Nouvelle association médicdmenteuse
La présente invention a pour objet une nouvelle association médicamenteuse destinée au traitement de l'hypertension et de l'insuffisance cardiaque globale.
L'invention concerne en particulier une association médicamenteuse contenant, comme substances actives, a) un activateur des canaux potassiques et b) un inhibiteur de l'enzyme de conversion de l'angio- tensine (inhibiteur ECA).
L'invention concerne également une composition pharmaceutique contenant a) un activateur des canaux potassiques et b) un inhibiteur ECA.
Les activateurs des canaux potassiques [ou composant a)] représente une classe de substances physiologiquement actives qui augmentent la perméabilité au potassium de la membrane cellulaire des muscles lisses. Un grand nombre de ces composés sont à présent connus et ont une application thérapeutique étendue, en particulier pour le traitement des troubles ou des maladies cardio-vasculaires, par exemple pour le traitement de l'insuffisance cardiaque chronique, et pour le traitement de l'asthme et des troubles obstructeurs du système respiratoire, de l'hypertension et des troubles du tractus gastro-intestinal. On utilise en général les activateurs des canaux potassiques comme vasodilatateurs, par exemple pour le traitement de l'hypertension.
On a peu publié de documents sur les propriétés pharmaceutiques et biopharmaceutiques des inhibiteurs de l'enzyme de conversion de l'angiotensine (ou composant b). Les inhibiteurs de l'enzyme de conversion de l'angiotensine sont des composés connus qui influencent la transformation de l'angiotensine I en angiotensine II et sont utiles pour abaisser la

pression sanguine élevée lorsque celle-ci provient de l'action de l'angiotensine II. Les inhibiteurs ECA, lorsqu'ils sont administrés seuls, doivent être utilisés en doses relativement élevées pouvant provoquer des effets secondaires indésirables.
La Demanderesse a trouvé de façon surprenante et inattendue que l'association médicamenteuse de l'invention comprenant le composant a) et le composant b) possède des propriétés pharmaceutiques avantageuses ainsi qu'un profil pharmacologique/thérapeutique spécialement favorable ou amélioré. On a trouvé en particulier que la co-administration d'un composant a) et d'un composant b) tels que décrits plus haut provoque une amélioration inattendue de l'activité antihypertensive et, de façon surprenante, une activité puissante contre l'insuffisance cardiaque globale.
Le composant a) provoque une augmentation de la pression veineuse centrale et réduit donc l'augmentation du débit cardiaque chez les animaux pré-traités avec le composant b). L'augmentation de la pression veineuse centrale en l'absence de dépression cardiaque (la contractilité cardiaque est évaluée avec précision en cousant une jauge de contrainte sur le myocarde) indique une augmentation du retour veineux provoqué par la dilatation artériolaire. L'effet réducteur du composant a) indique donc une compliance veineuse augmentée après inhibition de l'ECA.
L'activateur des canaux potassiques utilisé dans la composition de l'invention comme composant a) est de la classe des benzo [b] pyrannes et despyrannopyridines de formule

Les composés de formule I peuvent être utilisés sous forme de bases libre ou de sels. Les sels d'addition d'acides peuvent être obtenus par réaction des composés de formule I avec par exemple l'acide chlorhydrique, l'acide bromhydrique, l'acide malonique, l'acide maléique, l'acide fumarique, l'acide benzènesulfonique, l'acide toluène-sulfonique, l'acide méthane-sulfonique, l'acide malique, l'acide tartrique, l'acide campho-sulfonique, l'acide formique, l'acide oxalique, l'acide phosphorique, l'acide sulfurique, l'acide trifluoroacétique et l'acide trifluoro-méthane- sulfonique. Il est également identifié par la firme Schering comme SCH-33844. Ce composé est décrit à l'exemple 3 du brevet américain nO 4 470 972. Dans ce brevet américain il est indiqué que le composé exerce une activité antihypertensive. On peut utiliser un sel pharmaceutiquement acceptable. L'hydrate peut être utilisé.
Dans ce brevet américain, on décrit différents sels qui peuvent être utilisés à la place de la base libre, par exemple le chlorhydrate tel que décrit à l'exemple 4 de ce brevet ou l'hémimaléate tel que décrit à l'exemple 5 de ce brevet. La forme préférée est le monochlorhydrate monohydraté.
Les nouvelles compositions pharmaceutiques de l'invention peuvent être préparées selon les méthodes connues à l'aide des excipients pharmaceutiques habituels.
On sait que les activateurs des canaux potassiques et les inhibiteurs de l'enzyme de conversion de l'angiotensine sont utiles pour le traitement de l'hypertension.
Des associations médicamenteuses contenant les deux substances actives devraient également être utiles pour le traitement de l'hypertension. Contre toute attente, les recherches ont montré que les associations médicamenteuses spécifiques des deux substances actives ont des propriétés inconnues jusqu'à présent, ce qui les rend particulièrement intéressantes et appropriées pour le traitement de l'hypertension et de l'insuffisance cardiaque globale.

sous forme de base libre ou, si nécessaire, sous forme de sel, par exemple sous forme d'un sel d'addition d'acide.
La co-administration d'un activateur des canaux potassiques comme composant a) et d'un composant b) exerce une activité antihypertensive puissante et une activité contre l'insuffisance cardiaque globale comme il ressort des essais pharmacologiques.
L'activité de l'association médicamenteuse de l'invention dans le traitement de l'hypertension peut être mise en évidence chez les rats éveillés spontanément hypertendus selon la méthode décrite par Tschirki et coll., (Arzneimittelforsch. Drug Res. 18 (1968), 1285, et l'activité de l'association médicamenteuse de l'invention dans le traitement de l'insuffisance cardiaque globale peut être mise en évidence dans la méthode décrite par Salzmann et coll., dans Journal of Cardiovascular Pharmacology 7 (1985) 588-596.
Les associations médicamenteuses de l'invention sont donc appropriées pour une utilisation dans le traitement de l'hypertension et de l'insuffisance cardiaque globale.
L'activateur des canaux potassiques peut être administré par exemple de 1/3 à 100 % de la dose normale pour le traitement par exemple de l'hypertension ou de l'insuffisance cardiaque globale. La dose quotidienne appropriée est comprise entre environ 0,1 mg et environ 5 mg, en particulier entre environ 0,25 et 5 mg, avantageusement sous forme fractionnées 1 à 4 fois par jour, sous forme de doses unitaires contenant chacune entre environ 0,06 mg et environ 5 mg, ou sous une forme à libération prolongée. Le composé préféré est le composé (6) décrit plus haut. La dose appropriée est comprise entre environ 0,25 et environ 1 mg 1 à 2 fois par jour, par voie orale. Le composant b) peut être administré par exemple de 1/3 à

100% de la dose normale pour le traitement de l'hypertension ou de l'insuffisance cardiaque globale.
Les doses appropriées sont 2,5 mg, 5 mg et 15 mg, spécialement 2,5 et 5,0 mg. On utilise avantageusement 0,25 mg du composé a) et de 0,5 à 25 mg du composé b).
Le rapport pondéral approprié du composant a) au composant b) calculé par rapport à la base libre est compris par exemple entre environ 1 : 5 et environ 1 : 20, en particulier entre environ 1 : 10 et environ 1 : 15. C'est ainsi que pour le composé (6) en tant que composant a) et le spirapril en tant que composant b), le rapport pondéral préféré est compris entre environ 1 : 10 et environ 1 : 15.
Avantageusement, on administre les composants a) et b) sous forme d'une composition pharmaceutique contenant les substances actives en association avec un véhicule ou diluant pharmaceutique.
De telles compositions peuvent être avantageusement formulées selon les méthodes habituelles, par exemple sous forme d'une solution telle qu'une solution ou une suspension injectable, ou de préférence sous une forme solide, par exemple sous forme de comprimés ou de gélules. Si nécessaire, un composant ou les deux peuvent se présenter sous une forme à libération prolongée.
Si nécessaire, les substances actives peuvent être disposées dans un emballage afin de faciliter l'administration d'une dose particulière, par exemple dans un ordre particulier dans un blister.
Une composition appropriée peut également être placée dans un emballage contenant séparément le composant a) et le composant b) jusqu'à ce qu'ils soient utilisés pour une administration concomitante, ledit emballage contenant également le mode d'emploi pour une administration concomitante d'une quantité pré-déterminée du composant a) et du composant b).

The subject of the present invention is a new drug combination intended for the treatment of hypertension and global heart failure.
The invention relates in particular to a drug combination containing, as active substances, a) an activator of potassium channels and b) an inhibitor of the angiotensin converting enzyme (ACE inhibitor).
The invention also relates to a pharmaceutical composition containing a) an activator of potassium channels and b) an ACE inhibitor.
Activators of potassium channels [or component a)] represent a class of physiologically active substances which increase the permeability to potassium of the cell membrane of smooth muscles. Many of these compounds are now known and have broad therapeutic application, particularly for the treatment of cardiovascular disorders or diseases, for example for the treatment of chronic heart failure, and for the treatment of asthma and obstructive disorders of the respiratory system, hypertension, and disorders of the gastrointestinal tract. Activators of potassium channels are generally used as vasodilators, for example for the treatment of hypertension.
Little has been published on the pharmaceutical and biopharmaceutical properties of angiotensin converting enzyme inhibitors (or component b). Angiotensin converting enzyme inhibitors are known compounds that influence the transformation of angiotensin I into angiotensin II and are useful in lowering

high blood pressure when this comes from the action of angiotensin II. ACE inhibitors, when administered alone, should be used in relatively high doses which may cause unwanted side effects.
The Applicant has surprisingly and unexpectedly found that the drug combination of the invention comprising component a) and component b) has advantageous pharmaceutical properties as well as a specially favorable or improved pharmacological / therapeutic profile. It has been found in particular that the co-administration of a component a) and a component b) as described above causes an unexpected improvement in antihypertensive activity and, surprisingly, a potent activity against insufficiency overall heart.
Component a) causes an increase in central venous pressure and therefore reduces the increase in cardiac output in animals pretreated with component b). The increase in central venous pressure in the absence of cardiac depression (cardiac contractility is precisely assessed by sewing a strain gauge on the myocardium) indicates an increase in venous return caused by arteriolar dilation. The reducing effect of component a) therefore indicates increased venous compliance after ACE inhibition.

The activity of the drug combination of the invention in the treatment of hypertension can be demonstrated in spontaneously hypertensive awake rats according to the method described by Tschirki et al., (Arzneimittelforsch. Drug Res. 18 (1968) , 1285, and the activity of the drug combination of the invention in the treatment of global heart failure can be demonstrated in the method described by Salzmann et al., In Journal of Cardiovascular Pharmacology 7 (1985) 588 -596.
The drug combinations of the invention are therefore suitable for use in the treatment of hypertension and global heart failure.
The potassium channel activator can be administered, for example, from 1/3 to 100% of the normal dose for the treatment, for example, of hypertension or of global heart failure.
