Abstract

本发明涉及骨科用医疗器械领域,尤其是一种胸腰椎病变或损伤时,用于对胸腰椎进行固定的胸腰椎后路微动固定***。该***包括两个微动椎弓根螺钉、一个起连接两个微动椎弓根螺钉作用的钛合金连接棒及用于锁紧固定钛合金连接棒和微动椎弓根螺钉的锁紧顶丝。微动椎弓根螺钉由球头钉和钉座两部分组成,球头钉与钉座铆接,球头钉通过钉座固定,并且钉座与球头钉形成转动幅,使球头钉在钉座内在限定方向轻微转动,钉座前端的收口卡住球头钉的球头前部。本发明的胸腰椎后路微动固定***充分考虑国人的解剖学特点,所有设计参数直接来自国人脊柱解剖学数据,可满足胸腰椎内固定手术需求,固定及恢复效果良好。

Description

胸腰椎后路微动固定***
技术领域
本发明涉及骨科用医疗器械领域,尤其是一种胸腰椎病变或损伤时,用于对胸腰椎进行固定的胸腰椎后路微动固定***。
背景技术
腰椎融合手术是治疗由关节退变、椎间盘突出等各种疾病引发的腰椎不稳的通用手术,手术的关键是要植入内固定器械保证病变腰椎的稳定性,从而给予植骨融合必要的条件。其使用的内固定器械属于坚强***,植入腰椎后,会使得被植入节段活动度过分限制,从而导致相邻节段退变以及植骨融合不可靠甚至失败,同时还可能因为内部应力集中过大而导致***失效,出现松动、脱钉等现象。
在这种情况下,微动内固定***的需求应运而生。在最近的30年内,世界各国学者都不断的在研究微动内固定***,但是到目前为止,较成熟应用于市场的微动内固定***并不多,主要代表有两个,分别是Dynesys***和Graf韧带***。但是由多位学者的调查研究发现,Dynesys***和Graf韧带***都存在着比较严重的缺陷。因此,为了满足市场需要,有必要设计一种基于坚强固定的结构简单可靠的胸腰椎后路微动固定***。
发明内容
本发明针对现有产品的缺陷,提供了一种结构简单可靠,力学性能优越,使用方便,且适用于腰椎退变或融合手术恢复期,稳定性好的胸腰椎后路微动固定***。
本发明的技术方案是:
一种胸腰椎后路微动固定***,该***包括两个微动椎弓根螺钉、一个起连接两个微动椎弓根螺钉作用的钛合金连接棒及用于锁紧固定钛合金连接棒和微动椎弓根螺钉的锁紧顶丝。
所述的胸腰椎后路微动固定***,微动椎弓根螺钉由球头钉和钉座两部分组成,球头钉与钉座铆接,球头钉通过钉座固定,并且钉座与球头钉形成转动幅,使球头钉在钉座内在限定方向轻微转动,钉座前端的收口卡住球头钉的球头前部。
所述的胸腰椎后路微动固定***,球头钉转动范围控制在-15~15度。
所述的胸腰椎后路微动固定***,球头钉在与钉座铆接方向打磨出一对平行平面,使球头钉与钉座牢固铆接。
所述的胸腰椎后路微动固定***,钉座后部开有U型槽,微动椎弓根螺钉通过U型槽与钛合金连接棒固定连接,微动椎弓根螺钉与钛合金连接棒为柱面接触。
所述的胸腰椎后路微动固定***,钉座后部设有内螺纹部分,内螺纹部分安装锁紧顶丝,通过锁紧顶丝固定钛合金连接棒。
所述的胸腰椎后路微动固定***,钉座的前端收口在球头钉微动方向开有两个斜面。
所述的胸腰椎后路微动固定***,球头钉和钉座为一个组件,二者不可拆卸。
所述的胸腰椎后路微动固定***,采用Ti-6Al-4V材质的微动椎弓根螺钉、钛合金连接棒和锁紧顶丝。
与现有技术相比,本发明具有如下有益效果:
1、本发明提供的胸腰椎后路微动固定***由微动椎弓根螺钉、连接微动椎弓根螺钉的钛合金连接棒以及锁紧顶丝组成,锁紧顶丝用于固定钛合金连接棒,其结构简单,可靠性高。
2、本发明的微动椎弓根螺钉由球头钉和钉座组成,球头钉通过铆接工艺与钉座固定,球头钉可以在钉座内以铆接点为中心沿指定方向轻微摆动,球头钉可摆动角度控制,在-15~15度,满足腰椎固定节段在屈伸工况下的正常活动,促进病变腰椎的回复。
3、本发明的微动固定***继承了坚强固定***的优点,在满足固定效果良好的前提下实现微动,这种特性目前临床未曾使用。
4、本发明的微动固定***力学性能优越,与人体组织生物相容性好。
5、本发明符合人体脊椎运动原理,有助于脊柱正常功能的恢复。

具体实施方式
下面,结合附图对本发明进一步详细阐述。
参见图1、图2、图3(a)-图3(b),本发明胸腰椎后路微动固定***,包括两个微动椎弓根螺钉2、一个起连接两个微动椎弓根螺钉2作用的钛合金连接棒1及用于锁紧固定钛合金连接棒1和微动椎弓根螺钉2的锁紧顶丝3。
如图4(a)、图4(b)、图4(c)所示,微动椎弓根螺钉2由球头钉5和钉座4两部分组成,球头钉5与钉座4铆接成为一体,球头钉5可以在钉座4内在限定方向轻微转动,钉座4前端的收口10卡住球头钉5的球头9前部,防止球头钉5在体内被拔出,结构简单可靠。微动椎弓根螺钉2的外螺纹部分8设计成锥形(锥度范围为1~3度),可有效地提高微动椎弓根螺钉2根部的强度,微动椎弓根螺钉2根部一直是整个***应力集中的部位,这种设计可以降低微动椎弓根螺钉2从根部断裂的风险。
如图4(b)、图4(c)所示,球头钉5在与钉座4铆接方向打磨出一对平行平面,可提高球头钉5与钉座4铆接牢固度。
如图4(a)、图4(d)、图4(e)、图4(f)、图4(g)所示,钉座4后部开有U型槽6,微动椎弓根螺钉2通过U型槽6与钛合金连接棒1固定连接,微动椎弓根螺钉2与钛合金连接棒1为柱面接触,保证牢固可靠。钉座4后部设有内螺纹部分7,用于拧入锁紧顶丝3,通过锁紧顶丝3固定U型槽6中的钛合金连接棒1。球头钉5从钉座4后端装入,钉座4的前端收口10在球头钉5微动方向(转动方向)开有两个斜面11,控制球头钉5的微动角度。根据人体腰椎节段间的正常活动角度,该微动角度控制在-15~15度之间(即正常安装后的左右转动角度)。
其中,本发明的球头钉5与钉座4采用铆接固定,既对球头钉5提供一定的固定作用,又形成一个转动幅,实现球头钉5在体内的微动,使腰椎固定节段能够有轻微的屈伸活动度,改变坚强固定在腰椎节段的完全限制情况。
在进行胸腰椎后路微动固定***植入时,先确定患者脊椎的患病节段,根据需求植入微动椎弓根螺钉2,然后把钛合金连接棒1压入U型槽6,最后拧入锁紧顶丝3,植入过程安全快捷。
本发明的微动固定***可有效改善植入装置在体内的受力状态,减小应力集中,降低应力遮挡,并且可以给固定节段提供适当屈伸活动度。通过仿真分析技术预测在人体生理载荷下,植入装置的应力水平比坚强固定低。
实施例结果表明,本发明的胸腰椎后路微动固定***针对目前主流的坚强固定***存在的术后并发症问题,结合人体生物力学相关理论,是从力学角度提出的重建人体腰椎稳定的解决方案。该***充分考虑了国人的解剖学特点,所有设计参数直接来自国人脊柱解剖学数据,可满足胸腰椎内固定手术需求,固定及恢复效果良好。

Claims (9)

1.一种胸腰椎后路微动固定***,其特征在于,该***包括两个微动椎弓根螺钉、一个起连接两个微动椎弓根螺钉作用的钛合金连接棒及用于锁紧固定钛合金连接棒和微动椎弓根螺钉的锁紧顶丝。
2.按照权利要求1所述的胸腰椎后路微动固定***,其特征在于,微动椎弓根螺钉由球头钉和钉座两部分组成,球头钉与钉座铆接,球头钉通过钉座固定,并且钉座与球头钉形成转动幅,使球头钉在钉座内在限定方向轻微转动,钉座前端的收口卡住球头钉的球头前部。
3.按照权利要求2所述的胸腰椎后路微动固定***,其特征在于,球头钉转动范围控制在-15~15度。
4.按照权利要求2所述的胸腰椎后路微动固定***,其特征在于,球头钉在与钉座铆接方向打磨出一对平行平面,使球头钉与钉座牢固铆接。
5.按照权利要求2所述的胸腰椎后路微动固定***,其特征在于,钉座后部开有U型槽,微动椎弓根螺钉通过U型槽与钛合金连接棒固定连接,微动椎弓根螺钉与钛合金连接棒为柱面接触。
6.按照权利要求2所述的胸腰椎后路微动固定***,其特征在于,钉座后部设有内螺纹部分,内螺纹部分安装锁紧顶丝,通过锁紧顶丝固定钛合金连接棒。
7.按照权利要求2所述的胸腰椎后路微动固定***,其特征在于,钉座的前端收口在球头钉微动方向开有两个斜面。
8.按照权利要求2所述的胸腰椎后路微动固定***,其特征在于,球头钉和钉座为一个组件,二者不可拆卸。
9.按照权利要求1所述的胸腰椎后路微动固定***,其特征在于,采用Ti-6Al-4V材质的微动椎弓根螺钉、钛合金连接棒和锁紧顶丝。
