Abstract
本发明提供了一种含低剂量VEGF信号通路阻断剂的组合物及其用途。具体地,本发明提供了低剂量的VEGF信号通路阻断剂在用于制备肿瘤免疫治疗促进剂中的用途,以及含有低剂量VEGF信号通路阻断剂的组合物。本发明的实验表明,低剂量的VEGF信号通路阻断剂,能够诱导肿瘤血管的正常化,显著提高肿瘤免疫治疗的效果,并能促进T淋巴细胞对肿瘤组织的浸润。
Description
技术领域
本发明涉及医药领域,更具体地涉及一种含低剂量VEGF信号通路阻断剂,如阻断VEGF信号通路的抗体的组合物及其用途。
背景技术
1971年Folkman首次报道,实体肿瘤的体积超过2-3mm3时,单纯的氧气扩散就已经无法支撑瘤体的生长,必须有新生血管形成才能支持其继续生长。这些肿瘤新生血管与正常血管相比,有血管扩张、迂曲、囊状结构形成的现象、产生无规则连接的分支,血管密度分布不均匀;管径调节机制丧失导致异常分流,血管通透性和血管间隙均显著增加。异常的肿瘤新生血管最终会导致血管分布不均匀,部分区域血流停滞,加重肿瘤缺氧微环境的形成,继而影响肿瘤基因表型,激活新生血管生成因子,包括血管内皮生长因子(VEGF),促进肿瘤新生血管的生成,并造成肿瘤细胞基因的不稳定性,激活一些肿瘤细胞生存因子,导致肿瘤细胞对放疗和化疗的耐受,进一步促进肿瘤转移。更重要的是,肿瘤血管缺乏高内皮小静脉(HEV),因此T细胞侵润不足,使得免疫治疗策略无法得到有效实施。
嵌合抗原受体(chimeric antigen receptor,CAR)基因修饰的T淋巴细胞(以下简称CAR-T细胞)治疗恶性肿瘤技术被推选为2013年科学发明成就之首,在复发、难治的急性淋巴性白血病治疗中达到了超过90%的完全缓解率,现在这一代表国际最前沿和最新发展趋势的技术正在被推广到包括实体肿瘤在内的各种癌症治疗试验中。然而,由于功能异常的肿瘤血管和免疫抑制性的肿瘤微环境的存在,严重阻碍了静脉回输的CAR-T细胞归巢到肿瘤病灶部位和损害了CAR-T细胞在肿瘤内部的生存,使得这一革命性的技术在实体肿瘤治疗上的效果至今仍然十分不理想,CAR-T细胞应用于实体肿瘤的治疗在国际上仍然面临严重挑战。因此,本目前领域迫切需要开发新的、有效的利用CAR-T细胞治疗实体肿瘤的方法。
发明内容
本发明的目的在于提供一种基于肿瘤血管正常化和介入策略的CAR-T细胞治疗实体肿瘤的药物组合物。
在本发明的第一方面,提供了一种用于治疗实体肿瘤的药物组合物,所述药物组合物包含第一活性成分和第二活性成分,其中,第一活性成分为VEGF信号通路阻断剂,第二活性成分为肿瘤免疫治疗剂,
并且所述药物组合物为单元剂型,每个单元剂型中所述第一活性成分的含量为常规剂量的0.1至0.5(较佳地0.15-0.4,或0.2-0.25),其中,所述常规剂量为200-400mg/50kg体重。
在另一优选例中,所述常规剂量为每次给药的第一活性成分的总常规剂量,如200-400mg/50kg体重。
在另一优选例中,所述的一个单元剂型中第一活性成分的含量对应于每次给药的第一活性成分的总常规剂量的0.1至0.5。
在另一优选例中,所述的n个单元剂型中第一活性成分的总含量对应于每次给药的第一活性成分的总常规剂量的0.1至0.5,其中n为2、3、4或5。
在另一优选例中,所述的n个单元剂型是相同或不同的。
在另一优选例中,所述的单元剂型中所述第一活性成分的含量为10-160mg/剂,较佳地10-100mg/剂,更佳地10-80mg/剂,如10、20、30、40、50、60、70、80mg/剂。

在另一优选例中,所述的VEGF信号通路阻断剂包括阻断VEGF信号通路的抗体。

图4显示了常规剂量的抗血管生成药物治疗对肿瘤疫苗治疗的影响。图中,D7、D10、D13、D16分别表示首次接种后第7、10、13、16天,D40表示注射40mg/kg的抗血管生成药物DC101。
图5显示了经过低剂量阻断VEGF信号通路药物预处理的肿瘤患者接受CAR-T细胞介入治疗的病理切片分析结果。
具体实施方式
本发明人经过广泛而深入的研究,首次意外地发现,低剂量的VEGF信号通路阻断剂,如阻断VEGF信号通路的抗体可以显著提高肿瘤免疫治疗的效果、促进T淋巴细胞对肿瘤组织的浸润。实验表明,利用介入策略将CAR-T细胞注射到实体肿瘤组织内部,同时配合低剂量的阻断VEGF信号通路的抗体,诱导肿瘤血管的正常化,注射部位肿瘤出现明显坏死。在此基础上,完成了本发明。
VEGF
血管内皮生长因子(英文:vascular endothelial growth factor,简称:VEGF),早期亦称作血管通透因子(英文:vascular permeability factor,简称:VPF),是血管内皮细胞特异性的肝素结合生长因子(heparin-bindinggrowth factor),可在体内诱导血管新生(induce angiogenesis in vivo)。

VEGF信号通路阻断剂
如本文所用,“VEGF信号通路阻断剂”是指通过阻断VEGF的信号通路来抑制VEGF的血管调控功能。VEGF信号通路阻断剂包括阻断VEGF信号通路的抗体和酪氨酸激酶抑制剂(TKI)。
“阻断VEGF信号通路的抗体”是指特异性结合血管内皮生长因子的受体或其配体,是一种典型的抗血管生成药物。
目前临床上用的阻断VEGF信号通路的抗体药物有2种:雷莫芦单抗(Ramucirumab)和贝伐单抗(Avastin)。
雷莫芦单抗(Ramucirumab)是一种完全的人源性单克隆抗体,其对应的鼠源单克隆抗体是DC101。它可特异性地与血管内皮生长因子受体2(VEGFR2/KDR)结合,从而阻止受体被活化,所以它是一种VEGFR2的抑制剂。雷莫芦单抗已被美国食品药品监督管理局(FDA)批准用于治疗一些实体瘤,包括胃癌和转移性非小细胞肺癌(NSCLC)。
贝伐单抗(Avastin)是一种重组的人源化单克隆IgG1抗体,通过结合VEGF配体来阻断其与内皮细胞表面的受体(Flt-1和KDR)结合,从而抑制血管内皮生长因子的生物学活性而起作用。贝伐单抗已被美国食品药品监督管理局批准用于治疗一系列癌症,包括结肠癌、肺癌、肾癌和脑癌等。
在临床上,Ramucirumab的常规剂量是8mg/kg;Avastin的常规剂量是5mg/kg。根据实施例中的小鼠实验结果,申请人认为1/5至1/10是较佳的剂量,但不排除在不同肿瘤和肿瘤的不同阶段及其与不同的肿瘤免疫治疗手段结合时会有不同的剂量范围。
复方药物组合物和药盒
本发明提供了含有第一活性成分VEGF信号通路阻断剂,如阻断VEGF信号通路的抗体,和第二活性成分肿瘤免疫治疗剂;以及药学上可接受的载体的复方药物组合物。这类载体包括(但并不限于):盐水、缓冲液、葡萄糖、水、甘油、乙醇、粉剂、及其组合。药物制剂应与给药方式相匹配。本发明的药物组合物可以被制成针剂形式,例如用生理盐水或含有葡萄糖和其他辅剂的水溶液通过常规方法进行制备。诸如片剂和胶囊之类的药物组合物,可通过常规方法进行制备。药物组合物如针剂、溶液、片剂和胶囊宜在无菌条件下制造。本发明的药物组合也可以被制成粉剂用于雾化吸入。一种优选的剂型是注射制剂。此外,本发明药物组合物还可与其他治疗剂一起使用。
本发明还提供了一种用于提高肿瘤免疫治疗效果的药盒,所述药盒包括:
(a)第一药物组合物,所述第一药物组合物包含VEGF信号通路阻断剂,较佳地包含阻断VEGF信号通路的抗体,和药学上可接受的载体,并且所述第一药物组合物为单元剂型,并且所述药盒包括2-4个单元剂型,其中所述单元剂型中的VEGF信号通路阻断剂的含量为常规剂量的0.1至0.5(较佳地0.15-0.4,或0.2-0.25),其中,所述常规剂量为200-400mg/50kg体重;
(b)任选的第二药物组合物,所述的第二药物组合物为肿瘤免疫治疗剂;
(c)说明书。
本发明的药物组合物或药盒适用于***,尤其适用于治疗实体肿瘤。
需要注意的是,本发明的药物组合物或药盒含有低剂量的VEGF信号通路阻断剂,如阻断VEGF信号通路的抗体,其中,低剂量是指所述阻断VEGF信号通路的抗体的给药剂量A1与常规给药剂量A0相比,A1≤1/2A0,较佳地A1≤1/4A0。
在另一优选例中,本发明的药物组合物为单元剂型,每个单元剂型中所述活性成分的含量为日剂量的0.1至1(或0.25-1,或0.5-1),其中所述第一活性成分的日剂量为20-100mg(对于50kg的人而言)。
在另一优选例中,所述阻断VEGF信号通路的抗体的给药剂量为0.1-4mg/kg,较佳地0.2-2.5mg/kg,更佳地0.5-1mg/kg。
当然,所用的活性成分的有效剂量可随给药的模式和待治疗的疾病的严重程度等而有所变化。
治疗方法
本发明还提供了用本发明的两种活性成分或相应的药物对实体肿瘤进行治疗的方法,它包括给哺乳动物施用低剂量的第一活性成分阻断VEGF信号通路的抗体以及第二活性成分肿瘤免疫治疗剂,或者施用含有所述第一活性成分和第二活性成分的药物组合物。
当本发明两种活性成分被用于上述用途时,可与一种或多种药学上可接受的载体或赋形剂混合,如溶剂、稀释剂等,而且可以用无菌可注射溶液或悬浮液形式(在等渗介质中含有约0.05-5%悬浮剂)进行非肠胃给药。例如,这些药物制剂可含有与载体混合的约0.01-99%,更佳地约为0.1%-90%(重量)的活性成分。
本发明的两种活性成分或药物组合物可以通过常规途径进行给药,其中包括(但并不限于):肌内、腹膜内、静脉内、皮下、皮内、口服、瘤内或局部给药。优选的给药途径包括瘤内给药或静脉内给药。
从易于给药的立场看,优选的药物组合物是液态组合物。
此外,本发明的两种活性成分或药物还可与其他治疗癌症的药物(如顺铂、紫杉醇等)联用。
本发明的主要优点包括:
(a)低剂量的VEGF信号通路阻断剂,如阻断VEGF信号通路的抗体,可以显著提高肿瘤免疫治疗的效果。
(b)低剂量的VEGF信号通路阻断剂,如阻断VEGF信号通路的抗体可以促进T淋巴细胞对肿瘤组织的浸润。
(c)低剂量的VEGF信号通路阻断剂,如阻断VEGF信号通路的抗体,还可促进其他肿瘤治疗方法的效果,如化疗和放疗。
(d)本发明的药物组合适用于实体肿瘤的治疗。
下面结合具体实施例,进一步阐述本发明。应理解,这些实施例仅用于说明本发明而不用于限制本发明的范围。下列实施例中未注明具体条件的实验方法,通常按照常规条件,或按照制造厂商所建议的条件。除非另外说明,否则百分比和份数按重量计算。
通用材料
下述实施例中,阻断VEGF信号通路药物DC101购自Bio X Cell公司,用作对照的兔抗鼠IgG单抗购自Jackson ImmunoResearch公司。MCaP0008肿瘤细胞疫苗即丝裂霉素处理的MCaP0008肿瘤细胞,该肿瘤疫苗通过体内的抗原递呈细胞来诱导MCaP0008肿瘤抗原特异的免疫反应。抗血管生成药物贝伐单抗由医院提供;用于细胞免疫治疗的CAR-T细胞由博生吉公司生产。
实施例1
低剂量的抗血管生成药物对肿瘤血管的影响
构建原位MCaP0008乳腺癌模型,当MCaP0008乳腺癌小鼠肿瘤直径达到4-5mm时,给小鼠进行第一次低剂量(小鼠通用剂量的1/4)的阻断VEGF信号通路药物(DC101)以及兔抗鼠IgG单抗(对照)的治疗处理(设为0天),药物注射频率为每三天一次,剂量为10mg/kg(仅为小鼠通用剂量的1/4),共给药4次。在第11天对小鼠进行4%的多聚甲醛灌注,制备肿瘤组织冰冻切片(20μm),并对该组织切片进行内皮细胞标记物CD31分子以及周皮细胞标记物NG2分子的标记染色。利用共聚焦激光扫描显微镜随机选取肿瘤区域进行免疫组化图像采集(每个肿瘤组织4-6个区域,每组6-8个肿瘤),放大倍数20×。统计分析肿瘤血管密度,周细胞覆盖率。
结果如图1所示,在低剂量抗血管生成药物DC101(10mg/kg)治疗后的第11天,仍可观察到肿瘤血管的密度显著减少(图1A和图1B),和周细胞覆盖率显著增加(图1A和图1C)。结果表明低剂量抗血管生成药物可以诱导长效肿瘤血管正常化。
实施例2
低剂量的抗血管生成药物联合疫苗治疗对小鼠肿瘤的影响
为了研究肿瘤血管正常化对肿瘤免疫治疗的影响,进行了低剂量的抗血管生成药物联合疫苗治疗实验。当乳腺癌小鼠肿瘤模型MCaP0008的肿瘤直径达到3mm时,随机分组,腹腔注射5×106丝裂霉素预处理的MCaP0008肿瘤细胞疫苗或是等体积PBS(对照),注射时间分别为第7,9,12,14天。分别在第13,16,19,20天注射抗血管生成药物DC101(10mg/kg)或兔抗鼠IgG(10mg/kg)。从第13天开始测量肿瘤大小,每三天测量一次。绘制肿瘤生长曲线。每组10只小鼠。
结果如图2所示,单独的疫苗治疗对小鼠肿瘤生长的抑制效果不明显,而低剂量的抗血管生成药物诱导的肿瘤血管正常化可以显著增强肿瘤免疫治疗的效果。结果表明,与单独的疫苗治疗和低剂量的抗血管生成药物治疗相比,低剂量的抗血管生成药物联合疫苗治疗可以显著抑制小鼠肿瘤生长。
实施例3
低剂量的抗血管生成药物治疗对T细胞的肿瘤浸润的影响
为了研究肿瘤血管正常化增强免疫治疗的机制,对肿瘤组织中浸润的CD8+和CD4+T淋巴细胞进行分析。当MCaP0008乳腺癌小鼠肿瘤直径达到4-5mm时,给小鼠进行低剂量的DC101以及肿瘤细胞疫苗的治疗处理,DC101及对照兔抗鼠IgG的药物注射频率为每三天一次,剂量为10mg/kg,共给药4次。然后收集肿瘤组织,制备单细胞悬液,进而利用流式细胞分析仪分析肿瘤浸润CD4+T细胞和CD8+T细胞。
结果如图3所示,单独的低剂量阻断VEGF信号通路药物(组别:PBS/D10)可以显著提高CD8+和CD4+淋巴细胞的浸润能力,而低剂量阻断VEGF信号通路药物联合疫苗治疗(组别:疫苗/D10)后,CD4+淋巴细胞的浸润能力进一步显著增强。结果表明,低剂量的抗血管生成药物治疗可以促进T细胞的肿瘤浸润。
实施例4
常规剂量的抗血管生成药物对肿瘤疫苗治疗的影响
实验条件与实施例2相同,区别在于施用的抗血管生成药物的剂量为40mg/kg,即施用常规剂量的抗血管生成药物,研究其对肿瘤免疫治疗的影响。
结果如图4所示,单独的疫苗治疗对小鼠肿瘤生长的抑制效果不明显。同时,常规剂量的抗血管生成药物与疫苗的联合治疗对小鼠肿瘤生长的抑制效果与单独的常规剂量抗血管生成药物的治疗效果没有明显差异。上述结果表明常规剂量(高剂量)的抗血管生成药物,不能提高肿瘤疫苗的治疗效果。
实施例5
血管正常化预处理结合介入治疗的CAR-T临床治疗
CAR-T细胞治疗是一项十分有前景的能够有效***的新的细胞免疫疗法。接受CD19CAR-T治疗的复发或难治急性淋巴细胞白血病患者的完全缓解率达到90%。但是,CAR-T细胞在实体瘤治疗中的表现却并不能令人满意,国际上针对实体瘤CAR-T细胞治疗的尝试也并未成功。肿瘤组织内部血管分布不均匀、肿瘤内部供氧不足等因素严重阻碍CAR-T细胞***回输方案针对实体瘤的治疗有效性,造成***回输的CAR-T细胞不能完全有效的浸润到肿瘤组织内部,并造成CAR-T细胞不能在肿瘤内部被完全有效的活化、增殖、以及长时间的存留。因此,为了改善上述三点实体瘤的特殊性对CAR-T细胞治疗造成的不利影响,选择介入策略将CAR-T细胞注射到肿瘤组织内部,同时配合低剂量(1/5和1/10正常剂量)的Avastin(贝伐单抗,抗血管生成药物),诱导肿瘤血管正常化。
在通过临床医院伦理委员会审批同意后,对1名签署了知情同意书的MUC1+晚期实体瘤患者(晚期***癌)进行了临床科研试验。患者首先静脉注射1mg/kg(正常剂量的1/5,该剂量根据上述小鼠试验结果推测得到)剂量的阻断VEGF信号通路药物Avastin(贝伐单抗,靶向配体VEGF),2周后重复注射同等剂量阻断VEGF信号通路药物,然后1周后对该患者转移的肿瘤病灶在彩超指导下进行了MUC1CAR-T细胞的瘤内注射。CAR-T细胞注射1周后,再次注射阻断VEGF信号通路药物,但剂量减少为0.5mg/kg(正常剂量的1/10,该剂量根据以上小鼠试验结果推测得到)。CAR-T细胞注射3周后,手术摘取肿瘤组织,制作成石蜡切片,进行HE染色。
结果如图5所示,图中颜色粉红的椭圆组织是新鲜坏死的肿瘤细胞,可以看到坏死后的肿瘤细胞与周边组织形成的裂缝。结果表明,低剂量的阻断VEGF信号通路药物与MUC1CAR-T联合CAR-T细胞注射的肿瘤部位出血肿瘤明显坏死,表现出良好的治疗效果。

Claims (10)
1.一种用于治疗实体肿瘤的药物组合物,其特征在于,所述药物组合物包含第一活性成分和第二活性成分,其中,第一活性成分为VEGF信号通路阻断剂,第二活性成分为肿瘤免疫治疗剂,
并且所述药物组合物为单元剂型,每个单元剂型中所述第一活性成分的含量为常规剂量的0.1至0.5(较佳地0.15-0.4,或0.2-0.25),其中,所述常规剂量为200-400mg/50kg体重。
2.如权利要求1所述的药物组合物,其特征在于,所述的一个单元剂型中第一活性成分的含量对应于每次给药的第一活性成分的总常规剂量的0.1至0.5。
3.如权利要求1所述的药物组合物,其特征在于,所述的VEGF信号通路阻断剂包括阻断VEGF信号通路的抗体和酪氨酸激酶抑制剂。
4.如权利要求1所述的药物组合物,其特征在于,所述的阻断VEGF信号通路的抗体包括:雷莫芦单抗、贝伐单抗、或其组合。
5.如权利要求1所述的药物组合物,其特征在于,所述第一活性成分和第二活性成分是混合或单独存在的。
6.如权利要求1所述的药物组合物,其特征在于,所述的第一活性成分和第二活性成分的总含量为70~100wt%,按所述药物组合物中活性成分的总重量计。
7.如权利要求1所述的药物组合物,其特征在于,所述的肿瘤免疫治疗包括CAR-T细胞治疗、CAR-NK细胞治疗、肿瘤疫苗治疗、免疫检验点抑制剂治疗、或其组合。
8.一种低剂量的VEGF信号通路阻断剂的用途,其特征在于,用于制备一药物组合物或药盒,所述药物组合物或药盒用于提高肿瘤免疫治疗的疗效,
其中,低剂量是指所述VEGF信号通路阻断剂的给药剂量A1与常规给药剂量A0相比,A1≤1/2A0。
9.如权利要求8所述的用途,其特征在于,所述的药盒包括2-4个单元剂型,所述各单元剂型中的VEGF信号通路阻断剂的抗体的含量为常规剂量的0.1至0.5,其中,所述常规剂量为200-400mg/50kg体重。
10.一种用于提高肿瘤免疫治疗效果的药盒,其特征在于,所述药盒包括:
(a)第一药物组合物,所述第一药物组合物包含VEGF信号通路阻断剂,和药学上可接受的载体,并且所述第一药物组合物为单元剂型,并且所述药盒包括2-4个单元剂型,其中所述单元剂型中的VEGF信号通路阻断剂的含量为常规剂量的0.1至0.5(较佳地0.15-0.4,或0.2-0.25),其中,所述常规剂量为200-400mg/50kg体重;
(b)任选的第二药物组合物,所述的第二药物组合物为肿瘤免疫治疗剂;
(c)说明书。
