Abstract
一医疗装置(10)包括一具有多个节段(30)相互铰结地连接以至呈现一用于传送的伸直状态以及一屈曲或大致弯曲的配置状态的植入体。植入体可以具有数个啮合元件位于至少两个不同的节段上,例如,在大多数的节段上。一锚定在植入体末梢节段的细长锁定元件可以具有数个凸出物用于至少每个啮合元件上。施加在所述锁定元件上的张力使植入体从伸直状态偏向屈曲配置状态。当所述锁定元件挠曲到弯曲配置状态时,通过所述锁定元件的数个凸出物与所述植入体的数个啮合元件的匹配,所述植入体的数个屈曲节段是被锁定。形成一非常牢固的锁,以防止在配置后被打开。
Description
技术领域和背景技术
本发明要求申请人Siegal于2010年2月16日提交申请号61/304,857的美国临时专利申请的优先权。
本发明涉及数个植入体,像是用于椎骨手术,尤其特别涉及一具有非常稳固的锁定设备以防止所述植入体一旦被植入受验者体内后打开。
已有许多装置被提出来用在所述脊椎或其他部位内的植入物及其配置。一植入物的重要特点是它不仅要能有效率地传送,而且在配置之后要维持位置且在体内不打开。所述植入物的稳固、牢固以及可靠的定位是极其重要的。若所述植入物在配置之后打开,所述植入物可能会损伤周围组织,且毁掉所述脊椎手术的效果。
申请人于2009年2月12日公布的PCT专利公开号WO2009/019669教示了一植入物包括一植入体和一细长(elongated)的紧固元件,其以一伸直构造引入体内,且之后在体内呈现一大致弯曲的构造。前述公开全体通过引用并入此处。
因此,提供一种用于如脊椎手术的配置后不会在体内打开的植入物(和相关的方法),是极具有优势的。
发明内容
根据本发明的一特征,是提供一医疗装置包括(a)一具有多个节段相互连接以至呈现一伸直状态以及一屈曲(flexed)的配置状态的植入体,所述植入体形成至少两个啮合元件,至少两个不同的节段的每个节段具有一啮合元件;以及(b)一细长的锁定元件锚定在所述植入体的一节段上,所述锁定元件具有至少两个与所述至少两个啮合元件相应的凸出物,其中所述植入体和所述锁定元件用在锁定配置,当所述锁定元件挠曲(deflected)至所述屈曲的配置状态时,所述锁定元件的至少两个凸出物啮合且锁定于所述植入体的至少两个啮合元件,在连续的节段屈曲和锁定后,每个锁定的节段维持锁住状,所述锁定配置将相对于植入体的所述锁定元件有效的锁定,进而将植入物保持在所述屈曲的配置状态。
本发明的进一步特征是一医疗装置包括(a)一具有多个节段相互连接以至呈现一伸直状态以及一屈曲的配置状态的植入体,所述植入体具有至少两个槽(socket),至少两个不同的节段的每个节段具有一槽;以及(b)一细长的锁定元件锚定在所述植入体的一节段上,所述锁定元件具有至少两个与所述至少两个槽相对应的凸起物(bulges),其中配置所述植入体和所述锁定元件有锁定的功能,这样当所述锁定元件挠曲至所述屈曲的配置状态时,所述锁定元件的至少两个凸起物与所述植入体的至少两个槽相啮合且锁定,所述锁定配置将相应的具有多个节段的植入体的所述锁定元件有效的锁定,进而将植入物保持在所述屈曲的配置状态。
本发明的更进一步的特征是一种配置一医疗装置在主体内的方法,包括(a)提供一具有多个节段相互连接以呈现一伸直状态以及可挠曲以呈现屈曲的配置状态的植入体,多个所述节段的每个节段具有一啮合元件;(b)提供一锁定元件锚定在至少两个不同的节段上的其中一个节段上,且锁定元件沿着所述植入体延伸在所述植入体的拉长方向上,所述锁定元件具有与每一个啮合元件相应的凸出物;以及(c)使所述植入体进入主体内,并且施加向后张力在所述锁定元件上,以至使所述植入体挠曲至所述屈曲的配置状态,其中所述凸出物与所述啮合元件相应配合,以至使相关的节段锁在所述屈曲的配置状态中。

具体实施方式
本发明是一种可使用在人体内的植入物,例如在不同类型的脊椎手术中使用。所述植入物具有一稳固且牢固的锁定配置。本发明还包括一相应的方法用于通过一传送导管(conduit)用于配置所述植入物。所述植入物可以包括一植入体和一锁定元件。所述锁定元件可以包括在所述锁定元件上的凸出物,该凸出物沿着所述植入体的弯曲部与所述植入体的节段上的啮合元件相配合。所述凸出物可以是凸起物(bulges)、齿状物(teeth)、弹片(elastic tabs)等。所述啮合元件可以是台阶、插槽、矩形开口等。当每一个节段屈曲时,所述凸出物正好落在所述啮合元件上,而且已放置在体内的所述植入物不能打开。
与先前技术相比,先前技术所述植入物的锁定配置仅涉及锁住所述植入体中一个节段,本发明所述植入物可以涉及锁住所述植入物的已屈曲的所有节段,或在其他实施例中锁住所述植入物的已屈曲的至少两个、或至少三个、或至少三分之一、或至少四分之一、或至少一主要的节段。与先前技术进一步相比,先前技术所述植入体并不具有一牢固的锁定配置,本发明所述植入物可以以一足够牢靠且安全的方式锁住,当所述植入体在体内,一旦以屈曲的配置状态被锁住时,所述植入物不能被打开。与先前技术进一步相比,先前技术所述植入物可锁住单个能复原的齿状物,本发明所述植入体可以锁住在所述植入体的节段上的凸出物,与在所述锁定元件上的互补凹槽相配合。与先前技术相比,所述锁定配置可以包括多个节段上的多个凸起物与在至少两个节段上的多个插槽或凹槽的配合,以及在所有节段上的其他实施例。与先前技术进一步相比,所述凸起物至凸起物间的长度可以小于从一节段至其相邻的一节段间的所述屈曲区域至弯曲区域间的长度。这样一来,所述锁定元件的凸起物不可能在插槽中处于伸直状态,一旦所述节段屈曲进入所述屈曲的配置状态时,所述凸起物可以锁在插槽中。这使所述锁定配置额外的牢固。
根据本发明所述具有锁定机构的医疗装置和方法的原理和操作将参考附图和相应的描述来得以更好的阐明。
现转到附图,图1-5显示了一实施例的一医疗装置,所述医疗装置10包括一植入体20,植入体20具有多个在屈曲区域35相互连接的节段,例如是铰结地相互连接,以至呈现一伸直状态和一屈曲的配置状态。所述屈曲区域35可以包括一传统的铰链35、一体形成的铰链或另一种可提供需要的节段间弯曲的结构。作为所述锁定配置或锁定机构的部分,植入体20形成带有至少两个啮合元件40,至少两个不同的节段中每一个节段,例如,节段30A、节段30B均具有一啮合元件40。优选地,至少三个节段30具有一啮合元件40,且锁住一凸出物60。在其他优选实施例中,至少三分之一的节段30具有一啮合元件40,且锁住一凸出物60。在一更优选的实施例中,每连续四个的节段30中至少一个节段30具有一啮合元件40,且锁住所述细长元件50的凸出物60。在其他实施例中,半数以上的所述节段30具有一啮合元件。在一优选实施例中,所述节段30中除了一末梢节段30的每一节段可以具有一啮合元件40(这应该是可理解的,如同不能绝对排除所述末梢节段也具有一啮合元件)。
医疗装置10可以是一种植入物10,也可以具有一细长的锁定元件50,该锁定元件可锚定在所述植入体的一末梢节段上。在所述植入体的“一末梢节段”应该理解为能够屈曲的最末梢节段。锁定元件50可以具有与至少两个啮合元件40相应的至少两个凸出物60。优选地,锁定元件50可以具有至少一凸出物60,该凸出物与植入体20的每一啮合元件40中主要的啮合元件40相对应且啮合,以实行所述锁定配置。
通过植入体20和锁定元件50的配置,以施加张力在紧固元件50上,使紧固元件50趋向挠曲所述植入体20从伸直状态至屈曲的配置状态。细长的锁定元件50可以沿着一槽道穿过,该槽道沿着植入体20延伸。
通过植入体20和细长锁定元件50的配置作稳固及牢靠的锁定,来防止已经以屈曲状态定位在脊椎或者体内其他部位的植入物10打开。这种锁定可以是当锁定元件50挠曲到屈曲的配置状态(作为植入体20的一部分),植入体20的每一连续节段30可以屈曲,并且细长的锁定元件50中至少两个凸出物可以与植入体20的至少两个啮合元件40相啮合且锁住。在一个或多个连续节段屈曲且锁住之后,每个锁定的节段30可以保持锁住状。相应地,所述锁定配置有效地锁住相对于植入体20的锁定元件50,因此保持植入物10在体内处于所述的屈曲的配置状态。
例如,所述植入体中每至少两个、或每至少三个、或每至少三分之一、或每至少四分之一、或每至少一主要的节段、或至少每连续四个出一个的节段可以形成带有至少一个啮合元件,以至锁住所述锁定元件。进一步,所述锁定元件可以具有一凸出物,该凸出物与所述植入体的每一啮合元件相对应且啮合,以提供所述的锁定配置。另一种情况,锁定元件50可以具有多个多样化的凸出物60但少于一个凸出物用于植入体20的每个啮合元件40。在一些优选实施例中,所述锁定元件50可以比所述植入体的啮合元件多得多的凸出物。例如,每四分之一的凸出物60可以锁住一啮合元件。在一些情况下,甚至,每个节段30可以具有多个啮合元件40。
所述凸出物60和啮合元件40可以包括不同的类型,类型包括截面类型。总之,植入体20的啮合元件40广泛地定义为包括一台阶、一槽或其他类型可以承受并“夹住”锁定元件50的凸出物60(例如,一凸起物,齿状物或一弹片)。在图1-5所示的实施例中,啮合元件40可以描述为台阶40,该台阶可以夹住来自锁定元件50的齿状凸出物60。根据材料的机械特性选择凸出物的结构和尺寸,以提供足够的回复力和安全锁功能,并同时考虑生物相容性。所述相应的啮合元件可以作为任何适合位置的元件或凹槽来实施,例如一向上的凸出物或一凹陷的V型切口,或者两者的结合。
在图6-8中,所述啮合元件可以描述为插槽40,并且所述凸出物可以描述为凸起物60。术语“凸起物”不限于一圆形(rounded)凸起物,甚至在一些实施例中,例如像图6-8所示,凸起物60可以是球形。类似的,术语“插槽”也非必要指一圆形的插槽,尽管在图6-8中的插槽是圆形。进一步,可考虑凸出物60,包括凸起物60,可以是实心或中空。
在图9-12所示的实施例中,所述凸出物可以描述为弹片60,且啮合元件40可以为互补的凹槽。例如,每个节段30可以具有至少一互补的凹槽,且当每个节段以屈曲配置状态屈曲时,紧固元件50的至少一弹片可以锁住每个节段30的至少一互补的凹槽40。
在图9-12中,植入体200可以具有节段220和一拉链状的锁定元件210,锁定元件可以坚固地附着在一末梢节段220上,且锁定元件与所述植入物200的所有节段啮合。以一相对植入体200的向后方向拉着锁定元件210的最接近端。植入体200可以具有凹槽222或槽。锁定元件210可以是条状,可以使用金属材料(例如,镍钛合金,不锈钢)或塑料(例如,聚醚醚酮、超高分子量聚乙烯)来实施,以及具有一系列的弹片212,弹片锁在植入体200的相应啮合元件222上。
图11-12显示了实际的锁定机构在两个相邻的节段220之间。图11显示植入体200的两个节段220处于伸直状态,其中一V型切口或开口(在本实施例中是V型的)呈现在两个连续节段220之间。一旦相对于节段220以向后方向(根据图11中R箭头方向一致)拉着锁定元件210,节段220会转动是由于节段220的一轴与所述紧固元件20穿过的一槽道之间发生挠曲。作为转动的结果,节段220彼此之间紧密靠近,如图12所示。在图12中也显示了所述弹片212被锁在槽222中,于是,节段220彼此附着且牢固锁住。
在一些实施例中,隔离锁定元件50(或者210)的一张力元件可以用于以向后的方向拉所述植入体,且促使所述植入体的节段30屈曲。在那种情况下,所述张力元件将会第一个引起所述医疗装置10屈曲。接着,可以拉着锁定元件50并通过与啮合元件40相配合的凸出物60以锁住所述医疗装置。
在上述实施例中本发明所述锁定机构可以进一步通过使用多个锁定元件(50或210)来加强。在这种情况下,植入体(200或20)且在一节段内的多个啮合元件可以与植入体的每一节段(220或30)上的多个啮合元件(220或40)相啮合且锁定。在一例子中,所述多个锁定元件可以是并列(side by side)条状材料。
本发明所述医疗装置10可以进一步包括一槽道,槽道的大小被作成用以***述医疗装置,例如当所述医疗装置10处于伸直的形状,且以维持所述植入体20处于伸直的状态。所述槽道可以是伸直的形状,且可以紧贴植入体20的外部。尽管槽道没有在图1-3中所显示,但是一槽道应用于本发明的一植入体10的例子已显示在2006年7月13日公布的公开号WO2006/072941的图2-3中。通过植入体20和锁定元件50的配置来锁定,以至当所述锁定元件挠曲至所述屈曲配置状态时,每个连续节段屈曲,以及所述锁定元件的凸出物啮合且锁定于所述植入体的啮合元件。
在图6中,当植入体处于伸直的状态,锁定元件50的凸出物60,在图6中为凸起物60,可以不必适用于或者啮合于,且事实上是不适用于或啮合于啮合元件40,在图6中,啮合元件可以描述为植入体20的插槽40。在图7中,仅这些已经屈曲(30A、30B)的节段30可以具有它们的啮合元件或被凸起物60所占用的插槽40。进一步,在图8中,较容易看出凸起物60是在插槽40内,用于屈曲的节段30。仔细查看图6~图8及特别是图8,其揭示在所述植入物10中一凸起物60A至最近的凸起物60B之间的距离可以小于一节段30A的屈曲部分35A至另一相邻的节段30B的屈曲部分35B之间的距离。需注意的是在图6至图8所示的实施例中,从一凸起物60至另一凸起物60(例如,至相邻的凸出物60)之间的距离大体上是一致的,且同样地,从一插槽至另一插槽(例如,至相邻的插槽40)大体上是一致的。然而,在一需要医疗装置是螺旋或非平面或非均匀弯曲的实施例中,这些距离可以是不统一的,尽管在弯曲的节段上,插槽至插槽之间的距离仍然可以与凸起物至凸起物之间的距离相匹配。
注意到,当连续的节段30A、30B、30C和30D屈曲,紧固元件50的大致弓状造型可以跨越一个比植入体20的若干铰链35构成的大致弓状造型更小的弧形。这是因为在一弯曲配置状态中,植入物10的紧固元件50为大部分植入体20的内部,且特别是若干铰链35(hinges)的内部。在这方面,“内部”是描述成极为接近在弯曲配置状态下由植入物10(锁定组件50和植入体20)的弓状造型构成的弧形/环形中心。于是,处于所述伸直状态时,所述锁定元件的至少一些所述凸起物可以位于所述插槽外侧的部分铰链内,以及处于所述屈曲的配置状态的所述锁定元件的凸出物位于屈曲节段的插槽内。每一个已屈曲的节段30具有一凸起物60,以啮合相应的插槽。从图8可以看出,在屈曲配置状态下,每一个具有一插槽的屈曲节段,也具有一凸起物应用于内。
尽管附图所示的实施例中所述植入物在一二维平面(plane)上屈曲,以至植入体10的拉长方向可以说是与该铰链共面,且垂直于所述铰链,本发明等同于应用在非平面或螺旋式的植入物10,例如,植入物的拉长方向从一屈曲的末梢节段运行至处于屈曲的配置状态的一最接近屈曲的节段,该拉长方向不垂直于所述屈曲的节段的所述铰链。植入物的螺旋式配置且不具有本发明所述锁定配置的例子,在2006年7月13日公布的PCT专利申请公开号WO2006/072941(查看第17页的图9A、9B以及图10)中已有描述,其出版物因而以其整体引入这里作为参考。该出版物也描述了节段30和植入物10的一般特征,尽管其不具有本发明所述牢固安全的锁定配置。
参见图13,本发明也可以描述为一方法100用于配置一植入物在一主体体内。方法100可以包括第一步骤:提供一植入体,包括多个节段铰结地相互连接以至呈现一伸直状态,以及可挠曲以呈现一屈曲的配置状态,多个所述节段各自具有一啮合元件。在进一步步骤120中,方法100可以涉及提供一锁定元件锚定在一末梢节段上,沿着所述植入体延伸在所述植入体的拉长方向上。所述锁定元件可以具有凸出物,与所述植入体的每个啮合元件相对应。在方法的步骤130中,植入体可以进入主体的体内。进一步,向后的张力可以施加在所述锁定元件上,以至所述植入体挠曲至所述屈曲的配置状态。于是,所述凸出物可以与相应的啮合元件相配,以至锁住呈屈曲配置状态的相应节段。方法100的一些变化(version)也可以包括从锁定元件的锚中释放所述锁定元件,并将所述锁定元件从所述医疗装置中退出的步骤。
从结构上看,在节段30之间的屈曲区域35可以在宽范围的方法来实施。更优选地,节段间的相互连接可以是通过注入过程中或成型过程中与节段30一体形成的整体铰链35来获得的相互铰接,或通过从材料的起始块切除凹槽的方式来获得。所述凹槽可以是V型切口,平行的边,或者任意其他适合的形式。更优选地,可采用V型插槽,以至所述植入物的弯曲的配置形式具有在节段之间的间距基本闭合。然而,可选的实施方式,例如当以节段附着的可分离结构(例如“脊梁(backbone)”),提供铰接的相互连接时,也落入本发明的保护范围。在后面的例子中,所述脊梁可以是从节段自身的不同的材料制成,根据有意应用来选择。用于脊梁的材料包括但不限于,金属材料,多种塑料,其他聚合物,以及织物。
所述屈曲的配置状态也指弯曲(curved)的配置形式。然而,“屈曲”在这里不必涉及节段的真实弯曲,尽管在一些实施例中可能有如此的弯曲。而且,所述弯曲的配置形式不应该理解为代表一完美的弯曲,特别是检查相对少数的节段。于是,所述屈曲的配置形式,其被称为植入物10的弯曲的配置形式是大致弓形的形式,且它可以典型地延展转动大约180度以形成看上去大体为“U型”的形式。这里采用的术语“U型”总体上是指任意形状,只要具有转动大致180度(例如,180度加或减20度)的中间部分,而无需特定说明两侧部分的具体的形状、几何学或者范围(附加地,需注意的是,字母“U”自身在许多字体中是不对称的)。
值得重视的是,上面所描述的内容仅仅是是用作示例,还有许多其他实施方式都是可能的,他们都落入后附的权利要求所定义的本发明的保护范围之内。
Claims (23)
1.一种医疗装置,包括:
(a)一具有多个节段的植入体,所述节段相互连接以至呈现一伸直的状态和一屈曲的配置状态,所述植入体形成具有至少两个啮合元件,至少两个不同的节段中每一节段具有一啮合元件;以及

2.如权利要求1所述的一种医疗装置,其特征在于,所述锁定元件锚定在所述植入体的末梢节段上,以及配置所述植入体和锁定元件,以至施加在所述锁定元件上的张力倾向从所述伸直的状态挠曲至所述屈曲的配置状态。
3.如权利要求1所述的一种医疗装置,其特征在于,所述植入体的至少三个节段形成带有至少一啮合元件和一紧固元件,所述紧固元件与所述植入体的至少一啮合元件相对应且啮合,以至提供所述的锁定配置。
4.如权利要求1所述的一种医疗装置,其特征在于,所述植入体的至少三分之一的节段形成带有至少一啮合元件和一紧固元件,所述紧固元件与所述植入体的至少一啮合元件相对应且啮合,以至提供所述的锁定配置。
5.如权利要求1所述的一种医疗装置,其特征在于,所述植入体的每四个连续节段中有一个节段具有一啮合元件,且锁定所述紧固元件的一相应的凸出物。
6.如权利要求1所述的一种医疗装置,其特征在于,所述植入体的所有节段除了末梢节段形成带有至少一啮合元件,以及所述紧固元件具有一凸出物,凸出物与所述植入物的至少两个啮合元件相对应且啮合,以提供所述的锁定配置。
7.如权利要求1所述的一种医疗装置,其特征在于,一主要的节段形成带有至少一啮合元件,以及所述紧固元件具有一凸出物,凸出物与所述至少一啮合元件相对应。
8.如权利要求1所述的一种医疗装置,其特征在于,所述凸出物为齿状物,以及啮合元件为台阶。
9.如权利要求1所述的一种医疗装置,其特征在于,所述凸出物为弹片,以及啮合元件为互补的凹槽。
10. 如权利要求9所述的一种医疗装置,其特征在于,每个节段具有至少一互补的凹槽和至少一弹片,当每个节段弯曲处于所述屈曲的配置状态时,所述至少一弹片锁定至少一互补的凹槽。
11. 如权利要求1所述的一种医疗装置,其特征在于,进一步包括一导管尺寸以承受所述植入物,并且维持所述植入物处于所述伸直的状态。
12. 一种医疗装置,包括:
(a)一具有多个节段的植入体,所述节段相互连接以至呈现一伸直的状态和一屈曲的配置状态,所述植入体形成具有至少两个槽,至少两个不同的节段中每一节段具有一槽;以及
(b)一细长的锁定元件锚定在所述植入体的一节段上,所述锁定元件具有至少两个凸起物与所述至少两个凸起物相对应,
其特征在于,配置所述植入物和所述锁定元件有锁定功能,以至当所述锁定元件挠曲至达到所述屈曲的配置状态,所述锁定元件的至少两个凸起物与所述植入物的至少两个槽相啮合且锁定,所述锁定配置有效地锁定所述锁定元件相对于有多个节段的植入物,于是维持所述植入物处于所述屈曲的配置状态。
13. 如权利要求12所述的一种医疗装置,其特征在于,所述锁定元件锚定在所述植入体的末梢节段上,以及配置所述植入体和锁定元件,以至施加在所述锁定元件上的张力倾向从所述伸直的状态挠曲至所述屈曲的配置状态。
14.如权利要求12所述的一种医疗装置,其特征在于,所述植入体的至少一半节段形成带有至少一槽和一紧固元件,所述紧固元件具有一凸起物,凸起物与所有所述的槽相对应,以提供所述锁定配置。
15.如权利要求12所述的一种医疗装置,其特征在于,从凸起物至最近相邻的凸起物的距离比从一节段的弯曲区域至相邻的另一节段的弯曲区域的距离短。
16.如权利要求12所述的一种医疗装置,其特征在于,从凸起物至凸起物间的距离大体一致,以及从槽至槽间的距离大体一致。
17.如权利要求12所述的一种医疗装置,其特征在于,处于伸直状态的所述紧固元件的至少一些凸出物位于部分屈曲区域内,屈曲区域在所述槽的外侧,以及处于屈曲的配置状态的所述锁定元件的凸起物位于屈曲的节段的槽内。
18.如权利要求12所述的一种医疗装置,其特征在于,处于屈曲的配置状态的每一个屈曲的节段具有一槽,还具有一凸起物位于槽内。
19.如权利要求12所述的一种医疗装置,其特征在于,所述医疗装置是螺旋式。
20.如权利要求12所述的一种医疗装置,其特征在于,进一步包括一导管,导管的尺寸以承受所述医疗装置,以及维持所述植入体处于所述的伸直状态。
21.如权利要求12所述的一种医疗装置,其特征在于,所述凸起物和槽是球形的。
22.一种用于配置在主体体内的所述医疗装置的方法,包括:
(a)提供一具有多个节段的植物体,相互连接以至呈现一伸直状态,以及挠曲呈现一屈曲的配置状态,多个所述节段的每一个节段具有一啮合元件;
(b)提供一锁定元件锚定在至少两个不同的节段的一节段上,以及沿着所述植入体延伸在所述植入体的拉长方向上,所述锁定元件具有凸出物与每个所述啮合元件相对应;以及
(c)使所述植入体进入所述主体的体内,并施加向后的张力在所述的锁定元件上,以至所述植入体挠曲至所述屈曲的配置状态,
其特征在于,所述凸出物与相应的所述啮合元件相配合,以至锁住处于所述屈曲的配置状态的相应的节段。
23.如权利要求22所述的方法,其特征在于,进一步包括从所述锁定元件的锚中释放所述锁定元件,以及所述锁定元件从所述医疗装置中退出。
