Внутренняя поверхность которых соответствует наружной поверхности ацетабулярного компонента эндопротеза, а наружная поверхность соответствует поверхности костного дефекта. Толщина основания и перпендикулярной части ⊥-образного элемента переменна и убывает от места их пересечения к свободным краям. На основании и перпендикулярной части ⊥-образного элемента расположены отверстия на расстоянии не менее 5 мм друг от друга. Оценивают геометрию и объем дефекта костной ткани. Далее последовательно вводят, медицинские крепежные изделия, например винты в отверстия (1), (2), (3) расположенные на основании ⊥-образного элемента в случае необходимости с возможностью перепроведения, с целью повышения фиксации устройства в костном дефекте. Затем вокруг устройства медицинским инструментом, например импактором забивают ауто- или аллокость до полного заполнения костного дефекта с целью формирования новой вертлужной впадины. Затем медицинским инструментом, например направителем (фиг. 5) устанавливают ацетабулярный компонент путем импакциии таким образом, чтобы отверстие (5) в ацетабулярном компоненте совпало с отверстием (4) на перпендикулярной части устройства, тем самым достигают плотный контакт между ацетабулярным компонентом и перпендикулярной частью устройства. Пациент К, 1990 г. р. поступил в ФГБУ «НИИТО им. Я.Л. Цивьяна» Минздрава России в ноябре 2017 года после перенесенной автодорожной травмы. При поступлении предъявлял жалобы на боль в левом тазобедренном суставе, ограничение движений в левом тазобедренном суставе, укорочение и неопорность левой нижней конечности. В анамнезе больной оперирован в других медицинских лечебных учреждениях по поводу закрытого перелома лонной, седалищной костей слева со смещением отломков, закрытого перелома левой подвздошной кости, дна вертлужной впадины слева, перелома боковых масс крестца слева, разрыва крестцово-подвздошного сочленения слева (по классификации АО 61 В2, 62 С2). Полученное изображение анализируют, проводят измерения геометрии полученного дефекта: его размеры, форму, объем. На основании полученных данных определяют геометрию устройства. По внешней поверхности устройство соответствует размеру дефекта и составляет в диаметре в данном случае 62 мм, внутренняя поверхность составляет в диаметре 52 мм, а радиус перпендикулярной части - 26 мм. При этом толщина перпендикулярной части устройства составляет 32,6 мм, для достижения необходимой латерализации центра ротации. Ширина основания и перпендикулярной части устройства составляет 18 мм. Для данного пациента количество отверстий на устройстве определено в количестве 4-х штук, угол наклона отверстий составляет 136°. Полученную виртуальную модель устройства печатают на 3D принтере по технологии SLS. Вторым этапом производят замещение дефекта костной ткани вертлужной впадины с помощью заявляемого устройства. На 3 сутки после операции выполнена рентгенограмма тазобедренных суставов в прямой проекции (фиг. 12), на которой определено, что центр ротации левого тазобедренного сустава расположен на расстоянии 94 мм от центра туловища (вертикальная линия, проведенная через симфиз), что является допустимым в данном случае. Равность нижних конечностей восстановлена, это показано линией соединяющей малые вертела на правой и левой конечности. Пациент активизирован на 3 сутки с дозированной нагрузкой на оперированную левую конечность. Осложнений в раннем и позднем послеоперационном периодах не выявлено. Жалоб пациент не предъявляет. В связи с тем, что на устройство нанесено специальное покрытие происходит остеоинтеграция на границе «кость-устройство» это способствует вторичной фиксации, и как следствие, снижает риск повторной операции. У устройства высокие прочностные свойства в связи с тем, что оно монолитно и изготавливается на предоперационном этапе под конкретный костный дефект, что не требует проведения его моделирования в ходе операции. Отсутствует риск интерференции винтов при их проведении, в связи с тем, что направление отверстий не пересекаются и в ходе операции устанавливают запланированное на предоперационном этапе количество винтов, что повышает фиксацию устройства в костной ткани и как следствие повышает стабильность цементного вертлужного компонента эндопротеза. Отсутствует риск увеличения костного дефекта в связи с тем, что устройство позволяет провести винты с внутренней поверхности при положении устройства под углом менее 40° относительно плоскости наклона вертлужной впадины, тем самым отсутствует необходимость переустановки устройства и переподготовки костного ложа. Отсутствует риск увеличить дефект вертлужной впадины в связи с тем, что ввод винтов осуществляется в разных направлениях, с возможностью их перепроведения, что приводит к высокой первичной фиксации, тем самым не требуется переустановка устройства, с переподготовкой костного ложа. За счет соотношения геометрических размеров и объема, а именно сформированной между основанием и перпендикулярной частью ⊥-образного элемента выемки позволяет использовать максимальное количество алло/ ауто костной ткани, что приводит к полному восстановлению ее в дефекте, тем самым создает благоприятные условия для повторных реконструктивных операций. Минимален риск вывиха бедренного компонента, так как устройство позволяет регулировать внутреннее (медиализацию) и наружное (латерализацию) смещение центра ротации, что приводит к правильной биомеханики эндопротеза тазобедренного сустава. Позволяет правильно установить ацетабулярный компонент относительно плоскости наклона вертлужной впадины и произвести коррекцию инклинации ацетабулярного компонента за счет того, что фиксирующий винт проводят через отверстие (4) устройства и ацетабулярный компонент, в костную ткань, стягивая их в правильном положении, тем самым снижая риск вывиха бедренного компонента. Нагрузка с ацетабулярного компонента переходит на устройство, что приводит к благоприятным условиям для перестройки костного трансплантата в кость, с последующей остеоинтеграцией. Уменьшается время проведения операции, ее травматичность, период пребывания в стационаре, сроки восстановительного лечения и риск повторной операции. There is no risk of interference of the screws during their conduct, due to the fact that the direction of the holes does not intersect and during the operation, the number of screws planned at the preoperative stage is established, which increases the fixation of the device in bone tissue and, as a result, increases the stability of the cementitious acetabular component of the endoprosthesis. There is no risk of an increase in the bone defect due to the fact that the device allows screws from the inner surface when the device is positioned at an angle of less than 40 ° relative to the plane of inclination of the acetabulum, thereby there is no need to reinstall the device and retrain the bone bed. There is no risk of increasing the acetabulum defect due to the fact that the screws are inserted in different directions, with the possibility of re-conducting them, which leads to high primary fixation, thereby no reinstallation of the device is required, with the bone bed being retrained. Due to the ratio of geometric dimensions and volume, namely, the notch formed between the base and the perpendicular part of the образ-shaped element, the maximum amount of allo / auto-bone tissue can be used, which leads to its full restoration in the defect, thereby creating favorable conditions for repeated reconstructive operations. The risk of dislocation of the femoral component is minimal, since the device allows you to adjust the internal (medialization) and external (lateralization) displacement of the rotation center, which leads to the correct biomechanics of the hip joint prosthesis. It allows you to correctly set the acetabular component relative to the plane of inclination of the acetabulum and to correct the inclination of the acetabular component due to the fact that the fixing screw is passed through the hole (4) of the device and the acetabular component into the bone tissue, tightening them in the correct position, thereby reducing the risk of dislocation of the femoral component. The load from the acetabular component passes to the device, which leads to favorable conditions for the reconstruction of the bone graft into the bone, followed by osseointegration. The time of the operation, its morbidity, the period of hospital stay, the duration of rehabilitation treatment and the risk of reoperation are reduced. Patient satisfaction with surgical outcomes is enhanced.
