤(4)中生姜汁和上清液按照体积比1:5-8的比例加入。
古称“跌打损伤”为诸伤之总论，多因外力作用，或自身姿势不正确的情况下用力过猛而造成的。中医把凡因外力作用于人体而引起的筋骨伤损、瘀血肿痛、气血不和、经络不通以至脏器受损等，统称为跌打损伤。跌打损伤一般造成的软组织损伤、外伤肿胀疼痛、皮肉破损出血，其主要病理为淤血壅滞，血闭气阻，故以疼痛、肿胀为主要表现。不论是“痹症”还是“跌打损伤”其中主要的临床表现还是以局部疼痛、肿胀、关节活动不利等为主，如迁延日久还会出现其他相关并发症。因此本发明研制的膏药可用于风、寒、湿或热引起的痹症以及跌打损伤骨科疾病。
采用了上述技术方案后，本发明的有益效果是：
(1)本发明中生川乌、生草乌、洋金花、生南星、乳香、没药、红花、土元、具有活血、解痉止痛、消肿散结之功效，共为君药；蜈蚣、全虫、三七、蒲公英、栀子、天花粉具有清热解毒，解痉通络，共为臣药；湿性缠绵重浊，迁延日久，加入桑枝，羌活，独活，苍术，秦艽，木瓜，配伍祛湿通痹，加速陈旧性，慢性疾病的恢复，细辛，马钱子，皂刺，穿山龙，宽筋藤，伸筋草，防风，川穹，延胡索，大黄，七叶莲，鸡血藤，丝瓜络，海风藤，威灵仙具有舒筋通络，温经散寒、祛风除湿之功效，针对陈旧性顽固痹阻，具有较好的清除和疏通作用，使堵塞的经络疏通，气血得以运行，使祛风除湿，活血止痛的效果大大增强为佐。桂枝，牛膝，桑寄生，当归，骨碎补，香附，丹参具有益精补血，补肝肾强筋骨之功效，在祛邪的同时调补正气，使祛邪不伤正，加快受损组织的快速修复共为使药。方中君臣佐使相互配合，活血止疼，消肿散结，温经散寒，祛风除湿，调补筋骨，可治疗风、寒、湿、热以及外伤引起的多种相关类疾病；
(2)本发明的膏药应用范围广，可用于颈椎病或由颈椎病引起的其他症状，腰腿疼痛，肩周炎，关节增生，腿伸不利，半月板损伤，局部水肿，疼痛等一系列骨关节的疼痛，水肿，怕冷怕凉，跌打损伤等病症；
(3)本发明在制备过程中，首先将一部分中药组分粉碎，再用白酒浸泡，得到的药酒上清液与另一部分中药粉碎后的药粉混合，再加入姜汁混匀密封保存；姜汁具有温经散寒、调和诸药的作用，采用上述方法可使每一味中药充分发挥其作用；
(4)本制品制作完成后可置于容器内，随用随取，且不需要加热软化，药效稳定，极大的方便了患者的使用，贴敷时间短，见效快，取下后易清理。在实际应用中，用压舌板取出，平摊在医用胶布或者普通的壮骨膏药上(主要是代替胶布)，或先将药物涂抹在患处，之后用纱布缠绕固定。在所敷贴的部位上施以艾灸或用TDP神灯照射，用于辅助局部消炎，促进血液循环，增加疗效。

实施例3
一种外用膏药，按照重量份数计包括下列组分：生川乌43份、生草乌42份、生南星48份、洋金花20份、细辛10份、鸡血藤30份、七叶莲28份、桑寄生18份、全虫6条、蜈蚣6条、土元80份、三七25份、乳香100份、没药100份、延胡索30份、当归48份、威灵仙28份、红花100份、栀子48份、蒲公英28份、大黄98份、天花粉48份、马钱子28份。
其制备方法包括以下步骤：
(1)按照重量份数计将依次称量的生川乌、生草乌、生南星、洋金花、细辛、鸡血藤、七叶莲、桑寄生、当归、威灵仙、红花、天花粉、马钱子粉碎，加入60度的白酒，每5-7天搅拌一次，密封浸泡49天得到药酒；
(2)按照重量份数计依次称量土元、三七、乳香、没药、延胡索、蒲公英、栀子、大黄，再取蜈蚣6条以及焙干的全虫6条，将上述组分混合后粉碎，过100目筛，得到药粉；
(3)取步骤(1)中的药酒，过滤、除渣，收集的上清液，备用；
(4)取步骤(2)中的药粉，按照料液比1:1.5的比例再加入步骤(3)中上清液，搅拌，按照生姜汁与上清液1:6的比例加入，混匀，密封浸泡49天，即得。
实施例4
一种外用膏药，按照重量份数计包括以下组分：生川乌45份、生草乌42份、生南星45份、洋金花18份、细辛10份、鸡血藤30份、七叶莲30份、桑寄生20份、全虫6条、蜈蚣6条、土元80份、三七25份、乳香100份、没药100份、延胡索30份、当归50份、威灵仙30份、红花100份、栀子50份、蒲公英30份、大黄100份、天花粉50份、马钱子30份、桑枝15份、羌活20份、独活10份、皂刺30份、桂枝20份、苍术10份、秦艽10份、木瓜10份、牛膝20份、香附50份、川芎30份、丝瓜络15份、骨碎补30份、伸筋草15份、海风藤25份、穿山龙30份、宽筋藤30份、丹参30份。
其制备方法包括以下步骤：
(1)按照重量份数计将依次称量的生川乌、生草乌、生南星、洋金花、细辛、鸡血藤、七叶莲、桑寄生、当归、威灵仙、红花、栀子、天花粉、马钱子、桑枝、羌活20份、独活、皂刺、桂枝、苍术、秦艽、木瓜、牛膝、香附、川芎、丝瓜络、骨碎补、伸筋草、海风藤、穿山龙、宽筋藤、丹参粉碎，加入65度白酒，每5-7天搅拌一次，密封浸泡49天得到药酒；
(2)按照重量份数计依次称量土元、三七、乳香、没药、延胡索、蒲公英、栀子、大黄，再取蜈蚣6条以及焙干的全虫6条，将上述组分混合后粉碎，过100目筛，得到药粉；
(3)取步骤(1)中的药酒，过滤、除渣，收集的上清液，备用；
(4)取步骤(2)中的药粉，按照料液比1:2的比例再加入步骤(3)中上清液，搅拌，按照生姜汁与上清液1:8的比例加入，混匀，密封浸泡49天，即得。
为了更好的证明本发明具有较好的治疗效果，将实施例4中的配方用于治疗风、湿、寒、热引起的痹症或者跌打损伤骨科类疾病。
病历1
宋**，男21岁，2008年因外伤导致腰椎间盘突出，2013年因工作过程中不慎腰部肌肉拉伤，腰部无力，抬腿、弯腰困难，疼痛，自述因之前腰椎间盘突出时贴过不少膏药，自觉效果不佳。将本发明敷贴于腰部疼痛处，另做艾灸于敷贴处15分钟，治疗结束后，患者自述翻身坐立时症状有所缓解，信心大增。嘱咐其回家后多平卧，次日换药时自述病情好转1/3，疼痛明显减轻，可自行活动，弯腰时幅度有所增加，又贴敷三次后痊愈疼痛感全无，可弯腰90度以上，活动自如，嘱其日后多注意，至今五年未有复发。
病历2
李**，女51岁，2013年10月因外出徒步旅游活动后膝关节疼痛前来就诊，自述之前双侧膝关节均有不适，曾采取针灸推拿治疗，但时好时坏，本次想换种治疗方式试试。触诊：双侧膝关节肿大，内有积液，活动不利，站立弯曲时膝关节疼痛严重。采取现在患处贴敷本膏药，外用纱布固定，最外层包保鲜膜，烤TDP神灯20分钟，以促进汗液排放。本次治疗结束后，膝关节局部流出大量汗液，水肿消减2/3，屈膝活动自述好转许多，摘去保鲜膜后包纱布固定，嘱其注意休息，次日前来换药，自述疼痛大减，活动已无大碍，仅下蹲时仍有不适，遂又贴敷2贴加以巩固，随访，膝关节活动自如，疼痛消失。嘱其注意锻炼方式，后随访均未复发。
病历3
程**，女53岁，2015年3月因膝关节疼痛肿大活动不利来电咨询，自述是徒步爱好者，多年前膝关节反复不适，并未在意，于2015年年初疼痛症状加剧，前去当地附属医院，诊断为半月板磨损严重，局部水肿积液，予以膝关节微创手术治疗，因术后保养不当，膝关节疼痛加剧，行动严重受限，只能坐轮椅拄拐。将单侧膝关节敷贴本发明一次后，疼痛大减，随后贴敷用5次后，嘱其今后注意，后随访均为复发。

郑**，男28岁，2016年8月因打篮球将脚腕扭伤，因处理不当，导致脚踝肿大疼痛，行动受限。给予本发明敷于患处，以纱布包裹固定，次日自述疼痛有缓解1/3，嘱其2日换药一次，继续贴敷2次后行动自如痊愈，后随访多日均无复发。

郭**，男55岁，2017年7月因坐车出差多日，腰部无力、疼痛，活动受限，迁延双下肢酸胀，经诊断：腰部局部劳损，肿大，痛点明显，取俯卧位腰部患处敷贴本发明，再加以艾灸辅助，增强活血化瘀，温经通络的作用，20分钟后下床站立活动，自述症状好转一半，因次日出差，所以取2份的量的本发明带走，10日后出差回来自述已痊愈，随访多日未复发。
病历6
黄**，女54岁，退休棉纺织职工，因肩部活动受限疼痛前来治疗，自述患有此症状已有多年，之前因工作时好时坏。经诊断为：肩周炎。于患者患处贴敷本发明，2日更换一次，嘱其注意保暖，共贴敷6次，症状痊愈可自行活动。随访半年未有复发。
病历7
高**，男58岁，货车司机，因颈部不适，自感颈部漏风，怕冷，头晕恶心，经诊断：颈椎病，劳损严重，局部肌肉紧张粘连。先用火机烤本发明五秒钟后，将本发明敷于颈椎劳损部位，2日换一次，自感颈椎漏风好转，又贴敷4次后症状痊愈，后嘱其注意休息，随访多日未复发。
病历8
孙**，男19岁，高中在读，因课外活动打篮球不慎扭伤，处理不当导致脚踝部肿大疼痛，行动受限，药店自行购买膏药贴敷，半月余仍有疼感，肿大情况较之前减轻1/3但仍活动受限，贴敷本发明后次日疼痛基本消失，又贴敷3贴后痊愈，后随访无复发。

师**，女，48岁，农民，因腿部疼痛多年，近日疼痛加剧，前去当地医院就诊，诊断为膝关节半月板撕裂伤，并伴有骨刺，需手术治疗，因惧怕手术及费用，故采取保守治疗，敷用三次，自觉症状缓解，疼痛减轻，嘱其继续贴服，后回访一天比一天见好。

宋*，男26岁，推拿师，有腰椎间盘突出史(外伤)，因工作操作用力不当导致腰部急性扭伤，下肢无力，活动受限，次日来我处交流，嘱其操作方法，局部贴服本发明，再在局部隔药物艾灸15分钟，一日两次。次日，疼痛感大减，可做轻微活动，后继续贴服三日，腰部不适感全无，可自由活动。随访多日未有复发。

3.根据权利要求2所述的一种外用膏药，其特征在于所述外用膏药按照重量份数计包括以下组分:生川乌40-50份、生草乌40-50份、生南星42-50份、洋金花18-20份、细辛10-12份、鸡血藤30-32份、七叶莲30-35份、桑寄生20-23份、全虫6-8条、蜈蚣6-9条、土元80-83份、三七25-28份、乳香100-103份、没药100-103份、延胡索30-35份、当归50-53份、威灵仙30-33份、红花100-103份、栀子50-52份、蒲公英30-32份、大黄100-103份、天花粉50-55份、马钱子30-32份、桑枝15-18份、羌活20-23份、独活10-12份、皂刺30-33份、桂枝20-22份、苍术10-12份、秦艽10-12份、木瓜10-15份、牛膝20-23份、香附50-53份、川芎30-32份、丝瓜络15-18份、骨碎补30-33份、伸筋草15-18份、海风藤25-28份、穿山龙30-32份、宽筋藤30-33份、丹参30-35份。
4.根据权利要求3所述的一种外用膏药，其特征在于所述外用膏药按照重量份数计包括以下组分:生川乌45份、生草乌42份、生南星45份、洋金花18份、细辛10份、鸡血藤30份、七叶莲30份、桑寄生20份、全虫6条、蜈蚣6条、土元80份、三七25份、乳香100份、没药100份、延胡索30份、当归50份、威灵仙30份、红花100份、栀子50份、蒲公英30份、大黄100份、天花粉50份、马钱子30份、桑枝15份、羌活20份、独活10份、皂刺30份、桂枝20份、苍术10份、秦艽10份、木瓜10份、牛膝20份、香附50份、川芎30份、丝瓜络15份、骨碎补30份、伸筋草15份、海风藤25份、穿山龙30份、宽筋藤30份、丹参30份。
5.一种如权利要求1所述的一种外用膏药的制备方法，其特征在于所述制备方法包括以下步骤：
(1)按照重量份数计将依次称量的生川乌、生草乌、生南星、洋金花、细辛、鸡血藤、七叶莲、桑寄生、当归、威灵仙、红花、天花粉、马钱子粉碎，加入白酒，每5-7天搅拌一次，密封浸泡49天以上得到药酒；
(2)按照重量份数计依次称量土元、三七、乳香、没药、延胡索、蒲公英、栀子、大黄，再取蜈蚣5-9条以及焙干的全虫5-8条，将上述组分混合后粉碎，过筛，得到药粉；
(3)取步骤(1)中的药酒，过滤、除渣，收集的上清液，备用；
(4)取步骤(2)中的药粉，再加入步骤(3)中上清液，搅拌过程中加入生姜汁，混匀，密封浸泡49天以上，即得。
6.根据权利要求5所述的一种外用膏药的制备方法，其特征在于:步骤(4)中药粉和上清液按照料液比1:1.5-2的比例加入。
7.根据权利要求5所述的一种外用膏药的制备方法，其特征在于:步骤(4)中生姜汁和上清液按照体积比1:5-8的比例加入。
8.如权利要求1-4任一项所述的一种外用膏药在治疗痹症中的应用。
9.如权利要求1-4任一项所述的一种外用膏药在治疗骨科疾病的应用。
