Abstract
conjunto de elementos para realização de exame fenotípico para diagnóstico de tuberculose e método de diagnóstico, mais precisamente trata-se de conjunto de elementos (10) apresentado na forma de 'kit' para a realização de diagnóstico de tuberculose do tipo utilizado na área da saúde, mais especificamente, em laboratórios; dito conjunto de elementos (10) associado ao método de diagnóstico compõe a realização simultânea de quatros exames distintos, quais sejam: (a) isolamento da bactéria; (b) identificação da bactéria; (c) determinação da sensibilidade a isoniazida; (d) determinação da sensibilidade a rifampicina da bactéria; dito kit (10) é constituído de: i) pelo menos vinte peças de laminocultivo (20) formado por um par de lâminas (21) e (22) instalado num tubo cilíndrico (30) formado por duas aberturas distais (31) e (32) onde são acopladas respectivas tampas (40) e (41); cada lâmina (21)/(22) preve um par de poços (p1) e (p2) e (p1') e (p2'); ii) um frasco (50) do tipo conta gotas com solução digestora e descontaminante de hidróxido de sódio a 4% , sendo 4 gramas de hidróxido de sódio em 100 ml de água; iii) um frasco (60) provido de microesferas (70); iv) uma zaragatoa swab (80) constituído de um algodão absorvente hidrofílico com capacidade de absorção de cerca de 1.000 microlitros, o qual é fixado a uma haste plástica cilíndrica de 15 cm de comprimento; v) pelo menos vinte peças de tubos de ensaio (100) confeccionado em poliestireno cristal transparente com 10 cm de comprimento e 70 rnm de diâmetro, sendo que cada tubo (100) possui uma tampa (101) do tipo encaixe por pressão e identificação de volume de 3 ml a 5 ml; ditos tubos (100) são utilizados para acondicionar a solução de naoh 4% durante o processo de descontaminação do escarro; vi) uma caixa para incubação (90) dos laminocultivos (20) dotada de tampa (91), sendo que a porção interna prevê paredes divisórias transversais (91a) e longitudinais (91b) conformando nichos (n) para a acomodação dos laminocultivo (20) durante o transporte para a comercialização e para a estufa a 37°c após a semeadura no processo de incubação.set of elements for carrying out a phenotypic examination for the diagnosis of tuberculosis and diagnostic method, more precisely it is a set of elements (10) presented in the form of a 'kit' for carrying out a diagnosis of tuberculosis of the type used in the health sector , more specifically, in laboratories; Said set of elements (10) associated with the diagnostic method comprises the simultaneous performance of four distinct tests, namely: (a) isolation of the bacteria; (b) identification of the bacteria; (c) determination of sensitivity to isoniazid; (d) determination of the rifampicin sensitivity of the bacteria; Said kit (10) consists of: i) at least twenty pieces of laminoculture (20) formed by a pair of blades (21) and (22) installed in a cylindrical tube (30) formed by two distal openings (31) and ( 32) where respective covers (40) and (41) are attached; each slide (21)/(22) provides a pair of wells (p1) and (p2) and (p1') and (p2'); ii) a dropper-type bottle (50) with a 4% sodium hydroxide digesting and decontaminating solution, with 4 grams of sodium hydroxide in 100 ml of water; iii) a vial (60) provided with microspheres (70); iv) a swab swab (80) made of hydrophilic absorbent cotton with an absorption capacity of around 1,000 microliters, which is attached to a cylindrical plastic rod 15 cm long; v) at least twenty pieces of test tubes (100) made of transparent crystal polystyrene 10 cm long and 70 mm in diameter, with each tube (100) having a press-fit type lid (101) and identification of volume from 3 ml to 5 ml; said tubes (100) are used to condition the 4% naoh solution during the sputum decontamination process; vi) a box for incubating (90) the laminocultures (20) equipped with a lid (91), with the internal portion providing transverse (91a) and longitudinal (91b) dividing walls forming niches (n) for the accommodation of the laminocultures (20 ) during transport to commercialization and to the greenhouse at 37°c after sowing in the incubation process.

[001] Trata a presente invenção de conjunto de elementos para realização de exame fenotípico para diagnóstico de tuberculose e método de diagnóstico do tipo utilizado na área da saúde, mais especificamente, em laboratórios que realizam exames para diagnóstico bacteriológico de doenças infecciosas em, especial, o conjunto de elementos se apresentam na forma de 'kit' que permite a realização de exames fenotípicos para o diagnóstico da tuberculose, quais sejam: (i) isolamento da bactéria; (ii) identificação da bactéria; (iii) determinação da sensibilidade a isoníazida; (iv) determinação da sensibilidade a rifampicina da bactéria. Dito kit compreende o laminocultivo e outros elementos, tais como, frasco provido de solução digestora e descontaminante, frasco provido de microesferas de vidro, tubo de poliestireno, 'zaragatoa' e uma caixa para incubação dos laminocultivos. Dito procedimento é denominado de descontaminação, ou seja, eliminação da flora bacteriana - microbiota - indesejável do escarro. Os bacilos da tuberculose permanecem viáveis durante esses procedimento, pois resistem temporariamente ao pH alcalino extremo; e) Espremer o tufo de algodão contra a parede do tubo (100) para retirar o excesso de NaOH; f) Girar a tampa (40) em que se encontra presa à lâmina (21) contendo os meios de cultura cuidando para não tocar nas bordas e remove-la cuidadosamente do tubo transparente (30); g) Segurar firmemente a lâmina (21) e passar o swab (80) mediante movimentos rotatórios e em zigue-zague de maneira a espalhar bem o inoculo primeiro sobre a superfície do poço (pl) - controle - e a seguir sobre a superfície do poço (p2) - PNB; h) Com auxílio de um novo swab (80), repetir os procedimentos b, d e e. Segurar firmemente a lâmina (22) e passar o swab (80) mediante movimentos rotatórios e em zigue-zague de maneira a espalhar bem o inoculo sobre a superfície do poço (pl') - INH; i) Com auxílio de um novo swab (80) - terceiro repetir os procedimentos b, d e e. Segurar firmemente a lâmina (22) e passar o swab (80) mediante movimentos rotatórios e em zigue-zague de maneira a espalhar bem o inoculo sobre a superfície do poço (p2') - RIF. Se possível, utilizar lupa para melhorar a visualização das colônias pequenas; n) Se ocorrer o crescimento de colônias - micobactérias - na superfície do meio de cultura controle realizar a leitura e interpretação dos resultados entre 21 e 30 dias após a semeadura. Para que o resultado do método de identificação e de sensibilidade a INH a RIF seja válido é necessário que ocorra o crescimento suficiente de unidades formadoras de colônias - UFC - mais de 100 no meio de cultura controle sem droga. A resistência ou a sensibilidade é estimada comparando-se o número de colônias que crescem no meio com o fármaco com o número que cresce no meio controle, e expresso em percentual. Considera-se 1% como o ponto de corte definidor. Desta forma para que possa detectar pelo menos 1% de mutantes resistentes entre os bacilos inoculados - cálculo da porcentagem - será necessária a presença de pelo menos 100 UFC no meio de cultura controle; o) Quantificar o número UFC nos meios de cultura - Controle ou C, PNB, INH e RIF; p) Calcular a porcentagem de UFC desenvolvidas na presença de cada droga - PNB, INH e RIF - corn relação à média de UFC no meio de cultura controle. Se essa proporção é maior do que 1%, o isolado bacteriano é considerado resistente à droga em questão.
