Abstract
本发明涉及一种医疗器械,具体的说是一种髁状突骨折固定工具,包含用于旋入髁状突内将髁状突的骨折片固定在髁状突骨折断面上的螺纹段、与螺纹段相连的颈部和与颈部相连的帽部;其中,帽部用于锚缝固定断裂、脱位的颞下颌关节盘,且螺纹段、颈部和帽部构成镁合金螺钉。同现有技术相比,通过螺纹段可将复位后的髁状突骨折断端紧密贴合,同时可将颞下颌关节盘锚缝在螺钉的帽部,以实现对颞下颌关节盘的复位和锚固,在保证骨折良好愈合和颞下颌关节功能恢复的同时,还由于螺钉是采用镁合金材质,使得螺钉可逐渐被降解吸收以促进骨组织再生,从而克服了医用钛钉长期存留在髁状突内所造成的并发症,同时避免了二次手术取出钛钉所造成的不必要的创伤。

技术领域
本发明涉及一种医疗器械,特别涉及一种髁状突骨折固定工具。
背景技术
髁状突,位于下颌骨双侧顶端,是实现口颌功能的重要解剖结构,也是下颌骨解剖学上的一个薄弱部位。髁状突在生理状态下可将咀嚼压力通过颞下颌关节传导至颅底,在下颌收到撞击时此处易形成应力集中,从而易导致髁状突骨折,继而引起患者咬合关系错乱、张口受限、关节盘错位、关节强直等症状,严重影响患者的咀嚼、发音等生理功能。
由于髁状突骨折类型复杂多样,而髁状突高位斜形骨折以及矢状骨折是目前最常见的类型。由于在髁状突高位斜形骨折及矢状骨折区域中接骨板置入困难,而钢丝固定又不牢固,同时由于常伴有关节盘移位、折叠等软组织结构改变,因此对此类骨折常需使用医用钛钉将折裂的髁突复位固定至下颌骨断面上,恢复患侧髁状突及下颌骨的正常形态,使骨折断面不发生相对移动,有时还需在髁状突上置入一种特制的纯钛锚固钉从而将移位、折叠的关节盘复位并锚缝固定起来,从而提高骨折修复的稳定性和可靠性。
然而,医用钛钉长期留置在髁状突内会存在二次感染、金属微电流、缓慢移位等问题,而二期手术取出钛钉又存在手术创伤,取出困难等风险,极大地增加了手术难度。
发明内容
本发明的目的在于提供一种髁状突骨折固定工具,在有效对髁状突的骨折部位进行固定的同时,还可有效防止骨折部位二次感染、金属微电流及固位钉缓慢位移等术后并发症,并可有效避取出螺钉所造成的手术创伤。
为了实现上述目的,本发明在于提供一种髁状突骨折固定工具,包含用于旋入髁状突内将髁状突的骨折片固定在髁状突的骨折断面上的螺纹段、用于加力和锚固颞下颌关节盘的帽部及连接所述螺纹段和帽部的颈部;
其中,所述帽部用于锚缝固定颞下颌关节盘,且所述螺纹段、所述颈部和所述帽部构成镁合金螺钉。
本发明相对于现有技术而言,由于整个固定工具为一根镁合金螺钉,且是由螺纹段、颈部和帽部所构成,从而当螺钉的螺纹段在旋入髁状突的骨折部位后,通过螺纹段可将复位后的髁状突骨折断端紧密贴合,同时可将断裂、脱位的颞下颌关节盘锚缝在螺钉的帽部上,以实现对颞下颌关节盘的复位和锚固,从而避免了额外锚固钉的植入,在保证骨折良好愈合和颞下颌关节功能恢复的同时,还由于整个螺钉是采用镁合金材质,使得螺钉在行使固位功能的同时还可逐渐被降解吸收以促进骨组织再生,从而避免了医用钛钉长期存留在髁状突内所造成的并发症,及二次手术取出钛钉所造成的创伤和风险。
进一步的,所述螺纹段背离所述帽部的一侧为所述螺纹段的端部,且所述端部从端尾至端顶逐渐收拢,并在其端顶处形成一尖钉。通过螺纹段端部的尖钉可方便螺纹段旋入髁状突内。
进一步的,所述螺纹段的端部还开设有N个用于收集和排出骨屑的排屑槽;其中,所述N为自然数,且各排屑槽延伸至所述螺纹段端部的端顶。通过开设在螺纹段端部的排屑槽,可使得螺钉的螺纹段在旋入髁状突内时,可便于骨屑的收集和排出,从而进一步方便了螺旋段的旋入。
进一步的,当所述N大于1时,各排屑槽在所述螺纹段的端部上等距环绕设置。从而使得螺钉的螺纹段在旋入髁状突内时,可进一步提高螺纹段的排屑性能,保证排屑的均匀性,避免螺纹段在旋入时出现局部应力过大的现象。
进一步的,所述颈部的外径与所述螺纹段的外径一致,且所述颈部为一外表面为光滑面的圆柱体。从而使得螺纹段在旋入髁状突内后,通过颈部可将螺纹段和帽部隔开,避免帽部压迫到髁状突而造成复位后的髁状突骨折断出现不必要的位移。
进一步的,所述帽部包含与所述颈部相连的帽檐、设置在所述帽檐上用于带动所述螺纹段旋入所述髁状突内的加力件;其中,所述加力件上还开设有锚固孔用于将所述帽部锚缝在所述颞下颌关节盘上。并且,所述加力件为一正多边形结构。从而方便医护人员对加力件进行操作,避免打滑现象的发生,以使得螺纹段的旋入更为容易。
进一步的,所述帽檐设有所述加力件的一侧端面为上端面,所述帽檐与所述颈部相连的一侧端面为下端面;其中,所述帽檐整体从上端面到下端面逐渐收拢,且所述下端面的直径与所述颈部的直径相同。使得螺纹段在旋入髁状突内后,由于帽檐的下端面的直径与颈部的直径是相同的,从而可进一步避免帽部的帽檐直接压迫到髁状突,而造成复位后的髁状突骨折断出现不必要的位移。
进一步的,所述螺纹段的长度与所述髁状突骨折部位的宽度相适配。从而可使得螺纹段在***髁状突内后,可对复位后的骨折片与髁状突骨折断端进行有效固定。
进一步的,所述镁合金螺钉表面还涂覆有可降解高分子涂层、可降解陶瓷涂层及可降解复合涂层中的一种或多种。使得螺钉在髁状突内在承载负载的同时,还可释放一种或多种具有抗菌、消炎、促进骨再生作用的药物,从而加快髁状突骨折部位的愈合。
进一步的,所述髁状突骨折固定工具包含一根或两根所述的镁合金螺钉;其中,当镁合金螺钉为两根时,其中一根镁合金螺钉与髁状突骨折断面垂直,而另一根镁合金螺钉与之呈一倾角;
而当镁合金螺钉为一根时,所述镁合金螺钉螺纹段的旋入方向垂直于髁状突的骨折线,或所述镁合金螺钉螺纹段的旋入方向与髁状突的骨折线和髁状突断端外侧面垂线的角平分线一致。

具体实施方式
为使本发明的目的、技术方案和优点更加清楚,下面将结合附图对本发明的各实施方式进行详细的阐述。然而,本领域的普通技术人员可以理解,在本发明各实施方式中,为了使读者更好地理解本申请而提出了许多技术细节。但是,即使没有这些技术细节和基于以下各实施方式的种种变化和修改,也可以实现本申请各权利要求所要求保护的技术方案。
本发明的第一实施方式涉及一种髁状突骨折固定工具,该髁状突骨折固定工具为一根镁合金螺钉,如图1和图2所示,主要由螺纹段1、与螺纹段1相连的颈部2、与颈部2相连的帽部3构成。
其中,结合图3所示,螺纹段1用于旋入髁状突4内将髁状突的骨折片4-1固定在髁状突4的骨折断面4-2上,并且在本实施方式中,该螺纹段1为一个直径为1.5mm至2mm,长度为6mm至16mm的螺杆,且螺纹段1上的螺纹直径为2mm至2.5mm,螺距为0.75mm至1mm。颈部2用于在螺纹段旋入髁状突4内后隔开螺纹段1和帽部3,避免帽部3压迫到髁状突而造成复位后的髁状突骨折断出现不必要的位移,且该颈部为一个直径为2mm至2.5mm,长度为0.75mm至6mm的圆柱体的,且该圆柱体的外表面为光滑面。而帽部3在螺纹段1旋入髁状突4内后暴露在髁状突4的外部用于锚缝固定在颞下颌关节盘(图中未标示)上。并且螺纹段1、颈部2和帽部3均同轴设置。
并且该镁合金螺钉在制作过程成可选用镁元素作为主要的合金材质,而其他合金材料可选用钕、锌、锆、钙、钇、钆中的一种或多种元素,同时为了保证螺钉的机械强度,螺钉内部还可含有铜、锰、锶、铁、锡、铝、银等元素中的一种或多种。
通过上述内容不难发现,由于整个螺钉是由螺纹段1、颈部2和帽部3所构成,从而当螺钉的螺纹段1在旋入髁状突4的骨折部位后,通过螺纹段1可将复位后的髁状突骨折断端紧密贴合(即将髁状突的骨折片4-1与髁状突4的骨折断面4牢牢的贴合在一起),同时可将断裂、脱位的颞下颌关节盘锚缝在螺钉的帽部3上,以实现对移位、折叠的颞下颌关节盘的复位和锚固,有效的将复位后的髁状突骨折端稳固固定于正确的解剖位置,使其可承受一定的组织作用力,从而避免了额外锚固钉的植入,在保证骨折良好愈合和颞下颌关节功能恢复的同时,还由于整个螺钉是采用镁合金材质制成的,使得螺钉在行使固位功能的同时还可逐渐被降解吸收以促进骨组织再生,从而克服了医用钛钉长期存留在髁状突内所造成的并发症,及二次手术取出钛钉所造成的不必要的创伤和风险。
具体的说,在本实施方式中,螺钉的螺纹段1背离帽部3的一侧为螺纹段1的端部1-1,且端部1-1从端尾至端顶逐渐收拢,并在其端顶处形成一尖钉。从而使得螺纹段1在旋入髁状突内时,通过螺纹段端部1-1的尖钉可对螺纹段的旋入起到一定的导向作用,从而方便螺纹段1的旋入。
同时,如图1所示,螺纹段1的端部还开设有多个用于收集和排出骨屑的排屑槽1-2,且各排屑槽1-2从螺纹段端部1-1的端尾一直延伸至螺纹段端部1-1的端顶。由此可知,通过开设在螺纹段端部1-1的排屑槽1-2,可使得螺钉的螺纹段1在旋入髁状突4内时,可便于骨屑的收集和排出,从而进一步方便了螺旋段1的旋入。
并且,为了进一步方便螺钉螺纹段1的旋入,各排屑槽1-2在螺纹段1的端部上等距环绕设置。从而使得螺钉的螺纹段1在旋入髁状突4内时,可进一步提高螺纹段1的排屑性能,保证排屑的均匀性,避免螺纹段1在旋入时出现局部应力过大的现象。
此外,上述所提到的螺钉的帽部3如图1和图2所示,主要由与颈部2相连的帽檐3-1和设置在帽檐3-1上用于带动螺纹段1旋入髁状突4内的加力件3-2构成。其中,加力件3-2上还开设有锚固孔3-3,该锚固孔3-3用于将整个帽部3锚缝在颞下颌关节盘(图中未标示)上。由此不难发现,在实际操作时,医护人员可直接徒手转动加力件,或者也通过外部工具夹持住加力件3-2,并带动其进行旋转,从而使得螺纹段1可有效地旋入髁状突4内,以确保髁状突骨折片4-2与髁状突4的骨折断面4之间的紧密贴合。同时,为了方便医护人员对加力件3-2的操作,使得医护人员可轻松的将螺钉的螺纹段1旋入髁状突4内,在本实施方式中,加力件3-2的外形为一正多边形,而优选地可为正六边形,从而当医护人员在转动加力件3-2时,可避免打滑现象的发生,并由于加力件3-2为正六边形结构,还可方便的将其与外六角的旋转工具进行装配,从而进一步方便了医护人员的操作。
另外,值得一提的是,如图1和图2所示,帽檐3-1设有加力件3-2的一侧端面为上端面,而帽檐3-1与颈部2相连的一侧端面为下端面。而在本实施方式中,帽檐3-1整体从上端面到下端面逐渐收拢,整体为一碟状,其最大的端面直径为3mm至4mm,且帽檐3-1的整体高度为3-5mm。并且,需要说明的是,帽檐3-1下端面的直径与颈部2的直径相同,从而使得螺纹段1在旋入髁状突4内后,由于帽檐3-1的下端面的直径与颈部2的直径是相同的,从而可进一步避免帽部3的帽檐3-1直接压迫到髁状突4,而造成复位后的髁状突骨折断端出现不必要的位移现象。
通过上述内容可知,上述所阐述的内容为本实施方式中用于固定髁状突骨折部位的镁合金螺钉的结构,同时医护人员会根据患者实际的骨折情况,可选取一根镁合金螺钉对骨折部位进行固定,或者也可采用两根螺钉对骨折部位进行固定。当采用一根镁合金螺钉对髁状突的骨折部位进行固定时,可采用以下方式。
具体为,患者在经鼻腔插管全身麻醉后,采用耳前穿腮腺切口或改良的耳前角形切口,切开皮肤、皮下组织,分离或结扎颞浅动、静脉,显露并保护面神经分支,逐层分离至颞下颌关节囊外侧面。随后切开关节囊进入关节下腔,向下牵拉下颌支,显露髁状突高位斜形骨折或矢状骨折断面并在其前内下方寻找移位的骨折片4-2,将游离骨折片4-2复位至与髁状突骨折断面准确对位,如图3所示,并在核对咬合关系无误后,从髁状突骨折断端4-1外侧面设计一个钻孔位置,并用专用的钻针对外侧髁状突骨折断端4-1与内侧复位骨折片4-2进行全层钻孔。同时根据术前CT测量髁状突4内外侧宽度,选取长度适当的螺钉旋入其钻孔内对髁状突骨折断端4-1与复位骨折片4-2进行固定,需要说明的是,在选取螺钉时,应尽量保证螺钉螺纹段1的长度与髁状突骨折部位的宽度相适配,使得螺纹段1在***髁状突内后,螺纹段1可稍微超出髁状突4的内侧骨皮质层,从而可对复位后的骨折片4-2与髁状突骨折断端4-1进行有效固定。并且在完成固定后,检查关节盘(图中未标示)有无移位、穿孔、折叠现象,若有移位、折叠现象,即将其复位,并用缝线并配合加力件3-2的锚固孔3-3将其锚缝固定在帽部3的加力件3-2上,若有穿孔,即对其进行修补。最后,再次检查髁状突位置及咬合关系无误后冲洗创口,建立负压引流,分层缝合手术切口。
并且,需要说明的,在钻孔时,医护人员可根据术前CT测量髁状突内外侧宽度直接选用适当长度的2mm直径专用钻针对外侧髁状突骨折断端4-1及内侧复位骨折片4-2进行全层钻孔。或者先选用1.5mm直径专用钻针对外侧髁状突骨折断端4-1与内侧复位骨折片4-2进行全层钻孔,再以2mm直径专用钻针扩大外侧髁状突骨折断端4-1的钻孔,以增强对骨折界面两端的压力。
同时,为了保证镁合金螺钉在旋入时,能够对髁状突骨折断端4-1与复位骨折片4-2进行有效固定,在钻孔时,应保证螺钉螺纹段1的旋入方向垂直于髁状突的骨折线4-3,或螺钉螺纹段1的旋入方向与髁状突的骨折线4-3和髁状突断端外侧面垂线的角平分线(图中未标示)一致。
而当采用两根镁合金螺钉对髁状突的骨折部位进行固定时,可采用以下方式。
具体为,患者在经鼻腔插管全身麻醉后,采用耳前穿腮腺切口或改良的耳前角形切口,切开皮肤、皮下组织,分离或结扎颞浅动、静脉,显露并保护面神经分支,逐层分离至颞下颌关节囊外侧面。随后切开关节囊进入关节下腔,向下牵拉下颌支,显露髁状突高位斜形骨折或矢状骨折断面并在其前内下方寻找移位的骨折片4-2,将游离骨折片4-2复位至与髁状突骨折断面准确对位,如图4所示,并在核对咬合关系无误后,从髁状突骨折断端4-1外侧面设计两个钻孔位置,并用专用钻针对外侧髁状突骨折断端4-1与内侧复位骨折片4-2进行全层钻孔。同时根据术前CT测量髁状突4内外侧宽度,选取两根长度适当的螺钉旋入其钻孔内对髁状突骨折断端4-1与复位骨折片4-2进行固定。并在完成固定后,检查关节盘(图中未标示)有无移位、穿孔、折叠现象,若有移位、折叠现象,即将其复位,并用缝线并配合加力件3-2的锚固孔3-3将其锚缝固定在帽部3的加力件3-2上,若有穿孔即对其进行修补。最后,再次检查髁状突位置及咬合关系无误后冲洗创口,建立负压引流,分层缝合手术切口。
并且,需要说明的,在钻孔时,医护人员可根据术前CT测量髁状突内外侧宽度直接选用适当长度的2mm直径专用钻针对外侧髁状突骨折断端4-1及内侧复位骨折片4-2进行全层钻孔。或者先选用1.5mm直径专用钻针对外侧髁状突骨折断端4-1与内侧复位骨折片4-2进行全层钻孔,再以2mm直径专用钻针扩大外侧髁状突骨折断端4-1的钻孔,以增强对骨折界面两端的压力。
同时,为了保证镁合金螺钉在旋入时,能够对髁状突骨折断端4-1与复位骨折片4-2进行有效固定,在钻孔时,应保证两根螺钉螺纹段1在旋入后,其中一根镁合金螺钉5与髁状突骨折断面垂直,而另一根镁合金螺钉5与之呈一倾角,而作为优选的该倾角一般控制在30°至45°。
本发明的第二实施方式涉及一种髁状突骨折固定工具,第二实施方式是在第一实施方式的基础上作的进一步改进,其主要改进在于:在本实施方式中,镁合金螺钉表面还涂覆有可降解高分子涂层、可降解陶瓷涂层及可降解复合涂层中的一种或多种。由此可知,通过上述涂层,可使得螺钉在髁状突内承载负载的同时,还可释放一种或多种具有抗菌、消炎、促进骨再生作用的药物,从而加快了髁状突骨折部位的愈合。
本领域的普通技术人员可以理解,上述各实施方式是实现本发明的具体实施例,而在实际应用中,可以在形式上和细节上对其作各种改变,而不偏离本发明的精神和范围。
Claims (11)
1.一种髁状突骨折固定工具,其特征在于:包含用于旋入髁状突内将髁状突的骨折片固定在髁状突骨折断面上的螺纹段、用于加力和锚固颞下颌关节盘的帽部及连接所述螺纹段和帽部的颈部;
所述螺纹段、所述颈部和所述帽部构成镁合金螺钉。
2.根据权利要求1所述的髁状突骨折固定工具,其特征在于:所述螺纹段背离所述帽部的一侧为所述螺纹段的端部,且所述端部从端尾至端顶逐渐收拢,并在其端顶处形成一尖钉。
3.根据权利要求2所述的髁状突骨折固定工具,其特征在于:所述螺纹段的端部还开设有N个用于收集和排出骨屑的排屑槽;
其中,所述N为自然数,且各排屑槽延伸至所述螺纹段端部的端顶。
4.根据权利要求3所述的髁状突骨折固定工具,其特征在于:当所述N大于1时,各排屑槽在所述螺纹段的端部上等距环绕设置。
5.根据权利要求1所述的髁状突骨折固定工具,其特征在于:所述颈部的外径与所述螺纹段的外径一致,且所述颈部为一外表面为光滑面的圆柱体。
6.根据权利要求1所述的髁状突骨折固定工具,其特征在于:所述帽部包含与所述颈部相连的帽檐、设置在所述帽檐上用于带动所述螺纹段旋入所述髁状突内的加力件;
其中,所述加力件上还开设有锚固孔用于将所述帽部锚缝在所述颞下颌关节盘上。
7.根据权利要求6所述的髁状突骨折固定工具,其特征在于:所述加力件为一正多边形结构。
8.根据权利要求6所述的髁状突骨折固定工具,其特征在于:所述帽檐设有所述加力件的一侧端面为上端面,所述帽檐与所述颈部相连的一侧端面为下端面;
其中,所述帽檐整体从上端面到下端面逐渐收拢,且所述下端面的直径与所述颈部的直径相同。
9.根据权利要求1所述的髁状突骨折固定工具,其特征在于:所述螺纹段的长度与所述髁状突骨折部位的宽度相适配。
10.根据权利要求1所述的髁状突骨折固定工具,其特征在于:所述镁合金螺钉表面还涂覆有可降解高分子涂层、可降解陶瓷涂层及可降解复合涂层中的一种或多种。
11.根据权利要求1至10中任意一项所述的髁状突骨折固定工具,其特征在于:所述髁状突骨折固定工具包含一根或两根所述的镁合金螺钉;
其中,当镁合金螺钉为两根时,其中一根镁合金螺钉与髁状突骨折断面垂直,而另一根镁合金螺钉与之呈一倾角;
而当镁合金螺钉为一根时,所述镁合金螺钉螺纹段的旋入方向垂直于髁状突的骨折线,或所述镁合金螺钉螺纹段的旋入方向与髁状突的骨折线和髁状突断端外侧面垂线的角平分线一致。
