Procédé de fabrication d'un bridge et dispositif de fabrication mettant en oeuvre le procédé
La présente invention concerne un procédé de fabrication d'un bridge et un ensemble d'éléments sous la forme d'une trousse, panoplie ou assortiment pour appliquer ce procédé.
Les procêdés habituels de fabrication d'un bridge à base métallique font appel à une coulée à cire perdue sur mesure, pour constituer une armature métallique, et ensuite à une construction sur cette base d'une couche plus ou moins importante de matières cosmétiques (actuellement généralement de la porcelaine).
Ces procédés habituels nécessitent souvent une ablation importante de tissu dentaire (les dents support étant taillées pour recevoir une couronne recouvrant complëtement la dent). Cette opération constitue un travail long et difficile et entraîne une mutilation définitive de la dent et d'éventuels problèmes de gencives. Ces procédês nécessitent aussi des moulages très précis et sous-gingivaux de la zone où sera placée le bridge. Ces moulages sont coûteux en raison des matières utilisées et non récupérables et en raison du temps passê par un technicien de laboratoire dentaire.
De plus les bridges habituels sont réalisés en laboratoire souvent situé à l'écart du cabinet dentaire où l'on prend les moulages, où l'on prépare les dents comme piliers du bridge et où l'on cimente le bridge. Ces caractéristique permettent un procédé requérant une grande adaptabilité de la traverse: l'extrémité peut être réduite et le corps de la traverse peut être plié, meulé etc. pour s'adapter p.e. à l'occlusion.

Selon un autre aspect de l'invention , pour réaliser les ancrages radiculaires l'invention présente un nouveau type de traverse possédant en plus des qualités énoncées ci-dessus, un dispositif d'articulation ou d'emboîtement avec un nouveau type de pivots adaptés pour permettre cet emboîtement. Toutes les dimensions de pivots s'emboîtent sur toutes les dimensions de traverses étant donnê que l'articulation est standardisée. Ces deux nouveau types d'éléments permettent de réaliser au cabinet dentaire une armature rigide composée d'une traverse associée à un ou plusieurs pivots .

L'ensemble des simplifications obtenues par le procédé
suivant l'invention procure une réduction considérable des coûts de réalisation du bridge.

Comme déjà mentionné, selon un aspect particulier de l'invention, sur au moins une des faces de la traverse , une photogravure à l'acide d'un tramé constitue un moyen particulièrement efficace d'aide d'adhérence à la rêsine, cependant un sablage ou tout autre procédé mécanique, électrolytique, par électroérosion ou autre peut être éventuellement utilisé.
Ces éléments de fixation et ces pivots servent d'ancrage dans les dents formant les piliers du bridge.

La forme arrondie est donnêe à titre d'exemple , elle peut aussi être carrée, rectangulaire etc. Cela permet d' obtenir des traverses perforées d'épaisseur réduites par rapport au diamètre de la tête du pivot ( p.e. traverse de 1.6 mm.,. au niveau de l'épaississement 2 mm et pivot de 1.1 mm. ). Les proportions du dessin n'ont pas êté
respectées afin de mettre en évidence l'épaississement (typiquement 0,2 mm).
La figure 5b montre une forme de réalisation avantageuse d'une articulation traverse-pivot au moyen d'une traverse élargie et d'un pivot taraudé 8'. Dans ce cas, le procédé

- Le type de traverse A est vue en plan et est non perforée mais préréduite en 12 - Le type de traverse B vue par la tranche est perforée en 11 sur la tranche au centre et préréduite en 12.
- Le type de traverse C est perforée sur le plat au centre.
Les types de traverse B' et C' peuvent être dérivés par sectionnement, par exemple avec une pince adaptée, des traverses B et C correspondantes.
- Le pivot D à tête réduite (7,8) - Le pivot E à tête élargie (10) Ces 5 types d'éléments différents qui sont illustrés, grâce à leur possibilité d'emboîtement l'un avec l'autre, sont suffisants pour circonscrire la plupart ou toutes les formes d'armatures métalliques nécessaires pour la construction d'un bridge suivant le procédé de l'invention.

Une traverse de 1,6-2,6 mm est suffisamment résistante et suffit comme surface d'adhérence pour le composite. Au delà de 3 mm de largeur et de 2 mm d'épaisseur , la traverse devient encombrante.

La figure 8 illustre le compas modifié pour la mise à
longueur de la traverse. I1 permet de mesurer depuis le fond des évidements la distance entre les piliers. Pareil compas peut également faire partie du dispositif selon l'invention.
La figure 9 montre deux exemples de tramé gravable dans l'acier inox par photogravure à l'acide, afin de constituer la rugosité nécessaire pour l'adhérence de la résine.
La figure 10 montre un agrandissement d'une portion de traverse de tous les types ( A,B,C) laissant apparaître la découpe dentée trapézoïdale 6 (afin de réaliser une adhérence maximale) et les perforations éventuelles 11 et 11' (respectivement dans le plat et dans la tranche).
