Description
The present invention relates to a method for producing an implant having at least one area with a surface structure. Furthermore, the present invention relates to an implant produced by this method and an apparatus for carrying out the method.
For better anchoring of implants, especially endoprostheses in bone a variety of surface structures are known. These range from roughened surfaces to the equipment of the surface with complex structural elements.
Especially in the field of hip joint prostheses, it is advantageous if the implant parts, especially the hip socket mounted in the joint socket, can be anchored and rotationally fixed in the bone. While one group of sockets is anchored by screws, in a second group the fixation is done by means of bone cement.
In order to increase the Ausreissverhalten and the rotational strength of a cemented bone cement socket, the surface of such pans is usually provided with structural elements. These structural elements distribute occurring forces evenly and thus prevent rotation or even the tearing of the socket.
Under this aspect, various structural elements for Gelenkspfannen have been developed. Thus, for example, EP 0 186 471 shows a knee joint prosthesis with sawtooth-like grooves, which are mounted transversely to the direction of insertion, wherein the stability in the direction of the grooves is reduced. Another alternative provides barb-like pins on the prosthesis which penetrate into the femoral stump in the direction of insertion.
A general requirement of anchoring surfaces of this type is that they should once have a waxing-promoting structure and that they should on the other hand be produced economically.
DE 2 914 513 shows anchoring surfaces provided with wart-like protrusions and depressions to improve bone ingrowth. No information is given on the production and on the absolute dimensions of this structure. In the relative geometric conditions rounded forms are proposed on all sides, which allow only a limited primary anchorage.
Another structure is proposed in EP 0 381 351, in which on a hip joint shell on an outwardly projecting bead extending obliquely to the equator and offset in the circumferential direction punctures that intersect, creating a sort of randr anchoring surface with flattened protruding pyramids becomes. Again, the primary anchorage is limited, i. highly dependent on the preload in the bone cavity and the undersize of the bone cavity in the region of the equator.
In EP 1 648 350 a hip joint socket is described, which is provided in the equatorial region with a thread which consists of mutually separated by inclined grooves thread areas. In the production of the surface structure, the inclined grooves are first milled from the implant base body and then the thread is rotated.
A major disadvantage of the structures described is that they are produced by machining manufacturing process. However, the production of very fine, non-symmetrical or staggered structures is very complicated or even impossible with such methods.
EP 0 639 356 also proposes to provide the surface of a joint socket with ribs substantially perpendicular to the direction of introduction of the socket, which ribs are provided by a pressing tool with trenches running parallel to the direction of insertion. At the edges of the trenches, the ribs form shoulder portions, the tips of which are bent by the pressing process counter to the direction of insertion. This creates a barb structure that additionally anchors the socket.
By pressing method used only straight trenches can be produced. Equipping an implant surface with staggered trenches is not possible.
The object of the invention is to obviate the disadvantages of the known manufacturing method and in particular to provide a manufacturing method for an implant, which makes it possible to provide a structured surface with fine, asymmetrical or staggered arranged structural elements. This object is achieved by a method according to claim 1.
The inventive method includes the following steps: Presenting a basic implant body; Impressing a plurality of recesses, preferably arranged in rows, on a surface section of the implant base body; and Removing material between the rows of depressions so as to form raised structural elements on the surface portion delimited by the depressions and by the abraded locations.
The inventive method can be used to produce implants having at least one area with a surface structure. Alternatively, several areas of an implant can be equipped with surface structures. Several or all areas can be provided with the same structure. Alternatively, however, it is also possible to provide a plurality of regions with different surface structures using the method according to the invention.
In the method according to the invention, initially an implant base body is presented. The implant base body is understood as meaning both already finished implants and implant semi-finished products which are additionally to be equipped with at least one area with a surface structure, or else blanks which are to be formed into an implant. The implants are preferably endoprostheses. Particularly preferably, with the method according to the invention, sockets, in particular acetabular cups, are provided with at least one area with a surface structure. Furthermore, however, the method according to the invention can also be applied to other implants, such as, for example, knee prostheses, intramedullary nails, femoral or humeral stem prostheses, dental prostheses or the like.
In the next step, depressions are embossed in the implant base body. These recesses have a defined geometric spatial form and are introduced by a manufacturing process in the material. The arrangement and the shape of the recesses can be determined depending on the surface structure to be generated. However, the recesses are preferably arranged regularly, particularly preferably in individual, spaced-apart rows. The rows can be parallel or at a certain angle to each other. Also, the distance of the rows can be varied to each other, depending on the surface structure to be generated. In the case of round or dome-shaped implant basic bodies, the depressions can also be arranged in a spiral or helical manner around the basic implant body.
The recesses may be formed differently depending on the intended design of the surface structure. Preferably, the depressions are configured as cuboids, pyramids, tetrahedrons, wedges, cylinders, cones, truncated cones and / or in the form of an acute-angled saddle roof. In principle, however, the depressions can also be in the form of any polyhedron.
The dimensions of the recesses may vary depending on the configuration of the surface structure. The recesses on the implant surface preferably have a maximum edge length. a maximum diameter of 0.1 mm to 3 mm, more preferably from 1 mm to 2 mm. In addition, the depressions have a maximum depth of 0.1 mm to 3 mm, particularly preferably 0.5 mm to 2 mm, to the surface of the implant base body.
As "wells" in the context of the application notches, hollows or the like are understood.
Preferably, the impressing of the depressions is carried out by pressing at least one male on the implant body. This method step preferably takes place without prior heating of the implant base body in the sense of cold forming. However, depending on the material used, the embossing can also be carried out under the action of heat.
The embossing of the depressions is preferably carried out in a single or multi-axis press.
After the depressions are embossed into the implant body, material is removed in a further step between the rows of wells. This results in individual rows of structural elements separated from one another by the depressions and the removed material.
By combining the embossing and material removal can be very fine symmetrical as well as asymmetric or staggered arranged surface structures produce, which can not be prepared by a pure machining of the implant body or only with great expenditure of time.
The removal of material between the rows is preferably carried out by a machining production process. Particularly preferably, the removal of material takes place by turning or milling. Alternatively, other manufacturing methods may be used, such as electrochemical machining (ECM) or electroerodating.
Preferably, the implant base body is deformed by deep drawing before the impressions of the depressions. This can be produced with the inventive method Gelenkspfannen.
Particularly preferably, the deep drawing of the implant body and the embossing of the wells are sequential successively, the implant body remains in the same restraint. Since the implant base body does not have to be re-clamped between the two processing steps, the production of an implant with a structured surface is accelerated and simplified.
Preferably, the depressions are impressed by lateral rolling of embossing elements on the implant base body. Alternatively, the impressions of the depressions can also be effected by linearly closing jaws.
Another object of the invention is to provide an implant with an improved surface structure. This object is achieved with an implant according to claim 9.
The implant is preferably produced by the inventive method. The surface structure of the implant according to the invention has individual structural elements which are limited by material impressions and material erosions. Material impressions make it possible to produce very fine and asymmetrically or staggered structural elements which can not be produced by a purely machining operation or only with great expenditure of time.
The rows with the structural elements can be arranged parallel to each other. Alternatively, individual rows or even all rows may be arranged at an angle relative to at least one of their adjacent rows. In the case of round or dome-shaped implants, the rows can also be designed as at least one helix or helix extending one or more times around the implant.
The depressions can be arranged in a row in each case at the same distance from each other. Alternatively, however, the recesses may also be arranged at different distances from one another on a row. The distances between the recesses can be the same on all rows. Alternatively, the distances between the pits on one row may be different from the pits of an adjacent row.
Preferably, the recesses which separate the structural elements in a row from each other, relative to the recesses, which separate the structural elements in an adjacent row from each other, arranged offset relative to each other. The staggered arrangement of the depressions can be used to create surface structures which simplify the introduction of the implant into bone or which distribute the occurring forces better to the surrounding bone.
The wells of a row are preferably arranged in each case in the middle between two wells of an adjacent row. This results in a surface structure which allows a good anchoring of the implant in bone. In addition, forces acting on the implant are optimally distributed to the surrounding bone tissue.
The structural elements defined by the depressions on the individual rows are preferably shaped as cuboids, pyramids, tetrahedrons, wedges and / or in the form of any desired polyhedra. Particularly preferably, the structural elements are in the form of an acute-angled saddle roof. The shape of the structural elements depends on the shape of the rows and the depressions. By appropriate choice of shape can thus create a variety of forms of structural elements. The structural elements preferably have a width of 0.5 mm to 3 mm. Preferably, all structural elements have the same width. Alternatively, the width of the structural elements of different rows may be different. Further alternatively, the width of individual structural elements may vary in a row.
The implant is preferably a socket, wherein the structural elements are preferably arranged in a region near the equator. Alternatively, however, the structural elements can also be arranged on the entire surface. Particularly preferably, the socket has a plurality of parallel rows, which extend substantially at right angles to the direction of insertion of the socket into the bone. Alternatively, the rows can also be arranged in the form of a thread that winds around the cup several times or in the form of spirals.
Preferably, recesses of adjacent rows in the direction of the rows are offset from each other, wherein the distance by which the recesses are offset increases from one row to the next.
As a result, the structural elements can be arranged in certain geometric patterns on the surface of the socket. Particularly preferably, the distance by which the recesses are arranged offset from one another increases from row to row in such a way that the structural elements extend over a plurality of rows along a helical or circular arcuate line. By such an arrangement of the structural elements, a joint socket can be easily introduced by a slight rotation in the bone.
A further object of the present invention is to provide a device which enables a quick and efficient production of a socket with at least one area with a surface structure. This object is achieved with a device according to claim 16.
The device according to the invention comprises an opening for a socket base body and a press die. The opening is arranged between at least two embossing elements which are pivotally mounted in the stamping direction. The joint socket main body can be pressed into the opening by the press ram.
The embossing elements are formed as patrices of aufzuprägenden to the joint socket main body structure and can be pivoted in stamping direction from a first, open position to a second, closed position. The stamping elements are designed and arranged such that in the first, open position between them an opening remains free, which is adapted to receive the joint socket main body. The pressing in of the joint socket main body leads to the swiveling of the embossing elements in the second, closed position.
In this closed position, the dimensions of the opening correspond to the dimensions of the surface of the socket base body. However, the present on the embossing elements projecting contours of aufzuprägenden structure protrude into the opening. As a result, the structure is embossed into the surface of the socket base body. The presence of at least two pivotable embossing elements leads to an increase of the embossing force by the toggle action. Particularly preferred four to twelve embossing elements are arranged in stamping direction under the recording.
The embossing elements are preferably interchangeable. As a result, a multiplicity of different surface structures can be produced with a single device.
In a further embodiment, a drawing die is additionally arranged in the pressing direction before the recording. As a result, in the same press a blank made of flat material can first be converted to a joint socket main body and then pressed onto the embossing elements. This allows a particularly fast and efficient production of semi-finished Gelenkpfannen.
The device according to the invention is preferably used in a method according to the invention for producing a joint socket implant. By modifications, such a device can also be used for processing other implants, such as femoral stem implants, knee prostheses or the like.

Fig. 1azeigt den ersten Schritt. Fig. 1 shows a schematic representation of the inventive method in a cut perspective. Fig. 1a shows the first step. In this case, a plurality of depressions 4 is impressed on an implant base body 1. FIG. 1b shows the implant semi-finished product 3 obtained after the first step. The depressions 4, which have been embossed in the first method step, are arranged in rows. In the second process step, material is removed between the rows of depressions 4, preferably with a turning or milling tool. The structured surface 9 produced by the method shown can be seen in FIG. 1c. The structured surface 9 comprises a plurality of rows of structural elements 6 which are separated from one another by indentations 4. The individual rows of the structural elements 6 are spaced apart from one another by grooves 7. The grooves 7 are formed by the material removal in the second process step, while the depressions 4 were embossed in the first process step.
FIG. 2 shows a view of a surface profile 8 obtained by the method according to the invention on a curved surface. This surface profile can be arranged for example on a socket or a femoral shaft. The depressions 4, which separate the structural elements 6 of a row from one another, are offset relative to the depressions 4 which separate the structural elements 6 of an adjacent row from one another. In the example shown, the recesses 4 of adjacent rows are each offset such that they lie in the middle between two recesses 4 of an adjacent row. Alternatively, however, the recesses 4 of adjacent rows may be offset relative to one another by a greater or lesser distance.
FIG. 3 shows a preferred embodiment of a device 10 according to the invention. The device 10 has a thermoforming die 14, onto which a blank 13 made of flat material can be laid. Furthermore, the device 10 has a pressing die 12. First, the blank 13 is pressed by the pressing die 12 through the thermoforming die. In this case, the blank 13 is deformed into a basic implant body 11. Particularly preferably, the blank 13 is reshaped into a dome-shaped socket. In a further step, the embossing of the implant base body 11 takes place. In this case, the implant base body 11 can remain clamped in the same restraint. The clamped implant base body can be supplied by moving this step. In this step, the implant base body 11 is pressed into an opening 15, in which at least two embossing elements 16 are present. The embossing elements 16 are arranged pivotably in the pressing direction about the axes of rotation 17. In this case, the embossing elements 16 are arranged and configured such that the implant base body 11 can be inserted into the opening 15 appropriately. The implant base body 11 is pressed by the ram 12 into the receptacle 15. This results in the contact of the implant base body 11 with the projecting contours 19 of the structure to be imprinted, which easily project into the opening 15. By further impressions of the implant base body 11, the embossing elements 16 pivot in the pressing direction into a second position. The projecting contours 19 are thereby pressed into the implant base body 11. Preferably, two stamping elements 16 are respectively arranged on opposite sides of the opening 15, thereby reinforcing the embossing pressure of the stamping elements 16 by the toggle lever effect. Preferably, four to twelve embossing elements 16 are arranged in the device 10, wherein these particularly preferably completely embrace the implant main body 11. After the embossing step has taken place, the implant semi-finished product 18 can be ejected from the device, for example by means of ejecting means (not shown). In a further step, which shows the Fig. 3d, material is removed between the formed by embossing rows of recesses 4 by a lathe or a milling tool. Between the depressions 4 and the grooves 7 obtained by material removal, a multiplicity of raised structural elements 6 is thus produced.
4 shows an embodiment of a joint socket 21 produced by the method according to the invention. The joint socket is designed as a spherical cap. In the region near the equator, the joint socket has a surface structure which consists of structural elements 6. The structural elements 6 are arranged in a plurality of rows 7 spaced apart from each other by grooves. The structural elements 6 are separated from one another by depressions 4, the depressions 4 of one row being offset relative to the depressions 4 of an adjacent row. The depressions 4 are arranged offset from one another in series in such a way that the depressions 4 of a row are each offset by half the distance between the depressions 4 of an adjacent row. In the region near the pole, the joint socket 21 has no structural elements 6. For better insertion of the socket, a plurality of longitudinal grooves 23 are arranged in the region of the structural elements 6.
