技术领域
本发明涉及与泵***一起使用的眼科手术盒。更特别地,本公开涉及具有回流囊的眼科手术盒。
背景技术
在这个部分里的陈述仅仅提供与本公开有关的背景资料并且可能不构成现有技术。
通常已知在眼科手术过程中与泵***一起使用的眼科手术盒。每个眼科手术盒一般包括用于保持在眼科手术过程中从手术位置取出的抽吸的流体和组织的容器。一般眼科手术盒还包括抽吸管,在眼科手术过程中其被附接到手术机头上。
通常抽吸线具有根据外科医生的要求朝向手术位置向后回流抽吸的组织和流体的能力。为实现回流,典型地每个抽吸管包括回流囊和垫块夹紧点。眼科手术盒被置于其内的泵***通常包括柱塞,用于抵靠着垫块夹紧点压下回流囊。当要求回流时,第一柱塞在流量夹紧点处压下抽吸管道,以使抽吸路径管道与容器隔开。然后,第二柱塞压下回流囊,使得抽吸管内的抽吸的流体和组织反向流动大致等于压缩的位移量。对于多个抽吸管,每个抽吸管包括回流囊和垫块夹紧点,因此需要泵***中具有用于每个抽吸管的至少两个柱塞。
如果眼科手术盒不包括使抽吸路径与容器隔开的第一柱塞,当外科医生要求回流时,可能会产生吸力,降低了抽吸管内回流的效用。另外,在短时间内反复要求回流,可能会将空气从容器带入抽吸管,由于空气的可压缩性,进一步降低了反复要求回流的效用。
因此,需要一种包括最少部件并且具有最有效回流性能的改进的眼科手术盒。

下面的描述本质上只是示例性的并且不意于限制本公开、应用或用法。
根据本公开的一个实施例,在图1中示意了眼科手术盒10。眼科手术盒10包括具有收集抽吸流体和/或组织的内部体积的刚性壁容器12以及连接到刚性壁容器12上的主总管14。总管14包括经由刚性壁容器12的入口18与内部体积流体连通的抽吸管16。总管包括两个抽吸线20、22。抽吸线20通过抽吸总管24连接到抽吸线22。抽吸总管24还被连接到抽吸管16,以便在眼科手术过程中抽吸流体和/或组织可以在第一方向上从手术位置经由抽吸管16流至容器12的内部体积。
总管14还包括与这两个抽吸线20、22流体流通的回流囊(reflux bulb)26。在本实施例中,回流囊26通过抽吸总管24连接至抽吸线20、22。如已经知晓的,通过将冲洗流体引入到抽吸总管24内也可以得到回流。如下面所解释的,回流囊26被包括,用以在抽吸线20、22的至少一个中在与第一方向相反的方向上产生流体移动,即回流。在眼科手术过程中,回流对外科医生清除沿从眼睛到包括眼科手术盒10的手术***的抽吸路径的堵塞是有用的,例如,被抽吸通过带有针的手术机头(surgical handpiece)而被俘获的组织块造成堵塞。
使用时,眼科手术盒10被包含在眼科手术控制台中。眼科手术盒10被配置成与包含在眼科手术控制台30中的泵(图中未示出)连接,如图2所示。在特殊实施例中,泵是文丘里泵。在其它实施例中,可以使用不同类型的泵,例如真空泵或其它适合的泵。使用时,抽吸线20被连接至第一手术机头,抽吸线22连接至第二手术机头。手术机头可以包括晶状体乳化机头、玻璃体切除装置,或意于使用抽吸的其它类型的眼科机头。
图3示出了根据本公开的总管14的俯视图,包括多个黑体“×”以表示可能的夹紧点(pinch point),从而控制流体在抽吸路径中的流动。总管14被配置成当抽吸管16在A处并且抽吸线20在“B”处通过包含在眼科手术控制台30中的柱塞(图中未示出)夹紧(pinch)时,压下回流囊26致使抽吸线22中的流体在朝向手术位置(图中未示出)的方向上流动。相反,当抽吸管16在“A”处并且抽吸线22在“C”处被夹紧时,压下回流囊26引起抽吸线20中的回流。用这种方式,在控制台30内,眼科手术盒10只用一个回流囊26可以为两个抽吸线20、22提供回流。还可以通过只在A处夹紧抽吸路径同时在线20和22内提供回流。
应了解,根据本公开,在其它实施例中可以只包括一个回流囊用以在两个以上的抽吸线中提供回流。根据眼科手术控制台和/或根据要执行的眼科手术过程的类型,可以包括两个以上的抽吸线。另外,如上所述,当不同的抽吸线正在回流时,在控制台中对于每个额外的抽吸线只需要一个柱塞来隔开所述额外的抽吸线。
总管14优选包括通过抽吸总管24连接至抽吸线20、22上的冲洗管34。如图3中所示,回流囊26被从冲洗管34偏置。使用时,冲洗管34连接至冲洗源,例如,平衡盐溶液(BSS)瓶。因此,当回流囊26被压下时,来自冲洗管34的流体液流可以帮助抽吸线20、22中的流体在朝向手术位置的方向上移动。冲洗流体的流入还可以加快回流囊26的至少部分再膨胀。以这种方式,回流囊26通常装填流体,而不管最后的回流是什么时候发生的。因此,当回流囊26在短时间内多次压下时,冲洗管34保证连贯的回流,同时阻止将空气引入抽吸线20、22的其中一个或两者内。冲洗管34可以还通过“D”处的夹紧点而与回流隔开。通常地,冲洗路径被与抽吸线20、22隔开,除非需要回流或排出。但是,在某些实施例方式中,当回流囊26在预定的时间周期内只压下一次时冲洗管34可以被隔开。
本公开的总管可以还被用于对在眼科手术过程中使用的一个或多个不同技术提供不同的回流压力。如上所述,回流可以用于清除堵塞。另外地或可替代地,低压力回流可以用于沿视网膜清除血液,这是更精致的技术。回流囊产生的回流压力可以以多种方式增大或减小。
允许用户增大或减小在抽吸线20和22的至少一个中产生的回流压力的第一示例是具有机械柱塞50,如图4总所示,其通过脚踏板(图中未示出)的运动以适当的方式进行控制。用户使脚踏板在直线方向上移动得越远,柱塞50在压下回流囊26的箭头52的方向上移动得越快。回流压力与柱塞50压下球部26的速度成正比。在本示例和下面的示例中,柱塞50被包含在控制台30内允许柱塞与球部26相互作用的位置,如图4所示。
改变回流压力的第二示例是通过使控制台30能够改变柱塞50的移动距离。在本第二示例中,用户可以在回流压力的多个设置中选择,例如输入屏(图中未示出)上的全、中和轻的回流压力设置。如果用户选择全,则与中和轻回流压力设置相比,柱塞50将在箭头52的方向上移动最大距离并且最大程度地压下球部26。将球部26最大程度地压下导致与其它设置相比更大的回流压力。
实现可变回流压力的第三示例是通过控制夹紧点B和C处的抽吸夹管阀(图中未示出)而实现。如果B和C处的阀被设置成使得线20和22都被打开,那么将实现最小回流压力,但是如果一个线20或22被夹紧封闭,那么在打开的线中实现最大回流压力。
实现不同水平的回流压力的第四示例是通过构造不同材料、大小和形状的回流囊26而实现。因此,例如,如果球部26被由顺从的材料制成,那么其可以更多次地压下,与此相比,如果球部26被由相对更刚性的材料制成,则其能够压下的次数少些。当然,本示例设想柱塞50施加相同大小的力到不同材料的球部上。使用不同材料、大小或形状的回流囊26的本示例在设计执行特殊手术的回流囊时可能是有益的。
冲洗管34被通过冲洗总管36连接至冲洗入口管38和冲洗出口管40。在眼科手术过程中冲洗入口管38被连接至BSS或其它冲洗源。在眼科手术过程中,冲洗出口管40将冲洗流体传输至眼外科器械,例如套管。因此,在眼科手术过程中,眼科手术盒10可以将冲洗流体提供至手术位置并且提供至抽吸路径,而不需要对每一个设置单独的冲洗源。但是,应了解,眼科手术盒可以连接至用于冲洗手术位置和抽吸路径中的每一个的专门的冲洗源。
总管14优选可拆除地连接至刚性壁容器12上。在眼科手术过程中,容器可以充满抽吸流体和组织,以便理想地在眼科手术过程中继续下面的操作之前倒空容器。当总管14是可拆除的时,如图1中所示,刚性壁容器12可以方便地从泵28上拆除,而不需要从抽吸线20、22和/或包括在总管14中的冲洗入口/出口管38、40上断开一个或多个外科手术机头和/或其它眼科手术器械的额外操作。
回流囊26优选由硅形成。根据用户的应用和要求可以包括不同类型的材料。适合包括在流量控制装置中的其它材料可以是PVC、聚亚安酯和/或其它适合的外科手术材料。
在本公开的本实施例中,总管14包括与抽吸管16相邻定位的垫块42。当柱塞(图中未示出)压下抽吸管16时,垫块42提供接触表面以阻止柱塞移动,因此导致抽吸管16压缩并且形成夹紧点A。同样地,总管14包括与冲洗管34相邻定位的垫块44以形成夹紧点D。虽然没有示出,但可以提供形成夹紧点B和C的类似垫块。

Claims (8)

1.一种用于在眼科手术过程中收集抽吸的流体和/或组织的眼科手术盒,所述眼科手术盒包括:
刚性壁容器,其具有用于收集抽吸的流体和/或组织的内部体积;和
抽吸总管,其被连接到所述刚性壁容器上,所述抽吸总管被连接至与所述内部体积流体流通的抽吸管、用于连接第一手术机头的第一抽吸线、用于连接第二手术机头的第二抽吸线以及单一回流囊。
2.根据权利要求1所述的眼科手术盒,其中,所述眼科手术盒被配置成当所述抽吸管以及第一和第二抽吸线的其中一个被夹紧夹紧时,压下所述回流囊在第一和第二抽吸线的另一个中产生回流。
3.根据权利要求1所述的眼科手术盒,其中,所述抽吸总管形成在被可松开地连接到所述刚性壁容器上的主总管上。
