Abstract
Description
OSTEOSYNTHESIS PLANE
1. AREA OF APPLICATION
The present invention relates to a plate for osteosynthesis used in orthopedics in the treatment of broken bones by extracortical osteosynthesis, both in human and veterinary medicine.
2. BACKGROUND OF THE INVENTION
Extracortical osteosynthesis is used for fractures of different types and localization: fractures, hairs, screws, transverse, occipital and joints, regardless of the shape and the curvature of the marrow. In most cases, extracortical osteosynthesis retainers are different in shape and thickness, fixed to the bone by bone screws. However, osteosynthesis, with long, tightly adhering to the bone plates, leads to impaired blood circulation in the cortex below the plate. This leads to an increase in the likelihood of infection, which is considered a major complication in post-operative treatment, as well as an increase in treatment time. In addition, in order to achieve long-lasting and stable osteosynthesis, it is necessary to ensure a stable contact along the length of the plate between the plate and the bone fragments, which is guaranteed for different bone types and shapes, while allowing for the reduction of the number used screws.
It is known from US 3695259 a solution that offers a 1-shape cross-section of the bar to increase its strength as well as to better fit it to bones with different outer contours. The disadvantage of this solution is that for different sized bones, plates with different widths should be used. This does not guarantee a stable contact of the plate and bone fragments along the entire length of the plate, because the plate cannot provide relatively continuous contact in the relief of the bone. The latter leads to instability in the position of the bone fragments relative to each other.
. ..........-------------------------2 . ..........------------------------- 2
Also known is a plate of US 4429690, consisting of two parallel longitudinal rods, interconnected with a plurality of protruding connecting elements, spaced apart, and having openings for mounting cortical screws. A major disadvantage here is also that the bar cannot fit more tightly over longer lengths, thus not providing a uniform distribution of compression forces everywhere. In addition, this strap has a complex configuration, which complicates its manufacturing technology.
SU1037911 provides a U-shaped bar in which the side walls are provided at certain distances with teeth penetrating the cortex and having a bulging projection from the inner wall. Here, in addition to the shortcomings mentioned above, the presence of teeth leads to distortion of the periosteum and further injury to the cortex. In addition, the bar is sophisticated.
Such a solution is described in US 5053036. Here, the plate comprises an upper surface and a vaulted lower surface connected to side walls, the lower surface being provided with a plurality of contact elements - projections spaced apart from each other along the lower edges on both sides of the bar. The disadvantage of this solution is, similar to the ones described above, that the plate has a complicated construction. In addition, the forces created by the clamping screws are concentrated at the points of contact and the pressure applied to them is increased, which causes a point injury of the period. In addition, even here, a single bar cannot provide uniform contact in different bone sizes and configurations, thus reducing the stability of bone fragments with respect to one another.
4. TECHNICAL NATURE OF THE INVENTION
It is an object of the invention to provide a plate for osteosynthesis which provides a stable, relatively continuous contact between the bone fragments and the plate along the length thereof.
It is another object of the invention to provide, through the bar, a more uniform distribution of the compressive forces at the points of contact.
It is another object of the invention that the bar be suitable for use in bones with different cross-sections, as well as with different appearance and relief.
Last but not least, it is an object of the invention to provide a bar that has a simplified manufacturing technology.
Another subject is the bar to create an opportunity not to disturb the integrity of the period.
These and other problems are solved by the proposed osteosynthesis plate comprising an upper surface and a lower surface connected thereto with a deliberately curved smooth portion, with more than one cortical screw opening formed along the plate. At both lateral ends of the lower surface there are longitudinal end edges formed, with at least one further longitudinal edge parallel to them, so that lateral notches in the cross section are formed, separated by the vaulted smooth portion of the lower surface. The edges of the edges on each side form symmetrical surfaces at an angle to the symmetrical plane of the bar and not coinciding with the vault. These edges may have sharp or blunt tips. In this way, irrespective of the relief longitudinally and transversely to the bone, bone fragments are contacted with a specific portion of any edge and the compressive force created by the cortical screws is more evenly distributed along the plate. located at an angle to the plane of symmetry of the plank sections, it also allows the use of the plank for bones with different cross-sections, as well as with different appearance and relief, because, depending on the curvature of the cross-section, and the bone of at least one longitudinal edge is a major support in the attachment. In this way, the size of the plate is achieved for a greater number of different bones, regardless of their type and size. The blunt edges, in turn, do not disturb the integrity of the period.

6. EXAMPLES OF IMPLEMENTATION
7.The accompanying figures show some possible embodiments of the osteosynthesis plate according to the invention. The osteosynthesis plate 1 consists of an upper surface 2 connected to a lower surface 3. Figure 1 shows the connection of the surfaces 2 and 3 by side walls 4, which allows the bar to be fabricated. In FIG. 2, the connection of the two surfaces 2 and 3 is direct. Longitudinal end edges 5 are formed at the two lateral ends of the lower surface 3. There are three more longitudinal edges parallel to each of them on the lower surface 3 so that lateral notches 7 are formed in cross section, separated by a smooth vaulted portion 8 from the lower surface 3. The edges of the edges 5 and 6 on each side form symmetrical surfaces arranged at an angle F with respect to the symmetrical plane of the plate 1. The edges 5 and 6 are blunted in this case. Along the plate 1 in the vault 8, openings 9 are formed for cortical screws. The openings 9 to the upper surface are made with a spherical milling cutter 10, thus providing an opportunity for angular displacement of the cortical screw 11. Figure 4 shows the application of the plate 1 gripped with a cortical screw 11 to the bone. It is clear that when the shape of the bone is changed in the longitudinal direction and in the periphery, the proposed plate will provide permanent contact in the longitudinal direction by contacting different parts of the different edges 5 and 6 of the notches 7.
