Abstract
本发明公开了乙醛脱氢酶2作为蒽环类化疗药物处理肿瘤细胞时药物靶标的应用,通过药物筛选发现蒽环类化疗药可以特异性的杀死VHL缺失或突变的肾透明细胞癌细胞,对正常细胞毒性甚小,为肾癌的化疗治疗提供了一个有效选择,克服了目前临床中肾癌基本只依赖手术治疗的缺陷。进一步研究我们发现乙醛脱氢酶2介导蒽环类化疗药对肿瘤细胞的敏感性,提示乙醛脱氢酶2作为蒽环类化疗药物处理肿瘤细胞时药物靶标的应用,又由于40%的亚洲人群中都存在着乙醛脱氢酶2的缺失或突变,故使得本发明拥有更广泛的应用前景,而对于乙醛脱氢酶2不发生缺失或突变的患者,可以通过蒽环类化疗药与乙醛脱氢酶2的抑制剂联合使用,明显增加对肿瘤细胞的杀伤作用。

本发明涉及生物技术和医药领域,具体地讲,涉及乙醛脱氢酶2作为蒽环类化疗药物处理肿瘤细胞时药物靶标的应用。
背景技术
肿瘤是目前危害人类健康的最大杀手之一,目前临床上肿瘤的主要治疗手段是手术切除以及放化疗,化疗是利用化学药物杀死肿瘤细胞、抑制肿瘤细胞的生长繁殖和促进肿瘤细胞的分化的一种治疗方式,它是一种全身性治疗手段对原发灶、转移灶和亚临床转移灶均有治疗作用,但是化疗***在杀伤肿瘤细胞的同时,也将正常细胞和免疫细胞一同杀灭,还能导致胃肠功能紊乱、骨髓抑制等副作用,大大降低了患者的生存质量。现在化疗方案的改进就是尽量的减少化疗的副作用同时增进疗效,因此能找到一种特异性针对肿瘤细胞的化疗药物及其靶标是非常重要的。
肾细胞癌是泌尿外科常见恶性肿瘤之一,占成人所有肿瘤的2%-3%,全球每年有超过20万例新发病例及超过10万死亡病例。由于肾癌缺乏有效的化疗手段,临床目前只能依赖于手术治疗。肾细胞癌急需分子机制的深入研究,认识和发掘潜在的药物治疗靶点,发展靶向药物,提高患者的药物治疗水平。
在肾细胞癌中,肾透明细胞癌约占70%-80%比例。除了VHL缺失引发的遗传性透明细胞癌,约70%的散发性肾透明细胞癌患者癌组织中也存在VHL基因的突变、杂合性缺失或甲基化而导致pVHL蛋白的失活。研究发现,肾透明细胞癌的发病机制与VHL(VolHippel-Lindau)基因突变或缺失导致的失活紧密相关。VHL基因定位于染色体3p25-26区域,是典型的抑癌基因,其蛋白产物pVHL可与ElonginB、ElonginC、CUL2蛋白组成VBC(VHL-ElonginB/C-CUL2)复合物,该复合物被证实属E3-泛素蛋白酶***,介导人体内多种蛋白的降解。VHL基因失活可导致遗传性肾透明细胞癌的发生;目前发现的VHL靶蛋白中,对缺氧诱导因子(hypoxia-inducibletranscriptionfactor1-α,HIF1-α)的研究最为深入。失活的VHL蛋白无法正常调节HIF1-α的降解,从而使HIF1-α水平升高,而后者蛋白的升高可转位至细胞核内调节缺氧诱导基因的转录,导致其下游信号分子包括VEGF、血小板衍生生长因子(PDGF)、表皮生长因子(EGF)、转化生长因子(TGF)、***等的持续表达,与肾细胞癌的发生发展关系密切。
由于肾癌缺乏有效的化疗手段,临床目前只能依赖于手术治疗。肾癌不化疗的原因,首先是由于肾癌中一些多药耐药基因的高表达,导致其对化疗药物不敏感,其次虽然化疗在治疗肾癌能快速抑制症状,但是化疗抑制肾癌细胞的同时对正常细胞也会造成损伤,化疗带来的毒副作用多数患者难以忍受,因个体差异,多数患者在化疗过程中会出现全身乏力,容易受感染,倦怠,脱发,机体免疫力下降等。目前靶向治疗肾癌的药物均针对HIF1-α下游靶基因,包括索拉非尼、贝伐珠等药物已用晚期肾癌的靶向治疗并取得一定成绩,但是肾癌的药物治疗效果仍然不尽如人意,主要表现在病情复发、生存率提高甚微、毒副作用大等方面。因此肾细胞癌急需分子机制的深入研究,认识和发掘潜在的药物治疗靶点,发展靶向药物,提高患者的药物治疗水平。
蒽环类化疗药物是临床常用的抗肿瘤抗生素药物,主要包括阿霉素、表阿霉素等,是一类细胞周期非特异性化疗药。一般蒽环类化疗药物应用于一线或二线标准化疗方案中,经静脉给药方式应用于临床。可用于治疗的癌症包括乳腺癌,白血病,淋巴瘤,肺癌,肝癌,肾癌等。这类药物的主要副作用是心脏毒性,这极大程度地限制了它们的进一步使用。其他副作用包括骨髓抑制、呕吐、脱发等。
乙醛脱氢酶2(ALDH2)是NAD+依赖的乙醛脱氢酶超家族成员之一,目前已发现有19个乙醛脱氢酶家族成员,乙醛脱氢酶2是定位在线粒体内一种重要的醛类氧化酶,能将乙醇代谢中间产物乙醛氧化成乙酸。同时乙醛脱氢酶2可分解乙醛代谢产物4-羟壬烯醛,减轻乙醛及其代谢产物对细胞的氧化损伤。它是一个四聚体酶,大量表达在肝、肾和肺等组织,在线粒体功能活跃的心、脑组织等也有表达。近期有研究表明,该酶的激活对于心肌缺血再灌注导致的心肌细胞死亡有显著的保护作用。乙醛脱氢酶2基因具有多态性,其表达产物的487位氨基酸残基可以是谷氨酸(Glu487)或赖氨酸(Lys487),因此该基因可以有Glu487纯合子(乙醛脱氢酶2*1/1)、Glu487/Lys487杂合子(乙醛脱氢酶2*1/2)和Lys504纯合子(乙醛脱氢酶2*2/2)等几种等位基因。其中,乙醛脱氢酶2*1/2活性为正常的10%~45%,乙醛脱氢酶2*2/2活性为正常的1%~5%。东方亚洲人群中该基因突变率为40%以上。目前关于乙醛脱氢酶2与癌症关系方面的研究很少,基本都集中在统计学方面,有多项统计数据显示,携带ALDH2突变基因型者大量饮酒将显著增加患多种癌的危险性,包括肝癌,食道癌等。
发明内容
本发明的第一个目的是提供乙醛脱氢酶2和VHL基因作为蒽环类化疗药物处理肿瘤细胞时药物靶标的应用。
本发明的第二个目的是提供一种蒽环类化疗药物***的药物靶标。
本发明的第三个目的是提供蒽环类化疗药物的新的应用。
本发明的第四个目的是提供蒽环类化疗药物与乙醛脱氢酶2抑制剂联合使用在制备***药物中的应用。
通过药物筛选我们发现蒽环类化疗药可以特异性的杀死VHL缺失或突变的肾透明细胞癌细胞,对正常细胞毒性甚小,为肾癌的化疗治疗提供了一个有效选择。通过进一步的研究我们发现乙醛脱氢酶2介导了这一过程,从而发现了蒽环类化疗药物处理肿瘤细胞时药物靶标。进一步研究显示,蒽环类化疗药与乙醛脱氢酶2的抑制剂合用明显增加了对肿瘤细胞的杀伤作用,为化疗的联合用药提供了一种思路及方案。
为了解决上述第一个发明目的,本发明提供了乙醛脱氢酶2作为蒽环类化疗药物处理肿瘤细胞时药物靶标的应用。
作为一个优选,所述肿瘤是乙醛脱氢酶2表达缺失或突变的肿瘤。
本发明同时提供了VHL基因作为蒽环类化疗药物处理肾透明细胞癌药物靶标的应用。

为了实现本发明第四个发明目的,本发明提供了蒽环类化疗药物与乙醛脱氢酶2抑制剂联合使用在制备***药物中的应用。
作为一个优选,所述肿瘤是乙醛脱氢酶2表达的组织产生的肿瘤,包括肝癌、肺癌等(对于ALDH2基因突变或者缺失表达的患者,不必联合使用乙醛脱氢酶2抑制剂)。
肾透明细胞癌是典型的VHL基因缺失或者突变导致乙醛脱氢酶2表达缺失的肿瘤。
本发明的优点在于,化疗药物作为一种常用的抗肿瘤手段,有着毒副作用较大的缺点,我们发现作为一种常用化疗药物之一的蒽环类化疗药,包括柔红霉素,阿霉素,表阿霉素等,可以特异性的杀死抑癌基因VHL缺失的肾透明细胞癌细胞,为肾癌的化疗治疗提供了一个有效选择,克服了目前临床中肾癌基本只依赖手术治疗的缺陷。通过进一步的研究我们发现乙醛脱氢酶2介导了蒽环类化疗药对肿瘤细胞的敏感性,提示乙醛脱氢酶2作为蒽环类化疗药物处理肿瘤细胞时药物靶标的应用,又由于40%的亚洲人群中都存在着乙醛脱氢酶2的缺失或突变,故使得本发明拥有更广泛的应用前景,而对于乙醛脱氢酶2不发生缺失或者突变的患者,可以通过蒽环类化疗药与乙醛脱氢酶2的抑制剂联合使用,明显增加对肿瘤细胞的杀伤作用。

图3中,图3A显示用SILAC方法鉴定受VHL调控不受HIF1-α调控的蛋白的策略图以及质谱谱图。图3B显示用线虫进行药物筛选目的蛋白的流程图。
图4中,图4A显示在tm3232/TM3236品系的线虫中沉默掉VHL的表达之后,通过实时定量PCR检测VHL的mRNA表达情况,图中所示VHL即为沉默VHL的线虫。图4B显示tm3232/TM3236线虫用阿霉素处理后死虫和活虫的形态图。图4C显示用20ug/ul阿霉素处理线虫不用的时间,通过观察细胞形态,统计线虫死亡率。图4D显示在tm3232/TM3236品系的线虫中沉默掉ALH1(线虫的ALDH2基因)的表达之后,通过实时定量PCR检测ALH1的mRNA表达情况,图中所示ALH1即为沉默ALH1的线虫。图4E显示用20ug/ul阿霉素处理线虫不用的时间,通过观察细胞形态,统计线虫死亡率。图4F显示在沉默掉VHL表达的线虫中通过实时定量PCR检测ALH1的mRNA表达情况。

具体实施方式
下面结合具体实施例,进一步阐述本发明。下述实施例中所使用的实验方法如无特殊说明,均为常规方法。下述实施例中所用的材料、试剂等,如无特殊说明,均可从商业途径得到。应理解,这些实施例仅用于说明本发明而不用于限制本发明的范围。下列实施例中未注明具体条件的实验方法,通常按照常规条件,例如Sambrook等人,分子克隆:实验室手册(NewYork:ColdSpringHarborLaboratoryPress,1989)中所述的条件,或按照制造厂商所建议的条件。
实施例1:蒽环类化疗药物特异性的杀死VHL缺失的肾透明细胞癌细胞系。
我们以缺失表达VHL的肾透明细胞癌细胞系RCC4细胞为基础,转染VHL构建了表达VHL的细胞系RCC4-VHL,以及转染空载的RCC4细胞,如图1所示,RCC4细胞在常氧(N)和低氧(H)处理条件下,由于VHL缺失,HIF1-持续表达;而RCC4-VHL细胞由于VHL的表达,HIF1-仅在低氧条件下有累积。以RCC4和RCC4-VHL细胞为模型,我们对临床上常用的化疗药以及一些潜在的抗肿瘤药用CCK-8检测细胞生长抑制率。我们发现:蒽环类化疗药(红框内)特异性的抑制了RCC4的生长,对RCC4-VHL却影响很小(图2A)。我们又进一步用不同浓度和时间去验证,发现都有此差异(图2B)。再进一步的,用流式细胞仪检测细胞的凋亡情况,发现用蒽环类化合物DOX处理后,Annexinv阳性的细胞在RCC4中明显多于RCC4-VHL(图2C)。为了排除RCC4细胞系的独特性的因素,我们又选取了另外两种肾透明细胞癌细胞系,包括缺失表达VHL的786-0细胞和正常表达VHL的CAKI-1细胞,分别在两个细胞中过表达和敲除VHL,检测其对DOX的敏感性差异,发现与RCC4细胞一致,表达VHL的细786-0VHL细胞和CAKI-1与其对应缺失或者低表达VHL的细胞相比,对DOX诱导的细胞死亡都不敏感(图2D,E)。VHL作为泛素连接酶,调控靶蛋白的泛素化修饰和蛋白降解,其最重要的底物是HIF-1。因此随后我们又检测了HIF-1是影响DOX诱导的缺失VHL的肾癌细胞死亡。结果发现,在沉默HIF-1表达之后并没有影响细胞对DOX处理的敏感性(图2F),在用低氧模拟物CoCL2处理细胞后,细胞的生长抑制率也没有影响(图2G),提示HIF-1并没有在DOX引起的细胞死亡中发挥作用。
实施例2:通过蛋白质组学以及大规模筛选方法发现VHL可能通过调控乙醛脱氢酶2导致了细胞对DOX敏感性的差异。
实施例1提示影响蒽环类化合物在肾癌细胞敏感性的基因是HIF-1非依赖的,因此我们用常氧和低氧分别处理RCC4和RCC4-VHL细胞,并用基于SILAC的定量蛋白质组学的方法寻找只受VHL调控不受HIF-1调控的蛋白(图3A),共129个。
为了进一步找到其中那些影响阿霉素敏感性的蛋白,我们建立一个线虫药物筛选模型,筛选流程如图3B,首先我们验证我们的筛选模型是否可用,我们在线虫中敲除VHL,用DOX处理后线虫的死亡数量明显多于对照组(图4A,图4C),证明了我们建立的***适合于药物筛选,然后我们找到线虫中对应人类的VHL调控的蛋白,从线虫文库中找到对应包含敲除质粒的菌进行筛选,通过筛选我们发现线虫中敲除乙醛脱氢酶2(对应线虫中ALH1)之后,对阿霉素的敏感性明显增强(图4D,图4E)。进一步我们去探讨在VHL敲除的线虫对阿霉素的敏感性是否由乙醛脱氢酶2介导,于是我们检测了VHL敲除后乙醛脱氢酶2的表达,发现下调(图4F),提示VHL很有可能通过调控乙醛脱氢酶2影响到对阿霉素的敏感性。
实施例3:VHL可以转录调控乙醛脱氢酶2。
如图3所示,我们发现乙醛脱氢酶2(乙醛脱氢酶2)受到VHL显著地调控,而HIF-1不影响其表达。同时,我们对蛋白质组学的结果进行了验证,分别把RCC4和RCC4-VHL细胞放在常氧和低氧下处理,WB检测乙醛脱氢酶2的蛋白(图5A)和mRNA表达情况(图5B),乙醛脱氢酶2的蛋白和mRNA水平只受VHL的调控,并不受HIF-1和低氧调控。进一步,我们在RCC4-VHL细胞中敲除VHL(图5C),在786-0细胞中过表达VHL(图5D),在CAKI-1细胞中敲除VHL(图5E),发现VHL始终从转录水平正调控乙醛脱氢酶2的mRNA。另外,我们想知道这种调控是否是肾透明细胞癌特有的,于是选取了一株肝癌细胞系SMMC7721,敲除VHL之后(图5F),乙醛脱氢酶2的mRNA和蛋白水平也明显下调,证明了VHL对乙醛脱氢酶2的调控具有普遍性。
实施例4:乙醛脱氢酶2的低表达或者活性抑制可以使肿瘤细胞对蒽环类化疗药更敏感。
我们在RCC4-VHL细胞中敲除乙醛脱氢酶2,用不同浓度DOX处理细胞一天和两天,检测细胞生长抑制率(图6A),发现随着乙醛脱氢酶2的敲除,细胞的生长抑制率显著升高,Annexin5阳性细胞明显增多(图6B)。随后我们的RCC4细胞中过表达乙醛脱氢酶2,发现过表达乙醛脱氢酶2之后,细胞的生长抑制率随之降低(图6C)。接着我们有选取一株肝脏细胞系7701,敲除乙醛脱氢酶2后,检测细胞对DOX的敏感性(图6D),发现敲除乙醛脱氢酶2后细胞生长抑制率明显增高。为了进一步验证我们的结果,我们在乙醛脱氢酶2敲除的孕鼠中取出MEF细胞A-/-,于对照组一起分别用阿霉素处理,两天后检测细胞的生长抑制率以及凋亡情况(图6E,图6F),发现乙醛脱氢酶2敲除组的细胞生长抑制率明显高于对照组,凋亡细胞也明显多于对照组。在验证了ALDH2的表达量对阿霉素处理的细胞有影响之后,我们继续探讨了乙醛脱氢酶2的活性对阿霉素处理的细胞是否有影响,我们分别用乙醛脱氢酶2的活化剂alda-1和抑制剂daidzin与阿霉素同时处理RCC4-VHL细胞(图6G,图6H),发现alda-1可以降低RCC4-VHL细胞对阿霉素的敏感性,而daidzin却增强了这种敏感性,提示乙醛脱氢酶的活性越高,细胞对阿霉素越不敏感,反之亦然。
另外的蒽环类化合物也有类似效果,我们用另外一种蒽环类化疗药柔红霉素处理过表达乙醛脱氢酶2的RCC4细胞以后,与对照组相比,过表达乙醛脱氢酶2的RCC4细胞生长抑制率明显降低(图6C),表现出了很明显的耐药性。说明了这一类化合物都有类似效应。
以上所述仅是本发明的优选实施方式,应当指出,对于本技术领域的普通技术人员,在不脱离本发明原理的前提下,还可以做出若干改进和润饰,这些改进和润饰也应视为本发明的保护范围。
Claims (4)
1.蒽环类化疗药物在制备治疗乙醛脱氢酶2缺失或者突变的肿瘤药物中的应用。
2.蒽环类化疗药物在制备治疗VHL基因缺失或者突变的肾透明细胞癌药物中的应用。
3.蒽环类化疗药物与乙醛脱氢酶2抑制剂联合使用在制备***药物中的应用。
4.根据权利要求3所述的蒽环类化疗药物与乙醛脱氢酶2抑制剂联合使用在制备***药物中的应用,其特征在于,所述肿瘤是乙醛脱氢酶2表达的组织产生的肿瘤。
