Abstract

一种新型肩锁关节脱位固定板,属于骨科医疗器械技术领域,用于肩关节脱位的复位与固定手术。它由固定板部分和钩部分两部分组成,钢板固定板部分在远端即肩锁关节端有用于缝合肩锁韧带的3个专用小孔。固定板的主体成解剖型,和锁骨外侧端的解剖匹配;固定板的近端端有凹槽,有利于复位时顶棒顶压,使复位更简单。固定板的头端可以***肩锁关节后侧的肩峰下,使用时将固定板放在锁骨外侧端,将头端的钩插到肩峰下。该固定板具有便于修复肩锁韧带的优点,在肩锁韧带撕脱时,可以将韧带通过钢板上远端的3个小缝合孔缝合固定于锁骨远侧端;并且固定板头端不直接压迫韧带在锁骨远端的附着处,有利于韧带修复。该固定板具有使用方便、固定强度合适、有利于韧带修复、对肩锁关节囊及周围韧带损伤小的优点。

Description

肩锁关节脱位固定板
技术领域
本发明涉及一种治疗肩锁关节脱位的固定装置,属于骨科医疗器械技术领域。 
背景技术
在骨科医疗手术中,肩锁关节脱位是肩部损伤中的一种常见损伤。肩锁关节脱位多为直接暴力引起,如在交通事故中肩部直接撞击致伤。肩锁关节脱位时主要是周围的韧带损伤,其中主要是肩锁韧带。治疗肩锁关节脱位的方法有很多种,传统的如克氏针张力带固定、钢丝固定、普通锁骨钩固定板固定等。现有文献表明使用锁骨钩固定板是一种有效的治疗方式,但是由于固定板设计的原因,在肩锁关节脱位时,复位常常有困难;在上固定板的时候不利于肩锁韧带修复,在取固定板的时候又容易损伤肩锁韧带,固定板过于靠近锁骨远端时,会压迫肩锁韧带锁骨远端的附着处,如果远离韧带附着点时则很可能造成脱钩,内固定失败。为此,需要发明一种新的用于肩锁关节脱位的固定板,以便在治疗肩锁关节脱位时达到更好的治疗效果,防止复发,同时减少患者的痛苦,减轻患者负担。 
发明内容
本发明所要解决的技术问题是提供一种在进行肩锁关节复位和固定手术时能够便于复位、有很好的固定强度、对周围的组织损伤小、有利于肩锁韧带修复、减少肩峰下切割、取出时对肩锁关节囊及周围韧带损伤小的肩锁关节脱位固定固定板。 
解决上述技术问题的技术方案是: 
一种肩锁关节脱位固定板,它由固定板和钩两部分组成。固定板分左侧和右侧两种,设计原理一样,只是为适应左右侧的解剖结构。在固定板上沿板面纵轴方向分布有3个楼顶孔,都为锁定加压孔,其中第一个孔可以打入两枚钉,一枚锁定钉和一枚普通钉;另外两个钉皆为锁定加压孔,可以上普通钉或锁定钉。侧位观,固定板有向上的轻度弧度以符合锁骨远端的解剖形态。固定板近端有一个凹槽,为方便顶压和钳夹复位准备。固定板的头端有3个可以供缝合韧带的缝合孔。钩的起始端较长,独特的设计时固定板使用时不覆盖肩锁韧带附着点。 
采用这种结构的肩锁关节脱位固定板,使用时固定板不遮盖肩锁韧带间隙和韧带附着点处,为韧带和关节囊的修复提供空间。固定板近端凹槽为复位是钉棒着力点,通过使板贴服锁骨外侧端骨面即可以复位肩锁关节脱位。具有使用方便、固定强度大、 有利于韧带修复术、对肩锁韧带及肩锁关节囊周围软组织损伤小的特点,避免肩峰撞击综合症、患者术后恢复时间短,并发症少,减少脱位复发,避免关节融合术。减轻患者及社会的医疗负担。 
使用说明 
显露脱位的肩锁关节以后,将固定板的头端***肩锁关节后侧的肩峰下,顶棒顶住钢板近端的凹槽使肩锁关节脱位复位,可以通过钢板上第一个孔先在近端上一枚普通皮质骨钉使钢板进一步复位并维持,明确复位满意后再予螺钉加强固定。显露撕脱的肩锁韧带,如果韧带在中间断裂,则直接缝合;如果韧带在锁骨侧撕脱,则将韧带通过钢板上远端的3个小缝合孔缝合固定于锁骨远侧端;并且固定板头端不直接压迫韧带在锁骨远端的附着处,有利于韧带修复。该固定板具有使用方便、固定强度合适、有利于韧带修复、对肩锁关节囊及周围韧带损伤小的优点。

Claims (7)

1.一种肩锁关节脱位固定板,它由固定板和钩两部分组成。固定板分左侧和右侧两种,设计原理一样,只是为适应左右侧的解剖结构。在固定板上沿板面纵轴方向分布有3个楼顶孔,都为锁定加压孔,其中第一个孔可以打入两枚钉,一枚锁定钉和一枚普通钉;另外两个钉皆为锁定加压孔,可以上普通钉或锁定钉。侧位观,固定板有向上的轻度弧度以符合锁骨远端的解剖形态。固定板近端有一个凹槽,为方便顶压和钳夹复位准备。固定板的头端有3个可以供缝合韧带的缝合孔。钩的起始端较长,独特的设计时固定板使用时不覆盖肩锁韧带附着点。

3.根据权利要求1所述的一种肩锁关节脱位固定板,其特征在于:固定板有3个楼顶孔,都为锁定加压孔,其中第一个孔可以打入两枚钉,一枚锁定钉和一枚普通钉;另外两个钉皆为锁定加压孔,可以上普通钉或锁定钉。
4.根据权利要求1所述的一种肩锁关节脱位固定板,其特征在于:侧位观,固定板有向上的轻度弧度以符合锁骨远端的解剖形态。
5.根据权利要求1所述的一种肩锁关节脱位固定板,其特征在于:固定板近端有一个凹槽,为方便顶压和钳夹复位准备。
6.根据权利要求1所述的一种肩锁关节脱位固定板,其特征在于:固定板的头端有3个可以供缝合韧带的缝合孔。
7.根据权利要求1所述的一种肩锁关节脱位固定板,其特征在于:钩的起始端较长,独特的设计时固定板使用时不覆盖肩锁韧带附着点。
