FORMULAÇÃO MICELAR TERMORREVERSÍVEL CONTENDO QUERCETINA E VILDAGLIPTINA PARA ADMINISTRAÇÃO ORAL EM DIABETES TIPO 1. Uma melhor compreensão do Diabetes mellitus (DM) levou ao desenvolvimento de novas estratégias de tratamento que tiveram como alvo melhorar a função do pâncreas, como os Inibidores da DPP-IV (Vildagliptina). Somado a isto, sabe-se que a Quercetina é capaz de melhorar o estado hiperglicêmico. Entretanto, um passo limitante no uso da quercetina como um agente antidiabético coadjuvante é a sua baixa absorção intestinal. Para contornar esta limitação desenvolvemos uma formulação oral contendo Quercetina em associação com Vildagliptina formando um sistema micelar termorreversível baseado no copolímero tribloco poloxâmero 407 (P-407). A administração da Quercetina com a Vildagliptina produzirá contribuições excelentes considerando que não há relatos de trabalhos sobre a exploração de uma estrutura micelar contendo ambos os constituintes no desenvolvimento de uma formulação oral para o tratamento do diabetes tipo 1. Utilizamos 40 ratos distribuídos em 4 grupos: Grupos Controle e Diabético, receberam o volume correspondente a 1,0mL de formulação contendo o veículo;Grupo Diabético Tratado, recebeu o volume correspondente a 1,0mL da formulação contendo a Quercetina (50mg/Kg) em associação à Vildagliptina (10mg/Kg); Controle Positivo, recebeu uma injeção intraperitoneal de insulina. A better understanding of Diabetes mellitus (DM) has led to the development of new treatment strategies aimed at improving pancreas function, such as DPP-IV inhibitors ( Vildagliptin). Added to this, it is known that Quercetin is able to improve the hyperglycemic state. However, a limiting step in the use of quercetin as an adjunctive antidiabetic agent is its poor intestinal absorption. To overcome this limitation, we developed an oral formulation containing Quercetin in association with Vildagliptin forming a thermoreversible micellar system based on the triblock copolymer poloxamer 407 (P-407). The administration of Quercetin with Vildagliptin will produce excellent contributions considering that there are no reports of works on the exploration of a micellar structure containing both constituents in the development of an oral formulation for the treatment of type 1 diabetes. We used 40 rats distributed in 4 groups: Control and Diabetic Groups received a volume corresponding to 1.0mL of formulation containing the vehicle; Treated Diabetic Group received a volume corresponding to 1.0mL of formulation containing Quercetin (50mg/Kg) in association with Vildagliptin (10mg/Kg) ; Positive Control, received an intraperitoneal injection of insulin. [001] O Diabetes mellitus (DM) é um importante problema de saúde pública prevalente do ponto de vista social e econômico. Ao longo da última década, uma melhor compreensão do DM levou ao desenvolvimento de novas estratégias de tratamento que tiveram como alvo melhorar a função do pâncreas, tais como os medicamentos Inibidores da DPP-IV. Estes inibidores foram lançados no mercado com o intuito de promover um melhor controle sobre o diabetes mellitus tipo 2 (DM2), pois ao inibirem a enzima dipeptidil peptidase IV (DPP-IV) e aumentar os níveis endógenos das incretinas, estimulam a secreção de insulina de maneira dose-dependente e inibem a produção de glucagon.[001] Diabetes mellitus (DM) is an important public health problem prevalent from a social and economic point of view. Over the past decade, a better understanding of DM has led to the development of new treatment strategies that have targeted improving pancreas function, such as the DPP-IV inhibitor drugs. These inhibitors were launched on the market with the aim of promoting better control over type 2 diabetes mellitus (DM2), because by inhibiting the enzyme dipeptidyl peptidase IV (DPP-IV) and increasing endogenous levels of incretins, they stimulate insulin secretion in a dose-dependent manner and inhibit glucagon production.
[002] Apesar de esses inibidores terem sido extremamente estudados no tratamento do DM2, pouco se sabe do seu potencial no tratamento do diabetes mellitus tipo 1 (DM1). Entretanto, diversos autores relataram a ineficiência dos inibidores da DPP-IV na redução da glicose sanguínea no modelo de DM1, sendo assim, existe ainda a necessidade em buscar terapias adicionais que possam promover um melhor controle glicêmico. Apesar de a Quercetina apresentar um potencial antidiabético, este só se faz presente quando a mesma é administrada através de injeção intraperitoneal, pois um passo limitante ao uso da quercetina como um agente antidiabético é a sua baixa absorção intestinal. A administração da quercetina através de injeção intraperitoneal não satisfaz nossa proposta de elaboração de agente terapêutico oral para o tratamento do DM1. Sendo assim, a formulação não é óbvia, pois a mesma é necessária para permitir a absorção adequada da quercetina. Além disto, a associação dos dois constituintes (Quercetina e Vildagliptina) conferem efeitos hipoglicemiantes quando administrados na formulação proposta, conforme resultados apresentados neste relatório.[002] Although these inhibitors have been extensively studied in the treatment of DM2, little is known of their potential in the treatment of type 1 diabetes mellitus (DM1). Therefore, the formulation is not obvious, as it is necessary to allow adequate absorption of quercetin. In addition, the association of the two constituents (Quercetin and Vildagliptin) confer hypoglycemic effects when administered in the proposed formulation, according to the results presented in this report.

Está associado a complicações que comprometem a qualidade de vida e sobrevida dos indivíduos, além de envolver altos custos no seu tratamento. Nos anos 70 vários hormônios intestinais, denominados de incretinas foram identificados, como o GLP-1 (Glucagon like peptide-1), que foi reconhecido como um importante contribuinte para o controle do diabetes (TAMBASCIA, M. A. O complexo insulina/glucagon/incretinas e seu papel no controle Glicêmico. Assim, foi entendido que o processo glicorregulatório no diabetes é, também, o resultado da interação dos hormônios pancreáticos (insulina e glucagon) com hormônios incretinas, fazendo o diabetes ser visto como uma doença multi-hormonal. Baseado nesta informação, ao longo da última década, uma melhor compreensão do DM levou ao desenvolvimento de novas estratégias de tratamento em relação às incretinas que tiveram como alvo melhorar a função do pâncreas, tais como os medicamentos Inibidores da Dipeptidil Peptidase-IV. Apesar de estes inibidores terem sido extremamente estudados no tratamento do DM2, pouco se sabe do seu potencial no tratamento do DM1 (DUPRE, J. [005] Considerando que a redução da população de células beta é um dos mecanismos responsáveis tanto pelo estabelecimento quanto pela progressão do diabetes, se for demonstrado que a terapia baseada em incretinas é capaz de deter a progressão da doença, em longo prazo, através da preservação das células beta, então essa classe de medicamentos poderá ser utilizada desde o estágio de pré-diabetes até as fases mais avançadas da doença. Sendo assim, os inibidores da DPP-IV constituem não somente uma classe inovadora de agentes orais para o tratamento do diabetes tipo 2 (GREEN, B. D.; FLATT, P. R.; BAILEY, C. J. Portanto existe ainda a necessidade em buscar terapias adicionais que possam promover um melhor controle glicêmico. Neste sentido, sabe-se que a Quercetina apresenta um potencial antidiabético, melhorando o estado hiperglicêmico e reduzindo a incidência de complicações periféricas no diabetes (JEONG, S.; KANG, M.; CHOI, H. et al. [007] Um estudo comparativo da interação farmacocinética de quercetina e rutina após administração oral de uma formulação patenteada na Europa contendo extrato seco da planta Hipphophae rhamnoides e coadministração de quercetina e rutina foi realizado por Kammalla e colaboradores (2015). Os resultados do estudo demonstraram que existem diferenças na farmacocinética de quercetina e rutina quando estes fármacos foram administrados juntos a formulação fitoterápica. Na verdade, a biodisponibilidade de quercetina diminuiu e a biodisponibilidade da rutina aumentou a partir da formulação fitoterápica quando comparadas com a coadministração. Estes resultados provaram que existem interações farmacocinéticas inter e intra fitoterápica entre quercetina e rutina (Kammalla AK, et al. Comparative pharmacokinetic interactions of Quercetin and Rutin in rats after oral administration of European patented formulation containing Hipphophae rhamnoides and Co-administration of Quercetin and Rutin. Eur J Drug Metab Pharmacokinet. 2015.).[007] A comparative study of the pharmacokinetic interaction of quercetin and rutin after oral administration of a formulation patented in Europe containing dry extract of the plant Hipphophae rhamnoides and co-administration of quercetin and rutin was performed by Kammalla et al. (2015). The results of the study demonstrated that there are differences in the pharmacokinetics of quercetin and rutin when these drugs were administered together in the herbal formulation. Indeed, quercetin bioavailability decreased and rutin bioavailability increased from the herbal formulation when compared to co-administration. These results proved that there are inter and intra herbal pharmacokinetic interactions between quercetin and rutin (Kammalla AK, et al. Comparative pharmacokinetic interactions of Quercetin and Rutin in rats after oral administration of European patented formulations containing Hipphophae rhamnoides and Co-administration of Quercetin and Rutin. Eur J Drug Metab Pharmacokinet. 2015.).
[008] Para contornar esta limitação a maioria dos estudos que demonstram o efeito benéfico da Quercetina no controle glicêmico a administra através de injeção intraperitoneal, entretanto esta forma de administração não satisfaz nossa proposta de elaboração de agente terapêutico oral para o tratamento do DM1.[008] To circumvent this limitation, most studies that demonstrate the beneficial effect of Quercetin on glycemic control administer it through intraperitoneal injection, however this form of administration does not satisfy our proposal for the elaboration of an oral therapeutic agent for the treatment of DM1.
[009] Neste sentido, um dos aspectos importantes do desenvolvimento de uma formulação contendo concomitantemente Quercetina e Vildagliptina é a manutenção de concentrações adequadas destes fármacos no sítio de absorção no momento em que ocorre a liberação destes a partir da formulação. Neste contexto, um sistema polimérico de interesse são os géis aquosos termoreversíveis do copolímero polioxietileno-polioxipropileno-polioxietileno, ou poloxamero 407 (P-407). Em um destes estudos foi relatado o encapsulamento de quercetina em P407 (Pluronic F127) utilizando a técnica antissolvente supercrítico. Variações morfológicas e estruturais dos cristais de quercetina foram obtidas por co-precipitadas com P407 a partir de soluções de acetona, melhorando o comportamento de dissolução da quercetina em simulação de fluídos gástrico e intestinal. Nestas misturas, as concentrações de quercetina variaram de 10 a 20 mg enquanto as concentrações de P407 variaram de 10 a 90 mg, tendo o P407, conforme mencionado pelos autores, atuado como agente estabilizante na co- precipitação de quercetina. As partículas com tamanho reduzido foram obtidas quando a relação de proporção de quercetina:P407 foi de 10:20 mg e 20:20~34 mg, respectivamente. Na invenção descrita nesse documento, não só a relação de proporção de quercetina:P407 (50:160 mg) foi diferente, como também a técnica de preparação, o solvente utilizado e o estado físico final da formulação diferem consideravelmente do trabalho descrito acima. Neste trabalho, a capacidade de óleos, surfactantes e cosurfactantes solubilizar os fármacos mencionados foram avaliados. A melhor formulação foi adicionada de um agente formador de carreador sólido e, subsequentemente, liofilizada para gerar um produto auto-emulsificante sólido. In this work, the capacity of oils, surfactants and cosurfactants to solubilize the mentioned drugs was evaluated. The best formulation was added with a solid carrier forming agent and subsequently lyophilized to generate a solid self-emulsifying product. The authors reported that among the tested oils, oleic acid performed poorly as it failed to solubilize quercetin, which among the surfactants, P407 produced an emulsion with a good size and a good size dispersion, but required a relatively long time. to self-emulsify the Capmul® MCM EP oil phase in water.
