Dispositif pour la perforation de puits horizontaux parallèles dans les dents antérieures
et procédé pour l'obtention de ce dispositif.
La présente invention concerne un dispositif pour la perforation de puits horizontaux parallèles dans les dents antérieures; elle concerne également un procédé pour l'obtention de ce dispositif.
La nécessité de percer de tels puits se présente dans l'édification de bridges de conten- tion genre Koehler. I1 est, dans ee eas, nécessaire qu'au moins une partie des puits aient leurs axes rigoureusement parallèles entre eux. I1 en résulte une difficulté d'ajustement que le présent dispositif prétend résoudre d'une manière extrêmement simple et sûre.
Il est caractérisé en ce qu'il comprend un organe solidaire d'une pièce à main et une pièce moulée destinée à être mise en bouche pendant l'opération et contenant au moins un guide dans lequel une partie dudit organe est destinée à être introduite.
Le procédé pour l'obtention de ce dispositif est caractérisé en ce que, pour confec- tionner la pièee moulée, on prend un moulage des arcades du patient et en ce que l'on confectionne une pièce dont une partie est en substance plastique et dans laquelle on introduit et fixe le guide orienté dans la direction de l'axe des puits que l'on veut per cer parallèlement.
Le dessin annexé montre une forme d'exécution du dispositif en question, donnée à titre d'exemple.
La fig. 1 est une vue de côté d'une pièce à main munie de l'organe destiné à coopérer avec le ou les guides.
La fig. 2 est une vue en plan d'une pla que-base munie de guides et devant recevoir des bourrelets d'occlusion en substance plastique, l'ensemble constituant la pièce destinée à être mise en bouche pendant l'opération.
La fig. 3 montre un guide en perspective.
La fig. 4 est une vue en perspective de la bouche du patient et des éléments utilisés pendant le perçage d'un puits dans une incisive supérieure.
La pièce à main représentée à la fig. 1 se compose du tube 1 avec raccord 2, destiné à supporter la fraise 3.
En plus de ces parties, communes à toute pièce à main droite, il est prévu une douille 4, dans laquelle un organe avant la forme d'une tige coudée 5, que l'on nommera par la. suite baïonnette, peut coulisser axialement et tourner. On l'immobilise à la position voulue, au moyen de la vis moletée 6.
La douille 4 peut faire partie de la pièce à main, qui est alors de construction spéciale, ou au contraire être exécutée comme partie indépendante, susceptible de s'adapter facilement à n'importe quelle pièce à main, par vis, pièces ou tout autre moven.
A la fig. 2, on voit une plaque-base 7 en matière queleonque, résistante, ayant prati quenient le profil des arcades. Des guides eomme celui de la fig. 3, avec une partie tubulaire 8 et une semelle 9 peuvent y être fixés à la cire collante, par exemple en 10 et 11, tandis que sur et sous la zone ponctuée 12 viendront se placer des bourrelets d'occlusion en substance plastique.
Ayant établi un modèle en platre des arcades du patient, on préparera une plaquebase avec ses bourrelets d'occlusion et ses guides.
Le guide central, occupant la position 10, sera plaeé dans la région interincisives et son axe orienté en direction des puits horizontaux à percer dans les incisives centrales et latérales, direction que l'on détermine sur le modèle.
D'autres guides, par exemple tels que celui visible en 11, seront placés en regard des canines et orientés perpendiculairement à ces dernières.
La pièce ainsi préparée étant mise en bouche, le patient l'immobilisera en serrant les dents, comme cela ressort de la fig. 4.
On y voit la plaque-base 12 avec ses guides 10 et 11 et les bourrelets d'occlusion 13 et 14 que le patient serre entre les dents. Celles-ci apparaissent en réalité moins que sur le dessin, leurs extrémités venant s'insérer dans les creux correspondants des bourrelets.
Introduisant alors l'extrémité libre de la baïonnette 5, par exemple dans le guide 10, et amenant l'extrémité de la fraise 3 en face de l'incisive centrale 15, il suffit de serrer la vis 6 pour pouvoir percer avec facilité un puits d'axe rigoureusement parallèle à celui de la partie tubulaire du guide.
En desserrant la vis 6, il est possible, en raison du coude de la baïonnette et de sa faculté de tourner dans la douille 4, d'amener la fraise en regard d'une autre incisive et d'y percer, de la même manière, un trou rigoureusement parallèle au précédent.
On se servira par contre du guide latéral 11 pour percer la canine 16, et d'lm guide symétriquement disposé pour percer la' canine opposée.
L'endroit à perforer sur chaque dent sera au surplus préalablement repéré sur les dents du modèle en s'aidant de radiographies, afin d'éviter une perforation de la cavité de la dent.
REVENDICATIONS:
I. Dispositif pour la perforation de puits horizontaux parallèles dans les dents antérieures, caractérisé en ce qu'il comprend un organe solidaire d'une pièce à main et une pièce moulée destinée à être mise en bouche pendant l'opération et contenant au moins un guide dans lequel une partie dudit organe est destinée à être introduite.

The present invention relates to a device for perforating parallel horizontal wells in the anterior teeth; it also relates to a process for obtaining this device.
The need to drill such wells arises in the construction of Koehler-type bridges. It is, in this case, necessary that at least part of the wells have their axes strictly parallel to each other. This results in an adjustment difficulty which the present device claims to resolve in an extremely simple and safe manner.
It is characterized in that it comprises an organ integral with a handpiece and a molded part intended to be placed in the mouth during the operation and containing at least one guide into which part of said organ is intended to be introduced.
The method for obtaining this device is characterized in that, in order to make the molded part, a molding of the patient's arches is taken and in that a part is made, a part of which is in plastic substance and in which is introduced and fixed the guide oriented in the direction of the axis of the wells that we want to drill parallel.
The attached drawing shows an embodiment of the device in question, given by way of example.
Fig. 1 is a side view of a handpiece provided with the member intended to cooperate with the guide or guides.
Fig. 2 is a plan view of a pla that-base provided with guides and to receive occlusion beads made of a plastic substance, the assembly constituting the part intended to be placed in the mouth during the operation.

Fig. 4 is a perspective view of the patient's mouth and the elements used during the drilling of a well in an upper incisor.
The handpiece shown in FIG. 1 consists of tube 1 with connector 2, intended to support cutter 3.
In addition to these parts, common to any right hand piece, there is provided a socket 4, in which a front member in the form of a bent rod 5, which will be called by the. bayonet suite, can slide axially and turn. It is immobilized in the desired position, using the knurled screw 6.
The sleeve 4 can be part of the handpiece, which is then of special construction, or on the contrary be executed as an independent part, capable of easily adapting to any handpiece, by screws, parts or any other moven.
In fig. 2, we see a base plate 7 in some material, resistant, having prati quenient the profile of the arches. Guides like that of FIG. 3, with a tubular part 8 and a sole 9 can be fixed there with sticky wax, for example at 10 and 11, while on and under the punctuated zone 12 will be placed occlusion beads made of plastic substance.
Having established a plaster model of the patient's arches, we will prepare a baseplate with its occlusion beads and guides.
The central guide, occupying position 10, will be placed in the interincisor region and its axis oriented in the direction of the horizontal wells to be drilled in the central and lateral incisors, a direction which is determined on the model.
Other guides, for example such as the one visible at 11, will be placed opposite the canines and oriented perpendicular to the latter.
The piece thus prepared being placed in the mouth, the patient will immobilize it by clenching his teeth, as shown in fig. 4.
We see the base plate 12 with its guides 10 and 11 and the occlusion beads 13 and 14 that the patient squeezes between his teeth. These appear in reality less than in the drawing, their ends being inserted into the corresponding hollows of the beads.
Then inserting the free end of the bayonet 5, for example in the guide 10, and bringing the end of the bur 3 in front of the central incisor 15, it suffices to tighten the screw 6 to be able to easily drill a well of axis strictly parallel to that of the tubular part of the guide.
By loosening the screw 6, it is possible, due to the bend of the bayonet and its ability to turn in the sleeve 4, to bring the bur opposite another incisor and to drill there, in the same way , a hole strictly parallel to the previous one.
On the other hand, the lateral guide 11 will be used to pierce the canine 16, and of a symmetrically arranged guide to pierce the 'opposite canine.
The location to be drilled on each tooth will also be previously identified on the model's teeth with the help of x-rays, in order to avoid perforation of the tooth cavity.
CLAIMS:

Device for perforating parallel horizontal wells in the anterior teeth, characterized in that it comprises a member integral with a handpiece and a molded part intended to be placed in the mouth during the operation and containing at least one guide in which part of said organ is intended to be introduced.
